Transoesophageal echocardiogram identifying the source of endoleak after combined open/endovascular repair of a type 3 thoracoabdominal aortic aneurysm.

Open repair of thoracic aortic aneurysms is associated with significant morbidity and mortality. The introduction of endovascular repair has reduced both the morbidity and mortality. However, endovascular stent repair can be complicated by endoleaks. We report here the successful treatment of a type 2 endoleak following endovascular repair of a thoracoabdominal aortic aneurysm, using transesophageal echocardiography to assist in the localization of the thoracic endoleak.

Use of medical-grade methyl triacetoxy silane crosslinked silicone for facial prostheses.

The urethanes, PVC, acrylic resins, and silicones are still the most popular among a large number of materials being used to fabricate facial prostheses. Each material has advantages and disadvantages. The ideal material is not yet available. It was found that Silastic medical adhesive type A, a preparation of methyl triacetoxy silane crosslinked silicone, possesses a wide range of biologic, clinical, and technical advantages over other materials. However, the poor adheophilic properties of this material remain its most significant disadvantage. A processing technique for methyl triacetoxy silane crosslinked silicone is presented. Further investigations are being conducted to improve this material.

A comparison of the color stability of conventional and titanium dental porcelain.

Simulation of naturally appearing enamel in fixed prosthodontics is best achieved with porcelain. This study investigated the color stability of two different dental porcelains, Ceramco and Procera, after they were exposed to 900 hours of accelerated aging. Twenty titanium plates provided the substrate to which the Procera porcelain samples in shades A3, A3.5, C3, and D3 were fired, five in each shade. An equal number of metal-ceramic alloy (Olympia) plates were cast to the same dimensions to form the 20 substrates to which Ceramco porcelain was fired. The four shades of porcelain were again added to each of the samples. Commission International de l'Eclairage L a b readings were recorded with a Minolta Chroma Meter II before and after exposure to 900 hours of accelerated aging simulation. After the aging process the color differences between the pretest and posttest condition were calculated. Although statistical comparisons demonstrated a difference in color change (p < or = 0.05) between Ceramco and Procera porcelain in three of the shades tested, critical remarks of color refer to both types having "slight" color change. In terms of color stability, neither porcelain had "noticeable" changes after 900 hours of accelerated aging.

Two-year clinical evaluation of lithia-disilicate-based all-ceramic crowns and fixed partial dentures.

OBJECTIVES: The aim of this study was to evaluate the clinical performance of crowns and fixed partial dentures (FPDs) made with the Empress 2 system over a 2-year period. METHODS: Twenty anterior or posterior all-ceramic (Empress 2) crowns and 20 anterior or posterior, three-unit fixed partial dentures were fabricated for 15 patients. Evaluations of the restorations were performed at baseline and once a year during the 2-year follow-up period. U.S. Public Health Service criteria were used to examine the marginal adaptation, color match, secondary caries and visible fractures in the restorations. Survival rate of the restorations were determined using Kaplan-Meier statistical analysis. RESULTS: U.S. Public Health Service criteria showed 100% Alpha scores concerning recurrent caries for both crowns and FPDs. No crown fractures were observed during the 2-year follow-up, however, 10 (50%) catastrophic failures of FPDs occurred. Five (25%) failures occurred within the 1-year clinical period and the others (25%) within the second year. SIGNIFICANCE: Single unit Empress 2 all-ceramic crowns exhibited a satisfactory clinical performance over 2-year period. Furthermore, the high fracture rate of Empress 2 FPDs limits the usage of Empress 2 for the fabrication of all-ceramic FPD.

Embolization of iliac artery aneurysms following abdominal aortic aneurysm repair with a bifurcated graft.

Six iliac artery aneurysms in four patients were percutaneously embolized. All patients had previous abdominal aortic aneurysm repair using a bifurcation graft with distal anastomoses to external iliac arteries. The iliac aneurysms involved the oversewn common iliac arteries in all patients. Embolizations were performed via an ipsilateral common femoral arterial approach, with metal coil occlusion of aneurysm inflow and outflow. All aneurysms were successfully thrombosed. Follow-up colour flow Doppler examinations showed continued aneurysm thrombosis in all patients. One patient developed post procedure buttock claudication, which improved over time; there was no other procedure related morbidity. In conclusion, we describe a technique for percutaneous embolization of iliac aneurysms following abdominal aortic aneurysm repair with a bifurcated graft. We have demonstrated the safety and efficacy of this approach.

Five-year clinical evaluation of Procera AllCeram crowns.

STATEMENT OF PROBLEM: Esthetic demands of patients and practitioners for an all-ceramic crown in both anterior and posterior regions of the dental arches has prompted the development of the Procera AllCeram crown. Long-term clinical trials that evaluate the strength and naturalness of the Procera AllCeram crown are lacking. PURPOSE: This prospective study was initiated to evaluate the clinical performance of 100 Procera AllCeram crowns after 5 years in service. MATERIAL AND METHODS: One hundred Procera AllCeram crowns were fabricated for 58 patients (20 men and 38 women). Patients were treated by 4 general dental practitioners. Crown placement involved both the anterior and posterior regions of the dental arches. Crowns were examined at baseline and once a year during the 5 years that followed and evaluated at each appointment with the California Dental Association's quality assessment system. RESULTS: Of the 97 crowns remaining in the study after 5 years, only 3 crowns had experienced a fracture through the veneering porcelain and the aluminum oxide coping material. Two additional crowns were replaced as a result of fractures of only the veneering porcelain. One crown was replaced as a result of recurrent caries. All remaining crowns were ranked as either excellent or acceptable for surface/color, anatomic form, and marginal integrity. CONCLUSION: The 5-year clinical observations and ranking with the California Dental Association's quality assessment criteria supported the conclusion that Procera AllCeram crowns may be used in all areas of the mouth.

Color stability of facial silicone prosthetic polymers after outdoor weathering.

STATEMENT OF PROBLEM: Color deterioration of silicone prosthetic elastomers affects the life expectancy of facial prostheses in a service environment. PURPOSE: This study investigated the color stability of 3 nonpigmented silicone elastomers (Silskin 2000, Elastosil M3500, Ideal) exposed to outdoor weathering for 1 year. MATERIAL AND METHODS: A tristimulus colorimeter was used to evaluate the color differences (DeltaE*) of the silicone elastomers every 2 months. Ten disk-shaped specimens for each material were tested. Data were analyzed with a repeated measures ANOVA and Student-Newman-Keuls test at a significance level of alpha = .05. RESULTS: All silicone elastomers showed visually detectable, mean color differences (DeltaE* > 2.0), that ranged from 2.13 to 3.98. Silskin 2000 showed greater color differences (P <.05) compared with Elastosil M3500 and Ideal, which showed similar behavior (P >.05). CONCLUSION: Outdoor exposure of silicone facial elastomers for 1 year resulted in visually detectable color differences. Duration of exposure and silicone elastomer were significant factors that affected color stability. Silskin 2000 was significantly less color stable than Elastosil M3500 and Ideal, which were not statistically significantly different from each other.

Pulsed electromagnetic fields promote bone formation around dental implants inserted into the femur of rabbits.

The present study examined the effect of applying a pulsed electromagnetic field (PEMF) on bone formation around a rough-surfaced dental implant. A dental implant was inserted into the femur of Japanese white rabbits bilaterally. A PEMF with a pulse width of 25 microseconds and a pulse frequency of 100 Hz was applied. PEMF stimulation was applied for 4 h or 8 h per day, at a magnetic intensity of 0.2 mT, 0.3 mT or 0.8 mT. The animals were sacrificed 1, 2 or 4 weeks after implantation. After staining the resin sections with 2% basic fuchsin and 0.1% methylene blue, newly formed bone around the implant on tissue sections was evaluated by computer image analysis. The bone contact ratios of the PEMF-treated femurs were significantly larger than those of the control groups. Both the bone contact ratio and bone area ratio of the 0.2 mT- and 0.3 mT-treated femurs were significantly larger than the respective value of the 0.8 mT-treated femurs (P < 0.001). No significant difference in bone contact ratio or bone area ratio was observed whether PEMF was applied for 4 h/day or 8 h/day. Although a significantly greater amount of bone had formed around the implant of the 2-week treated femurs than the 1-week treated femurs, no significant difference was observed between the 2-week and 4-week treated femurs. These results suggest that PEMF stimulation may be useful for promoting bone formation around rough-surfaced dental implants. It is important to select the proper magnetic intensity, duration per day, and length of treatment.

Physicomechanical and cytotoxic properties of room temperature vulcanizing silicone prosthetic elastomers.

The physicomechanical and cytotoxic properties of two newly introduced room temperature vulcanizing (RTV) silicone prosthetic elastomers, Ideal and Silskin 2000, were investigated. Another RTV silicone, Elastosil M3500, was also investigated as a potential facial material. The in vitro cytotoxicity was assessed with the agar diffusion test and mouse fibroblast cells (L929). The properties investigated were tensile strength, percentage elongation, modulus, tear strength, hardness, and color changes (deltaE*). The properties tested were selected because of their clinical significance in fabricating facial prostheses. The results indicate that Elastosil M3500 has a better combination of high tear strength, elongation at break, and low hardness than Ideal and Silskin 2000. All materials demonstrated a low cytotoxic profile. Elastosil M3500 warrants further attention with clinical trials.

Computer analysis of titanium implants in atrophic arch and poor quality bone: a case report.

The oral implantologist usually uses a panoramic radiograph for the evaluation of bone tissue around implants. The development of computed tomography combined with computer software has allowed for the bone-to-implant interface to be illustrated in greater detail with cross-sectional and pseudo-color images. An implant patient has titanium fixtures in an atrophic arch and poor quality bone four years after implantation. The implants were loaded with fixed metal/resin restorations seven days after surgery. Integration of the implants and the anatomic structures near the implant sites are described with an imaging technique.

Optimal mitral annular and subvalvular shape change created by the Coapsys device to treat functional mitral regurgitation.

We have reported that the Myocor Coapsys (Myocor, Inc, Maple Grove, MN) device treated functional mitral regurgitation (MR) by reducing mitral annular dimension and repositioning papillary muscles. This study was conducted to evaluate the optimal Coapsys device sizing level. The Coapsys device was implanted in seven dogs after induction of MR by rapid ventricular pacing. The device consists of anterior and posterior pads connected by a subvalvular cord. The device was tightened in 5% increments of the left ventricular epicardial to epicardial dimension up to 40%. Hemodynamic and echocardiographic measurements were repeated at each tightening level. The Coapsys significantly reduced or eliminated functional MR, and the reduction was maximized at the 30% tightening level or lower in all cases. Although the left ventricular end diastolic volume decreased significantly, forward stroke volume was maintained until the 35% tightening level. The forward ejection fraction significantly increased from 33 +/- 24% at baseline to 62 +/- 42% at 40% tightening level. Mean aortic pressure decreased slightly but significantly. The Coapsys device can be applied over a broad range of tightening levels with significant reduction in MR without negative physiologic impact. This feature makes the device usable [corrected] in a variety of clinical settings.

A rare case of Listeria monocytogenes presenting as prosthetic valve bacterial endocarditis and aortic root abscess.

Listeria monocytogenes is a gram-positive bacillus that is rarely associated with infections in the general population. Those susceptible to this pathogen include neonates, pregnant women, and the immunocompromised. The most common clinical manifestations of listeriosis are bacteremia and meningitis. Endocarditis caused by L. monocytogenes is rare with less than 60 cases reported in the world literature. We report the case of an 81-year-old man who suffered aortic prosthetic valve listeria endocarditis, and examine the literature regarding this rare manifestation of human listeriosis.

Histological evidence of osseointegration in human retrieved fractured hydroxyapatite-coated screw-type implants: a case report.

Histological evidence of clinically successful dental implants is very rare. This case report presents histological evidence of osseointegration in human implants retrieved because of fractures at the connected portion between the abutments and fixtures due to a car accident. The duration of functional loading of the implants was 18 months. Two hydroxyapatite (HA)-coated screw-type implants were removed with part of the healthy bone from the mandibular left molar region. A block was prepared using cutting and grinding equipment to obtain a central section approximately 50 microm thick, which was stained with basic fuchsin and methylene blue. Histological examination revealed that the bone was dense and in close relation with the HA coating of the implants. The interspaces of each thread of the implant were filled with mineralized bone. Peri-implant soft tissue was not observed in the section. A high degree of osseointegration was noted, with a bone-to-implant contact of 87.5% (implant corresponding to tooth 36) and 97.4% (implant corresponding to tooth 37). The connection between the 30 and 50 microm HA coating and the metal was uniformly tight and constant. In conclusion, the histological evidence showed a high degree of osseointegration in two HA-coated screw-type dental implants retrieved after functional loading for 18 months.
