In the past decade, the leading form of specific lesion in children is tuberculosis of intrathoracic lymph nodes, whose proportion in the pattern of clinical forms is as high as 85.7%. The period of 1990 to 2002 showed a 3-fold increase in the detection of bilateral lesion involving more than 3 groups of lymph nodes; the specific processes were complicated by 3.5 times more frequently. The proportion of generalized forms of primary tuberculosis increased from 5 to 15%. Inclusion of mediastinal computed tomography into a package of studies significantly evaluates the magnitude of enlargement of lymph nodes. The polymerase chain reaction test for MBT DNA in the diagnosis of tuberculosis of intrathoracic lymph nodes in the absence of bacterial isolation yields a positive result in 73.3% of the children, which is an additional tool in the comprehensive study, which verifies the activity of a tuberculous process.
According to the literature, bovine tuberculosis in man has been an illness of minor importance in Norway, unlike in Sweden and Denmark. This situation cannot be explained, since in former days infected cattle were a problem in the southeastern part of the country in particular. No case of bovine tuberculosis in humans has been reported in Norway since 1940. Recently we have observed a 29-year-old female immigrant from India with cervical lymph node tuberculosis caused by M. bovis. A second case is a 77-year-old Norwegian male. In the 1920s, at the age of ten, he was infected in Norway by drinking raw milk from tuberculous cattle. He developed tuberculosis of the mesenterial lymph nodes. Neither of these patients had tuberculosis in any other part of their body. Previous Norwegian reports, covering eight cases, are summarized.
Histiocytic necrotizing lymphadenitis, also known as KIKUCHI disease is rare self-limited condition of young adults. Manifestations include enlargements of the cervical lymph nodes, sometimes with fever, and can be associated with other non-specific signs. The aetiologie of this affection is still unclear. This is a case report of a 27 year-old man who presented cervical and sus-clavicular lymphadenopathy diagnosed first as lymph nodes tuberculosis before the diagnostic of KIKUCHI disease was made.
INTRODUCTION: Mediastinal diseases are mostly diagnosed by CT and MRI. The applicability of ultrasound is limited by the surrounding air- and bone-containing thorax, which permits only restricted echo windows. Transesophageal endoscopic ultrasonography circumvents this problem and ensures visualization of parts of the mediastinum. PATIENTS AND METHODS: We report our results in 38 patients with pathological mediastinal findings who were examined by endoscopic ultrasound between 1988 and 1993. The diagnoses were established by imaging and/or histological procedures. RESULTS: The following mediastinal diseases were diagnosed in 38 patients: aberrant right subclavian artery (n = 3), right aortic arch (n = 1), aortic aneurysm (n = 6), cysts (n = 4), retrosternal struma (n = 3), mediastinal lymph node tuberculosis (n = 1), Hodgkin's/non-Hodgkin's lymphomas (n = 11), lymph node involvement in bronchogenic carcinoma (n = 8), mediastinal inflammatory fibrosarcoma (n = 1). Altogether, 37/38 pathological findings were demonstrated endosonographically. CONCLUSIONS: The results in this small group of patients with pathological mediastinal findings show that endoscopic ultrasound can give additional information to conventional imaging methods. A prospective comparative study is necessary to evaluate this procedure in comparison to the established imaging techniques.

A case of parenchymal and intraparotid lymph node tuberculosis is reported in a young Asian male. The diagnosis was suggested pre-operatively by the contrast enhanced CT (CECT) appearance. There were no systemic symptoms of tuberculosis and this is a characteristic feature of the 100 cases reported in the literature. The presence of thick walled rim enhancing lesions with a central lucency on CECT should suggest the diagnosis. Filling defects with or without thin walls are non-specific findings and are seen in tumours and other inflammatory processes. In an appropriate clinical setting, thick walled round rim enhancing lesions with a central lucency are characteristic of tuberculosis.

The diagnosis and management of peripheral lymph node tuberculosis remains a major problem in most of the developing countries. We retrieved 584 cases of tuberculous lymphadenitis from a total 1124 lymph node aspirations done over a period of 3 years (1995-1998). Overall acid-fast bacillus positivity was 37.4%, being highest in the cases in which purulent material was aspirated. Fine needle aspiration (FNA) of tuberculous lymphadenopathy provided a high level of diagnostic accuracy as shown by 1.7% false negative and a zero false positive rate. FNA is reliable as an initial evaluating procedure for diagnosis of tuberculous lymphadenitis making it suitable for wider application in developing countries with scant resources.
