[Diffuse recto-colic malacoplakia: association with cryptogenic colitis and remission following long-term antibiotic therapy].

Diffuse digestive malakoplakia appears exceptional. A case of rectocolic malakoplakia with multiple localisations is reported in a 22 year old man presenting with inflammatory bowel disease. The most prominent clinical features were deterioration of his general condition, fever, and rectal bleeding, with fistula. Endoscopy revealed pseudotumoral masses and multiple colorectal ulcerations. Diagnosis was based on histological examination of colorectal biopsies. Clinical and histological remission was obtained after colonic diversion associated with broad spectrum antibiotherapy. These findings raise the problem of the possible association between inflammatory bowel disease and malakoplakia. They also confirm that, as previously reported, favourable outcome in digestive malakoplakia is possible.

[Comparison between hypo- and hyperglucidic diets on protein sparing in major visceral surgery (author's transl)].

The authors compare the protein sparing effect of two diets, exclusively intravenous, including the same protein intake, but a different caloric intake, 21 calories/gm nitrogen for diet "A" (20 cases); 138 calories/gm nitrogen for diet "B" (20 cases). This has been observed during the six post-operative days of major visceral surgery: oesophagectomy, total gastrectomy, colic or rectocolic exeresis, sequestrectomy for acute pancreatitis, lots having been drawn for the diets. Daily nitrogen balances have been made and plasmatic and urinary levels of amino-acids have been measured before surgery and on the third and fifth post-operative days. Statistical exploitation is done by variance analysis (linear model of three factors) with a 99% confidence ratio: 1) Patient factor has no influence whatsoever on cumulative nitrogen balance. 2) Time factor arises only on the fourth post-operative day and only in the hypocaloric diet, leading to catabolism. 3) Metabolic condition is determinant. On no cancerous disease, superiority of hypercaloric diet is well demonstrated. On cancerous disease, nitrogen loss is only significantly different on 4th and 5th post-operative day: hypercaloric diet gives a better nitrogen balance.

[Primary sclérosing cholangitis (author's transl)].

Primary sclerosing cholangitis is a rare affection. Two characteristic cases are described. A total of 80 cases have been reported in the published literature, and lesions are found varying from lacalized to diffuse forms. The affection is often associated with other disorders, especially hemorrhagic ulcerative rectocolitis, suggesting an auto-immune pathogenicity. Treatment is mainly surgical and consists, in the localized forms, of complete excision whenever possible and a by-pass operation. The diffuse forms, which are difficult to treat by surgicall methods, have a poorer prognosis and usually progress towards secondary biliary cirrhosis.

[Ruptures and "spontaneous" perforations of the rectum and sigmoid: a report on two cases and review of the published literature (author's transl)].

Though ruptures and "spontaneous" perforations of the rectum and sigmoid are sometimes confused they are very different affections. Ruptures of the rectum or the rectosigmoid junction are lesions due to effort in chronically constipated subjects, frequently having rectal prolapses. They are due to a particular type of trauma. Perforations of the rectum, rectosigmoid junction, or the sigmoid loop arise from a previous acute ulceration of the intestinal wall. Systematic histological examinations may confirm the etiology. A case is reported of rupture of the rectum due to effort. The second case was a double perforation, of the rectum and the rectosigmoid junction, due to a non-specific acute rectocolitis.

[Pseudomyxoma peritonei associated with appendiceal mucinous cystadenoma. Report of a case and review of the literature].

Mucinous cystadenoma of the appendix is a rare entity. Its incidence is low, between 0.1 and 0.3%. Constitutes are found incidentally al surgery and is generally asymptomatic. We present a case of a 60 years old woman which was complicated by a pseudomyxoma peritonei, idiopathic ulcerative rectocolitis and infiltration of the muscular layer of the colon sigmoid wall with poorly differentiated adenocarcinoma. We reviewed the literature, emphasize modern concepts about its terminology and its possible pathogenesis.

[Surgical indications in ulcerative colitis: European approach].

Ulcerative colitis can only be cured by surgery. Colonic perforation and rectocolic cancer are absolute indications for surgery. Severe attacks of ulcerative colitis are classically managed for 5 days by intensive medical treatment, including intravenous and rectal corticosteroids. Colectomy must be performed if this treatment fails. Although not universally accepted, endoscopy is of great help to select patients needing early surgery. Finally, chronic continuous and cortico-dependent forms will obviously benefit from surgery. Proctocolectomy with ileal pouch-anal anastomosis clearly makes certain surgical decisions somewhat easier.

[Contribution of immuno-hematology in understanding autoimmune phenomena].

Acquired hemolytic anemia (AHA) can be considered under four aspects: 1) The nature and specificity of involved autoantibodies correspond to weakly differentiated structures of high frequency but normal human blood group antigens. 2) The pathologic role of autoantibody can be experimentally demonstrated by parallel measures of the specific red cell survival. 3) In certain cases of AHA--secondary to infections (Mycoplasma pneumoniae, infectious mononucleosis) or drugs (methyl dopa), or related to tumours (dermoid ovarian cyste, ovary teratoma) or tissue damages (haemorragic rectocolitis)--the relation between a clinical precise circumstance and the production of autoantibody can actually be demonstrated. 4) In idiopathic AHA, in addition to various autoantibodies, an immunological disorder is observed, including the production of allo- and heteroantibodies and, in 50% of the cases, a global deficit in immunoglobulin. 5) On these bases, the nature of autoimmunity is discussed. If specific modifications can explain secondary AHA, they can hardly account for idiopathic AHA. Cellular non-specific anomalies should be considered.

[Body Image Index in the Rorschach test in ulcerative proctocolitis].

The use of the Rorschach test allows the body image to be examined using the Body-Image Index formulated by Fisher-Cleveland. This index is obtained by calculating the responses to Penetration and Barrier. The hypothesis is that the diseases which affect the inside of the body are more greatly correlated to a high P score and a low B score (high P-low B type personality). The paper examines the Body-Image Index in a sample of 41 patients suffering from ulcerous rectocolitis comprising 24 males and 17 females with a mean age of 32 years. The personality profile obtained is of the "high P-low B" type with a P/R% ratio which is clearly higher than the central mean value (28.7% vs 8%) and a B/R% ratio which is clearly lower than the central mean value (4.2% vs 18%). The personality of the patients in this group was marked by a tendency to anxiety and a tendency to express aggression within their bodies (anger-in) which was field-dependent, difficulty in tolerating stress, a tendency to react somatically to conflicts, a low degree of body image stability and a constant need for an external holding object.

The surgical treatment of vascular and gastrointestinal implications in Behçet disease. Report of four cases.

This study reports 4 cases of Behçet Syndrome which came under our observation and were surgically treated for vascular or gastrointestinal manifestations. These cases included 3 males and 1 female, mean age 26.5 years, of which 2 were treated with implantation of prosthesis for aneurysm of the abdominal aorta with recurrence of Behçet vasculitis and prosthesis substitution three years later in one case, 1 with total colectomy for ulcerous rectocolitis, while the last case (female) required excision and plastic reconstruction, for a perforated duodenal ulcer. Immediate and long-term results of the surgical treatment of the gastrointestinal and vascular manifestations and/or complications were satisfactory, whereas the long-term results of the medical treatment of the basic disease were unsuccessful in 50% of cases. BS is a rare affliction, whose pathogenesis is still poorly defined and in which the results of medical treatment cannot be regarded as satisfactory to date. Surgical treatment of its vascular and gastrointestinal manifestations and complications, on the other hand, was effective in our experience.

[Morbidity estimates of beneficiaries of national health insurance for seven chronic conditions].

This work is aimed at providing improved coefficients applicable to the statistics of Health Insurance concerning the long-lasting conditions in order to assess morbidity of Health Insurance members concerning diseases invoked legislation on long-lasting conditions. To reach this objective, we have applied the consequences of successive equivalence (between diseases and their specific treatment, and between the use of special prescriptions for exempting diseases and the acknowledgment of long-lasting conditions by Health Insurance) to the results of a survey on medicines carried out by the Caisse Nationale d'Assurance Maladie des Travailleurs Salariés in 1993 on the basis of a representative sample. Improved coefficients were equal to the inverse of the proportion of special prescriptions within the whole of the prescriptions including specific treatment. Those coefficients and their confidence intervals have been calculated for seven affections: non-insulin dependent diabetes [1.56-1.67], coronary disease [1.51-1.61], Parkinson's disease [1.51-1.85], manic-depressive psychosis [1.49-1.89], haemorrhagic rectocolitis and Crohn's disease [1.49-2.04], insulin dependent diabetes [1.10-1.20] and tuberculosis [1.61-3.33]. The critical analysis of our work and its comparison with the rare literature on the same subject confirms the value of our results. The application of those improved coefficients to the figures of prevalence and incidence provided by Health Insurance makes it possible to set confidence bounds to the statistics of morbidity concerning the people who are affiliated to that system of Health Insurance and treated for these diseases however serious they are.

[Pyostomatitis-pyodermatitis vegetans uncovering a case of Crohn disease].

BACKGROUND: Pyostomatitis-pyodermatitis vegetans is an uncommon condition associated with chronic inflammatory bowel disease in 75% of the cases, usually hemorrhagic rectocolitis. CASE REPORT: A 48-year-old man was referred for recent development of pustulous lesions of the lips and buccal mucosa and weight loss. He complained of abdominal pain and intermittent diarrhea which had persisted for more than one year. During the last three months, a pseudotumoral plaque with a pustulous rim had developed over the two distal phalanxes of the right middle finger in association with ungueal lysis and nodular, vegetating and crusted lesions on the lateral aspect of the left arm. Small pustules covered the entire buccal mucosa excepting the tongue and the glans forming a typical snail trace aspect. Bacterial and mycological samples were negative. The histology reports for skin and mucosa were similar: epithelial hyperplasia, intra- and subepithelial granulocyte micro-abscesses and polymorphous infiltration of the superficial derma with numerous neutrophils and eosinophils. There was a discrete interkeratinocytic fluorescence at direct immunofluorescence but indirect immunofluorescence was negative. Anti-desmogleine 1 immunolabeling showed typical normal skin uptake and immunotransfer was negative. Digestive tract endoscopy and histopathology examination of the bowel specimens confirmed the diagnosis of Crohn's disease. Clinical manifestations improved dramatically with prednisone. DISCUSSION: This case of pyostomatitis-pyodermatitis vegetans involved several aspects rarely reported in the literature: a) the cutaneomucosal signs were inaugural; b) the association with Crohn's disease; c) the presence of lesions to the genital mucosa; d) the unusual localization of the inaugural skin manifestations. This clinical entity has now been clearly distinguished from pemphigus vegetans. There was however a long debate on the similar clinical, histological and even immunological expressions. We suggest that pyostomatitis-pyodermatitis vegetans belongs to the spectrum of neutrophilic dermatoses and other authors even propose it is a clinical form of pyoderma gangrenosum.

[The solitary rectal ulcer today. A review of the literature].

The solitary rectal ulcer (SRU) is a benign lesion of adults of either sex, which presents with chronic constipation, peculiar defecatory disorders, rectal prolapse and smaller psychological abnormalities. The characteristic appearance of this disease is a "neither being always ulcerate, nor always solitary" lesion, but often with polypoid or granular feature, typically localized in anterior rectal wall, a few inches from anal channel. Distinctive histopathological specimens are localized mucosal distortion, hypertrophic proliferation of muscularis mucosae and obliteration of lamina propria by fibroblasts and muscle fibres from the muscularis mucosae. Very few intermittent or recurrent symptoms are rectal bleeding and mucous discharge with defecations, difficulty of a complete ampullar evacuation and sometimes pelvic or rectoperineal pain. Clinical picture and endoscopic biopsies led to diagnosis. Barium enema, defecography, transrectal ultrasound, manometry and electromyography have an additional role. Medical treatment is performed by high-fiber diet, but biofeedback training is very helpful. Surgical management is as an excisional surgery, as a rectopexy if there is prolapse. Fecal diversion and rectocolic resection are considered only for patients with obstinate and severe symptoms. Even in patients who seem to advocate a surgical approach it is important to heal a dyskinetic puborectalis muscle.
