[Memory complaints among community-dwelling elderly in Japan: comprehensive health examination for the community elderly for prevention of the geriatric syndrome and a bed-ridden state ("Otasha-kenshin") part III].

PURPOSE: Previous studies have indicated that memory complaints may predict cognitive decline and dementia among the elderly. The present study was therefore conducted to clarify memory complaint characteristics among elderly dwelling in an urban community in Japan. METHOD: The participants analyzed in the present study were 453 men and 385 women aged 70 to 84 years living in an urban Japanese community. Data on problems related to memory complaints, cognitive decline (below 24 points on Mini-Mental State Examination), depression (measured by Mini-International Neuropsychiatric Interview), hearing and vision problems, I-ADL (measured by TMIG Index of Competence), self-rated health, age, sex, and years of education were collected at a comprehensive mass health examination for the elderly ("Otasha-kenshin"). RESULTS AND DISCUSSION: Twenty-seven percent of male respondents and 32% of female respondents reported having current trouble remembering things (reported as "frequently" or "sometimes"). We collected specific descriptions of the memory complaint difficulties the subjects were experiencing. A quarter of the responses indicated problems with forgetting persons' names, a fifth with forgetting where things had been left, 15% with leaving things behind, and a quarter with prospective memory failure. The results of multivariate logistic regression analysis to explore correlates showed that in men self-rated health and cognitive decline, and in women hearing problems and self-rated health were significantly and independently related to the memory complaint. These findings suggest that in addition to cognitive decline, self-rated health and hearing problems may influence memory complaints, and that memory complaints in men may be a reliable, simple indicator of cognitive decline. We now need to carry out a longitudinal study to clarify predictive validity.

[A case of Mycobacterium intracellulare lung disease that was detected by health examination and was successfully cured by chemotherapy--including discussions on the diagnostic criteria of the disease].

A case was 56 years old woman, and she did not have any subjective symptom. She received multiphasic health screening, and abnormal shadow was detected on her chest radiograph. Chest radiography revealed infiltrations in the middle lobe. Computed tomography (CT) of the thorax showed clusters of small nodules in the middle lobe. The bronchial washing specimen showed acid-fast bacilli identified as Mycobacterium intracellulare by DNA-DNA hybridization (DDH) method. This case was diagnosed as Mycobacterium intracellulare lung disease. The patient received combination therapy with rifampicin, ethambutol, and clarithromycin for one year with radiological improvement. CT findings were characteristic and useful for the early diagnosis of MAC infection, which led to cure of the disease by chemotherapy.

Lower urinary tract symptoms in 1,912 apparently healthy persons of both sexes.

We aimed to evaluate lower urinary tract symptoms (LUTS) in apparently healthy persons of both sexes using the International Prostate Symptom Score (IPSS) and quality of life index (IPSS-QI) as related to aging. A total of 1,912 apparently healthy participants (1,052 women and 860 men) were enrolled in this study. They were informed as to the nature of the study and asked to fill out the IPSS questionnaires. The mean age of women was 46.8 years (range 18.0 to 83.0) and that of men was 48.5 years (range 19.0 to 76.0). The storage and voiding symptom scores gradually increased in parallel with increasing age in both sexes. The proportion of participants with moderate to severe symptoms (IPSS 8 or greater) increased from 5.6% in women younger than 30 years to 8.9, 15.7, 20.8, 21.0% and 28.6% for women 30 to 39, 40 to 49, 50 to 59, 60 to 69 and 70 years or older, respectively; the corresponding distribution for men was from 8.8 to 15.9, 18.7, 38.1, 48.6 and 68.8%, respectively. In both sexes, the majority of persons showed 1 or less nocturia. The distribution of IPSS-QI was very similar within each category of the IPSS severity in both sexes, showing that LUTS was equally bothersome to both sexes. These data should be taken into account to evaluate LUTS in both sexes.

[Longitudinal decline of forced expiratory volume in one second in non-smoking Japanese women].

We prospectively studied 1,696 clinically healthy non-smoking women to assess the influence of aging on longitudinal decline in forced expiratory volume in one second (FEV10). All the subjects took an annually medical examination involving a pulmonary function test from 1995 to 2001. The mean value of annual decrease in FEV10 (slope) was 19.6 mL/year. Multiple regression analysis revealed that initial age, height, and FEV10 were significant parameters determining the slope. This study clearly showed the aging-related longitudinal decline in pulmonary function in non-smoking Japanese women.

[The Aging Male's Symptoms (AMS) scale in Japanese men attending an infertility clinic].

There is a gradual decline in testosterone and free testosterone with age. Physical and psychological changes can occur due to this decline of androgens--a syndrome known as "partial androgen decline in the aging male" (PADAM). Male infertility and erectile dysfunction (ED) can also be caused by androgen deficiency. Thus, male infertility, ED and PADAM are interrelated. We evaluated the prevalence of PADAM symptoms in 215 infertile patients in an infertility clinic using the Aging Males Symptom (AMS) scale. Results of the evaluation of 301 men, (30-39 years old) using the same scale, who were part of a multiphasic health screening program, served as controls. The total score of the infertile patients was lower than that of the controls. Especially, the scores of the psychological and somatic subscales were significantly lower in infertile patients (P=0.009, P=0.012, respectively). Thirty three (15.3%) of the 215 infertile patients had ED. Although the score of sexual subscale was higher in the ED patients than in the controls, the scores of the psychological and somatic subscales were not significantly different from those in the controls. In conclusion, PADAM symptoms in Japanese infertile patients were not severe. Moreover, general function was better preserved in the infertile patients than in the controls. The psychological and somatic functions of ED patients were also well preserved.

[Gastroenterological endoscopic findings in alcoholic liver disease patients--comparison of daily drinkers at physical check up].

We compared findings of alcoholic liver disease patients used by gastroenterological endoscopes (31 patients hospitalized in Hyogo Prefectural Amagasaki Hospital: AA group, and 156 patients hospitalized in Suma Red Cross Hospital: AL group) with physical checkup patients (48 daily drinkers: A group, and 99 non drinkers: C group). Esophageal varices were seen by the majority of AA group, and the frequencies of esophageal cancers were also high in the gullet changed to morbid state. Although these things were seen AL group, there were few as compared with AA group. Moreover, frequencies of esophagitises in AL group were higher than C group (p < 0.05), though it was the same level in C group and AA group. In a strange stomach trouble, as for AL and A group, there were a lot of gastric erosions compared with C group (p < 0.05). However, AL group compared with the other groups, the gastric polyps were few (especially C group, p < 0.01). In the duodenum, changing to a morbid state, ulcers were high frequency (5/31) in AA group compared with the other groups (p < 0.05). Moreover AA and AL groups had a lot of polyps high frequency compared with C group (p < 0.05), and duodenitises were high frequencies in AL group compared with A and C group (p < 0.05, p < 0.01). It seemed stronger that the digestive organs changing to morbid state was tendency depended on alcohol consumption and the influence of smoking was not able to be disregarded.

The positive known association design: a quality assurance method for occupational health surveillance data.

Quality control must be an integral component of an occupational health surveillance program. The positive known association design offers the occupational health physician a method to test, on a population basis (ie, high periodic medical surveillance examination participation rates by the employees), the quality of periodic medical surveillance data. Several well-established biological associations were evaluated and observed in this study, including a dramatic relation between white blood cell counts and smoking. We highly recommend that the positive known association design be incorporated in the quality assurance procedures of occupational health surveillance programs.

[A study of falls experienced by the aged among users of preventive health examination services].

In order to determine what factors are related to falls and the effect on the aged, a study was conducted on subjects aged 60 years and over who had undergone preventive health examinations in the western region of Shizuoka Prefecture. Subjects (n = 305) had an average age of 64.4 +/- 3.8 years, and all were interviewed regardless of their history of falls. For the 66 cases who had experienced falls, further questions prepared in advance, were included. The following results were obtained: 1) There were 66 subjects (21.6% of the subjects) who had experienced falls with some experienced multiple falls for a total count of 91 falls. 2) The frequency of falls of female subjects was significantly higher than that of males. Further, with respect to the type of injuries suffered, in the case of many of the female subjects, these took the form of contusions. 3) The majority of subjects who experienced falls were engaged in farming and most of the falls were work-related. 4) Results of this study showed that, while not statistically significant, cerebrovascular diseases are closely connected to falls and increases the falls coefficient among the aged.

Comparison of two pre-admission testing methods for elective surgery patients.

Patients admitted for elective surgery (inguinal hernia, varicose veins and hemorrhoids) were studied in order to evaluate the impact of performing pre-admission testing (PAT) by utilizing an automated multiphasic health testing (AMHT) technology on the rate of repeating the pre-operative routine laboratory tests during hospitalization. A slightly lower but statistically significant rate of repeated tests was found among patients who performed PAT by AMHTS compared with those who performed the tests via the conventional ambulatory system. This result suggests that performing routine tests before hospitalization in a single authorized AMHTS facility is preferred since it saves the patient time and reduces the need for repeated tests.

[An assessment of the acceptance of a face graph as a health education device].

With the aging of society, an increasingly greater number of people receive health evaluations organized by the community. Results of health evaluations are usually obtained as multiphasic numerical figures which are usually written down directly into a prescribed results form by which the community health personnel inform examinees of their health status. Most of aged examinee, however, cannot fully comprehend their own health status expressed as multiphasic numerical figures. Community health personnel also have difficulty in explaining these numerical results. Although the cartoon of face has been shown to be an effective device to help understand multiphasic numerical values, its use is not yet popular among community health personnel. The use of a face graph as an aid for health information and education activities by community health personnel was studied using a simple face drawing system developed for lap-top microcomputers. In this study, the acceptability of this face was evaluated by 29 community health nursing students and 31 community health nurses, studying and/or working in Nagasaki Prefecture, Japan. The first evaluation was made of a newly developed prescribed health evaluation result form using a face graph (facial form). All subjects evaluated the acceptability of this facial form by comparing it to the usual numerical form on the basis of the following five items: (1) discrimination of abnormal range, (2) evaluation of the degree of abnormality, (3) discrimination of normal range, (4) provocation of interest in one's own health status, and (5) stimulation of familiarity in one's own health status.(ABSTRACT TRUNCATED AT 250 WORDS)

[Health status and vaccination status of 4th grade students in the Rhine-Lahn district].

Within the framework of the school examination of fourth-formes in 1988, 997 children of the primary schools in the Rhein-Lahn district were examined by the Board of Health of Lahnstein. Nearly every 5th child had to be referred to established specialists because of suspicious diagnoses. Primarily this was a matter of pathological skeleton findings. About 20% of the children were overweight. About 75% of the pupils complied with our request to show us their vaccination certificates. After a part of them had been revaccinated, complete vaccination status was available in three quarter of the children. The significance of medical examination in school is underlined by the great number of children with medical problems. First of the area of the spinal column is alarmingly high. The health-political significance results from the possibility of an early prevention of degenerative diseases of the spinal column.
