[Male and female sexuality--differences according to sex and age].

Sexual physiology, sexual behaviour and attitude to sexuality vary according to sex and age. The sexual reaction cycle is very uniform in men, but much more variable in women; at a more advanced age the reactions of this cycle are delayed in both sexes. While hormones have been proved to influence male sexuality, their influence on female sexuality has not been clarified. Sexual appetite changes with age. It reaches a climax earlier in men than women, but also declines more in men with increasing age. In the last three decades the number of women who masturbate has considerably increased. The difference between the sexes as far as attitude to sexuality is concerned is particularly evident in those with sexual difficulties: men usually see their problems in an isolated way as a purely physical disorder, whereas women are usually aware of the connection between their sexual problems and personal, partner or work problems.

[Diagnostic problems in suspected sexual abuse of handicapped preschool children].

The diagnostic process in four developmentally retarded pre-school girls who, on grounds of sexual behaviour inappropriate for their age, were suspected by their nursery school teachers of having been sexually abused, is here described as a way to understanding the behavioural sympomatology. Suspicion was confirmed only in the case of one of the girls. It has been shown that the early development of relationship disorders and sensomotory retardation either alone or in connection with maternal experience of violence may lead to characteristic forms of masturbation, even when sexual abuse has not occurred. The building of a patient-therapist rapport during the diagnostic process is here even more crucial than in the case of non-retarded children. It is only on the basis of the development in the child of an improved ability to form relationships that the therapist may gain psychological access to the child and thus insight into its traumatising experiences.

Quantitative observations of hymens in prepubescent females selected for non-abuse.

The maximum hymenal opening was evaluated quantitatively in 111 prepubescent females during routine physical examinations in a pediatric subspecialty office. For comparison, an additional 53 females referred by child protective agencies were also examined. Analysis of data show "non-abused" groups may be separated statistically from "abused" groups on the basis of area of hymenal opening. The mean area for the "non-abused, non-masturbate" group was 6.4 mm2. The upper limit of area (mean + 3 S.D.) of hymenal opening in this group was 24.1 mm2. A child having a hymenal opening diameter of 6.94 mm or less has a 99% chance of being in the "non-abused" group. The area of hymenal opening for "non-abused" groups did not change with increasing age, height or weight. A skilled pediatrician knowledgeable in the area of sexual abuse may obtain clinically relevant information with ordinary office equipment and trained personnel. Regular and repeated observations of genitalia during routine health maintenance examinations are vital baseline measurements for the physical and mental health of young female patients.

Sexual behaviour and knowledge of normal children as perceived by their parents.

This study set out to examine sexual behaviour and knowledge of normal children as perceived by their parents. The study was carried out in two settings; a) general practice and b) an acute paediatric admission ward. Families where there was a history or suspicion of child abuse or neglect were excluded. The parents (62 mothers and 22 fathers) of 84 children aged three to twelve years, from a variety of social backgrounds, were interviewed. The majority of parents saw mothers as the main and most important source of information on sexual matters for their children. Parents believed that their children were able to talk freely to them about sexual matters, and most had discussed general information about reproductive matters without going into detail with their children. Parents perceived their children as having little interest in the genitalia of others, except during nappy changing or bathing of preschool siblings. Few parents were aware that their children masturbated although many mentioned "nervous fiddling" with genitals by male children. More explicit sexual behaviour among children, such as simulated intercourse, were distinctly unusual. The parents of 14% of boys and 44% of girls were unaware of what word (correct or slang) their child used to describe their own genitalia. The results of this study suggest that explicit sexual activities or preoccupations that come to adult attention are distinctly unusual in normal children. While parents can talk in a general way with their children about reproductive matters, specific details about sexual behaviour tend not to be discussed.(ABSTRACT TRUNCATED AT 250 WORDS)

Detection of interleukin-8 (IL-8) in seminal plasma and elevated IL-8 in seminal plasma of infertile patients with leukospermia.

OBJECTIVE: To determine if interleukin-8 (IL-8) is a normal constituent of seminal plasma and if leukospermia is a factor determining its elevation. DESIGN: Seminal plasma from 58 men obtained by masturbation was examined for the presence of IL-8 using an IL-8 specific sandwich ELISA. Semen samples were obtained from 34 infertile men without leukospermia, 10 infertile men with leukospermia, and 14 proven fertile men. The correlation of amount of IL-8 in seminal plasma with some spermiogram parameters and the amount of polymorphonuclear (PMN) elastase was statistically evaluated. RESULTS: Immunoreactive IL-8 was observed in the seminal plasma of all 58 subjects. The IL-8 titer in seminal plasma of patients with leukospermia (6.16 +/- 0.82 micrograms/L) was significantly higher than that in seminal plasma of patients without leukospermia (2.35 +/- 0.34 micrograms/L) and fertile men (1.64 +/- 0.29 micrograms/L). There was a high degree of correlation between PMN elastase and IL-8 levels in seminal plasma. CONCLUSIONS: These findings demonstrate IL-8 to be in seminal plasma and elevated IL-8 levels in infertile patients with leukospermia.

[Sexuality. Cultural, social and clinical aspects].

The deep changes in values, beliefs and behaviour, both individual and group, have been examined, referred to the sexuality, by various methods and prospects. Sexual differentiation, typical of the plast, has been rediscussed and modified. As a result of these changes we can talk about old myths, concerning the conception of sexuality in the past, and new myths that concern the present. In this work we have reviewed the different aspects of male and female roles: masturbation, the obsession abort performance, the orgasm and, not last, the sexuality in old age condition.

[Reproductive health in adolescent students: knowledge, attitudes, and behavior in both sexes, in a community of Santiago].

Nine hundred forty eight teenagers (600 females) from a public school of one of the poorest communities of Santiago were surveyed about knowledge and practices on sexuality and reproductive health. Twenty four percent of females and 40% of males did not talk about their problems at home, instead they talked preferentially with their friends. Half of the sample attributed a risk a pregnancy to the first sexual intercourse; 67% did not know the infertile phase of the menstrual cycle and 20% did not identify two sexually transmitted diseases. Sixty percent considered rhythm technique as a safe fertility control method, 60% of males and 50% of females considered masturbation as risky, 57% of males and 49% of females estimated that condoms could be reused and between 5 and 16% of the sample correctly identified AIDS modes of transmission. Fifty percent had a couple and 23% of males and 17% of females had a sexual intercourse in the last six months; of these, 45% of women and 27% of men used a contraceptive method. Four percent of women and 6% of men became involved in a pregnancy and 57% of these terminated in abortions. It is concluded that knowledge about sexuality and reproductive biology among low income teenagers is scanty.

Accumulating experience in a child abuse clinic.

OBJECTIVE: To examine the patient profile encountered in the first year of operation of the Child Abuse and Neglect Clinic of the Transvaal Memorial Institute. DESIGN: Record review of all cases presenting to the Clinic from May 1988 to April 1989. RESULTS: Females comprised just over 80% of the 227 patients. Sexual abuse was the presenting complaint in 89.8%. Most were young, 7% under 3 and 55% under 10 years of age. Almost one-third of the boys and 5.0% of the girls had chronic signs of anal abuse. Of the girls 56% had signs of chronic and 10% signs of acute vaginal abuse. Where the certainty of sexual abuse was high, 60% of the girls and 45% of the boys had suffered penetrative abuse. The perpetrators were almost invariably known to the child; biological family members accounted for 38% of perpetrators, and if all relations are included (biological, step and 'common law'), family members were the perpetrators in 66% of cases. Strangers were the perpetrators in only 7% of our cases. The majority of perpetrators were male. Behaviour problems were recorded in 73% of cases. Many different problems were noted; the most common were school problems (21%), masturbation (19%), 'clingy' behaviour (12%), and withdrawal and depression (11.5%). CONCLUSIONS: Certainty of diagnosis should be specified. We use four categories: proven, highly suspected, unproven but still suspected, and no abuse. For sexual abuse we also differentiate between penetrative, non-penetrative, 'type uncertain' and no abuse. Training of other health personnel in child abuse management is now a priority in our setting.

RNA and protein synthesis in the nonspermatozoal cells of normal human semen.

A series of semen samples from recently fertile men was obtained by masturbation. They were washed and placed in short term culture in BWW medium, and labelled either with tritiated uridine or tritiated tyrosine. Control samples were labelled in the presence of actinomycin D and puromycin respectively, and examined by light and electron microscopy. Ejaculated spermatocytes, spermatids of various stages and certain anucleate bodies proved to be labelled with tritiated tyrosine and hence to be protein synthetic. Ejaculated spermatocytes were also active in RNA synthesis. Macrophages and neutrophil leucocytes both heavily incorporated tritiated uridine and tritiated tyrosine. The significance of these results is discussed in relation to the possible role of exfoliated sperm precursors in ejaculates.

[A survey on sex knowledge and medical demand of the cases in Kaohsiung city adolescent health clinic].

The purpose of this survey is to study the sex knowledge of adolescents, and to analyze their need in sex consultation and related health problem in Kaohsiung city. A total of 1,000 study subjects were recruited from May 1991 to May 1994. A self-administered questionnaire and medical records were used to obtain related information. The results were as follows: 1. The male to female ratio of study subjects was 1:2. Half of them were students, 44.1% had sexual experience, and the mean +/- standard deviation of age at first sex experience was 19.1 +/- 2.8 years. 2. The average percentage of correct knowledge was as low as 67.6%. It indicated an inadequacy of sex knowledge in adolescents. Females had better knowledge than males. Subjects who had graduated from senior high schools had better knowledge than those graduated from middle schools. No association was found between the score of sex knowledge and occupation. 3. 12.5% of study subjects believed that "masturbation" might cause neurasthenia, kidney malfunction and loss of memory, while 11.9% recognized that "nocturnal emission" might result in abnormal ejaculation, mental aberration and guilty feelings. Furthermore, 3.8% considered that kissing, hugging or sleeping together might cause pregnancy. 4. Most adolescents felt that "guidance on sex knowledge" was the major reason for them to visit the adolescent clinic and "obstetric and gynecological problems" was the second most important. 5. Males and females have the same prevalence of sex experience, and 50.6% of them did not practice contraception. 88.9% subjects had a high risk of getting pregnancy. 6. Males had a higher percentage of more than one heterosexual partner than females did. There was a higher percentage of people with a high education level having a fixed heterosexual partner than that of those with a low education level.

Non-gay-identifying men who have sex with men: formative research results from Seattle, Washington.

Non-gay-identifying men who have sex with men are at risk for human immunodeficiency virus (HIV) infection. To understand these men and to develop interventions to reduce their HIV risks, the authors interviewed staff at agencies that serve non-gay-identifying men who have sex with men, business people who interact with them, and the men themselves. Interviews were augmented with focus groups of non-gay-identifying men who have sex with men and field observations at sites identified as places where they meet to negotiate or have sex. These qualitative data suggested 73 possible groups, which were consolidated into 16 broader "sectors," and then formally ranked by level of HIV risk, ease of access to the sector, psychosocial risks, and influence of other local interventions or research activities. The authors identified six priority groups of non-gay-identifying men who have sex with men (and sites where members of these groups could be approached): hustlers, closeted men, experimenters, incarcerated or formerly incarcerated men, men of color, and heterosexually identified bisexuals. Masturbation and oral sex were reportedly common, but anal and vaginal sex were also noted; condom use was rarely reported. Risk behaviors among non-gay-identifying men who have sex with men persist for a variety of reasons and may require a variety of intervention approaches.

Vaginal lubricants for the infertile couple: effect on sperm activity.

OBJECTIVE: To determine the effects of natural vegetable oils and vaginal lubricants on sperm motion and viability. DESIGN: Four widely used vaginal lubricants (K-Y Jelly, Astroglide, Replens, and Touch) and two vegetable oil products that have been used as vaginal lubricants were purchased through local vendors. Sperm was obtained by masturbation without lubrication from normal, healthy donors. Lubricants were mixed with sperm from individual donors and the effects on sperm motility were determined at 1, 15, 30, and 60 minutes. SETTING: Southwestern Fertility Associates of The University of Texas Southwestern Medical Center at Dallas. MAIN OUTCOME MEASURES: Sperm motility was evaluated by manual motility counts and by computer-assisted semen analysis. Sperm viability was evaluated with Hoechst 33258 dye. The effects of the various lubricants were compared with those of a spermicidal agent, Gynol II (negative control) and Ham's F-10 (positive control). RESULTS: Commercial lubricants inhibited sperm motility by 60-100% after 60 minutes of incubation. Sperm exposed to Replens or Astroglide were nonmotile and nonviable after incubation for 60 minutes, similar to the control, nonoxynol-9 containing product Gynol II. Canola oil had no detrimental effects and was indistinguishable from Ham's F-10 in terms of sperm viability and motility. CONCLUSIONS: For couples with infertility, the use of vaginal lubricants during intercourse is not recommended. In cases where a lubricant is required, careful selection can maximize sperm motility and viability.

Clinical correlates of anorexia nervosa subtypes.

OBJECTIVE: Past research has suggested that subtypes of anorexia nervosa (restricting vs. binge eating/purging) correspond to differing degrees of impulsivity and personality psychopathology. We further investigated potential differences between these diagnostic subtypes. METHOD: We compared restricting anorexics (n = 100) to binge/purging anorexics (n = 71) with regard to symptom presentation and history, impulsivity, eating and related attitudes (Eating Disorders Inventory), and personality psychopathology. RESULTS: We found that the two diagnostic subtypes did not differ in age at intake, body mass index at intake, or age at first diet. Binge/purging anorexics were more likely than restricters to have attempted suicide or engaged in stealing, but the two groups did not differ in likelihood of having engaged in self-injury or in reported drug problems. The two anorexia nervosa subtypes did not differ in age at onset of menses, likelihood of having engaged in masturbation or sexual intercourse, age of onset of sexual activity, or satisfaction with current levels of sexual activity. Likewise, the two groups of anorexia nervosa patients did not differ with regard to scores on the Eating Disorders Inventory or Millon personality measures. DISCUSSION: Results are discussed in relation to the results of past research as well as historical fluctuation in the diagnosis of anorexia nervosa subtypes. We propose that subtyping may be less useful than consideration of eating disorder symptoms along continua.

[Psychological aspects of erectile dysfunction].

It is widely accepted opinion that male sexuality comprises more than an erected penis. This fact should be considered in the management of erectile dysfunction. Therefore, the valuation of sexual disturbances have to be essentially included in the general examination programme of erectile impotence. Several causes are known to contribute to an erectile dysfunction. The anamnesis can be taken as an important tool for classification of the erectile dysfunction. The following questions are helpful in the clinical practice: erectile dysfunction during sexual intercourse only but also during masturbation; spontaneous erections; dependence of the erectile impotence on special situations, partners and events; questions for somatic and psychosocial risks. Sexual therapy represents a complex intervention will be aim of improvement of the functional disturbances in consideration of the general risks for sexual impairments.

Health promotion of adolescents.

China has been making progress in adolescence health care, carrying out directed investigations and academic exchanges, as well as training. Since 1949, both growth and development of Chinese children and adolescents have accelerated significantly. Menarche and the secondary sex characteristics of girls now appear earlier than before. The average age of menarche is 12.5 years (1991) and boys average first emission is 14.33 years (1991). In China, the commonly encountered adolescent health problems are menstruation hygiene, menstruation dysfunction, emission, masturbation, teenage pregnancy, acne, obesity, smoking, alcohol drinking, drug abuse, and suicide. Causes of death of adolescents in China has significantly changed, all deaths caused by infectious diseases have dropped significantly. Of all death causes today, accidental injury is the leading one. Sexually transmitted diseases and tuberculosis have shown a rebound recently. The rate of smoking among middle school students in Beijing increased from the 1980s to 1990s, with male students' smoking at significantly higher rates than female students. Adolescents is a transitional period from dependent childhood to independent adulthood. Good physical and mental health of children and adolescents makes for good health in adulthood, therefore adolescence is a very important period in one's life. We need to go a step further and develop more detailed data on adolescent health and provide more health care for adolescents.

[An exploration of sexual knowledge, attitudes and behavior in aboriginal elementary school students in the Ping-Tung area].

The purpose of this study was to investigate sexual knowledge, attitudes and behavior of fifth and sixth grade students in aboriginal elementary schools in the Ping-Tung area. A structured questionnaire was administered to 1091 students who were selected by cluster sampling. The results showed: (1) The sexual knowledge score was low but sexual attitudes showed a positive trend. (2) 64.7% and 67.4% of students had at some time seen pictures of male or female sexual organs. (3) About 61% of students had seen sexual magazines or videotapes. (4) 66.2% of male and 88.1% of female students had heard about wet dreams or menstruation before their first experience; more than half of the students thought that wet dreams need treatment. (5) 17.8% of students had masturbation experience, and after that 59.3% of students had fear or guilt feeling. (6) Female students had significantly higher knowledge and attitude scores than male students, Demographic variables produced no significant difference in the above scores. (7) 42.4% of students most desired to know what phenomena indicate sexual maturity. (8) Sex knowledge had significantly positive correlation with sex attitude.

Some roots of persistent homosexual fantasy and the quest for father's love: conflicted parental identifications in a male patient: fragment of an analysis.

This is a clinical paper, which includes material from sessions, presenting the process of the analysis of a young adult male whose narcissistic character patterns, related to and evolved from failed attempts to integrate conflicting parental identifications. These unintegrated mother and father identifications contributed to life-long latent homosexual fantasies. Rodney's analysis indicates that for a boy/man, even a mother who has the qualities of a good enough parent, is not good enough to enable him to reach a nonconflicted manhood. A mother cannot provide for the boy the male model he needs and is searching for, the male who would affirm him in his maleness. Rodney wanted a father on whose shoulders he could stand to become a man. Rodney's persistent homosexual fantasy life, his quest for father's love, and his search for a masculine object to identify with stem from a combination of several factors. Rodney's regression to the negative oedipal phase was probably stimulated when father left the family. Rodney was eleven at the time. He felt overwhelmed experiencing himself as the oedipal victor. Unconsciously, Rodney feared his exacerbated incestuous wishes. He projected them upon his mother and subsequently incorporated them in his fantasies. His regression to more infantile dependency feelings was defensive. Rodney's father was an unsuitable object for identification. He was disinterested in Rodney and emotionally unavailable. Rodney, however, sought his father, whose lack of loving acknowledgement resulted in a lack of affirmation of Rodney's masculinity. Mother provided for Rodney the loving acknowledgement he lacked in his relationship with father. She was emotionally sustaining, an energetic, vibrant personality, who was seen by Rodney as a "superior human being." Rodney consciously idealized his mother toward whom he unconsciously also had ambivalent feelings. Rodney's identification with mother was not counterbalanced by the presence of a strong, loving father figure whom he could have used as a suitable model. This led to the development in Rodney of a strong sense of effeminization. Rodney in his homosexual fantasies assumed the so-called "feminine victimized" role. The regression to the negative oedipal phase contributed to an exacerbation of erotic, father-directed feelings, intensified by the identification with mother. Rodney was fixated in his quest for father's love. In addition, Rodney's unconscious guilt related to father and mother directed incestuous impulses, and his intense aggressivesadistic feelings contributed to the masochistic cast of his masturbation fantasies. Rodney's narcissistic aims and the quality of his narcissism changed during the analysis. His grandiosity almost disappeared. Rodney's goals became realistic and he acquired the skills necessary to achieve them. Inhibitions related to the "fear of success" were worked through. This enabled Rodney to compete successfully. His healthy narcissism derives from the success of his many achievements. Though Rodney remained a basically narcissistic personality, he did derive great pleasure from being a giving person. This was one of the many ways in which he identified with his mother. At the present, Rodney's identifications are selective and do not evoke intrapsychic conflict.

Sexual behavior changes with vulvar vestibulitis syndrome.

OBJECTIVE: To study the changes in normal sexual behavior that accompany the vulvar vestibulitis syndrome and to gauge an appropriate classification for the resulting sexual dysfunction. STUDY DESIGN: Following full medical screening, questionnaires were administered to assess sexual history, illness behavior and vulvar discomfort. Forty patients who met the criteria of vestibulitis syndrome composed the study cohort. RESULTS: The average duration of reported vulvar pain was 33 months (range, 3-240). The cohort showed no significant differences when compared with normative data on the sexual history form in their level of sexual desire, duration of intercourse or chance of female orgasm with intercourse or masturbation. Patients were: (1) highly likely to experience vulvar pain with intercourse (P < .001), (2) highly likely to have significantly reduced arousal potential (P < .001), (3) highly likely to complain of vulvar symptoms preventing intercourse from occurring (P < .001), (4) highly likely to have a reduced interest in intercourse and to have negative feelings toward it (P < .001); and (5) highly likely to refuse a partner's sexual advances (P < .001). CONCLUSION: In the case of vulvar vestibulitis, the most appropriate Diagnostic and Statistical Manual of Mental Disorders, fourth edition, classification appears to be sexual dysfunction due to a general medical condition.
