Bacillus cereus septic arthritis following arthrography.

Organisms of Bacillus species usually are common laboratory contaminants and nonpathogenic in humans. Recently, however, it has been suggested that cultures growing common Bacillus species may indeed represent significant infections and should not always be disregarded. A 24-year-old man developed Bacillus cereus septic arthritis following routine arthrography, which is an example of a serious orthopedic infection that can be caused by this supposedly nonpathogenic organism.

Arthrography for the assessment of pain after total hip replacement. A comparison of arthrographic findings in patients with and without pain.

Arthrography in twenty-five painful hips of twenty-one patients after total hip replacement and in fifty-three asymptomatic hips of forty-two patients after this procedure showed that arthrographic evidence of loosening was not always associated with pain. More than one-fifth of the asymptomatic hips had arthrographic evidence of loosening. Furthermore, the loosening shown by arthrogram could be confirmed in only seven of twelve hips that were subsequently explored. In addition, there was no apparent relationship between arthrographic loosening and the presence of a Trendelenburg sign. This study casts doubt on the value of an arthrogram in the diagnosis of the cause of pain after total hip replacement.

[Magnetic resonance arthrography for the diagnosis of intraarticular adhesions of TMJ adhesions].

PURPOSE: To evaluate the value of magnetic resonance arthrography (MRAr) on the diagnosis of intraarticular adhesions of the temporomandibular joint(TMJ). METHODS: 30 consecutive patients (31 TMJs) diagnosed as internal derangement of TMJ were examined by MRAr and arthroscopy. The findings of interpreting MRAr were recorded as positive, suspicious and negative according to the MRAr radiographic criteria. After comparing the findings of MRAr with those of arthroscopy, the numbers of true positive, true negative, false positive and false negative were obtained. Through SPSS11.0, receiver operator characteristic curve(ROC curve) was made, and the area under the ROC curve was calculated. According to the area, the diagnostic accuracy of MRAr was evaluated. RESULTS: Surgery results confirmed that 22 TMJs in 31 TMJs had adhesions. According to the above criteria, we found 20 TMJs were positive, 3 TMJs were suspicious, and 8 TMJs were negative. After comparing the findings of MRI with those of arthroscopy, true positive in 19 TMJs and false positive in 1 TMJs were detected among 20 positive TMJs, adhesion in 2 TMJs and no adhesion in 1 TMJs were detected among 3 suspicious TMJs, and true negative in 7 TMJs and false negative in 1 TMJs were detected among 8 negative TMJs. The area under ROC curve was 0.91(0.86,0.95), P<0.05. CONCLUSION: It is concluded that: (1)The diagnostic accuracy of MRAr is excellent, much better than that of PFA and MRI. (2) MRAr is an ideal method to diagnose intraarticular adhesions of TMJ.

Double-contrast CT arthrography of the cruciate ligaments.

The value of computed tomography (CT) in evaluating the cruciate ligaments was assessed by studying an amputated knee in order to find the most suitable position for detecting minimal defects of the ligaments. The ACL or PCL, or both, of 60 patients (61 knees) were then examined by CT and the findings were compared with those of arthroscopy. Defects 3mm in length were shown by CT. A sufficient quantity of air and an adequate amount of positive contrast medium were required to obtain an accurate picture of the ligaments. For the ACL, the sensitivity of CT was 96.6%, specificity was 95.5%, and accuracy was 96.1%. For the PCL, sensitivity was 78.6%, specificity was 91.7%, and accuracy was 84.6%. Ruptures of the cruciate ligaments shown by CT were classified into four types. Their CT images coincided significantly with their arthroscopic findings. This study shows double-contrast CT arthrography to be a valuable method for evaluating the cruciate ligaments, and especially the ACL.

[Double contrast arthrography as a screening method of suspected meniscal lesions].

On reevaluation after one year of experience with double-contrast-arthrography (DCA) we found in 83 consecutive patients with suspected meniscal lesion a true positive diagnostic frequency of 0.84, a true negative diagnostic frequency of 0.75, a true positive nosografic frequency of 0.87 and a true negative nosografic frequency of 0.70. This gives an increased diagnostic accuracy compared to the initial evaluation of the arthrografies. No complications of the examination were registered. As DCA demands fewer resources than arthroscopy, this method is suggested as a screening procedure in patients with possible tears of the menisci.

[Positive contrast arthrography in the diagnosis of ligament lesions in the upper ankle joint].

There are a great number of open questions in the diagnosis and the management of ligament lesions of the ankle joint. On the basis of the data of the literature and the author's experiences the advantages of the contrast arthrography of the ankle-joint may be resumed in the following: 1. No narcosis is necessary. 2. More exact determination of the site of the lesion is possible. 3. Therefore proper treatment may be chosen. 4. The incidental technical errors are more easily to be verified. (Paraarticular administration of the contrast medium is immediately demonstrable, whereas faulty observance of the prescriprions becomes not always evident from the roentgenogram). 5. No comparative roentgenograms are necessary. 6. The X-ray staff is not exposed to X-ray radiation.

Double contrast arthrography of the knee.

The author's experience with 280 double contrast knee arthrograms is presented. The accuracy of the procedure in 101 operated cases was 100 percent for normal menisci and 88 percent for abnormal. It is suggested that arthrography can be regarded as an integral part of the investigation of any knee in which a serious meniscal abnormality is suspected.

[The labrum acetabular. Apropos of 121 arthrographies of the hip in adults].

The authors report a retrospective study of 121 arthrographies of the hip in adults, excluding prostheses. The study was based on normal pathological values for angular measurement of the labrum acetabular: ECB angle (mean value: 15 degrees) enlarging the lateral cover of the head of the femur, VCE angle (mean value: 30 degrees) and a mean global VCB angle of 45 degrees. A certain equilibrium was observed between the covering capacity of the acetabulum and the covering capacity of the labrum acetabular. Numerous anomalies of the labrum were observed, including fissures in about 30% of cases. A large number of fissures were associated with other degenerative anomalies of the hip joint. A few fissures resulting in detachment of a strip of labrum were isolated and the possibility of their traumatic etiology and their pathogenic nature in clinical symptoms are discussed.

Clinical and roentgenological studies on malalignment disorders of the patello-femoral joint. Part I: Classification of patello-femoral alignments using dynamic sky-line view arthrography with special consideration of the mechanism of the malalignment disorders.

Dynamic sky-line view arthrography at 30, 60, and 90 flexion degrees was carried out on 1,242 knees of 621 cases who claimed trouble with at least one knee. The patient's age range was from 15 to 35 years. Alignment of the patello-femoral joint was classified into four groups; Grade I: The patello-femoral joint is congruent in all the three views. Grade II: The view taken at 30 degrees shows the joint space open in the medial facet but closed in the view at 60 degrees. Grade III: The medial interspace is open at 60 degrees but closed at 90 degrees. Grade IV: The medial joint space is open in all three views. Grade distribution of all cases studied was: Grade I, 62.1%; Grade II, 25.8%; Grade III, 6.7%; and Grade IV, 5.4%. Clinical disorders thus occur at high percentages in the groups of Grades III and IV. Based on this observation, the mechanism of the malalignment disorders of the patello-femoral joint is discussed.

The diagnostic potential of vibration arthrography.

The detection and interpretation of vibration emission from the locomotor system is a sensitive, noninvasive method for the objective study of human joints. The projects reviewed here aimed to identify and categorize vibration signals from various joints, chiefly the neonatal hip and the adult knee. Using a vibration detection and computerized analysis system, various studies have suggested that vibration arthrography will be a potentially valuable diagnostic service for the orthopedic surgeon of the 1990s.

Early diagnosis of acute rupture of the quadriceps tendon by arthrography.

Arthrographic study of the knee was performed in six patients for diagnosis of acute rupture of the quadriceps tendon. All of the patients gave a history of attempting to recover from a fall. Pain and tenderness over the suprapatellar area were common findings in all patients. Four patients had a positive arthrogram demonstrating extravasation of the radiopaque material outside the knee along the quadriceps tendon. Of these four patients, two had complete loss of quadriceps function, but the other two were able to partially extend the knee when the gravity was eliminated. Two patients had negative arthrograms; both had loss of active extension of the knee against gravity but were able to extend the knee when the gravity was eliminated. All four patients with a positive arthrogram had rupture of the quadriceps tendon, which was demonstrated at the time of surgery. In one patient with a negative arthrogram, exploration of the knee revealed an intact extensor mechanism. Arthrography of the knee is an aid to the diagnosis of rupture of the quadriceps tendon and when indicated can be performed in the emergency room.

[A system for improving the quality of the image in knee arthrography].

A simple and inexpensive technique for arthrography is described. A standard fluoroscopic unit with overhead tube is used. A special cassette holder allows the use of cassettes with a screen-film combination normally used for mammography. An exceptionally good combination of factors important for a good image quality is obtained.

Complementary role of wrist arthrography in non-union of scaphoid fractures.

Plain radiographs and wrist arthrography of 11 patients with non-united fractures of the scaphoid were retrospectively studied. The arthrographic feature of non-union was found to be opacification of the fracture line and, secondly, of the midcarpal compartment, whereas in patients with fibrous or cartilaginous union no opacification of the fracture line was obtained. There was no single plain radiographic feature that could distinguish non-union from fibrous or cartilaginous union.

Fluoroscopic technique for double-contrast knee arthrography.

The fluoroscopic double-contrast technique of arthrography of the knee is useful in documenting the extent of suspected injury to the knee. It is of particular value in demonstrating small and large lesions involving the menisci, cruciate ligaments, patellar cartilages, as well as the articular cartilages of the femoral condyles and tibial plateaus. This examination is instrumental in the diagnosis of the patient with an atypical history of injury and unusual physical findings. A simple restraint device allows the fluoroscopist complete control over the stress applied to the knee while positioning the patient for filming. Fluoroscopic spot radiographs of excellent contrast and sharp detail can be obtained of each of the various structures, applying the stress needed. When the radiologic technologist takes a little time to become more knowledgeable about the anatomy of the knee jount, and the rationale for the various views, the knee arthrographic examination is easily understood.

[The reliability of arthrography of the knee joint via medium format technique (author's transl)].

Reports on 217 arthrographies of the knee joint with operative control. Partly or entirely erroneous roentgen diagnoses were made in 4.8-16.3% of the cases, depending upon the technique by which the patients had been examined. Lateral meniscus lesion was wrongly diagnosed three times as often as the medial lesion. The lowest incidence of wrong diagnoses occurred when examining via the medium format technique.
