Imminent concerns of filial caregivers reporting recent experiences of crisis.

The purpose of this descriptive study was to generate information about imminent concerns of adult children that could serve as initial context for development of a meaningful framework for coping with an ongoing parent care situation. Ninety-two adult children pre-selected for self-reports of crisis were interviewed about their concerns and goals for caregiving and asked to discuss experiences of crisis in the previous six months of caregiving. Key issues pertaining to their experiences were extracted from notes of the interviews and classified according to their common properties. Cohen's kappa for interrater reliability of the classifications was .79 while percentage agreement was 98 percent. The results of the study point to the presence of multiple issues pertaining to significant life and death events, relational burdens, early experiences with the family of origin, and focal patterns of distress and coping. Synthesis of the findings reveals overriding concerns for making improvements in one's family of origin, created family of adulthood, and middle-aged self. Findings have implications for experiential adult education in the area of filial maturity and caregiving.

Genetic diversity of Streptococcus suis serotypes 2 and 1/2 isolates recovered from carrier pigs in closed herds.

The aim of this study was to compare, by randomly amplified polymorphic DNA (RAPD), the diversity of Streptococcus suis serotypes 1/2 and 2 isolates recovered at slaughter houses from the tonsils of clinically healthy pigs. The pigs belonged to herds with or without clinical signs of S. suis disease. Overall, a low diversity was observed among isolates of serotype 1/2. A representative isolate recovered from a diseased animal presented a relatively high similarity (85%), with most isolates recovered from carrier pigs, from herds either with or without clinical signs of S. suis disease. For serotype 2 isolates, a relatively high degree of heterogeneity was observed in the whole population. Two subpopulations were observed for serotype 2 isolates, which arose from herds with clinical signs. Interestingly, the representative isolate coming from the diseased pig was included in a small closed cluster, with 2 isolates recovered from carrier pigs belonging to the same herd. On the other hand, most of the S. suis serotype 2 isolates originating from herds with no history of S. suis disease, were closely related (90% similarity). Furthermore, they presented different RAPD patterns from those originating from animals from the herd presenting S. suis clinical signs due to this serotype. Results suggest that, in the herds studied, clinical manifestations due to serotype 2 are probably related to the virulence of a specific isolate. Conversely, for the herd affected with serotype 1/2, clinical manifestations of the disease were more likely to be the result of inherent herd factors than the virulence of the specific isolate.

The extrastriate generators of the EP to checkerboard onset. A source localization approach.

The cortical origin of the pattern onset EP has been investigated over a time window which covers the entire positive-negative-positive complex of the pattern onset EP. On the basis of a dipole source localization approach, the position, orientation and strength of the underlying sources of the pattern onset EP were estimated. For large check stimuli, chosen to have a weak edge specific component in the response, still two components are needed to account for the variance of the responses. Each component corresponds to a single dipole source, and both originate in the extrastriate cortex. These components dominate, respectively, the initial and the late positive peaks of the pattern onset EP. The equivalent dipole sources of the two components show different behaviors with respect to the position of the stimulus in the visual field. The topography and behavior of the equivalent dipole source underlying the early positive component suggest an origin in area 18. The invariance with stimulus location of the dipole source underlying the late positive component suggests an origin beyond area 18. The different topographies of the components also account for the differences in surface distribution of the pattern onset EP to large check stimulation of the upper and lower sectors of the visual field.

Cytology of bovine foetal fluids.

Cell contents of foetal fluids obtained separately from amniotic and allantoic cavities of live cows by transvaginal amniocentesis and by aspiration from pregnant uteri immediately after slaughter were studied using Papanicolaou's staining procedure. The samples obtained belonged to cows which were between ten to 24 weeks of gestation. Cell scrapings from foetal and membranous surfaces normally in contact with the foetal fluids were also stained simultaneously with foetal fluid smears by Papanicolaou's method to determine the probable origin of the cells detected in the amniotic and allantoic cavities. The cell types were classified according to morphology and tinctorial patterns and their origin was deduced on the basis of similarity to cell scrapings from the surface layers of foetus and foetal cavities. The evaluation of foetal fluid cells revealed that a majority of cells are of foetal origin and that the cytological features of these cells may be useful in assessing the condition of the foetus.

Two cytologic patterns in invasive ductal adenocarcinoma of the pancreas.

OBJECTIVE: To clarify 2 cytologic patterns: severe ductal dysplasia (SD)/carcinoma in situ (CIS) and invasive components of invasive ductal adenocarcinoma of the pancreas (IDAP). STUDY DESIGN: Tumor samples from 12 patients with IDAP were examined cytologically and histologically. Cytologic specimens were obtained by fine needle aspiration (ENA) and imprint smears from resected pancreases. RESULTS: In the 12 IDAP cases, roughly 2 cell populations, SD/CIS-type cells and invasive component-type cells, were detected cytologically. The former composed a small portion of the tumor cells, exhibited a small nuclei without anisonucleosis and had a decreased total quantity of chromatin as compared with invasive component-type cells. SD/CIS-type cells were found in small papillary-cohesive and compactly packed clusters and had clearly defined cytoplasmic borders, a nucleus individually enveloped in well-preserved cytoplasm and small amount of cytoplasm (< 15 microm in short diameter) without prominent anisocytosis. The latter comprised mostly of the tumor cells, were present characteristically as a loose and discohesive population and contained a combination of hyperchromatin, large nuclei (> 15 microm) and abundant cytoplasm (>21 microm). Histologically, these 12 cases exhibited IDAP with SD/CIS. Hence, the invasive component-type cells appeared to originate with ordinary invasive spread of IDAP, and the SD/CIS type cells appeared to originate with noninvasive intraductal spread. CONCLUSION: Two cytologic patterns originating with SD/CIS and invasive components of IDAP could be differentiated.

Endogenous and ectopic expression of noggin suggests a conserved mechanism for regulation of BMP function during limb and somite patterning.

The gene noggin, originally cloned in Xenopus, encodes a secreted factor expressed in the Spemann organizer, where it functions to oppose the ventralizing influence of bone morphogenetic proteins (BMPs). Noggin protein acts by binding directly to BMPs, thereby preventing them from interacting with their receptors. Here we describe the pattern of expression of the chicken noggin gene during somite and limb development, two tissues in which BMPs have been postulated to play essential patterning roles. We find that noggin is expressed in dynamic restricted patterns consistent with an important role in the modulation of BMP signaling. Using a replication competent retrovirus we have ectopically expressed noggin in developing somitic and limb bud mesoderm and observed phenotypes consistent with complete block of BMP activity. This includes suppression of lateral somite differentiation and, in the limb, complete inhibition of chondrogenesis and local suppression of programmed cell death. In addition, we find that ectopic noggin expression in the limb has no effect on anteroposterior limb pattern, suggesting that BMPs are unlikely to play a significant role in this process. Taken together,, our results indicate that noggin is a key regulator of vertebrate limb and somite patterning and suggest that the antagonistic Noggin-BMP interaction is a widely used mechanism to modulate BMP signaling during multiple inductive events in vertebrate embryogenesis.

Using the 1983 norms for the MMPI: code type frequencies in four clinical samples.

MMPI responses obtained from a large random sample of midwestern adults who were 18 through 99 years old and had no physically or mentally handicapping condition yielded a pattern significantly different from the pattern of the original norms. From these data, two new kinds of normative tables have been developed: Norms that reflect, for each sex, the response pattern of the general adult population, and a set of tables, separate for each sex, that allow comparisons to be based on age. In addition, the traditional scoring procedures based on a linear transformation that maintains any underlying skewness of the raw score distribution has been replaced by procedures that yield normalized T scores. The changes that are apparent at the item and scale level are also evident in the frequency with which certain 1- and 2-point codes occur in normal and clinical samples. These changes are apt to make our interpretive statements more meaningful because they are based on contemporary norms.

Redox potential and electrostatic effects in competitive inhibition of dual-path electron transfer reactions of spinach plastocyanin.

Redox inactive ions with high positive charges lower the rate constant for oxidation of several plant plastocyanins (PC) by small positively charged inorganic reaction partners. The rate constant decrease is commonly attributed to competitive inhibition where the redox inactive ions are bound to the negatively charged remote electron transfer (ET) site of PC and block this site sterically. We have investigated the effects of the inhibitor [NH3)5Co(NH2)Co(NH3)5]5+ on the ET reactions of spinach PC with [Co(phen)3]3+ (phen = 1,10-phenanthroline) and the electrically neutral analogue [Co(phen-SO3)3] (phen-SO3 = 5-sulfonato-1,10-phenanthroline) at the ionic strengths mu = 0.1 M and 0.03 M. Inhibition of the [Co(phen)3]3+ reactions is notably smaller for PC(II) reduction than for PC(I) oxidation. This is indicative of a redox potential increase of PC(II)/PC(I) on inhibitor attachment. The effect amounts to 16 mV at mu = 0.1 M and 31 mV at mu = 0.03 M. These data, and analysis in terms of ET theory show that inhibition cannot be caused solely by steric blocking. Driving force and interreactant electrostatic work terms are equally important. The PC(I)/[Co(phen-SO3)3] reaction exhibits a more entangled pattern. The rate constant first increases slightly with increasing inhibitor concentration, then drops, and approaches a constant value not far from the original value. This pattern is in line with association between the negatively charged -SO3- groups of the Co(III) complex and the inhibitor, and ET of the associate at both ET sites of PC.

Investigation of the bifurcation level of the common carotid artery and variations of the branches of the external carotid artery in human fetuses.

Variations in the position of the bifurcation of the common carotid artery (CCA) and the origin or branching pattern of the external carotid artery (ECA) are well known and documented. In this study, the bifurcation levels of the CCA and origin variations of the branches of the ECA have been extensively investigated in human fetuses. Bilaterally, 40 carotid bifurcations and 40 ECA and their branches have been examined. A latex solution, which had been diluted with water at a ratio of 25% and colored with red India ink, was injected into the aortic arch. Fetuses were kept at room temperature for 24 h before the ECA and its branches were dissected under the microsurgery microscope. Cases of variation were determined and photographed. The bifurcation level of the CCA was determined to be 55% at the C3 level, 35% at the C4 level, 10% at the C5 level on the right side and 60% at the C3 level, 40% at the C4 level on left side. The distribution of the ECA trunks was determined as follows: A linguofacial trunk was present in 20% of the cases, a thyrolingual trunk in 2.5%, a thyrolinguofacial trunk in 2.5% and an occipitoauricular trunk in 12.5%. Beyond this the ascending pharyngeal artery (APA) was observed in one fetus to originate from the internal carotid artery (ICA). Knowledge of variations in the origin and course of the ECA and its branches is of great importance in surgery and radiological examinations.

[Biology of periodontal regeneration: a review and a hypothesis for future researches].

Since the end of the seventies several studies have been carried out about the possibility of regeneration of periodontal osseous defects. The results of such researches supported by histological tests allowed to establish which surgical techniques could yield periodontal regeneration in a predictable way. In spite of a quite large diffusion of these sugical techniques in the treatment of periodontal osseous defects, some biological aspects of periodontal regeneration are still unknown. The most important among them is the origin and the differentiation pathway of the synthetic cells that shall provide for the reconstruction of the deep periodontium. Particularly, the phenotypic pattern and the origin of cementoblasts are not clear. Our researches focused on pericytes, a mesenchyma-derived cell population with remarkable differentiation capacities which have a microvascular location. We considered the hypothesis that pericytes could play an important role in regeneration of the alveolar bone, of the cementum and of the periodontal ligament. Should pericytes be actually the precursors of osteoblatsts, fibroblasts and cementoblasts involved in periodontal regeneration, new therapies, such as cultivated cells reimplantation or the targeted use of growth factors could be feasible. Future researches shall verify these observations.

[The outer membrane protein (OMP) patterns of Escherichia coli isolates of predominant O serogroups originated from chichens in different regions in China].

The purpose of this study is to determine the outer membrane protein (OMP) patterns of avian Escherichia coli isolates with predominant serogroups originated from 18 provinces, autonomous regions and municipal cities in China. Total of 204 isolates belonging to O18, O78, O2, O88, O11 and O26 serogroups respectively, were tested. The outer membrane proteins of these isolates were isolated with the improved N-lauroylsarcosine method and analyzed by SDS-PAGE. 4 different OMP patterns were identified with these isolates of predominant serogroups. 3 different OMP patterns with 56 isolates of O18, 4 with 54 isolates of O78, 2 with 28 isolates O2, 1 with 26 isolates of O88, 3 with 22 isolates of O11 and 1 with 18 isolates of O26 respectively, were found. Isolates with OMP pattern 1 were discovered in 6 different serogroups, and OMP-3 pattern was also shared by O18, O78 and O11 serogroups isolates. These results indicated that the OMP patterns of avian pathogenic Escherichia coli isolates of O18, O78, O2, O11 serogroups which were isolated from different regions in China were heterogeneous, and all of O88 and O26 serogroups isolates just belonged to OMP pattern 1. Moreover, the OMP pattern 1 was presented in isolates of six different predominant O serogroups.

Population genetics of Trypanosoma brucei and the epidemiology of human sleeping sickness in the Lambwe Valley, Kenya.

Numerical taxonomy was used to review isoenzyme variation in isolates of Trypanosoma brucei obtained from cattle, tsetse, humans and wildlife from the Lambwe Valley, Kenya. From isoenzyme information alone, it was possible to classify isolates as to source through the use of linear discriminant functions analysis, with an error rate of only 2% in humans, and 14% over all groups. Differentiation was mostly dependent on patterns in the enzymes ASAT, PEP1, and ICD. Parasites from non-human sources, especially tsetse, were characterized by high isoenzyme diversity, and many unique zymodemes. Observed frequencies of genotypes for ICD, ALAT, and ASAT did not agree with expected frequencies based on random mating of a diploid organism. Deviations were particularly large for tsetse isolates, and were mostly due to a deficiency of one homozygote. Cluster analysis revealed complex relationships among isolates, with patterns evolving through time. Major human zymodemes from the 1970s clustered together with most wildlife isolates from East Africa. This chronic human-wildlife transmission cycle was characterized by ASAT pattern I. Other, minor human zymodemes were associated with a human-cattle transmission cycle characterized by ASAT pattern VII. These original chronic transmission cycles appeared to change in 1980 with the appearance of two new zymodemes in humans. These zymodemes involved changes in ALAT and/or PGM to patterns typical of tsetse and cattle isolates. A resultant epidemic was halted with repeated aerial spraying of endosulfan in 1981. Since then, a variety of new zymodemes of unknown human infectivity have appeared. The origins of these changes are discussed in terms of genetic exchange in tsetse, adaptation to human and cattle transmission cycles, and selection resulting from chronic use of insecticides.

[Anatomo-radiological study of the middle colic artery in children from 0 to 1 year].

The authors tried to establish the most frequent origin and distribution patterns of the middle colic artery. The search consisted of fifty cases and it was possible to distinguish five fundamental types of vascular origin and two basic types of distribution. The use of the radiological method for the anatomical study of small vascular structures proved us the great importance comparing it with the traditional dissecation.

Dendritic reticulum cell pattern as a microenvironmental indicator for a distinct origin of lymphoma of follicular mantle cells.

Follicular dendritic reticulum cells (DRCs) are known to be normally present in primary follicles and both follicular centres and mantle zones of secondary follicles of peripheral lymphoid tissue. Involved frozen biopsy tissue specimens from eight cases of intermediate lymphocytic lymphoma/mantle zone lymphoma (ILL/MZL); eight cases of follicular centre cell lymphomas (FCCL) of the centroblastic/centrocytic type; and seven cases of well-differentiated lymphocytic lymphoma (WDLL) consistent with chronic lymphocytic leukaemia (CLL) were analysed immunohistologically with R4/23 (DRC-1) monoclonal antibody reactive with 'bystander' DRCs. As opposed to FCCL and most WDLL/CLL cases, the DRCs consistently formed a loose, ill-defined meshwork with a radiating or blurred outline in all MZL cases and one ILL. On the basis of the observed findings as well as from those reported in literature, the hypothesis is proposed that ILL/MZL originates from the follicular mantle zone and represents a distinct lymphoma entity owing to its peculiar immunostaining pattern of DRCs that allows it to be separated from both FCCL and WDLL/CLL. Moreover, the absence of DRCs in the microenvironment of other B-cell malignancies such as prolymphocytic leukaemia and hairy cell leukaemia, analogously with most CLL cases, would speak in favour of their different--possibly extrafollicular--compartment of origin within lymphoid tissue.

Evaluation of normal variations in S-T segment patterns by body surface isopotential mapping: S-T segment elevation in absence of heart disease.

S-T segment elevation is commonly observed in the electrocardiogram of normal persons. To study the possible origins of such patterns, 45 normal volunteers were examined. Electrocardiographic potentials were registered from 150 torso electrodes and processed to construct isopotential maps at 2 ms intervals throughout the QRS-T interval. The maximal potentials recorded from any of the 150 electrodes were 198 +/- 76.4 and 272.1 +/- 84.2 microV at instants 40 and 80 ms into the S-T segment, respectively. Maximal voltages recorded by the six standard precordial V leads at these respective time points were 109.7 +/- 57.0 and 163.6 +/- 66.9 microV. Torso maximal potentials were significantly stronger than were those sensed by V leads; the two were significantly correlated but predictability was limited. The duration of overlap between the onset of ventricular recovery and the end of the excitation was determined from isopotential maps and ranged rom 4 to 16 ms. There was no significant correlation (p greater than 0.05) between these values and either torso or V lead potentials at either 40 or 80 ms into S-T segment. These data suggest that (1) standard precordial leads do not accurately predict maximal torso potentials during the S-T segment, and (2) the degree of overlap between repolarization and depolarization is not a major determinant of precordial voltage. Hence, the rationale for use of the term "early repolarization" to describe this clinical condition is not substantiated.

Variegated expression of a mouse steroid 21-hydroxylase/beta- galactosidase transgene suggests centripetal migration of adrenocortical cells.

5'-Flanking sequences (6.4 kb) of the mouse steroid 21-hyrodxylase (21-OHase) A gene linked to a LacZ reporter gene directed appropriate cell-specific expression in cultured Y1 adrenocortical tumor cells and in the adrenal cortex of transgenic mice. The transgene expression initiated at the same stage of adrenal development as the endogenous 21-OHase gene (embryonic day 11.5). Although the endogenous 21-OHase gene is expressed throughout the adrenal cortex, the 21-OHase/beta-gal transgene showed a strikingly variegated pattern of adrenocortical expression in all 10 transgene-expressing mouse lines examined. This presents as radial stripes of beta-gal staining transcending the classical zonal structure of the adrenal cortex but paralleling the columnar arrangement of cells of the zona fasciculata on the centripetal organization of the adrenocortical blood supply. To the extent that the variegated pattern of 21-OHase/beta-gal transgene expression depicts adrenocortical cell lineage, these results suggest that all cells within an individual stripe have a common clonal origin; the radial pattern of clonally derived cells argues that cellular migration maintains the adult adrenocortical cell population. Adrenal glands of developing embryos also exhibited a variegated pattern of 21-OHase/beta-gal transgene expression. However, this presented as islands of beta-gal reporter staining within the developing gland, suggesting that the rapid embryonic adrenal growth phase, which precedes the establishment of the classic adrenocortical zonal structure, may be governed by cellular mechanisms distinct from those responsible for maintenance of the adult adrenocortical cell population.

Specificity and crossreactivity of immune serum and hybridoma antibodies to various species of avian coccidia.

The species-specificity and crossreactivity of serum antibodies (Ab) from birds immunized specifically with six different species of coccidia and of 24 hybridoma antibodies (Ab) developed against four species of chicken and two species of turkey coccidia were determined by use of the indirect immunofluorescent antibody (IFA) test on air-dried sporozoites. With few exceptions, the immune chicken sera were found to crossreact with all species of coccidia tested. Seven of the hybridoma Ab were species-specific, while the other 17 Ab demonstrated varying degrees of crossreactivity. Similar types of IFA patterns were seen with both the species-specific and crossreactive hybridoma Ab. Some of the crossreactive hybridoma Ab produced one type of IFA pattern with the sporozoites against which they were originally raised and different patterns with other species of sporozoites. The development of the hybridoma Ab has made it possible to identify the species of coccidia found in mixed infections and check the purity of laboratory strains.

[Development of scirrhous cancer of the stomach; a speculation from clinical materials obtained].

In order to get information about the biological behavior of scirrhous gastric cancers in their growth, we examined 1) retrospective and 2) prospective follow-up cases of scirrhous cancer, 3) scirrhous cancers on the gastric remnant, 4) those misdiagnosed as early cancer and 5) similarity in sex ratio, location and histological type (backgrounds of growth) between early and advanced cancers by multivariate analysis. The results obtained were as follows: 1) In the five well documented cases of scirrhous cancer being available for retrospective study (see Table 1), superficial depressed lesions were seen previously. And four of the five were non-ulcerative (without converging folds) and the rest one was ulcerative. In these five cases intervals between elastic and contracted appearance (E-C interval) in their growth was very short. The shortest was only 40 days (see Fig. 1). 2) In contrast, in a case who had been followed up from localized advanced to scirrhous cancer prospectively, the term was relatively long, namely one year and half. 3) In eight cases who had recurred as scirrhous cancer on the gastric remnant and been resected, time intervals between the initial operation and recurrence detected were diverse, and there was no relationship between the time intervals and clinicopathological features of the primary lesion (Table 2). 4) Six cases of scirrhous cancer were misdiagnosed as early by radiology and/or endoscopy (Table 3). In these six, gastric wall was not contracted macroscopically in spite of the presence of diffuse scirrhous invasion histologically ("elastic scirrhous cancer"). 5) According to the multivariate analysis (Hayashi's quantification theory No. 3) examining the similarity in backgrounds of growth between 937 cases of early and 2065 of advanced cancers, non-ulcerative II c type of early cancers, particularly in younger generation showed close similarity to scirrhous cancers. The above results may suggest following consideration about development of scirrhous cancers: Most of the original lesions correspond to II c type of early cancers without converging folds (non-ulcerative) in younger generation (results 1 and 5). Growth patterns from the original to the final gross appearance are classified into two groups, slow (through locally advanced stage as result 2) and rapid (direct change from early lesion as result 1). Because E-C interval is very short in the rapid growing cases (result 1) and scirrhous cancer is not always contracted macroscopically (result 4), it is possible to consider the presence of some endogenous factors from cancer cells, which make "elastic scirrhous cancers" contracted rapidly and simultaneously.(ABSTRACT TRUNCATED AT 400 WORDS)
