Microcomputer-generated reminders. Improving the compliance of primary care physicians with mammography screening guidelines.

Recent studies have documented that physician compliance with recommended periodic health screening improves with reminders to physicians. These reminders, however, are often costly to maintain and modify. This study investigates the influence of a microcomputer tickler system on the ordering of mammograms. All women (N = 1262) aged 40 years and older who made visits to an outpatient office during a 6-month period were randomly assigned to one of two groups. For the experimental group, the date of the last mammogram ordered and recorded in the clinical database was printed on the encounter form generated for each patient visit. No information regarding previous mammograms was printed for patients in the control group. Women in the experimental group were more likely to have a mammogram ordered during the study period (19% compared with 12%, P = .001) and, as a result, were more likely to be in compliance with mammography guidelines at the study's completion (27% compared with 21%, P = .011). Microcomputerized data storage and retrieval systems may help increase physicians' attention to preventive health screening recommendations.

Microcomputer program in Chinese to manage and analyze antimicrobial susceptibility results.

A chinese microcomputer program which analyzes the minimal inhibitory concentration (MIC) of antimicrobial susceptibility and presents the results in tabular and chart forms is described. This program also computes MIC50, MIC90, and geometric mean MIC and displays these values for a large number of antimicrobials in a summary table. The set of MIC values used for different groups of organisms or antimicrobial agents can be altered without changing the program. A text file of the results is written into the disk to enable editing before final presentation. The program features several major enhancements over previously published ones.

[Ameliorative catalytic model of age-specific prevalence of leprosy simulated by using microcomputer].

This paper advances an ameliorative catalytic model to imitate age-specific prevalence rates for leprosy by using microcomputer for eight prefectures in Jiangsu province with analyses of results. The author considers that the distribution of age-specific prevalence of leprosy tallies with the ameliorative catalytic model. It is a curve of S shape which slowly goes up at beginning then faster rising and reaching a plateau after the point of peak. The curve is reflected in characteristics with a longer latent period, longer course of the disease and lower incidence. Through analysing and comparing the practical leprosy prevalence in eight prefectures with three parameters of the catalytic model, it is obviously that the prevalence was positive correlation with parameter a and negative correlation to parameter b, k. Therefore parameters--a, b, k in the two-stage catalytic model respectively represents the force of infection, the speed of eliminating the disease and an indicator of the effect of control disease. Catalytic models may be used to simulate and analyse the disease data in various periods, regions or masses for comprehensive evaluation of the force of infection, ability to eliminating disease in a population and the effect of control programme.

A microcomputer-based, net-lending interlibrary loan system.

A microcomputer-based, net-lending interlibrary loan system was developed at Lane Medical Library, Stanford University. The system, designed to generate the monthly billing invoices and all necessary statistical reports, has reduced the time required for logging-in procedures and compilation of monthly, quarterly, and annual statistics. User menus, help screens, and choice fields were developed explicitly for library staff who have little or no computer experience. The program was written using the DataEase database management software running on IBM PC, XT, AT, or compatible with a minimum of 512K RAM. Described are features of this automated interlibrary loan management system and its use in a net-lending interlibrary loan department. It focuses on data entry in the "Library Directory" and "ILL Log Sheet," details of billing invoices, and statistical reports, and flexibility in modifying tax rates, borrowing fees, and other parameters.

Microcomputer-assisted monitoring of medication-dispensing errors.

A microcomputer-based program for collecting and monitoring data on medication-dispensing errors made by pharmacy personnel is described, along with the policies and procedures for dealing with the responsible employees. The program was implemented at a 964-bed teaching hospital. When a medication-dispensing error is suspected, the director of pharmacy initiates a medication discrepancy follow-up report form. The appropriate supervisor fully investigates the incident, completes the form, and returns it to the director, who assigns a point value to the error on the basis of whether it injured or had the potential to injure the patient. Information from the report is then entered into an automated file. Data are sorted by the computer, allowing the performances of pharmacy personnel to be monitored. Progressive educational or disciplinary actions are taken depending on the number of points accumulated annually. Responsibility for an error is not assigned unless there is absolutely no question as to its origin, and dispensing-error points are not carried over from year to year. Since the program was implemented more than three years ago, the average number of reported medication-dispensing errors has decreased from 20 to 6 per month. Departmental procedures for monitoring and analyzing dispensing errors and for initiating disciplinary actions against personnel who commit many dispensing errors have been successful in creating greater awareness of the need to prevent such errors and in reducing their number.

Microcomputer database management for surgical residents.

Surgical residents must record procedures performed and may choose to keep files of photographic slides, bibliographic references, and a curriculum vitae. Four databases that store this information are produced with an inexpensive and easily obtained microcomputer software program. A surgical procedure database is modeled after the procedure list recommended by surgical boards. This list can be viewed while one enters data, thereby enabling production of accurate and complete records. In the second database, photographic slides are assigned sequence numbers and slide content is designated using both procedure codes and key words, allowing structured and personal recall of data. Data can be printed in many report formats, including that used by the boards of surgery for final submission of reports of residents' operations at the completion of residency. The bibliographic and CV databases contain highly segmented citation data. This structure enables manipulation of data to satisfy the sequence requirements of journals or institutions to which articles or CV are submitted. Database maintenance consumes a few minutes daily and requires a minimum of experience with computers. By providing ease of access to organized data, these databases enhance the potential for critical review of clinical experience by both residents and program directors.

A microcomputer program for real-time data acquisition in the perinatal physiology laboratory.

This report describes a real-time data acquisition system using the IBM PC-AT microcomputer, and software specifically engineered for the perinatal physiology laboratory. The system samples up to sixteen analog signals at a rate ranging from 1 to 1024 Hz. Calibrated digital values are displayed on the computer monitor and stored on disk. The system can perform algebraic computations and estimate derivatives. Pattern recognition algorithms are included for the detection and characterization of uterine contractions and fetal breathing movements. Two different noise-rejection filters are implemented. Output of signals via a digital-to-analog converter is supported. The system has been used to record a broad variety of physiologic variables in a number of different studies.

A microcomputer-based algorithm for digitized image simplification and information extraction.

Spatial analysis of objects often requires significant image simplification prior to information extraction and application of a decision-making algorithm. Much decision making based on images (e.g., histologic diagnoses) requires identifying patterns in complex backgrounds (image simplification) and comparison of those patterns to other patterns (decision making). Automated extraction of information from images commonly requires the extraction system to recognize edges (contours) of structures and their internal discontinuities (such as gradations in density) and to selectively suppress irrelevant data in order to conserve memory and speed computation; data from homogeneous image areas occupy memory, but are noncontributory or redundant. This paper describes the development of a microcomputer-based algorithm that deletes all homogeneous information from overlaid digitized images, generating contours in the place of nonhomogeneities. Contours corresponding to different areas or objects depend on color differences between an object and its surroundings. Any set of contours can be deleted almost instantaneously, leaving only those of interest. Contours can be highlighted by an operator-driven interactive process if desired and can be deleted and retrieved until an appropriate image is obtained. This contour-generating and image-simplification algorithm facilitates three-dimensional reconstruction of an object from serial images by reducing the number of calculations required and yielding a cleaner final image.

[Use of microcomputers in epidemiological analysis].

The possibility of the effective use of microcomputers in the practical work of a sanitary and epidemiological station for the purpose of organizing epidemiological surveillance has been shown. The programs developed in this investigation are recommended for introduction into the practical work of sanitary and epidemiological stations servicing districts with a population of 100,000-200,000 and more.

A microcomputer method for recording and analysing behavioural elements.

The trend in ethology, psychopharmacology and behaviour genetics is to get increasingly higher resolution of the behaviour of animals, since it increases the sensitivity of the tests used. Consequently the higher resolution requires an increased data-logging efficiency, and a decreased time-investment for data handling. In this paper a method for inexpensive microcomputers is presented, which enables an individual experimenter to obtain frequency, relative duration and its variance from as many behavioural elements as desired.

Clinical decision analysis using microcomputers. A case of coexistent hepatocellular carcinoma and abdominal aortic aneurysm.

Many difficult medical decisions involve uncertainty. Decision analysis-an explicit, normative and analytic approach to making decisions under uncertainty-provides a probabilistic framework for exploring difficult problems in nondeterministic domains. As the methodology has advanced, clinical decision analysis has been applied to increasingly complex medical problems and disseminated widely in the medical literature. Unfortunately, this approach imposes a heavy computational burden on analysts. Microcomputer-based decision-support software can ease this burden.

[Determination of the indices of genetic similarity and diversity using programmable microcomputers].

A complex of programs for microcomputer permitting to compute the similarity and diversity indices of two or few populations, herds, intrabreed types, lines and related groups of animals in automatic operation has been suggested. The absolute values of occurrence of factors and the gene frequencies in polymorphic systems of serum proteins and erythrocyte antigens of blood groups are used in calculation.

[Demonstration of several functions of motoneurons using microcomputers].

In order to display some electrophysiological properties of the motoneurons, a special program for a microcomputer has been designed. The images in the monitor resemble those of an oscilloscope screen. The microelectrode penetration in the cell membrane, magnitude of the resting potential, inhibition and facilitation phenomena and the time-course of a spike potential are shown. The sequences of program's sentences (detailed in the text) can be exhibited in the screen also. Related theoretical basis of neurophysiology and a discussion of the present work are included.

[Interactive microcomputer program for calculation and normal values for routine clinical neurophysiological research].

A BASIC program for a 128 KByte microcomputer is described which is deviced to support every day clinical neurophysiological investigations. It provides calculating routines and normal values with a hard copy print option for visual, auditory, and somatosensory evoked potentials, peripheral motor and sensory nerve conduction velocities, and needle EMG potential analysis. All important arm and leg nerves and muscles are included. By its simplicity the program gives easy access to changes according to individual requirements.

Personal microcomputer-aided three-dimensional reconstructions.

A fast, cheap and simple method is described which enables three-dimensional reconstructions to be produced from short series of sections. The reconstruction is formed and displayed either as a full line or hidden line diagram or shaded model. It may be rotated, magnified or reduced, all with the aid of a personal microcomputer, peripherals and software, the total cost of which does not exceed several hundreds of US dollars.

[Processing of the results of experiments on molecular DNA-DNA hybridization by using the Elektronika BZ-34 programmable microcomputer].

The programs for microcomputer "Electronika" BZ-34 permitting to compute the molecular DNA-DNA hybridization data with thermal stability duplex analysis have been developed. The proper different programs are offered for analysis of results obtained in different conditions of DNA hybridization. These programs are especially useful for accelerated calculation of large series of experimental data.

[Analysis of 13C-NMR spectra of oligosaccharides using microcomputers].

A microcomputer programme, ANMROL, has been developed to check whether structure of an oligo- or polysaccharide may be elucidated unambiguously from the 13C NMR data. Principles of the programme are described and its application for the series of linear and branched oligosaccharides is discussed.

[A medical terminal based on a microcomputer].

Herein are described purpose and field of application of the medical DVK-2M microcomputer based terminal. This system will be used to accumulate, preliminary process and feed the medico-physiological information to the consulting-diagnostic centre. Were studied the channel data feed control protocol and protective algorithm of the transferred information including error correction. Information on the medical network structure is proposed as well.
