Muscle involvement in association with filarial chyluria.

Twenty unselected consecutive patients of filarial chyluria were evaluated clinically, electromyographically, and histopathologically for neuromuscular dysfunction. None of the patients showed clinical evidence of muscle wasting or weakness suggesting myopathy, although generalized muscle weakness was complained by all of them particularly while climbing the stairs or on getting up or during lifting heavy weights. Electromyographic abnormalities were found in nine patients and slight histopathological abnormalities in two. The average duration of motor unit potentials and the mean amplitude were reduced, compatible with myopathy. There were no fibrillation potentials. Histological abnormalities consisted of a marked variation in muscle fibre size, sarcolemmal nuclear proliferation, and mild interstitial fatty infiltration. None of the patients showed evidence of clinical neuropathy, abnormalities in conduction velocity along the nerves or signs of segmental demyelination in the biopsy of the nerve. Our data suggest that muscle weakness in these patients is myopathic in nature without peripheral nerve involvement, and is possible due to hypoproteinaemia and hypolipidaemia, as these patients lose excessive amounts of protein and fat in their urine.

Hematogenous dissemination of ingested polyvinyl chloride particles.

Persorption of large, hard, insoluble, solid particles in micrometer size range is observed with regularity. It is a normal biological effect on the food passage through the intestine. Persorbability of particles is determined by the size of the particles and by their consistency and hardness. PVC particles in micrometer size range are very suitable as models to study persorbability in animals and the hematogenous dissemination of PVC particles. After oral administration of PVC particles to animals, many PVC particles can be found in the blood. Through the bloodsteam they are disseminated in all organs, where individual particles can be found for a long time later.

Traumatic chylous knee effusion.

A 47-year-old patient presented with a chylous knee effusion and traumatic infected skin lacerations. The diagnosis of septic arthritis was considered because of purulent looking joint fluid, spuriously high synovial fluid white cell count, and signs of acute knee synovitis. Synovial fluid lipid analysis showed increased total lipids due to high concentration of neutral lipids, mainly triglycerides, and repeated knee radiographs showed a small fracture of the tibial spine. Joint trauma with subchondral fracture can be associated with frank chylous effusion, which may mimic acute septic arthritis.

Lymphatic abnormalities in Alagille's syndrome.

Chylous pleural effusions developed in a patient with Alagille's syndrome who had dysplasia of the lymphatic system. Lymphatic abnormalities are not a recognised feature of Alagille's syndrome.

Chylothorax after high translumbar aortography.

Two patients who had high translumbar aortograms for suspected arterial disease developed shortness of breath 48 hours after aortography and were found to have chylothoraces. Conservative management was initially adopted for both, but surgery had to be resorted to, although the first patient died before arriving in the operating theatre. The literature and the management of this condition are reviewed.

Chylopericardium as a complication of mitral valve replacement.

A patient developed chylopericardium after mitral valve replacement. Although there was early relapse with cardiac tamponade it eventually resolved with conservative management consisting of tube drainage initially and a medium chain triglyceride diet for two months.

Life threatening chylous pleural and pericardial effusion in a patient with Behçet's syndrome.

Chylothorax and chylopericardium secondary to thrombosis of the superior vena cava and the innominate and subclavian veins were diagnosed in a patient with Behçet's syndrome. Immunosuppressive treatment, diet, and underwater seal drainage led to a diminished volume of pleural fluid and pericardial fluid and to a diminished concentration of triglyceride in them; pleurodesis with tetracycline was then performed.
