[Esophagocoloplasty in the treatment of total caustic stenosis of the esophagus].

Since October 1986 until January 1988 we have treated 3 patients with corrosive strictures of the esophagus, by substitution of esophagus with vascularized colon. Two females (4 and 8 years) and one male (5 years) had been treated on others hospitals by means of gastrostomy and periodically retrogradual dilatations; in two of them for more than 24 months. The surgical treatment was indicated upon the evolution time, the degree of dysphagia and the size of the affected segment. The coloesophagoplasty by retrosternal approach with proximal anastomosis in the neck, has been the operative technique performed in the male case. On the others two cases, colon interposition by extrapleural thoracic approach, combined with laparotomy was carried out. We think that the coloesophagoplasty by retroesternal way should only be advised in cases were superior anastomosis in the neck is indicated, conditioned by the height of the stricture. On the other hand, colon interposition by extrapleural approach, is the elective way for the others cases.

[A new treatment concept in severe caustic injuries and burns of the eye].

In severe burns of the eye the combination of surgical and drug therapy has proved to be useful. Initially, necrotic epibulbar tissues were excised. Large areas of denuded and in many cases ischemic sclera were covered with Tenon flaps, which were prepared and advanced from the parabulbar undamaged connective tissue. For several months an artificial epithelium was used to protect the denuded and to a large extent acellular corneal stroma. Subsequently, drug therapy including corticosteroids, Healon and prostaglandin synthesis inhibitors was applied to reduce the inflammatory reaction of the burned anterior eye segment. Regeneration of the conjunctival and the corneal epithelia can be improved by epidermal growth factor, autologous fibronectin and proteinase inhibitor (Trasylol). This therapy avoids some of the complications that could not be managed up to now and thus provides more favorable conditions for corneal grafts later.

[Caustic injuries of the esophagus].

Several different agents can cause lesions of the esophagus if they are ingested. An esophagoscopy is necessary for proper evaluation of the damage. Our experience over the last 12 years substantiates that lye and strong alkalis are the most dangerous substances to swallow. The decision to do an esophagoscopy should be based on certain criteria listed in the paper.

[Variations of the etching patterns of enamel surfaces after treatment with various caustics].

Conditioning the enamel is extremely important for the prophylactic treatment of caries with sealers. The possibility of dentition depends on the depth and pattern of etching. Incisors and molars were etched with seven etching agents. The depth of etching was examined via microradiography; the etching pattern, with the scanning electron microscope. The best etching agent proved to be a 50% solution of H3PO4 + 2% solution of NaF; The depth of etching was greater in the molars than in the incisors. The etching pattern produced with this agent was also the most even. The etching pattern could not be predicted on the basis of the etching agent.
