[Age-related characteristics of tumor development in ovaries transplanted into the rat spleen].

Female rats were ovariectomized at the age of 3 or 14-16 months, one of the ovaries being autologously transplanted into the spleen. The frequency of tumor development in transplanted ovaries was identical in both age groups. Older rats developed ovarian androblastoma earlier and with higher frequency, while granulosa cell tumors of the ovaries prevailed in the younger age group. 35.8% rats with intrasplenic tumours had other neoplasms. Age is thought to be the key factor influencing the proliferative activity of spleen-transplanted ovarian tissues in response to carcinogenic challenge.

[A study of morphopathological changes in the ovary caused by parabiotic union in rats].

Male and female Wistar rats from the same brood were joined parabiotically and histopathological changes in the ovary of the reactor were observed from the 8th to the 41st week. Only one tumor, a granulosa cell tumor appearing in the 28th week, was seen to form. Histogenetically, it appeared to have arisen from the follicular granulosa cells. In many cases, stromal luteinization progressed until the entire ovary appeared to consist of luteinized tissue, but in no case were tumor features noted. Changes followed the same process as in autotransplantation of the ovary into the spleen or as in irradiation; eventual tumor formation was judged likely from the observations in some cases. The cause of the extremely low rate of tumor formation in this experiment is thought to be inhibition of gonadotropin production by estrogen transferred from reactor to partner via capillary communicating branches.

Clinicopathologic review of 118 granulosa and 82 theca cell tumors.

The clinical course and histologic features of 118 granulosa cell tumors and 82 theca cell tumors were reviewed. Although the 2 cell types are related, important differences exist in their behavior. Theca cell tumors (TCTs) are virtually benign, while granulosa cell tumors (GCTs) exhibit less malignant activity than most ovarian carcinomas. Characteristically detected at an early stage, GCTs may recur locally years after the initial diagnosis. Survival figures depend on tumor stage. Recurrence of GCTs is associated with a high mortality rate. The cornerstone of treatment is total abdominal hysterectomy and bilateral salpingo-oophorectomy, regardless of age. Adjuvant radiation or chemotherapy may be helpful when lesions are higher than stage lai. Therapy for recurrence usually requires operative excision combined with radiation, chemotherapy, or both.

Fine structural evidence on the origin of gonadotropin-induced ovarian tumors in mice.

A nonluteinized granulosa cell ovarian tumor and a purely luteinized ovarian tumor, induced by gonadotropin stimulation in mice, were studied ultrastructurally to obtain evidence concerning the possible origin of these tumors, whether from granulosa cells only or from granulosa and theca. Evidence including the presence of cells sharing features of both nonluteinized and luteinized cells, the presence of desmosomes and other junctional complexes in both cell types, and the presence of partially extracted lipid droplets in the luteinized cells suggests that the luteinized cells represent granulosa-lutein cells rather than theca-lutein cells. These findings are consistent with the concept that these tumors represent a spectrum of differentiation from nonluteinized granulosa cells to luteinized granulosa cells. The lack of theca and theca-lutein cells in both tumors implies that theca cells are not involved as progenitor cells.

Fibrothecoma of the broad ligament.

We present a patient with a primary extraovarian fibrothecoma, an extremely rare lesion originating in the broad ligament. Possible sources of origin and significance are discussed.

Diagnosis and management of bilateral theca lutein cysts in a normal term pregnancy.

Theca lutein cysts are benign neoplasms known to be associated with twins, molar pregnancy and erythroblastosis fetalis, but they are only rarely associated with a normal singleton pregnancy. Their natural course is postpartum spontaneous regression. Documented cases were noted because of abdominal pain or dystocia. This paper reports theca lutein cysts which were asymptomatic and were first noted ultrasonographically in a singleton gestation at term. In a few published cases and in a larger number of unpublished cases, apparent confusion as to the benign nature of these cysts has led to unwarranted bilateral oophorectomy. Asymptomatic theca lutein cysts undoubtably occur more commonly than they are reported. With the increasing use of ultrasonography and cesarean section, this condition probably will be observed more frequently in the future. At the time of laparotomy in late pregnancy with ovaries which appear pathological, tissue biopsy and evaluation is of critical importance in order to avoid unnecessary castration in this group of young women.

Trophoblastic growths: morphologic aspects and taxonomy.

The hydatidiform mole-gestational choriocarcinoma complex constitutes a graft qua tumor whose natural invasion, rejection and resolution share features of normal pregnancy and of neoplasms of other tissues. Trophoblastic neoplasms sometimes arise from morphogenetically abnormal chorions, notably the "partial mole," among others. Thus, they provide another demonstration of the association of teratogenic and carcinogenic events. Gestational choriocarcinoma is a complication of pregnancy, not a sequel to it. Although these tumors may arise from the trophoblast of any pregnancy, presenting symptoms during gestation or following it, they are much rarer than the innocuous conditions, normal and abnormal, with which they may be confused. A bone fide malignant neoplasm is less likely to be misdiagnosed as a banal phenomenon than is the reverse. Morphologically, the trophoblast is the key, and emphasis should be placed on its behavior. Taxonomy should follow suit, employing the simple, explicit terms chorioma and choriocarcinoma, graded to express the neoplastic attributes of the constituent epithelium.
