Treatment planning for dental implants: considerations, indications, and contraindications.

During the last 10 years the initial concept of osseointegrated implant dentistry as essentially treatment of the heavily resorbed edentulous mandible in older patients has expanded to encompass almost every combination of prosthetic problem. Many factors must be taken into account when planning treatment to ensure a flexible approach which is necessary given the uncertainty in predicting the outcome of difficult cases. The complex, and therefore inherently costly, nature of implant dentistry will inevitably lead the patient to have very high expectations of treatment. It is extremely important for the operator to determine these expectations at an early stage, and if there is any doubt about being able to satisfy them then he or she may be wiser not to proceed (as in all other areas of dentistry, skillful case selection is the key to success). In any event, the patient must be fully appraised of the complications, the potential risks and the benefits of treatment so that they can proceed on the basis of informed consent.

[Is Letigen contraindicated in hypertension? A double-blind, placebo controlled multipractice study of Letigen administered to normotensive and adequately treated patients with hypersensitivity].

The aim of this study was to study how the slimming drug, Letigen, containing ephedrine (E) 20 mg and caffeine (C) 200 mg (E + C) affects blood pressure in normotensive and hypertensive patients treated with adrenergic beta-receptor blocking drugs and/or other antihypertensive agents, during a period of six weeks. We followed 136 patients in a double blind, randomised, placebo controlled study of five parallel groups of overweight patients from general practices. The systolic blood pressure was reduced significantly (5.5 mmHg) in the patients treated with antihypertensive agents other than betablockers, plus E + C. In normotensive patients treated with E + C the systolic and diastolic blood pressure declined significantly (4.4/3.9 mmHg). A mean loss of weight of 4 kg over six weeks was significant for all groups. This study does not support the assumption that E + C should cause rises in blood pressure in either normotensive or hypertensive obese patients. The antihypertensive effect of the betablockers was not reversed by E + C.

[Colonoscopy: indications, contraindications and complications].

The bioengeneering technological development associated with new medical information transformed colonscopy today in one of the most complete method of investigation on colorrectal diseases. The authors aim to describe the use of colonoscopy as a diagnostic and therapeutic method, its main indications and limitations. The wide utilization of colonoscopy as therapy is emphasized in the polipectomy, the identification of bleeding areas and the hemostasy. Some intervening facts are discussed as copious bleeding, perfuration and post polipectomy syndrome. Finally, the authors report the low morbidity and mortality rates in their personal experience with colonoscopy during the last decade.

Is acute cholecystitis a contraindication for laparoscopic cholecystectomy?

Two-hundred and ninety-four patients underwent laparoscopic cholecystectomy between May 1993 and April 1997 in the Clinical and Surgical Gastrointestinal Unit at Santa Casa Hospital of Porto Alegre. Of all, 47 were operated on for acute cholecystitis (AC). The mean age was 47.1 years, 70.2% were females and 29.8% were males. The mean operative time was 142.8 minutes. Routine intraoperative cholangiography was successfully done in 38 (80.8%) and conversion was necessary in 3 (6.4%). All patients received prophylactic antibiotics for 24 hours. No intraoperative or postoperative complications were detected. The results in this series showed that laparoscopic cholecystectomy is a possible and safe technique for the treatment of the AC.

Simultaneous rhinoplasty and maxillomandibular osteotomies: Indications and contraindications.

Functional and esthetic correction of a dentofacial deformity may require not only maxillary or mandibular osteotomies but also a rhinoplasty. Rigid internal fixation makes it possible to perform rhinoplasty and maxillary osteotomies simultaneously. Nevertheless, to plan rhinoplasty correctly it is of paramount importance to predict changes of the nose that will follow osteotomy of the maxilla. The authors present their experience concerning the surgical technique, advantages, and disadvantages of rhinoplasty in combination with orthognathic surgery.
