We encountered a patient with diffuse pleural mesothelioma pointed out by a tumor shadow 7 mm in diameter on a chest X-ray film obtained during a regular physical checkup. Computed tomography (CT) scans disclosed several small tumors, slightly thickened pleura, and a small amount of pleural effusion. A cytological examination of the effusion, as well as a bronchoscopic examination, were both unable to detect any evidence of malignancy. The patient was referred to the respiratory medicine department of our hospital, where further bronchoscopic examinations were performed but no conclusive diagnosis was obtained. The patient was therefore referred to our department. CT scans revealed enlarged tumors, markedly thickened pleura, and an increase in pleural effusion. This time a cytological analysis of effusion samples was positive for malignant mesothelioma cells. A pleuropneumonectomy was performed, but tumor cells were partially exposed on the dissected surface. Recurrent tumors were disclosed by CT scans 15 months later. Our conclusion was that a thoracoscopic examination should have been performed for diagnostic purposes at an earlier stage.
The paper proposes a variant how to solve the problem of complex automation of the work of antituberculous service by using the concept of building up a multilevel automatic informational and analytical system of antituberculous service at the regional level developed at the regional level. The programme product "statistical phthisiologist AMP" for the lower level of the area (urban) antituberculous dispensary which has been developed within this project is given.
Development of a simple and effective smoking cessation program is needed to provide cessation counseling during health checkups. A new cessation program, which consists of brief individual counseling and 4 follow-up telephone calls, was developed based on the stage model for life-style change. This program was performed during health checkups in the town of Nose to evaluate its usefulness. Smoking status questionnaires were completed to assess the smoking habits of subjects and to evaluate their smoking stage before the counseling session. Then, stage-matched cessation counseling was provided using a self-help guide. During the counseling, carbon monoxide measurement of expired air and Health Risk Appraisal feedback were performed to enhance self-perception of smoking. Follow-up calls were provided for only those clients who set a quit date during the individual counseling. It was easy to implement this program, and it required between 15 and 20 minutes to conduct. The cessation rate was 19% at 8 months after the health checkups. This result was more effective than data for other programs reported previously. Therefore, this program was effective and could be used at health checkups. This trial had no control group, so further studies are needed to clarify the efficacy and effectiveness of this program. In addition, training courses for health professionals must be developed to disseminate this program into general use.

This report gives an account of the patterns of requesting of multiple biochemical tests in a large general hospital and the frequency of abnormal test results in different hospital departments. There are appreciable differences between some areas in the frequency of requests for multiple testing and in the proportions of abnormal results. If it is assumed that clinicians should be aware of abnormal values and their progress, facilities for multiple testing are desirable and economically justifiable.
We report a case of non-functioning retroperitoneal paraganglioma detected incidentally by health examination in a 60-year-old female without any clinical manifestation. She was admitted to our hospital for the purpose of evaluation of a renal mass detected by ultrasound sonography. Computed tomography, magnetic resonance imaging and angiography revealed the heterogeneously-enhanced 10 x 10 cm mass in the left retroperitoneal space without any distant metastasis. It was suspected to be a renal cell carcinoma. All laboratory data, including those from endocrinological examinations, were within normal ranges. Radical nephrectomy was performed to resect en bloc the mass and the left kidney. The pathological and immunohistochemical examinations of the mass which was completely separated from the kidney and the adrenal gland, showed characteristic features of paraganglioma with vascular and capsular invasion, with a final clinical diagnosis of the pathologically-malignant non-functioning paraganglioma occurring between the left kidney and the adrenal gland.

The Danish Twin Registry was initiated at the Copenhagen University Institute for Human Genetics in 1954 by professor Tage Kemp and his assistants, Bent Harvald and Mogens Hauge. The project was until 1960 primarily financed by US National Cancer Institutes. Twin pairs both of same and different sex, born 1870-1910 were drawn from the birth registers and followed-up through the national registers until death or present place of living. Later on same sex pairs born 1911-30 were added. Besided cancer a multitude of other disorders were registered. Intensive studies on identical twins grown up apart were undertaken by Niels Juel-Nielsen, on criminal behaviour by Karl O. Christiansen, on smoking habits by Elisabeth Raaschou-Nielsen, on peptic ulcer by Kaj Gotlieb Jensen, on schizophrenia by Margit Fischer. Among early remarkable results should be mentioned the modest influence of genetic versus environmental factors in most cases of cancer, the strong genetic impact in mental and moral qualities, the approximate genetic/environmental ratio of 50/50 for the orgin of criminal behaviour and a corresponding ratio of 25/75 for life expectancy. In 1971 the Danish Twin Registry was moved to Odense Universty.
There is a final purpose of the standardization of the external quality control for the resolution of the difference between facilities. Two solutions are promoted in order to unify the evaluation level. They are that it unifies the reference value and common evaluation method used common display. However, it is the prior settlement that there is some a problem on both method and that absolute value is made to agree using standard substance.

A search was made for predictors of sudden cardiac death (SCD) among 197 men, age 40 to 79, who had multiphasic health checkups and experienced SCD 2 days to 5 1/2 years later (mean 18.1 months). Two age-sex-race-matched control groups, one matched and one unmatched for standard coronary risk factors, have been compared to the cases. In this ambulatory population, SCD occurred largely in persons with prior diagnosed heart disease. Standard coronary risk factors were confirmed as predictors of SCD. Predictors among other laboratory and quantitative tests include chylous serum, uric acid, hemoglobin, leukocyte count, pulse rate, diminished lung volumes, and hearing loss. Some of these had predictive value independent of standard risk factors. Pain tolerance was not related to SCD or to death in, versus out of hospital. Most of these predictors were not related to terminal symptom duration; this suggests a relationship more to the underlying atherosclerotic process than to the terminal fatal mechanism.
A 50-year-old man was admitted to our hospital because of an abnormal shadow, detected during an upper gastrointestinal examination. A chest radiograph showed an infiltrating shadow in the right middle lung field. A chest CT showed a fistula communicating between a bronchus (rt. B6) and the middle of the esophagus. Resection of the fistula was performed by video-assisted thoracoscopic surgery (VATS). Isolation of the fistula was straightforward, and there was no evidence of inflammation or adherent lymph nodes around it. Histologic examination of the resected specimen revealed that the fistula lumen was covered with squamous epithelium and muscularis mucosa. These findings suggested that this case could be categorized as Braimbridge type II. In this case, the chest CT showed the esophagobronchial fistula clearly, and was useful for the diagnosis.
