[Iodized polyvidone (betadine) and prevention of postoperative infection. A multicenter study].

A multicentre study of a new preparation of povidone iodine (5% betadine) was carried out on 150 patients undergoing cataract surgery. Pre-operatively the eye was prepared by daubing the peri-ocular skin and a two-minute instillation into the conjunctival fornices. No other antiseptic was applied. Tolerance to the product was evaluated in all the patients: clinical evaluation postoperatively and during a 60-day follow-up period confirmed that the preparation was harmless to the eye. Microbiological investigations (comprising conjunctival swabbing, bacterial culture and count of the bacterial species isolated, before and after application of the product) performed on 60 patients showed a significant fall in the number of colonies after treatment.

Effect of extremely low frequency of electromagnetic fields on cell proliferation, antioxidative enzyme activities and lipid peroxidation in 3T3-L1 preadipocytes--an in vitro study.

The exposure to extremely low frequency electromagnetic field (ELF-MF, frequencies less than 200-300 Hz) can alter the transcription and translation of genes, influence the cell proliferation rate and affect enzyme activities. Moreover, the hypothesis that ELF-MF increases free oxygen metabolites generation has been proposed. Since recent in vivo studies suggest that electric and magnetic fields are able to affect adipose cells metabolism. The aim of the study was to examine the effects of ELF-MF (frequency of basic impulse 180-195 Hz, induction 120 microT) on cell proliferation, antioxidative enzyme activities and malondialdehyde (MDA) concentration in 3T3-L1 preadipocyte cell culture. We found that ELF-MF application lasting 36 minutes daily failed to influence cell count after 24h and 48 h of incubation. After 24 h, in the ELF-MF treated group, manganese- and copper-zinc-containing superoxide dismutase (MnSOD and Cu/ZnSOD) isoenzymes media activities were decreased, catalase activity was increased, whereas there were no significant differences in glutathione peroxidase (GSH-Px) and glutathione reductase (GSSG-Rd) activities in comparison to the control. After 48 h of incubation, all enzyme activities were reduced, except for GSSG-Rd, in which no changes were noticed. MDA concentration at 24 h after incubation with the exposure to ELF-MF was significantly higher in comparison to the control, without ELF-MF. After 48 h of incubation, MDA levels were significantly lower in both groups with no differences between the groups without and with ELF-MF. We conclude that ELF-MF influences antioxidative enzyme activities and increases lipid peroxidation in 3T3-L1 preadipocyte cultures.

Lack of specificity for detection of breast lesions with radioactive cesium chloride.

Detection of breast disease at an early stage was evaluated with 131CsCl. The tissue distribution study showed that malignant tumors could be distinguished from both benign and normal tissue by the degree of 131Cs concentration. However, the external counting method was not sensitive enough to warrant general application to patients with a suspected malignant tumor of the breast. False-positive breast study findings using radionuclides, such as 131CsCl, require close follow-up study in view of possible malignant changes in benign disease of the breast.

Evaluation of hand applied naled thermal fog for Wyeomyia control.

Tests on the effect of hand applied naled thermal fog, both as a single treatment on one day/week and a single treatment on 3 successive days, did not control Wyeomyia vanduzeei and Wy. mitchellii. Five-min landing/biting counts in a native oak/palm woodland demonstrated that single applications produced an average landing rate decrease of 13%. Treatments 3 days in succession did not suppress the landing rate.

[Development and evaluation of an instrument to assess the psychosocial environment].

It is an instrument to explore the psico-social environment that carries out a thorough inventory of the elements in such environment, it groups them in general categories that, from the process of stress (taken as the point of the departure), are the following: stressing situations, psicosocial support and facing conducts. And they are expressed in four levels or spheres of action: society, family, occupation and individual. It was intended to satisfy the need to count on relatively simple material, self-managable, widely applicable, standardized and that describes in an integral way the environment. The validity of contents and concepts was estimated through the Delphos technique. Though the concurrent validity is established by its application with another similar instrument, when this late was not available, some scales that could work as standards were use. The t Student test was used to evaluate the discrimination capacity among subgroups. In this, statistical differences were noticed for the same items with a significant value of 0.01. It was observed that the older they were there was an increasing set of stressing situations in the society and occupation spheres (correlation quotient of 0.80). According to sex, there were more stressing family situations in women. According to income, it was found that the bigger number of stressing situations and fewer psicosocial supports pertained to a higher income (correlation quotient of 0.91). It is deduced that the instrument, though it has some limitations, can be widely used. The only requirement is that it should be applied to individuals over 18 years old, of any sex and socio-economical class, and they should be literate.

Circadian variation in mitotic influx in a keratinized epithelium. The stathmokinetic technique used in vivo on the hamster cheek pouch epithelium and analyzed by periodic regression analysis.

An in vivo study of the hamster cheek pouch epithelium using the stathmokinetic technique (Colcemid) demonstrated a circadian variation in mitotic influx. Based on measurements of all nucleated epithelial cells the diurnal mean was estimated in two separate experiments as 0.34%/h +/- 0.02 (SE) and 0.27%/h +/- 0.02 (SE) respectively. 3HTdR was injected in the latter study (a double labelling experiment). The significant difference between the two experiments is, however, probably due to biological variations. The maximal values for the mitotic rate were found during the light (resting) period, as were the maximal values for the mitotic index. The mean mitotic influx for the 'light period' was estimated as 0.5-0.4%/h, and for the 'dark period' as 0.2%/h. Independent analyses demonstrated the necessity of a circadian-dependent correction of the 1 and 4 h values of accumulated metaphases. The 1 h value was significantly too high during the light as well as the dark period. The 4 h value was found to be too low, but only significantly so during the dark period. Basing the estimation of mitotic rate on the 3 h accumulation value produced only very similar results to those found by using all four accumulation periods. The use of overlapping experiments proved that only cells entering mitosis after Colcemid application were arrested, so that when arrested metaphases were counted the accumulation line was correctly drawn through the origin. In the latter study (the double labelling experiment) both S- (Møller and Keiding 1982) and mitotic influx were estimated, the estimates being 0.55%/h +/- 0.03 (SE) and 0.27%/h +/- 0.02 (SE) respectively. Even considering possible methodological problems, the discrepancy between the S efflux and the mitotic influx indicates cell death and/or differentiation from G2.

Controlled and clinical evaluations of the anthelmintic activity of a levamisole pour-on formulation against gastrointestinal nematodes in cattle.

Beef-type calves (n = 107) with naturally occurring helminth infections were used in a series of 3 controlled experiments to study the anthelmintic activity of a new pour-on formulation of levamisole. In the 1st experiment, the efficacy of a single treatment of levamisole pour-on at a rate of 10 mg/kg of body weight was determined, using a placebo pour-on in a similar volume as a control. In the 2nd experiment, calves were treated with levamisole pour-on at doses 0 (nontreated control), 7.5, 10, or 12.5 mg/kg of body weight. In the 3rd experiment, the anthelmintic efficacy of the optimal dose of levamisole pour-on (10 mg/kg of body weight) was compared with that of injectable levamisole at the recommended rate (6 mg/kg of body weight). In all 3 studies, levamisole pour-on, at a dosage of 10 mg/kg of body weight, was efficacious (90% to 100%) against the adult forms of Trichostrongylus axei, Haemonchus placei, Ostertagia ostertagi, Cooperia spp, Oesophagostomum radiatum, Nematodirus spp, and Bunostomum phlebotomum. Efficacy was moderate to good against Capillaria bovis, and poor or inconsistent against Strongyloides papillosus and Trichuris ovis. Clinical tests of anthelmintic efficacy and local tolerance of levamisole pour-on were conducted in 371 beef and dairy cattle in Mexico. Fecal egg counts were consistently reduced 2 weeks after a single application of levamisole pour-on at a dosage of 10 mg/kg of body weight. Local reactions were limited to various degrees of dry hair coat for up to 2 weeks at the application site. Hair loss or skin abnormalities were not observed in any of the treated cattle.

[The reaction of calves to parenteral administration of cyclopheosphamide].

One single injection of 50 mg/kg live weight cyclophosphamide or more to calves was followed by a latency of few days after which pneumonia or enteritis developed and caused death within one or two weeks. Cyclophosphamide application led to pronounced changes in the white blood count characterised, at the beginning, by rise of neutrophilic granulocytes and decline of lymphocates. Primary rise of granulocytes then was followed by almost complete disappearance of them. In those calves that survived the parameters of the white blood count were not restoredto normal until several weeks had elapsed. While an injection of 30 mg/kgcyclophosphamide usually was tolerated without any visible clinical reaction, it was also accompanied by the above pronounced changes in the white blood count. The activity of the reticulohistiocytary system, as recordable by means of ink and bacterial clearance, was not markedly affected by one single injection of 30mg/kg or 40 mg/kg cyclophosphamide. Calves with cyclophosphamide treatment exhibited unambigous humoral immune response, yet somewhat delayed or reduced in comparison to controls. While one single intravenous injectionof 30 mg/kg cyclophosphamide alone failed to trigger any clinical disease, it proved to be capable of rendering calves more susceptible to pneumonia pathogens. It, therefore, might be suitable for supporting experimental infection and thus facilitating the study of enzootic pneumonia of calf which usually is difficult to reproduce.

[Effect of acetylsalicylic acid on platelet aggregation and capillary bleeding in healthy cats].

The influence of acetylsalicylic acid (ASA) in vitro and in vivo on several parameters of the hemostatic system was evaluated in healthy cats. The aggregation response of cat platelets in vitro was not influenced by 10 micrograms ASA/ml plasma, whereas ASA concentrations of 35, 70, 100 micrograms/ml and of 1 mg/ml consistently inhibited the platelet aggregation induced by 0.1 (n = 4 cats) and 0.25 micrograms collagen/ml (n = 6 cats). The aggregation response to ADP and thrombin was partially (n = 2) or completely (n = 8) inhibited by an ASA concentration of 5 mg/ml. Administration of ASA at a dosage of 10 and 25 mg/kg BW orally and intravenously yielded a plasma concentration of salicylic acid of 30-42 micrograms/ml (10 mg/kg BW) and 50-70 micrograms/ml (25 mg/kg BW). All the dosages of ASA produced an inconsistent inhibition of the collagen-induced platelet aggregation (1/5 and 2/5 for the respective groups), whereas the response to ADP and thrombin was not altered. Capillary bleeding time was not significantly altered after the ASA administration. Platelet count and screening tests were not influenced by ASA application. According to the results of this study, ASA as a platelet aggregation inhibitor is not useful for the prophylaxis of thromboembolism in cats.

[Treatment of multiple myeloma with high-dose chemotherapy and transplantation of autologous hematopoietic stem cells and subsequent maintenance therapy with interferon alfa-2b or interferon alfa 2b and dexamethasone. Report of the ongoing study of the "4W" Czech Myeloma Group].

We report our results with high-dose chemotherapy in previously untreated multiple myeloma patients (4 courses of VAD chemotherapy, collection of PBSC after priming with cyclophosphamide, 5 g/m2, high-dose chemotherapy with melphalan, 200 mg/m2). Second transplantation was indicated only for patients who did not achieve remission after the first high-dose therapy (paraprotein lower than 25% of the pretreatment value). For the second transplantation melphalan (200 mg/m2) with methylprednisolone (1.5 g for 5 days) were used as conditioning regimen. After high-dose therapy all patients were randomized into two arms of maintenance therapy: interferon alpha-2b or sequential maintenance therapy (interferon alpha-2b for 3 months followed after 4 week pause by 40 mg of dexamethasone days 1-4, 10-13 and 20-23. The administration of interferon alpha was resumed four weeks after the last dexamethasone for next three months. The maintenance therapy continued for 48 months or until the progression. Fifty-five patients were enrolled in the study from January 1996 to August 1997. Thirty-five patients have undergone the first transplantation and 57% of them reached complete remission. There were 10% of non-responders after the first high-dose regimen. The mean time to reach white blood cell count above 1 x 10(9)/L after the application of high dose melphalan and platelets more than 50 x 10(9)/L were 12.2 (range 6-16 days) and 12.4 (range 0-25 days), respectively. Grade 4 mucositis according to SWOG classification requiring total parenteral nutrition was presented in 40% of the patients. The mean number of 1 unit of platelets and 2 units of packed red blood cells transfusions were given within the posttransplant period. Early transplant related mortality was 3%. This paper describes the response and tolerance of each particular step of therapy. The follow-up has been too short to evaluate event-free and overall survivals.

Sodium hypochlorite as a tissue solubilizing agent compatible with liquid scintillation counting.

The use of hypochlorite to prepare biological samples, containing incorporated radiolabeled molecules, for liquid scintillation counting was investigated. Hypochlorite both solubilized and decolorized a variety of animal tissues, but fat aggregates, cartilagenous tissue, and plant material resisted digestion. Although hypochlorite induced chemiluminescence, addition of acidified stannous chloride to the counting cocktail alleviated the spurious counts. The reported procedure, modified to suit particular experiments, may be applicable in a variety of situations where quantitation of biologically incorporated radiolabeled molecules is desired.

Application of triphenyltetrazolium chloride in microbial limit test of pharmaceuticals and cosmetics.

An alternative method was developed for counting viable microbial contaminants in pharmaceuticals and cosmetics with insoluble materials and low bioburden. The method uses triphenyltetrazolium chloride (TTC). The inhibitory effect of TTC on microbial growth observed by others was eliminated by adding it as an overlay, after incubation of sample in culture medium for 48 h. Six samples of pharmaceutical suspensions and 6 samples of cosmetics were evaluated by most-probable number (MPN) and pour-plate techniques with TTC in comparison with MPN with subculturing. The equivalence between MPN technique with TTC and subculturing was demonstrated for all samples and between pour-plate and subculturing for 10 samples. The differences were probably due to some error inherent in both techniques and not from application of TTC. Comparison of coefficients of variation showed that plate countings were more precise than the MPN method, as expected.

Application of non-radioactive BACTEC system to sterility testing and hygiene.

The applicability of the non-radioactive BACTEC NR 660 system for the detection of low viable counts of aerobic and anaerobic bacteria (including non-fermenters and aerobic spore-forming bacteria) and fungi was investigated. The BACTEC system together with the usual BACTEC NR 6A and NR 7A media was able to detect most of the organisms without supplementation of the media; for growth of lactobacilli and other acidophilic organisms, sterile orange juice was used as a supplement. Microbial growth was detected from pure cultures and also from artificially contaminated beverages and ointment bases. Detection times varied according to the types of metabolism of the respective strains and their initial viable counts, but usually they were detected after 20 to 42 h of incubation. The BACTEC NR 660 system seems to be applicable not only for blood cultures in medical microbiology but also in pharmaceutical and food industry.

Recurrent injury event-time analysis.

Public health decision making based on data sources that are characterized by a lack of independence and other complicating factors requires the development of innovative statistical techniques. Studies of injuries in occupational cohorts require methods to account for recurrent injuries to workers over time and the temporary removal of workers from the 'risk set' while recuperating. In this study, the times until injury events are modelled in an occupational cohort of employees in a large power utility company where employees are susceptible to recurrent events. The injury history over a ten-year period is used to compare the hazards of specific jobs, adjusted for age when first hired, and race/ethnicity differences. Subject-specific random effects and multiple event-times are accommodated through the application of frailty models which characterize the dependence of recurrent events over time. The counting process formulation of the proportional hazards regression model is used to estimate the effects of covariates for subjects with discontinuous intervals of risk. In this application, subjects are not at risk of injury during recovery periods or other illness, changes in jobs, or other reasons. Previous applications of proportional hazards regression in frailty models have not needed to account for the changing composition of the risk set which is required to adequately model occupational injury data. Criteria of applicability for autoradiography of tritium.

Autoradiography is an effective tool for the imaging of radionuclide distributions in various samples. In sophisticated applications with special preparation and development of sample-emulsion combinations and subsequent grain counts it can be highly quantitative, but it requires carefully controlled conditions and a variety of counter-checks, for example through scintillation spectroscopy. Less refined applications use X-ray films as detectors, and their seeming simplicity tends to invite artefacts and misinterpretations. Particular care needs to be taken, if one deals, or presumes to deal, with the low-energy beta-emitter tritium. Because of the short electron ranges the film must be in intimate contact with the sample, which tends to produce chemographic artefacts; without added spectroscopic measurements it is impossible to discriminate the spurious signals from a blackening of the film due to tritium. Recent statements concerning autoradiographic tritium measurements in tree samples have created considerable public concern and have demonstrated the pitfalls of uncritical use. This paper presents order-of-magnitude criteria for the detection threshold in the autoradiography of tritium; they can serve as an exclusion principle for some of the more extravagant misinterpretations.

Microdroplet fluorochromatic assay for the enumeration of white cells (WBCs) in WBC-reduced blood components: validation and application for evaluating newly developed WBC-reduction filters.

BACKGROUND: Sensitive and accurate counting methods are required to assess the residual white cells (WBCs) in WBC-reduced blood components. The Nageotte hemocytometer, widely used for this purpose, is cumbersome, and its efficacy is dependent upon the skill of the operator. The performance of a simple fluorochromatic assay using tissue-typing microdroplet trays is presented here. STUDY DESIGN AND METHODS: Undiluted samples of blood components were mixed with a fluorochromatic dye in trays. WBCs were counted under an epifluorescence microscope. The accuracy and sensitivity of this method were compared with those of the reference Nageotte hemocytometer method by using serial dilution of samples of platelets and red cells containing known concentrations of WBCs and by calculating the standard curves. The Nageotte hemocytometer and the microdroplet fluorochromatic assay (MFA) were also compared in terms of count correlation and reproducibility in 320 paired counts of plateletpheresis samples. MFA was used to evaluate a newly developed WBC-reduction red cell filter. RESULTS: The MFA for platelets and red cells was linear to 0.1 and 0.03 WBCs per microL, respectively. The linear regression line of log10 MFA versus log10 Nageotte method had a slope of 0.963, intercept of -0.04, and r2 of 0.968. The Nageotte method gave an estimation of WBC content 12 to 20 percent greater than that of the MFA. The MFA, with a larger neat sample volume, showed precision comparable to that of the Nageotte method. The filters demonstrated a median WBC reduction of 4.8 log10. CONCLUSION: The MFA is a sensitive and accurate method for quality control processes to assess the residual WBCs in WBC-reduced blood components.

Intensity discrimination as the driving force for loudness. Application to pure tones in quiet.

Loudness functions and their associated neural-count functions are derived for steady-state tones at 250 and 1000 Hz from measurements of intensity discrimination obtained under gated and continuous conditions. The calculations are based on a multichannel generalization of the McGill-Goldberg counting model [J. Acoust. Soc. Am. 44, 576-581 (1968)]. Using the data for just noticeable differences (jnd) in intensity as input, the generalized version gives an integral relation between the neural-count function N(x) and the intensity-jnd function, where x = I/I0 and I0 is the reference intensity. Loudness functions are generated through the prescription L(x) = AN(x)--B. To determine how the detailed shapes of the intensity-jnd functions affect the form of the loudness function within the model, integration was performed over the intensity-jnd functions with and without a power-function approximation. Over a range of intensity levels from 20-95 dB, the best agreement between the calculated and measured loudness functions is obtained from the unaltered intensity-jnd functions. Consistent with psychophysical evidence and several models of intensity coding, the results predict that the output of the whole auditory nerve is unnecessary to maintain the large dynamic range observed for loudness and intensity discrimination.
