Dynamic histomorphometry of alveolar bone remodeling in the adult rat.

Alveolar bone normally undergoes remodeling on one side of the socket and modeling on the opposite side as the tooth migrates at a rate of 6.7 micrometer per day. Periodontal ligament width, however, remains constant. Because of this very high turnover rate, this bone is a good model to study bone modeling and remodeling activities. This study was undertaken in order to measure the different cellular events occurring during tooth migration along the alveolar bone of the rat. The histomorphometric measurements performed on this model permitted us to calculate the duration of each phase of the remodeling cycle, i.e., resorption lasts about 1.5 days and reversal about 3.5 days. Since the duration of the forming phase is about 1 day (Guyomard and Baron, '74), the total duration of each remodeling cycle is about 6 days. This time is very short compared to 60--120 days in adult human trabecular bone. Additionally, in this model each osteoclast resorbs 2--4 times its own volume of bone per day. Based on this knowledge, it will be possible to measure accurately the effects of experimental conditions on bone cells and bone remodeling in this rat alveolar bone model.

Primary tuberculosis of the oral cavity.

Two cases of primary tuberculosis of the mouth are reported. Painless ulceration of long duration and enlargement of the regional lymph nodes that did not respond to conservative and antibiotic therapy were the main manifestations of the disease in both cases. In the first case the tubercle ulcer was located in the lower buccogingival sulcus of a 17-year-old girl. In the second case the lesion occurred as a chronic periodontal inflammation around the gingiva of the left lower second molar tooth. When the tooth was extracted, a painless ulceration appeared around the socket, which was filled by granulation tissue. Histopathologic examination; bacteriologic, serologic, and blood tests; and chest x-ray film confirmed the diagnosis. Predisposing factors that might favor primary inoculation of tubercle bacilli into oral mucosa are also discussed. Finally we emphasize that in such cases it is essential to attempt to locate a possible primary site elsewhere in the body before oral tuberculosis is considered the primary disease.

[The role of fibrinolysis in the pathogenesis of alveolitis after tooth extraction. Preliminary report].

We registered fibrinolytic activity of blood within fresh wounds after tooth extractions by means of measuring fibrinolysis in vitro. In cases which later developed alveolitis we found higher degrees of fibrinolysis. Women taking oral hormonal contraceptives showed fibrinolysis to a greater extent than other women or men. We found more fibrinolytic activity in cases with longer extraction times. Possible starting points for prevention of alveolitis are discussed.

[The bacteriology of normal wound healing following tooth extraction with special reference to anaerobic microorganism diagnosis].

In 10 patients with indication to tooth extraction swaps from the microbiotopes "sulcus/gingival pocket", "root canal", "socket" and "extraction wound 2 and 7 days after extraction" were investigated bacteriologically. The anaerobic-aerobic mixed flora with predominance of anaerobes verified in the microbiotopes "sulcus/gingival pocket", "socket" and "extraction wound 2 days after extraction" was identically. The flora of the root canal was distinguished by a smaller amount of microbes with predominance of aerobes. In the 7 days old extraction wound only single aerobes could be verified whereas anaerobes were missed totally.

[Histopathological observation of periodontal tissue by tipping movement toward the hydroxyapatite-packed socket].

Tipping movement in orthodontic therapy, which is often performed even in the absence of the adjacent tooth, is represented by the movement following tooth extraction. Such tipping movement in the absence of the adjacent tooth depends not only upon the magnitude of orthodontic force as classified into Stoner 4D, but also upon the condition of the extracted wounds. To evaluate the clinical applicability of HAp-packing used in combination with tipping movement, histopathological investigation was performed in the periodontal tissue with tipping movement toward the adjacent to HAp-packed socket. HAp packing into the extracted wound cavity produced no significant changes in quantitative tipping movement compared with non-HAp packing. HAp packing into the extracted wound cavity, followed by tipping movement, produced compressed sides not only for the moved tooth but also for the anchor tooth. Resorption and disappearance of alveolar septum were observed on the compressed sides of both these teeth for all experimental sides. HAp in an amount enough to meet the narrowing of the extracted wound cavity, associated with tipping movement was discharged from the suture surface. In the same region, fibrous connective tissue lay between the dental root and the HAp mass, which were not adjacent to each other. The fibrous connective tissue presented temporal bone regeneration so as to surround the tooth. Apart from the moving distance, the tissue change showed no essential difference either for a weak force or for a strong force. The above results revealed the usefulness of HAp packing into the extracted wound cavity for tipping movement of the adjacent tooth in the direction of the wound cavity and immediate move ability after extraction.

The effect of excision of the root apex on the intrusive mobility of anterior teeth in adult monkeys (Macaca fascicularis).

Intrusive tooth mobility was recorded on anterior teeth in four adult male animals by linear variable-differential transformers. Reflection of the labial mucosa caused a significant increase in mobility at 4 N when the loading rate was slow (1 N s-1) but not when fast (100 N s-1). No significant additional increase in mobility was recorded after the apices were removed from eight incisors and canines. A further study on five incisors was made in which the displacement at 4 N load was noted half hourly during a control period of 5 or 6 h. Some weeks later, the sequence was repeated but the apices were resected after 3 or 4 h. As there was no increase in mobility for the majority of teeth following apicoectomy and, for two teeth studied in detail where there was an increase, that change was short lived; it seems that compression in the fundus of the socket is not a mechanism of importance in supporting the tooth against small intrusive loads.

[The repair process in tooth extraction wounds after implantation of microcrystalline collagen fragments. Histological study in rats].

The author studied histologically, the healing process after dental extraction in rats which sockets were filled with fragments of microcrystalline collagen. Thirty six rats (Rattus Novergicus, albinus, Wistar), young, male, weighting from 90 to 110 g and separated into two groups, control and experimental, of eighteen animal each. All animals were anaesthetized with 10% cloral hidrate, 3 ml/Kg body weight and the right maxillary incisors were extracted. In control animal the sockets were sutured after clot filling, and the experimental sockets were sutured after filling with microcrystalline collagen. Three animals of each group were killed after periods of 1, 3, 6, 14, 21 and 24 days after surgery. The following conclusions were drawn: 1. Microcrystalline collagen caused an evident inflammatory reaction in the inicial phases of the healing process of the sockets. 2. Microcrystalline collagen was biodegraded 6 to 14 days after surgery. 3. Microcrystalline collagen delayed the repair process of the sockets.

Vertical root fracture treated by bonding fragments and rotational replantation.

These case reports describe a new treatment method for complete vertical root fractures. This method is composed of two steps. First, the fractured tooth is intentionally extracted atraumatically, and then the separated fragments are bonded with an adhesive resin cement. The restored tooth is replanted by rotating approximately 180 degrees into the original socket and fixing with an orthodontic wire. For both cases reported here, bone regeneration was observed at the 3-year follow-up. Since this modality adopts the respective advantages of both intentional replantation and adhesive treatment, it might be the treatment of choice in cases of vertical root fracture.

Alveolar osteitis (dry socket)--a review.

The literature on "dry socket" is reviewed. The diagnosis, incidence and predisposing factors are discussed in relation to its aetiology. Treatment regimes are outlined; in particular, the use of anti-fibrinolytic drugs.

Effects of mesiodistal inclination of implants on stress distribution in implant-supported fixed prostheses.

PURPOSE: Patterns of von Mises stress values surrounding implants supporting fixed prostheses in the posterior edentulous maxilla were evaluated using 3-dimensional finite element analysis. MATERIALS AND METHODS: Implants were placed in maxillary bone in 2 different configurations. In the first configuration, implants were placed in the first premolar, second premolar, and second molar regions; in the second configuration, implants were placed in the second premolar and second molar regions, and a mesial cantilever was extended to the space of the first premolar tooth on the superstructure. On the implant placed in the socket of the second molar, 3 different inclinations were used (0, 15, and 30 degrees). Loading was applied in the vertical, oblique, and horizontal axes. RESULTS: Inclination of the implant in the molar region was found to result in increased stress. Significant increase in stress on the implant embedded in the premolar region was also seen in the design with the cantilever as compared to the conventional prosthesis design. DISCUSSION: The stress concentrations observed at the neck of the implant were similar to results reported in the literature. CONCLUSION: The highest stress value obtained in the study was 194.2 MPa with oblique loading. This value did not exceed the endurance limit of pure titanium, which is 259.9 MPa.
