Experiences with DHE: an order entry and result reporting case study.

Organization of hospital information systems that are currently in use is characterized by a multitude of simple, unfortunately less integrated components. For the integration of the already available as well as new components into a hospital information system, the "Committee European de Normalisation" (CEN) proposed a basic "Conceptual Architectural Framework". Based on the CEN proposal, in this paper we present first results of a case study in order to investigate the usability of the Distributed Healthcare Environment (DHE), as a standard architecture for hospital information systems. Another aim of this case study was to design an Order Entry and Result Reporting System and implement it by using DHE.

Neural network analysis of biochemical markers for early assessment of acute myocardial infarction.

Neural network analysis was applied for early diagnosis/exclusion of acute myocardial infarction and prediction of infarct size. Eighty-eight patients admitted with onset of chest pain within 8 hours were included. Frequent blood samples for measurement of myoglobin, CK-MB and troponin-T were obtained and used in the development of a set of neural network components of a decision support system. The results indicate that this approach could provide useful support for assessment of patients with suspected AMI.

From natural language to formal language: when MultiTALE meets GALEN.

In the GALEN project, the syntactic-semantic tagger MultiTALE is upgraded to extract knowledge from natural language surgical procedure expressions. In this paper, we describe the methodology applied and show that out of a randomly selected sample of such expressions, 81% could be analysed correctly. The problems encountered are summarised and areas of further investigation identified.

Software quality assessment for health care systems.

The problem of defining a quality model to be used in the evaluation of the software components of a Health Care System (HCS) is addressed. The model, based on the ISO/IEC 9126 standard, has been interpreted to fit the requirements of some classes of applications representative of Health Care Systems, on the basis of the experience gained both in the field of medical Informatics and assessment of software products. The values resulting from weighing the quality characteristics according to their criticality outline a set of quality profiles that can be used both for evaluation and certification.

Taking HL7 to the next level.

For HL7 to reach the next level of standards capability, the organization must continue their model-centric development process so systems using the standards can achieve interoperability--sematic as well functional. The HL7 Message Development Framemwork specifies thi process detail and anchors future HL7 standards with a common reference information model. That model will evolve through a process of discussion, amendment, voting to continuously harmonize proposals from committees that have specific spheres of knowledge.

Engineering in software testing: statistical testing based on a usage model applied to medical device development.

When a population is too large for exhaustive study, as is the case for all possible uses of a software system, a statistically correct sample must be drawn as a basis for inferences about the population. A Markov chain usage model is an engineering formalism that represents the population of possible uses for which a product is to be tested. In statistical testing of software based on a Markov chain usage model, the rich body of analytical results available for Markov chains provides numerous insights that can be used in both product development and test planing. A usage model is based on specifications rather than code, so insights that result from model building can inform product decisions in the early stages of a project when the opportunity to prevent problems is the greatest. Statistical testing based on a usage model provides a sound scientific basis for quantifying the reliability of software.

Doppler Monte Carlo simulations of light scattering in tissue to support laser-Doppler perfusion measurements.

Doppler Monte Carlo (DMC) simulations of the transport of light through turbid media, e.g., tissue, can be used to predict or to interpret measurements of the blood perfusion of tissue by laser-Doppler perfusion flowmetry. We describe the physical and mathematical background of Doppler Monte Carlo calculations, and present some comparisons with measurements, performed with experimental flow models, built to mimic skin tissue characteristics and for the calibration of perfusion instruments,which are important goals of the Biomed-I concerted action. The measurements deal with coherence effects.

Processing of laser-Doppler signals from free flaps.

As conventional graphical laser-Doppler flowmetric (LDF) registrations from free flaps are difficult to interpret we explored the use of refined computerized signal processing to enhance the reliability of the blood flow supervision postoperatively. From eleven free flaps LDF data were collected using a software programme and a personal computer for analysis. Findings were compared with the clinical outcome. Nine flaps healed whereas one had wound problems and one suffered a partial necrosis. From the nine uneventful flaps, a peak within the range of frequencies from 0.04 to 0.23 Hz was seen. In the remaining two, such a low frequency peak could hardly be observed. Frequency analysis using computerized processing of LDF signals thus has the capacity to demonstrate the status of the flap perfusion. The slow wave vasomotion component seems to be of particular importance. Other frequency components warrant further investigation. A custom made monitoring device would be of great clinical value.

An institution-based process to ensure clinical software quality.

Clinical software can have a major impact on the delivery of care. It is imperative that clinical software undergo regular quality review, to evaluate the clinical correctness of the specification, the technical correctness of the software, problems that have arisen, and maintenance of the software as conditions change. We have developed a process using existing hospital review groups to perform clinical review, and using a project specification form and analysis of likely problem areas to effect technical review.

Assurance: the power behind PCASSO security.

The need for security protection in Internet-based healthcare applications is generally acknowledged. Most healthcare applications that use the Internet have at least implemented some kind of encryption. Most applications also enforce user authentication and access control policies, and many audit user actions. However, most fall short on providing strong assurances that the security mechanisms are behaving as expected and that they cannot be subverted. While no system can claim to be totally "bulletproof," PCASSO provides assurance of correct operation through formal, disciplined design and development methodologies, as well as through functional and penetration testing. Through its security mechanisms, backed by strong system assurances, PCASSO is demonstrating "safe" use of public data networks for health care.

Clinical informatics case study. Computerized protocols for ventilator management in ARDS patients. Case study.

The implementation of the computerized protocol for mechanical ventilation management should be considered successful. Of the 11 sites, only 4 encountered major difficulties. One site could be considered an implementation failure. At this site the protocol software, which was loaded onto the existing computerized patient documentation system, did not initially function smoothly and it severely affected the trust at the clinical site. At the other three sites system usage was minimal due to time requirements of the study rather than a failure to accept the decision support system. Implementation methods suggested by authors such as Whitten and Bentley were found to be successful. Methods of particular use included obtaining buy-in of key personnel, contacting and working with IS personnel early in the project, providing adequate training and reference materials for clinical and support personnel, and providing extra training for some users, thereby creating a "super-user" role. Finally, factors for success of a clinical trial to evaluate a decision support system are slightly different from those for successful use of the system. Time necessary for research functions such as patient screening and recruitment must be considered at the outset and planned for accordingly.

The use of the arcs software system to store and examine sleep research results.

Ready access to research results is essential if nurses are to keep current with the scientific knowledge available to guide practice. Sigma Theta Tau International has supported the development of a software system that facilitates ongoing storage of information from research reports and can be used to gain online access to continually updated databases of research results called "knowledgebases." One such experimental knowledgebase, the Sleep Promotion Knowledgebase, includes studies of correlates of poor sleep in adults as well as sleep interventions and outcomes. The authors describe the creation of the Sleep Promotion Knowledgebase using arcs BUILDER to store information about research reports and the scientific findings they contain. The use of arcs MAPPER to examine sleep promotion interventions also is described. The strengths and limitations of the software system are identified, and the potential uses of arcs to disseminate sleep research results are discussed.
