Aggressive angiomyxoma of the pelvis and perineum: case study.

Aggressive angiomyxoma (AAM) is a rare locally aggressive tumor affecting the pelvis and perineum of young females. Histopathologically, it is characterized by fibreoblasts in a myxoid background with vascular proliferation, scanty mitotic figure and no real capsule. AAM needs to be considered in the differential diagnosis of vulval mass in a reproductive age woman. We describe the first Ethiopian case of huge, recurrent, AAM of the pelvis and perineum in a 35 year old para II lady and stress that early diagnosis offers a better option for treatment by wide local excision and low recurrence rate.

Osteomyelitis of the pelvis and proximal femur: diagnostic difficulties.

Osteomyelitis of the pelvis or proximal femur may still be diagnosed late because the infection is deeply placed and investigations may concentrate solely upon the possibility of septic arthritis. Periacetabular infection was diagnosed in 16 children between 1994 and 1998. A high index of suspicion and the use of appropriate imaging will ensure that the condition is not allowed to progress, although in this series one child underwent an unnecessary appendectomy, and a subsequent sepsis of the hip joint was drained in another case. Radiographs of the pelvis were rarely abnormal within 7 days of the onset of symptoms and an ultrasound scan focused on the hip joint may miss the periarticular changes.

[More effective exenteration of small pelvis organs in colorectal cancer: is it possible?].

21 exenterations of small pelvis organs were performed from 1991 to 2000 for locally-spread cancer of the rectum. Temporary passage feces and urine before radical operation was performed in 6 patients. 10 patients underwent infralevator and 11 patients--supralevator exenteration of small pelvis organs. Continent cypass of urine was carried out in all the patients (in 2--delayed), and of feces--in 8 (in 2--delayed). Substitution of the urinary bladder was performed in 11 patients, formation of reservoir with "dry" urinostoma--in 10.

[Ectopic spleen in pelvis minor--a case report].

A case of ectopic spleen diagnosed as a left adnexal tumor in a 19 year old female was presented. Considering the rareness and lack of typical symptoms, diagnosis of this pathology still is very difficult, and establish most often intraoperatively. In every case of ascertainment irregular mass in minor pelvis one should differentiate it among: colonic diverticulosis, wandering kidney, coproliths, colon and mesentery tumors and ectopic spleen. We consider that ultrasonography plays the special role in these cases. In every case of stating the pathologic structure in minor pelvis it seems advisable not to content only with estimation of genital organs, but also locate such as: kidneys, liver, spleen. Such a procedure allows to state preoperative diagnosis more precisely.

Selective arterial embolization in the treatment of an aneurysmal bone cyst of the pelvis.

This case report deals with an aggressive aneurysmal bone cyst of the pelvis in a 20-year-old man causing considerable destruction of the left ilium and the anterior column of the acetabulum, with extension into the pelvis. The treatment of large pelvic aneurysmal bone cysts is challenging because of local destruction of adjacent structures and because of the risk of severe intraoperative bleeding. Therefore selective preoperative embolization may be a valuable tool in the management of aneurysmal bone cysts.

[Pelvic digit: a case report with reference to the differential diagnosis of pelvis abnormalities].

Pelvic digit was incidentally detected on plain pelvis radiography of a 44-year-old male patient who presented with right-sided hip pain. Three-dimensional reconstruction of computed tomography scans provided a more clear illustration of the anatomy and localization of this anomaly. To avoid unnecessary investigation methods and treatment, this entity should be kept in mind when an atypical bone structure is noted around the pelvis

Osteosarcoma of the pelvis. A clinical and histopathological study of twenty-five patients.

Twenty-five patients who had an osteosarcoma of the pelvis were treated at the University of Florida between 1967 and 1990. Two of these patients had underlying Paget disease, and five had received previous radiation therapy to the pelvis. Common problems in this series of patients included delays and errors in establishment of the diagnosis, underestimation of the extent of the tumor on the radiographic staging studies, histopathological findings of local extension next to and into pelvic structures, widespread invasion into major pelvic veins, and microscopic foci of tumor in otherwise normal tissue. These problems led to difficulty in gaining local control of the tumor with any type of operative procedure. Of the eighteen patients who had a resection, only four had a contamination-free wide margin, and a local recurrence developed in thirteen. Only one patient, who had no evidence of disease eleven years after treatment, was alive at the time of writing. Because of the tendency for venous invasion, the radiographic staging studies should include a thorough evaluation of the blood vessels adjacent to the tumor.

Computed tomographic evaluation of benign and inflammatory disorders of the female pelvis.

The use and importance of CT in the evaluation of patients who have inflammatory conditions of the pelvis has increased in recent years. Although its role may be limited in women of childbearing age, CT may provide definitive diagnostic information concerning benign pelvic masses frequently present in this age group. CT is particularly valuable in detecting infectious complications in the postpartum and postoperative pelvis. In patients who have known or suspected inflammatory conditions of the bowel, CT is the imaging modality of choice to identify extraluminal complications such as abscess formation. CT is frequently diagnostic in cases of diverticulitis and appendicitis and is of recognized value in guiding percutaneous drainage procedures, often sparing emergent surgical intervention.

Diagnostic imaging of carcinoid metastases to the abdomen and pelvis.

BACKGROUND: Functional and anatomical imaging methods are currently the standard approach to the evaluation of carcinoid tumor extent. Due to the nature of carcinoid tumors ,there is not a single imaging procedure that is sufficient to detect the malignancy. The aim of the study was to compare the value of CT, MR and functional imaging study with(111) In-Octreotide (SRS) in carcinoid detection and evaluation of its extent as well as to work out the imaging studies diagnostic algorithm that could enhance diagnostic efficacy. MATERIAL/METHODS: A total of 34 patients with carcinoid tumor confirmed clinically, biochemically and/or histopathologically were enrolled into the study. Computed tomography (CT) examinations of the abdomen and pelvis were performed before and after administration of a contrast medium, the triple-phase liver scanning included. Magnetic Resonance Imaging (MRI)of the liver was performed according to the standardized protocol before and after administration of a contrast medium .Planar scintigraphy with(111) In Octreotide and SPECT were performed. In each case liver as well as lymph node metastases in the abdomen and pelvis were searched for. All results were reviewed independently. Clinical picture at follow-up and further imaging and/or histological studies confirmed or excluded the diagnosis of the disease. RESULTS: Twenty nine patients were diagnosed with carcinoid tumor. Liver metastases were found in 26 patients and lymph node metastases were detected in 18 of them. Complete diagnostic conformity as far as the diagnosis and extent of the disease are concerned was obtained only in 7 cases. Incorrect assessment of the disease extent was the source of discrepancy between the imaging studies results in most of the remaining 22 patients. The SRS and MR results of liver metastases detection were mostly consistent with one another. Both imaging modalities evaluated the extent of hepatic metastases incorrectly in 22%of the patients,as compared to 43% of the patients for CT. The highest proportion percentages of incorrect results of lymphatic involvement detection were 41%, 29%and 43%for MR, SRS and CT, respectively. CONCLUSIONS: The results of our study have indicated that application of a complex imaging algorithm enables higher accuracy of hepatic and lymph nodes metastases detection in carcinoid tumor patients. Combining the results of MR and SRS in evaluating the extent of the carcinoid disease seems to represent higher diagnostic value than relying on one of this method in combination with CT images.

[Laparoscopy-assisted exenteration of the pelvis in a locally advanced rectal carcinoma].

INTRODUCTION: In this pilot study, based on three case-reviews, the authors report on their initial experience with the laparoscopy-assisted total exenteration of the pelvis, conducted in patients with locally advanced rectal carcinomas. METHODS: In two patients with histologically confirmed rectal carcinomas, the neoadjuvant radiochemotherapy was initiated, due to the locally advanced tumor finding, aiming to downstage and locally control the disorder. The pelvic exenteration was indicated, based on the control findings of the imagining methods. In one patient, due to repetitive enterorrhagies, the primary surgical management was indicated. RESULTS: The surgical procedures were multidisciplinary, assisted by urologists. In all three cases, the laparoscopic resections, followed by planned conversions and constructions of the ileouretheral conduit and the terminal sigmoidostomies (resp. with their maintenance), were completed. Prior to the procedures, in all the three patients, the presence of distant metastases was excluded. No peroperative complications were recorded in the case-reviews. In one case, postoperative re-suturing of the wound was inevitable, due to its dehiscence. In this patient, the enzymatic acute MI, without the ECG correlate, was diagnosed. CONCLUSION: The laparoscopy-assisted pelvic exenteration may be a suitable modification of the classical surgical procedure in selected patient groups with locally advanced colorectal carcinomas. Good visualisation and exact orientation in the region of the small pelvis, as well as other generally accepted advantages of miniinvasive procedures, are the pros of the method.

Clinicopathological features of unusual vascular lesions of the pelvis, retroperitoneum and colon in females: a report of five cases and review of the literature.

Vascular lesions comprise benign and malignant neoplasms as well as non-neoplastic conditions that may be located in various sites but only rarely in the pelvis or the retroperitoneum. In our study we describe five diverse and unusual cases of vascular lesions of the pelvis, retroperitoneum and colon in female patients: a case of retroperitoneal angiosarcoma, pelvic hemangioendothelioma, pelvic angiomyxoma, retroperitoneal lymphangioleiomyomatosis and a case of diffuse cavernous hemangiomatosis of the colon, with emphasis on their clinicopathological features and differential diagnosis. The recent literature on the subject is also briefly reviewed.

[Initial experience with the use of oral gadolinium-DTPA in nuclear magnetic resonance tomography of the pelvis].

The use of an oral contrast medium has so far not become a matter of routine in MR of the abdomen. In the present study the use of orally applied gadolinium-DTPA was examined in respect of tumorous diseases in the minor pelvis. 18 patients with tumours in the minor pelvis were examined before and after oral administration of gadolinium-DTPA (Gd-DTPA). 10 ml/kg body weight of a gadolinium DTPA solution were applied in a concentration of 1.0 mmol/l. T1-weighted and T2-weighted sequences were carried out before application and T1-weighted sequences after application. Oral application gadolinium-DTPA resulted in enhancing the signals of the filled intestinal portions. In 54% of the cases the sequences showed a sharper delineation between tumour and intestine. In 19% the delineation between pathological tissue and intestine on contrast examination was as well defined as in T2-weighted contrast images; in 27% of the cases oral administration of gadolinium-DTPA did not yield any additional information. No significant side effects were seen.

[Combined interventions in non-organic neoplasms of the small pelvis].

Experience with 62 combined operations on patients with nonorganic new growths of the true pelvis is discussed. They accounted for 33.3% of 227 radical interventions undertaken for this disease. The ages of the patients ranged from 4 months to 64 years. Ten patients were admitted after exploratory laparotomies. Benign new growths were diagnosed in 14 and malignant new growths in 48 patients. Different variants of operative approaches were used. Most frequently the tumor was resected together with bones, iliac vessels, genitalia, and urinary bladder. Various complications were noted in 39 patients who were operated on. Hemorrhage was the most frequent and menacing complication--in 26 patients. Two patients died. Recurrences were found in 8 of a group of 60 patients discharge from the clinic. After combined operations for malignant nonorganic tumors of the true pelvis 3-year survival was 36.0%, 5-year survival was 29.4%.

[Continuous intra-arterial infusion chemotherapy for malignant bone and soft tissue tumors in pelvis and trunk areas].

We evaluated the effectiveness of intra-arterial infusion chemotherapy for 21 cases of malignant bone and soft tissue tumors in the pelvis and trunk areas. Eight cases are alive for one or two years without operation. In all cases, pain was decreased and quality of life improved. We concluded that intra-arterial infusion chemotherapy is one of the useful treatments for malignant bone and soft tissue tumors in the pelvis and trunk areas.

[Nonorganic retroperitoneal neoplasms of the pelvis].

The results of observations over 116 patients with nonorganic retroperitoneal tumors of the pelvis in the diagnosis of which computerized and ultrasonic tomography were used among other methods showed these methods to expand the possibilities of early diagnosis significantly. Analysis of 83 operations conducted on patients with nonorganic tumors of the pelvis revealed the main reasons why a radical operative intervention failed to be performed. Hemorrhage and pyo-inflammatory processes in the pelvic fat were the most common and serious complications in the postoperative period.

[Injuries of the pelvis associated with injuries to other organs].

In polytraumatized patients with fracture of the pelvis and injury to the other organs, the clinical picture is dominated by a hypovolemic shock, which is difficult to control. Surgical intervention must be quick in cases of external or internal bleeding. The importance of reaching a quick diagnosis cannot be emphasized enough, upon which the life of the patient is greatly dependent. During the seven year period 23 patients with injury to the pelvis were treated. 10 had injuries to the surrounding organs (brain commotion, injury to the urethra and vagina, as well as the lower extremities). The causes were: automobile accidents (8), falling during play (6), from height (5), during sports (4). Two injured girls are individually described (2 1/2 and 8 years old), who were surgically treated successfully. There was no mortality.

Up-to-date surgical management of tubal infertility due to inflammations of the pelvis.

A total of 87 refertilization operations in the distal tubal segment are reported. Tubal obstructions developed following inflammations of the pelvis. Hysterosalpingography and laparoscopy were used to learn about the patients' anatomy and these served as bases for assessing the changes of pregnancy following recanalization. The interventions were made by applying up-to-date microtechnical methods and instruments. Among the causes of tubal infertility, the interruption of the first pregnancy showed a marked prevalence. The 20 pregnancies that ensued correspond to a success rate of 23%. Mature deliveries occurred in 17 cases. A considerable proportion of the pregnancies occurred within a year of the operation. The effectiveness is mainly influenced by adhesions in the pelvis and the characteristics of the distal tubal segment. The surgical solution is worth attempting when the expected chances are over 20%.
