The term acute tubular necrosis refers to those forms of acute renal failure that occur in association with ischemic or hypoxic injury or exposure to nephrotoxic substances. This article discusses the various alternative classifications that are based solely on the clinical setting in which the acute renal failure occurs, and the types of therapies used in the management of them.

Increasing knowledge of the pathophysiology of respiratory distress syndrome has led to improvements in clinical management. Future advances in prevention and therapy, including administration of agents to prevent prematurity or to accelerate lung maturation, provision of surfactant replacement, and new techniques of mechanical ventilation, will further decrease mortality and morbidity.

The effect of the sitting, supine, supine with legs elevated 10 degrees, and 10 degrees head-down tilt postures on renal fluid and electrolyte handling was investigated in 14 patients with hypoalbuminaemic fluid-retaining states and in 14 normal individuals. Basal (sitting) urine volume, creatinine clearance, and urinary electrolyte levels were significantly lower in patients than in controls. In patients the values of these variables increased progressively from the sitting to the supine to the legs elevated to the head-down postures. The percentage rise was higher in patients than in controls, to the extent that, in the head-low position, only creatinine clearance values remained lower in patients than in controls. The head-down posture acts as a physiological diuretic, enhancing diuresis by improving renal function in normal individuals and in patients with ascites and oedema due to hypoalbuminaemia; it also corrects the abnormal fluid and sodium retention in these patients.
The intent of the work was to study the structure-activity relationships of AVP receptor antagonists bearing a chiral ring as a partial structure since such studies had been reported for only achiral compounds. In the present paper, we deal with compounds consisting of the chiral tricyclic hetero ring (1,2,3,3a,4,5-hexahydropyrrolo[1,2-a]quinoxaline and 1,2,3,10,11,11a-hexahydro-1H-pyrrolo[2,1-c][1,4]benzodiazepine) and 2-phenylbenzanilide analogues. These compounds exhibited a highly selective affinity for V2 receptor, and their stereochemical configuration had a great influence on V2 receptor binding. VP-343 (N-[4-[[(2S,3aR)-2-hydroxy-2,3,3a,4-tetrahydropyrrolo[1,2-a] quinoxalin-5(1H)-yl]carbonyl]phenyl]-4'-methyl[1,1'-biphenyl]-2-ca rboxamide), VP-365 (N-[4-[[(11aS)-2,3,11,11a-tetrahydro-1H-pyrrolo[2,1-c][1,4]benz odiazepin-10(5H)-yl]carbonyl]phenyl][1,1'-biphenyl-2-carboxamide) and VP-339 (N-[4-[[(11aS)-5-oxo-2,3,11,11a-tetrahydro-1H-pyrrolo[2,1-c][1,4]+ ++benzodiazepin-10(5H)-yl]carbonyl]phenyl][1,1'-biphenyl]-2-carboxami de) were the most potent compounds in vitro and in vivo. The IC50 values of VP-343, VP-365 and VP-339 against V2 receptor were 0.772, 1.18 and 0.216 nM, respectively. The ED300 values (dose required to increase three times the urine volume of the control rats; oral administration) of VP-343, VP-365 and VP-339 were 0.22, 0.31 and 0.78 mg/kg, respectively.

RATIONALE AND OBJECTIVES: The authors performed this study to assess the effects of furosemide-induced diuresis on paramagnetic contrast material enhancement at magnetic resonance (MR) imaging of the kidney, liver, spleen, and psoas muscle. MATERIALS AND METHODS: Twenty-five patients (average age, 44.9 years; age range, 23-74 years; 13 men, 12 women) who were suspected of having unilateral renal hydronephrosis received 0.1 mmol/kg contrast material with a standardized injection and imaging protocol to assess organ signal intensity at 0-5 minutes after injection. All patients had a normal serum creatinine level. Imaging was performed with a 1.5-T magnet by using a fat-suppressed fast spoiled gradient-echo pulse sequence and a 70 degrees flip angle. Eight patients received 40 mg of furosemide 10 minutes before contrast material injection. RESULTS: The areas of the renal cortical and medullary signal intensity curves minus baseline in the unilateral normal kidneys were significantly greater in the group who received furosemide (P = .026 and P = .037, respectively). The areas of the renal cortical and medullary signal intensity minus baseline in the unilateral hydronephrotic kidneys were also significantly greater in the group that received furosemide (P = .036 and P = .026, respectively). There was a statistically significant increase in splenic enhancement (P = .02) and a tendency for increased liver (P = .09) and psoas muscle (P = .08) enhancement. CONCLUSION: Furosemide-induced diuresis appears to potentiate the cortical and medullary MR renogram, as well as the MR splenogram. A rapid shift in water compartmentalization from the intracellular to the extracellular space and increased renal water content with diuresis are possible explanations.
