In six acutely anesthetized, mechanically ventilated mongrel dogs, we evaluated sequential changes (over 30 minutes) of sustained forelimb venous occlusion (occlusion pressures 30 to 65 mm Hg) on strain gauge dimension, radionuclide blood pool emissions, and venous, subcutaneous, and intramuscular pressures (Wick technique). Forelimb intravascular volume (assessed by decay-corrected radionuclide counts/unit time) changed by a mean of only -1.2 +/- 2.8% (+/- SEM) and thus was ignored in the calculation of filtration rate. Forelimb (distal to the occlusion site) hematocrit changed insignificantly (45.3 +/- 1.4% at the control point to 48.2 +/- 1.1% at 30 minutes), colloid osmotic pressure rose slightly 19.4 +/- 1.4 mm Hg to 22.7 +/- 1.6 mm Hg, p less than 0.01), and serum osmolality remained unchanged. During sustained occlusion, venous pressure remained constant, subcutaneous pressure rose (-0.7 +/- 1.2 mm Hg control vs 0.5 +/- 1.4 mm Hg at 30 minutes, p less than 0.05), and intramuscular pressure also rose (-0.4 +/- 1.1 mm Hg to 2.6 +/- 1.6 mm Hg, p less than 0.01). Driving pressure, defined by venous pressure - oncotic pressure less the average of intramuscular and subcutaneous pressure declined slightly over the 30 minutes of the study (27.8 +/- 5.5 mm Hg to 23.3 +/- 1.3 mm Hg, p less than 0.05). The relationship between either the initial driving pressure or the initial difference between venous and protein osmotic pressure correlated (r = 0.83 for both) well with strain gauge estimates of capillary fluid flux (evaluated by the change in forelimb strain gauge dimension over time and given as cc/100 cc forelimb volume/min).(ABSTRACT TRUNCATED AT 250 WORDS)
A new computerized plethysmograph allows segments of the vascular system to be examined accurately and automatically. Laser flowmetry allows blood flow measurements to be made of digital circulation for vascular abnormalities. With the 2 new instruments, large and small vessel circulations can be measured quickly and easily. Disease of these vessels can be readily detected. Thus, appropriate medical or surgical therapy can be investigated.

Seventy-two limbs in forty patients underwent Doppler systolic ankle pressure and Pulse Volume Recording (PVR) amplitude measurements intraoperatively. Control patients and patients undergoing abdominal aortic aneurysm (AAA) resections showed no significant decrease in Doppler systolic ankle/brachial pressure ratio (DSAB). PVR measurements were slightly decreased after declamping in the AAA patients. Femoropopliteal bypass was associated with a prompt increase in PVR and DSAB levels. In contrast, postreconstruction values in the extraanatomic (EA) and aortofemoral (AF) bypass groups were dependent upon the patency of the femoropopliteal segment. Intraoperative monitoring provides a quantitative assessment of the immediate success of arterial surgery.
The accuracy of occlusive impedance phlebography (IPG) depends on recognition of the artifacts that produce false-positive results. Fifty per cent of a series of studies of patients with normal legs showed abnormal results when the recording was made with the leg held straight. We therefore recommend that the patient's leg position be carefully monitored when an IPG test is being administered.
Computer-assisted analysis of the data from SLP and PVR study of the legs with angiographically documented arterial occlusive disease, projected against a background of normal limbs, was used to assess the diagnostic value of each study, including its individual variables, as well as their varied combinations. The accuracy of both SLP and PVR interpretation was significantly improved by standarization against brachial values, but even with the aid of superimposed physician interpretation, each only reached 86 per cent when cases with multilevel occlusive disease were included. The combination of these two studies, which compensate for each other's limitations in several respects, achieved an overall accuracy of 97 per cent, with no errors observed in diagnosing either normal limbs or those with isolated occlusive disease. Objective diagnostic criteria for both SLP and PVR interpreatation are presented in algorithmic form.

On line determination of airway resistance is carried out by an Apple II connected to a flow body plethysmograph. Recognition of the panting maneuver is performed in real time. Linear drift of plethysmographic volume V is eliminated by taking the derivatives V and Vm of V and mouth flow Vm. Linear regression of V on Vm yields an estimate R of airway resistance, at a volume close to functional residual capacity FRC. The values of FRC is given by linear regression of V on Pm, the derivative of mouth pressure. No significant difference has been found between the estimates of the specific conductance sG calculated by linear regression and by spectral analysis of V and Vm. As variability of sG can be kept below 10%, the regression technique appears to be reliable for routine patient testing and pharmacological studies.
