GH release after GHRH plus arginine administration in obese and overweight women with polycystic ovary syndrome.

Few and unclear data are available in the literature about the relationship between impairment of GH/IGF-I axis and polycystic ovary syndrome (PCOS). This study was aimed to evaluate the basal GH and IGF- levels, and GH release after challenge test in obese and overweight women with PCOS. Thirty patients with PCOS and other 30 healthy women matched for age, body mass index (BMI) and waist-hip ratio (WHR) were studied. Serum follicle-stimulating hormone (FSH), LH, PRL, E2, P, 17OH-progesterone (17OH-P), total T, delta4, DHEA-S, SHBG, GH and IGF-I levels were evaluated in each subject. A GHRH plus arginine challenge test was performed in all subjects. After provocative test, in PCOS and control women the GH levels were significantly (p<0.05) higher in comparison to basal values from 30 min to 120 min. At the same times, a significant (p<0.05) difference was observed between women with PCOS in comparison to healthy women. The mean peak value of GH resulted significantly (p<0.05) lower in PCOS women in comparison to healthy women. The total GH response (area under curve, AUC) to GHRH plus arginine test resulted significantly (p<0.05) lower in PCOS than in healthy women. These findings were statistically significant (p<0.05) also considering the distinction in obese and overweight women. The AUC for GH secretion was significantly lower (p<0.05) in obese in comparison to overweight subjects in the control group, whereas no significant difference was detected between obese and overweight women in the PCOS group. In conclusion, in PCOS women there is a BMI-independent alteration of the GH levels. Further investigations will be necessary to establish the real cause of these data.

Age, overweight and physical inactivity increase the risk of latent autoimmune diabetes in adults: results from the Nord-Trøndelag health study.

AIMS/HYPOTHESIS: Latent autoimmune diabetes (LADA) is a common form of diabetes, yet the risk factors are poorly characterised. The aim of this study was to investigate the influence of age, overweight and physical activity on the risk of LADA. METHODS: We analysed age, overweight and physical inactivity and the incidence of LADA in 38,800 men and women, observed between 1984 and 1986 and 1995 and 1997 as part of the Nord-Trøndelag Health Survey. We also compared such factors with incident cases of type 2 (n = 738) and 'classic' type 1 diabetes (n = 18). Patients classified as LADA (n = 81) had antibodies against GAD and were insulin independent at diagnosis. RESULTS: The proportion of those who were older, overweight and inactive before diagnosis was almost identical in LADA and type 2 diabetes patients. BMI >or=30 kg/m(2) was strongly associated with LADA incidence (relative risk [RR] = 15.0, 95% CI 7.51-29.97). The association was similar for type 2 diabetes (RR = 15.37, 95% CI 12.07-19.57) but not for type 1 diabetes. Similarly, age (>or=60 years) was an important risk factor for LADA (RR = 5.62, 95% CI 2.36-13.4) as well as for type 2 diabetes (RR = 6.78, 95% CI 5.07-9.06) in contrast to type 1 diabetes. Physical inactivity was associated with an increased risk of both LADA and type 2 diabetes. CONCLUSIONS/INTERPRETATION: This study suggests that increased age, overweight and physical inactivity are as strong risk factors for LADA as for type 2 diabetes. These findings suggest a role for insulin resistance in the pathogenesis of LADA.

Total knee arthroplasty without patellar resurfacing in active and overweight patients.

Overweight patients are often considered poor candidates for total knee arthroplasty (TKA). A retrospective study of this was done on 47 osteoarthritic knees treated by TKA without patella resurfacing between March 1991 and June 1993. The Hospital for Special Surgery (HSS) rating system was used for clinical evaluation, and radiographs to study the degree of osteoarthritis and radiolucency. Correlations between overweight, range of motion (ROM) and stage of patellar damage and other measured variables (HSS score, patellar pain and radiolucency) were studied. Overweight was not correlated with HSS score, radiolucency or patellar pain. ROM was significantly correlated with patellar pain and HSS score, with better results in patients with ROM between 90 degrees and 110 degrees. Therefore, we believe that TKA in osteoarthritic knees can lead to successful results, even in active or overweight patients.

Body mass index measurements and prevalence of overweight and obesity in school-children living in the province of Belgian Limburg.

UNLABELLED: In order to evaluate the distribution of body mass index (BMI) and the prevalence of overweight and obesity in school-children living in the province of Belgian Limburg, BMI percentiles for 9,487 children aged 3.2 to 16.0 years were calculated and compared with the Dutch 1980 values. Although the 5th and 50th percentile BMI values were comparable to or only slightly above the reference values, the 85th and 95th percentiles were much higher revealing an increased degree of obesity in the studied population. The prevalence of overweight and obesity, determined by calculating the proportion of children with a BMI value above the 85th or 95th percentile of the Dutch 1980 reference, ranged for overweight from 15% at the age of 3-4 years to 33% at the age of 12-13 years, and for obesity from 6% to 19%. CONCLUSION: this study shows a dramatic increase in upper body mass index percentiles and in the prevalence of overweight and obesity in children living in the province of Belgian Limburg. Strategies to prevent obesity in childhood should be a priority in child public health programmes.

Management of hypertension in overweight and obese patients: a practical guide for clinicians.

The association between obesity and cardiovascular disease is well established, and up to 60% of overweight or obese patients have hypertension. Dietary interventions associated with modest weight loss are effective in controlling blood pressure and in reducing use of antihypertensive drug therapy in overweight and obese patients. However, long-term maintenance of weight loss is achieved only in a small proportion of patients. Orlistat and sibutramine may help to achieve and maintain weight loss but may not be sufficient to control blood pressure in overweight and obese hypertensive patients. Consequently, antihypertensive drug therapy is often necessary in addition to weight loss interventions. Few studies have investigated different antihypertensive drugs, specifically in overweight and obese patients with hypertension. Based on studies involving obese and nonobese patients, first-line treatment options include a diuretic alone or an angiotensin-converting enzyme (ACE) inhibitor alone. If monotherapy is inadequate for blood pressure control, combination therapy with diuretic and ACE inhibitor and/or combining either of these drugs with a calcium channel blocker are reasonable treatment options. Additional studies to further clarify the management of these patients are warranted.

Being overweight and multiple fractures are indications for operative treatment of humeral shaft fractures.

Excellent clinical results can be obtained with non-operative treatment of humeral shaft fractures. In certain patients, operative stabilization is the treatment of choice. This study was initiated to determine the results of non-operative treatment in relation to multiple fractures and being overweight. From 1985 to 1992 we treated 35 humeral shaft fractures in 34 patients by non-operative methods. The median age was 51 (18-84) years. There were 12 women and 22 men. Nine were in overweight patients and 11 were in patients with multiple fractures. Fractures in overweight patients were followed for 158 (60-597) days and the Neer score was 61 (50-72) points. Patients with multiple fractures were followed for 178 (52-970) days and the Neer score was 72 (38-96) points. Single fractures in non-overweight patients were followed for 70 (35-412) days and the Neer score was 94 (65-100) points. These results show that humeral shaft fractures in certain patients may best be treated by operative stabilization.

Parity and the prevalence of overweight.

OBJECTIVE: To assess the association between previous term pregnancies and the prevalence of overweight in a group of urban women, controlling for the influence of age. METHODS: One thousand twelve women, living in middle and low socioeconomic areas of Mexico City, were interviewed at home and their reproductive histories studied. Height and weight were measured in a clinical setting using controlled procedures. Overweight (BMI > 25) was the dependent variable used to calculate odds ratios and to perform a multiple logistic regression analysis. RESULTS: Age and parity were significantly associated with the prevalence of overweight. Controlling for age, two or more previous pregnancies significantly increased the magnitude of the association. CONCLUSION: During the reproductive years parity seems to increase the risk of overweight in low and middle socioeconomic level urban women.

Calorimetric results in man: energy output in normal and overweight subjects.

In order to test the practicability of 24-hr investigations with the gradient-free Vienna Whole-Body Calorimeter, energy output was measured over 24-hr periods in 18 human subjects. Heat loss was partitioned into dry and evaporative components. Sixteen female subjects were divided into normal-weight (less than 100% according to the Broca index), overweight (100%--120%), and obese (over 120%) groups. A male with severe hypothyroidism, and a female with no signs of impairment of thyroid function who had weight problems that were suspected to be due to low energy expenditure, were studied separately. Subjects reported that the calorimeter chamber was sufficiently comfortable for at least a 24-hour investigation. Overweight and obese subjects showed both greater total heat output and greater inter-individual variability than the normal weight group. Normal and overweight subjects were on steady levels of food intake that were representative of usual intake. For normal subjects there was a relatively close correspondence between energy intake and output, but not for overweight subjects. Thyroid hormone therapy produced a large increase in energy output in the hypothyroid patient. Energy expenditure was found to be unusually low in the patient with weight problems and was increased by about 50% after thyroid hormone administration.

A study of the self-perception of being overweight in adult inhabitants of Cracow.

The self-assessment of body weight (normal vs overweight) reported in an interview in a group of 783 men and 1085 women aged 32-83, was compared with weight and height measurements taken a few days after the interview. Among subjects with body mass indices revealing excess weight (over 27 kg/m2 in men and 45 kg/m in women), 24% of men and 15% of women reported having normal body weight. The proportion of overweight persons unaware of their condition was related to age, smoking habit, education and physical activity. In men with hypertension, the proportion of subjects unaware of being overweight was smaller by 25% compared with others. This was not the case in women. A history of cardiovascular disease, diabetes, dyspnea or poor health status had no effect on the awareness of being overweight.

Metabolic risk factors and markers of cardiovascular and renal damage in overweight subjects.

BACKGROUND: The prevalence of overweight and obesity in the United States has dramatically increased. Obesity clusters with a variety of hemodynamic and metabolic disturbances that increase the risk of cardiovascular disease. In this study we evaluated whether overweight subjects with hypertension also manifest hemodynamic and metabolic abnormalities compared with individuals of normal weight. METHODS: In a cohort of 129 patients with essential hypertension we measured the relationship between body mass index (BMI), blood pressure (BP), insulin sensitivity, lipid profile, and markers of organ damage including thickness of the carotid artery (IMT) and urine albumin excretion (UAE). A total of 41 normotensive, age-matched, healthy individuals served as control subjects. RESULTS: Hypertensive individuals showed higher levels of serum triglycerides, insulin area-under-the-curve (AUC), UAE, and greater IMT than normotensive subjects. Overweight hypertensive subjects showed higher levels of serum triglycerides, LDL cholesterol, glucose AUC, insulin AUC, UAE, and IMT than hypertensive subjects with normal body weight (BMI <25). Night-time systolic BP was higher and night-time fall in BP was lower among overweight than among normal-weight hypertensive patients. Simple regression analysis showed that BMI was correlated with age, UAE, BP, insulin and glucose AUC, serum triglycerides, cholesterol, and IMT in hypertensive subjects. However multiple regression analyses showed that BMI significantly correlated only with UAE. CONCLUSIONS: The study results show that increased body weight clusters with a variety of hemodynamic and metabolic abnormalities in hypertensive subjects. However multiple regression analyses showed a significant correlation only between BMI and UAE, a marker and predictor of cardiovascular and renal disease.

Maternal pre-pregnancy overweight and obesity and the risk of cesarean delivery in nulliparous women.

PURPOSE: To examine the effect of maternal pre-pregnancy overweight and obesity on the risk of term cesarean delivery in nulliparous women. METHODS: The authors examined data from 641 nulliparous women with a term pregnancy that participated in the Pregnancy, Infection, and Nutrition Study from 1995 to 2002. Unadjusted and adjusted risk ratios and 95% confidence intervals (CI) were computed for normal weight (BMI 19.8-26.0 kg/m(2)), overweight (BMI 26.1-29.0 kg/m(2)), and obese (BMI>29.0 kg/m(2)) women. Normal weight women served as the referent population. RESULTS: The unadjusted risk ratio for cesarean delivery for overweight women compared with normal weight women was 1.4 (95% CI, 0.97, 2.1) and for obese women compared with normal weight women was 1.4 (95% CI, 1.03, 2.0). After controlling for maternal height, education, weight gain during pregnancy, and labor induction, the adjusted risk ratio for cesarean delivery among overweight women was 1.2 (95% CI, 0.8, 1.8). The adjusted risk ratio for obese women was 1.5 (95% CI, 1.05, 2.0). CONCLUSION: Our analysis confirms that there is a moderate association between maternal pre-pregnancy obesity and an unplanned term cesarean delivery. However, the risk is not as large as previously reported.

[Which alternative method to dual-energy X-ray absorptiometry for assessing body composition in overweight and obese adolescents?].

CONTEXT: Professionals in charge of overweight and obese children and adolescents need a simple, reliable and precise method for assessing body composition. OBJECTIVES: To compare body composition as assessed by dual-energy X-ray absorptiometry (DXA), bioelectrical impedance analysis (BIA) and the skinfold thickness (SFT) method in overweight and obese adolescents, and to establish and validate new predictive equations of body composition from BIA measurements using DXA as standard method. SUBJECTS AND METHODS: Body composition was assessed in 143 obese adolescents (Z-score = 3.2 +/- 1.4) aged 12 to 17 years by DXA, BIA (RJL System, Analycor and Analycor XF models) and SFT (Siri and Slaughter's equations). New prediction equations of fat mass (FM) as assessed by DXA were computed from BIA measurements in a calibration group, and validated in an homologous group of subjects. Results. - The Bland-Altman test showed that compared to DXA, BIA underestimated FM by 2.8 +/- 2.0 kg and 2.3 +/- 2.1 kg using the RJL System and Analycor impedancemeter, respectively (P < 0.001). With the Analycor XF model, FM was underestimated by 3.3 +/-2.6 kg in boys, and over-valued by 0.6 +/- 2.4 kg in girls. On the contrary, the predictive equation of Wabitsch et al. overvalued FM by 6.2 +/- 2.9 kg. The SFT method overvalued FM by 2.1 +/- 5.0 kg in boys and underestimated FM by 2.3 +/- 3.5 kg in girls using Slaughter et al. equation, while Siri's equation underestimated FM by 4.0 +/- 2.9 kg (P < 0.001). The alternative to the DXA method to assess FM was BIA with new prediction equations including gender, body weight, height(2)/resistance and reactance. CONCLUSION: DXA, BIA and the SFT method were not directly interchangeable. The SFT method was inadequate to assess body composition in overweight and obese adolescents. BIA and new prediction equations could be an alternative to the DXA method in overweight and obese adolescents.

[Overweight and dietary habits among adolescent: schoolchildren study in the department of Hauts-de-Seine].

Several epidemiologic studies show in a concordant way that the prevalence of the overweight and that of obesity increase in France. Obesity is an important risk factor in public health. The main aim of the National Program of Nutrition and Health is to stop the increasing prevalence of childhood obesity. In this framework, we conducted a prevalence study among 1507 schoolchildren (boys: 51,6%; girls: 48,4%) aged from 11 to 17 years old. A self-questionnaire was used to collect information concerning the weight, height and diet. The findings show that 17,6% of collegians are either over weighted or obese, 7,7% of the girls and 10,0% of the boys are in overweight; 5,5% of the girls and 11,7% of the boys are obese, according to the French charts of body mass index (BMI). This study shows a decreasing frequency of the overweight with the age in both males and females: 28,9% at the 11 years old and 13,2% at the 17 years old. Preventive actions are needed at early stages to try to stop and, if possible, to reverse the present increase of overweight. In this context, the itinerant forum of health promotion carried out in the Hauts-de-Seine department with school doctors can play a determining part.

Abnormal HDL subclasses distribution in overweight children with insulin resistance or type 2 diabetes mellitus.

BACKGROUND: Small HDL particles have emerged as significant predictors of incident type 2 diabetes mellitus (T2DM) in adults with impaired glucose tolerance (IGT). However, no previous study has investigated HDL size in pediatric subjects with these clinical conditions. METHODS: We studied the HDL size distribution by native polyacrilamide gradient gel electrophoresis in 106 overweight children, 47 with T2DM, 43 with normal glucose tolerance (NGT), 16 with IGT, and 39 healthy weight controls. RESULTS: Diabetic children had significantly lower proportions of HDL2b and HDL2a, and higher proportions of HDL3b and HDL3c than the other 3 groups. Overweight subjects showed HDL size distributions similar to those of controls. However, insulin-resistant children had lower proportions of HDL2b, and HDL2a, and higher proportions of HDL3b when compared with the insulin-sensitive overweight subjects. Multiple linear regression analyses showed that homeostasis model assessment correlated inversely with HDL2b and HDL2a, and directly with HDL3b, while BMI was independently associated only with HDL3a. CONCLUSIONS: This study showed that HDL size distribution was shifted toward smaller particles in T2DM pediatric patients and in overweight children with insulin resistance, independent of their glucose tolerance status. Insulin resistance was the main factor associated with these HDL size abnormalities. This parameter could be useful as an early risk marker of incident diabetes and, probably, of coronary heart disease.

Short-term effect of buformin, a biguanide, on insulin sensitivity, soluble fraction of tumor necrosis factor receptor and serum lipids in overweight patients with type 2 diabetes mellitus.

AIMS/HYPOTHESIS: The UK Prospective Diabetes Study (UKPDS) showed that biguanide therapy in overweight patients reduced the risk for any diabetes-related endpoint and all-cause mortality. Biguanides lower the blood glucose values without stimulation of insulin release. We have investigated the short-term effect of buformin on insulin sensitivity, solved tumor necrosis factor receptors (sTNFRs), and serum lipids in overweight subjects with type 2 diabetes mellitus (DM). METHOD: Thirteen overweight subjects with type 2 DM were examined. The subjects who were fed 20 kcal/kg body weight were divided into two subgroups according to whether they were treated by buformin (Buformin group), or dietary therapy alone (Diet group). Six patients were in Buformin group and seven patients were in Diet group. We calculated insulin-mediated glucose uptake by the liver and peripheral tissues using euglycemic hyperinsulinemic clamp combined with an oral glucose load before and after buformin treatment or diet therapy for 2 weeks. RESULTS: Fasting plasma glucose, total cholesterol (T-chol), LDL-cholesterol (LDL-chol), and sTNFR2 were significantly decreased, and hepatic glucose uptake significantly increased from 32 +/- 7 to 42 +/- 7% (P < 0.05) in Buformin group but did not changed significantly in Diet group. However, the glucose infusion rate thought to express insulin sensitivity in peripheral tissue, TNF-alpha, sTNFR1, fasting plasma insulin, C-peptide, and NEFA levels did not change significantly in both the groups after treatment. CONCLUSION/INTERPRETATION: Buformin improved insulin sensitivity in the liver and decreased T-chol, LDL-chol, and sTNFR2. The mechanism of action for buformin likely involves inhibition of TNF-alpha. Buformin lowers insulin resistance and risk factors for cardiovascular disease including serum lipid and will therefore, be useful in management of overweight type 2 DM patients.

Breastfeeding and overweight: longitudinal analysis in an Australian birth cohort.

OBJECTIVE: To examine adiposity in relation to breastfeeding using longitudinal analysis in an Australian birth cohort. STUDY DESIGN: Repeated surveys from 16 weeks gestation to 8 years in a cohort (N = 2087) recruited through antenatal clinics. Overweight was defined by National Center for Health Statistics 95th percentiles for weight-for-length at 1 year and body mass index (BMI) at 3, 6, and 8 years. Overweight was examined using Generalized Estimating Equations with results summarized as OR. BMI Z scores were analyzed in mixed models. RESULTS: At 1 year, infants breastfed >12 months were the leanest group (mean Z score -0.16, 95% CL -0.28, -0.04; not breastfed 0.16, 95% CL 0.02, 0.29; breastfed < or = 4 months 0.31, 95% CL 0.22, 0.40; 5-8 months 0.17, 95% CL 0.06, 0.27; 9-12 months 0.11, 95% CL 0.01, 0.22). From 1 to 8 years, children breastfed < or = 4 months had the greatest risk of overweight (OR 1.29, 95% CL 0.89, 1.97) and the highest prevalence of maternal obesity, smoking, and lower education. CONCLUSIONS: Infants breastfed >12 months were leaner at 1 year but not at 8 years. Breastfeeding < or = 4 months was associated with greatest risk of overweight and adverse maternal lifestyle. Familial factors may modify associations between breastfeeding and adiposity beyond infancy.

Left atrial diameters in overweight children with normal blood pressure.

OBJECTIVE: To measure left atrial (LA) diameter in normotensive, overweight children and to determine the variables that independently influence LA diameters. STUDY DESIGN: A cross-sectional study on 22 overweight children (age, 13.40 +/- 1.22 years) and 18 control children (age, 13.40 +/- 2.19 years) was performed. LA diameter, left ventricular (LV) geometry, and LV systolic/diastolic function were measured through echocardiography. Data were compared between the two groups. Multiple regression analysis, with LA diameter as dependent variable, was conducted. RESULTS: In the overweight group, LV diastolic dimension, LV mass index, peak S, S/D ratio, Am, maximal LA diameter (34.1 mm vs 25.4 mm), middle LA diameter (26.9 mm vs 18.1 mm), and minimal LA diameter (22.5 mm vs 16.1 mm) were increased and Em/Am ratio was decreased. In multiple regression analysis, body mass index (R(2) = 0.7040, 0.7085, 0.7406 in respective maximal, middle, and minimal LA diameter) and systolic blood pressure (R(2) = 0.0540, 0.0477, 0.030 in respective maximal, middle, and minimal LA diameter) were significant independent correlates of all three measures of LA diameter. CONCLUSIONS: LA diameter in normotensive overweight children is increased. Body mass index and systolic blood pressure were significant correlates of LA diameter.

Fluvastatin improves endothelial dysfunction in overweight postmenopausal women through small dense low-density lipoprotein reduction.

Small dense low-density lipoprotein (sdLDL), which are often associated with obesity, are considered as the most atherogenic and have been shown to impair endothelial function. It is not known whether reduction of sdLDL by pharmacological intervention can improve endothelial function. Thirty-four consecutive postmenopausal women with >/=5.70 mmol/L total cholesterol were placed into either an overweight (body mass index [BMI] >/= 25.0, n = 22) or a normal-weight (BMI < 25.0, n = 12) group, and forearm blood flow (FBF) was measured using strain-gauge plethysmography during reactive hyperemia before and after fluvastatin treatment. At baseline, the peak FBF during reactive hyperemia in the overweight group was less than that in the normal-weight group (mean +/- SD, 13.6 +/- 4.4 v 22.2 +/- 4.0 mL/min/100 mL, P <.01). The maximal FBF after nitroglycerin was similar in both groups. In the stepwise multiple regression analysis, only the concentration of sdLDL was the predictor for peak FBF (standard coefficient = -0.517, P =.0115). The nonsignificant parameters for the correlations in the model were age, BMI, systolic blood pressure, the homeostasis model assessment of insulin resistance (HOMA-IR), hemoglobin A(1c) (HbA(1c)), and LDL-cholesterol. Fluvastatin treatment was associated with the recovery of the peak FBF in the overweight group but it did not influence that of the normal-weight group. Changes in sdLDL fractions by fluvastatin correlated well with the peak FBF recovery. These results suggested that an increased sdLDL was linked to endothelial dysfunction in overweight postmenopausal women and fluvastatin treatment improved endothelial dysfunction by decreasing the atherogenic sdLDL fraction in this population.
