[Transport aspermia following retroperitoneal lymphadenectomy and its treatment].

After retroperitoneal lymphadenectomy disturbances of ejaculation (retrograde ejaculation, transport aspermia) are typical postoperative complications. An own case with transport aspermia is reported, in whom a retrograde ejaculation could be induced after intravenous application of the alpha-sympathomimetic drug midodrin. After masturbation the semen was collected from the bladder and the spermatozoa were inseminated. Immediately after the first instrumental insemination a pregnancy was obtained.

Influence of 1,25 dihydroxycholecalciferol on sexual dysfunction and related endocrine parameters in patiens on maintenance hemodialysis.

It has been postulated that hyperparahyroidism is in part responsible for sexual dysfunction in dialyzed patients and that this could improved by 1,25(Oh)2D3. In a single blind study patients on maintenance hemodialysis were treated after a control period with 1,25 (OH)2D3 and with placebo in random order. Sexual performance was assessed with a detailed semistructured psychiatric interview protocol in 14 and several endocrine parameters were analyzed in 15 patients. Under control conditions sexual function was disturbed in 11/14 patients. Plasma testosterone was moderately decreased in men, PRL and PTH levels were distinctly elevated in both sexes. 1,25(OH)2D3 raised serum calcium levels significantly from 9.6 to10.6 mg/100 ml ( P < 0.01) and lowered PTh from 1.39 to 0.82 ng/ml ( P < 0.01). However, no improvement in sexual function (such as libido, frequency of intercourse or masturbation) was found, and apart from a slight rise of testosterone in men and moderate fall of PRl in men and women endocrine parameters remained unchanged. It is concluded that in spite of an improvement of secondary hyperparathyroidism, treatment with 1,25(OH)2D3 for 2 to 4 months was of no value in improving sexual dysfunction in these hemodialysis patients.

Influences of culture on sexuality.

Religion is a cultured phenomenon, a subculture within our larger cultural system. Different religions have different teachings about what constitutes sexual morality, while members within a specific religious denomination may also have different beliefs and practices. Religiosity, or acceptance of the teachings of a particular religion, is more important as a determinant of sexual behavior than a specific religion per se. Orthodox Judaism, traditional Catholicism and traditional Protestantism are alike in their condemnation of masturbation, abortion, homosexuality, and premarital and extramarital coitus. More liberal members of these religions may not tolerate these activities, but may espouse them as necessary means to maintain or attain health. Nurses assess the beliefs that clients hold in regard to sexual morality and also identify if the client is experiencing guilt about past sexual practices. Interventions are planned with the client within the framework of the client's religious and spiritual beliefs and practices. To do otherwise is to invite distrust and distress in the client. Nurses intervene with sensitivity, compassion, and respect for beliefs and values that may be different from their own.

[Foreign bodies in the rectum].

Extraneous matter in the rectum is that which has become detached either accidentally or deliberately (by mental patients) and retained in the posterior part of the intestine, or that has been introduced from outside via the anus (most often deliberately: rectal masturbation). There are countless different objects introduced in this way, and they indicate an eroticism bordering sometimes on insanity. In many cases it is difficult to extract these objects, which are often large, sometimes brittle, or, more rarely, sharp, and almost always slippery. Because of this the problem of extraction has given rise to many incontestably original procedures with which it is as well to be familiar in order to avoid having to recourse to extraction via the abdomen, a procedure which is not without risks.

Sexuality and headache.

Ten percent of 362 headache sufferers reported sexual arousal during migraine attack. Clinical investigations on sexuality in 16 headache sufferers, according to some studies showing correlations between idiopathic headache and sexual behavior, were performed. Patients responding by questionnaire listed each sexual experience, headache attack, and number of sleeping hours every day for 1 month. In both men and women, the number of coiti, erotic dreams, and sleeping hours were similar in headache sufferers and controls, while the frequency of masturbation was significantly reduced in the former. Sexual excitement and fantasies appeared more often in female headache sufferers than in controls, while the opposite occurred in the male group. Among the clinical analogies between the crises of migraine and morphine abstinence, sexual arousal may be included.

Sexuality in pregnancy: what determines behavior?

A group of 155 puerperal volunteers were surveyed by anonymous questionnaire about personal and sexual histories before and during pregnancy. Results were viewed in regard to the influences of prior life experience and pregnancy itself upon attitudes, behavior, and response to sex in pregnancy. Although many differences among the population could be found the general trend of attitude, behavior, and response was to a progressive decline in coital frequency toward the end of pregnancy. Masturbation persisted or increased in inverse relationship to attitudes regarding coitus. Other past or present individual circumstances had no statistically significant effect on these trends. Poor conjugal relationships tended to deteriorate more rapidly than good ones. The author concludes that pregnancy strongly influences human behavior regardless of many individual or group characteristics. Sexually experienced and satisfied women compensate better in changed circumstances than inexperienced or sexually unfulfilled women. Rewarding sexual circumstances retard the rate of decline in interest and activity experienced by most women during late pregnancy, but do not avert this decline. Pregnancy is a potent influence over sexuality irrespective of an individual's conditioning.

A comparison of university student experience with various sexual outlets in 1974 and 1980.

This study compared 1974 and 1980 university student experience with various sexual outlets. Fifty single males and fifty single females in 1974 and 1980, randomly selected from all the single males and single females taking a sexuality course at University of Northern Iowa in those years, answered a questionnaire requiring yes or no responses to twenty-four questions on experience with various sexual outlets. Fewer students in 1980 than in 1974 had: 1) sexual experience with a person of their own sex as an adult, 2) group sex experience, and 3) sexual contact with an animal but more students in 1980 than in 1974 had stimulated a partner's genitals orally. Within each student group, more males than females had experienced 1) masturbation and 2) pre-marital intercourse with other than an intended marriage partner, while more females than males had experienced 1) anal intercourse as a passive partner and 2) being a victim of rape or attempted rape.

Sexual behaviors in retarded children and adolescents.

Literature reports on the sexual behaviors of mildly retarded adolescents are reviewed. Retarded adolescents often participate in masturbation and homosexual exploratory behavior. The retarded adolescent's heterosexual interests are of great concern to parents. The retarded adolescent is vulnerable to suggestibility, poor judgment and a failure to foresee the consequences of his actions. Parents are usually acutely distressed by the retarded youth's sexual behaviors, and they may develop an attitude that these behaviors are "bad." There is a need to provide appropriate sex education for retardates and to counsel their families about the management of sexual behaviors which occur during the adolescent years.

Sexual adjustment after lower extremity amputation.

Sexual adjustment following lower extremity amputation was evaluated by interviewing 60 adults with recent amputations, 39 men and 21 women, after they had become independent in ambulation with a prosthesis. Among the men, 77% reported a substantial decrease in the frequency of sexual intercourse following amputation, while only 38% of the women reported a decrease. The decreased frequency was greater for nonmarried men than for married men, greater for men with above-knee amputations than for men with below-knee amputations, and greater for male amputee patients in whom phantom phenomena persisted compared to those in whom phantom phenomena no longer existed. The decreased frequency of intercourse for male amputee patients was unrelated to the patient's age, education, and etiology of amputation. There was no significant change in other aspects of sexual activity, including oral-genital relations, masturbation, homosexuality, and extramarital relations.

[Inhibitions or neurosis and masculine characteristics in women. Contributions to analysis of denial].

The first part of this paper describes the general qualities of the psychical apparatus in two groups of women, male character and inhibition or neurosis. A different attitude towards the realization of the sexual difference in the phallic phase originates these two groups. The girl with a male character identifies herself with the little boy and maintains an autoerotic attitude. In the case of inhibition or neurosis the girl undervaluates herself and renounces her pleasure in masturbation. In the second part we develop a pattern of analysis, the theory of the places within the psychical apparatus: ideal, wish object, assistant and rival of the subject. The ideal is defined as warrantor for the being, for the feeling of oneself. When this ideal-cathexis happens the subject is put in the place of assistant of the ideal, in order to be recognized, to get the identification, the being. The place of rival is occupied by the one whose wishes are identical with the subject's, with whom the former has an exclusion relationship. The place of wish object appears in a heterosexual link, as the consequence of a libidinous cathexis. The distribution of these places in inhibitions or neuroses and male characters is considered in the next three parts of the paper. In inhibitions and neuroses the girl takes her mother as ideal, and her desire is to complete her, in identification with her father. The latter becomes a hostile character for the girl, and his fate is destruction, loss of meaning. The idealized link with her mother, however, progressively becomes hostile while the Ego of the girl develops. The girl then looks for shelter in worthless and multiplied men, and afterwords in a single man, idealized. These characteristics already belong to male characters. An analysis of the metapsychology of denial (Verleugnug), main defense mechanism against the registry of a traumatic perception, is offered in the next part. This defense preassumes an equivalent of an action aimed at putting something in the place of the traumatic registry, therefore maintaining the idealization of somebody as ideal, warrantor for the being. This idealization is questioned by an existence and value judgement. A link is then established between the Ego and three positions: the ideal, whose idealization the Ego tries to keep (love link); a hostile ideal, who assures the traumatic perception (hostile link) and an assistant of the hostile ideal (link of denounciation and disqualification). Denial implies the splitting of the Ego together with the existence of two preconscious organizations with different rules. One of these systems is connected only with primary identifications and the other also includes object-cathexes. In the last item we study how denial works in inhibitions and neuroses and in male characters. The girl may deny as regards either her own body (male characters) or her father's (inhibitions and neuroses). What really happens in both groups is a double denial, and the real definitive Ego, the reflexive judgement, is left out...

[Cerebral dysfunction and disorders of sexual behaviour (author's transl)].

The effects of epilepsy and focal brain lesions on sexual behaviour are described. Fits may be provoked by hypervenilation in epileptics during sexual activity, but also by orgasm and other sexual stimuli. Epileptic auras consisting of tactile genital sensations and sexual feelings, including orgasm, also occur. Post-ictal sexual automatisms consist of masturbation or other poorly structured activity, imitating the gestures of sexual intercourse. Focal brain lesions most commonly produce a reduction in sexual activity. However, frontal lobe lesions may produce disinhibition with inappropriate sexual behaviour and temporal lobe lesions may be associated with true hypersexuality, transvestite and transsexual behaviour. Eleven cases, which illustrate the various associations, are described.

Psychosexual characteristics of male university students in Brazil.

Male freshmen (N = 268) at the State University of Campinas (UNICAMP) in the state of Sao Paulo in Brazil responded to a questionnaire concerning aspects of their sexuality from the onset of adolescence. Topics such as physical changes, first ejaculation, masturbation, homosexual manifestations, and sexual practices were investigated, as well as present attitudes toward virginity, intercourse, birth control, abortion, and prevention of AIDS. The quality of the relationship established with parents and the possibilities for dialogue about sex in the family circle were also surveyed. The majority of the subjects revealed satisfactory development in most areas of their sexuality, as well as a good relationship with their parents. Nevertheless, great difficulty was found in dialogue about sex within the family circle. Of these young men, 32% were still virgins at the end of their freshman year, but a considerable number of those already sexually active reported behavior which was inappropriate for the prevention of pregnancy and AIDS. The results are analyzed using psychoanalytic theories of psychosexual development.

AIDS related attitudes and sexual practices of the Jakarta WARIA (male transvestites).

As part of a community based educational campaign to convey the risk of HIV infection and AIDS to commercial sex workers in Jakarta, over 600 male transvestites (WARIA) were questioned about their sexual behavior patterns and their knowledge and attitude towards HIV infection and AIDS. Most expressed a genuine fear about AIDS, but they stated they did not have sufficient information to determine if their risk of infection, at this time, was great enough to command a change in their sexual conduct. Among the group who felt they were not at risk for HIV infection, 40% had 8 or more different sex partners per week. Low risk receptive oral sex, thigh massage (simulated vaginal sex) and masturbation of the client were routine activities but high risk receptive anal sex without condoms was most common. Despite such high risk behavior, the WARIA community remains free of HIV infections. However, if they refuse to accept the endorsement of their peer leaders and they fail to alter their sexual behavior because of an unawareness of their risk, that status will change soon.

[Occurrence of deviant tendencies in phalloplethysmographic and vulvoplethysmographic examinations in sexually dysfunctional patients (case reports)].

The author demonstrates on four case-histories the presence of sexual deviant trends as the cause of sexual dysfunctions in conventional heterosexual relations in men and women and the irreplaceable part played by vulvo- or phalloplethysmography used for their detection. In a man treated for some time on account of impaired erectile potency and in a woman who visited the surgery with her husband and presented complaints diagnosed as secondary frigidity and anorgasmy, PPG or VPG examination revealed a homosexual orientation. Both these patients knew of their homosexual orientation but did not tell anybody. The mentioned male patient intentionally dissimulated homosexuality, the woman attempted in vain to achieve heterosexual adaptation. In two young men with primary disorders of erectile potency PPG examination revealed sadomasochistic tendencies. In the first one the impaired erectility created the impression of "debutant's failure" when starting a partnership, in the second one it seemed to be due to adverse psychosocial factors. During an interview following the examination both described spontaneously and in detail erotic dreams and masturbation fantasies with sadomasochistic contents. They were not aware of the association of their specific sexual motivation structure. It was typical for all four patients that they never disclosed previously their deviant sexual tendencies and only the specific situation during the PPG or VPG examination, which they had voluntarily after previous instruction, they were motivated to confess their problems.

[Sexuality in Ancient Egypt].

The present article explores the sexuality in ancient Egypt. In particular in this article are presented the ways of concubinage (marriage, concubinage, adultery), the incest, loves of the pharaohs and of the common people, the freedom of choice in garments, the status of the hetairas and of the whores, the sexual perversions (male and female homosexuality, necrophilia, sodomism, bestiality, rape, masturbation, exhibitionism), the operations of the genitals (circumcision, excision, castration) and finally the level of knowledge in gynaecology, fertility, contraception and obstetrics that even today demands our admiration.

Preliminary observations of DSM-III-R axis I comorbidity in men with paraphilias and paraphilia-related disorders.

BACKGROUND: We describe a comorbidity study of DSM-III-R-defined Axis I diagnoses comparing male outpatient paraphiliacs to men with nonparaphilic forms of sexual impulsivity designated as paraphilia-related disorders. METHOD: Data were prospectively collected from 60 consecutively evaluated outpatient males, aged 21-53 years, seeking treatment for the principal disorders of paraphilias (N = 34) and/or paraphilia-related disorders (N = 26). Subjects completed a semistructured psychiatric Intake Questionnaire and Sexual Inventory. The lifetime prevalence of DSM-III-R Axis I diagnoses, including disorders of sexual impulsivity, was assigned by follow-up psychiatric interviews. RESULTS: Both groups of men were diagnosed with an elevated lifetime prevalence of mood disorders (76.7%), especially early-onset dysthymia (53.3%); psychoactive substance abuse (46.7%), especially alcohol abuse (40.0%); and anxiety disorders (46.7%), especially social phobia (31.6%). The predominant forms of sexual impulsivity reported by both groups were "nonparaphilic" paraphilia-related disorders: compulsive masturbation (73.3%), protracted promiscuity (70.0%), and dependence on pornography (53.3%). CONCLUSION: There were no major differences in lifetime Axis I diagnoses to differentiate men with paraphilic disorders from those with paraphilia-related disorders. Both groups were likely to acknowledge multiple paraphilias and/or multiple paraphilia-related disorders suggesting that sexual impulsivity has diverse manifestations, which can include culturally "deviant" as well as "normative" behaviors. Several hypotheses regarding the possible etiologic relationship between depressive disorders and sexual impulsivity are suggested.

Psychosexual characteristics of female university students in Brazil.

Freshmen women (N = 240) at the State University of Campinas (UNICAMP) in the state of Sao Paulo, Brazil responded to a questionnaire concerning several aspects of their sexuality from the beginning of adolescence. Topics such as body changes during puberty, menarche, menstruation, masturbation, sexual identity, virginity, sexual relations, maternity, contraceptives, abortion, and attitudes toward AIDS were investigated, as well as the level and sources of information about sex and the quality of the subjects' relationships with their parents. The majority of the subjects revealed satisfactory development in most of the aspects surveyed, as well as in their relationships with their parents except where sexual issues were concerned. Nevertheless, most of the students reported menstrual disturbances, and a significant number of the sexually active reported inappropriate behavior in relation to contraception and AIDS prevention. The results of the research are analyzed using psychodynamic theories of sexual development.
