Measuring the impact of modern matrons in the ward setting.

With the The NHS Plan (Department of Health, 2000) the government made it clear there was a need to reintroduce the role of matron to establish a clear responsibility for standards in the ward setting. The role of modern matron is now considered to be a fundamental part of clinical governance, ensuring the quality of the care provided is of the highest standard. Staff at South Essex Partnership NHS Trust instituted an audit to provide an accurate picture of the impact of modern matrons.

[Molecular anthropology and the origin of modern human].

Since Watson & Crick put forward the double-helix model of DNA structure and hereditary mechanism in 1953, it is generally accepted that this event marks the birth of modern molecular biology. This new field of biology has experienced a flourishing development in the past 50 years. On one hand, the development of molecular biology has been deeply influencing many relative fields; on the other hand, its own proceeding pace has been accelerated by the reaction from the other fields. Anthropology is one of the fields most deeply impacted by the theory and method of molecular biology. Most importantly, molecular anthropology was born as a result of combination of molecular biology, anthropology as well as paleoanthropology. This new branch provides reliable method and vital direction for paleoanthropology. This paper systematically reviews the history, principle and method of molecular anthropology. Two hypotheses on the origin of modern human, which include "out-of-African theory" and "theory of multiregional evolution" are also discussed for the purpose of showing how molecular anthropology is applied in paleoanthropology.

[Comparison of 2 techniques of anesthesia induction in modern closed-circuit anesthesia].

The application of low flow anesthesia goes back already for more than 65 years when Ralph Waters introduced and applied cyclopropane with a very simple canister technique. The guide-lines for the clinical use of the closed circuit anesthesia were published in Chicago and Los Angeles by Professor Lin and Professor Lowe 15 years ago. We examined and followed these procedures on a modern anesthesia machine and easily achieved a good clinical performance. Our results were consistent with the modern anesthesia standard in closed circuit t.i.: economical benefits, environment savage of waste gases, heat production and humidification and last but not least in both techniques a quick anesthesia plane on 10-12 minutes that was consistent with theoretical considerations.

The modern methods of gastric imaging.

The major aim of the review is presenting contemporary diagnostic methods applied in gastric imaging as well as their place in clinical treatment. The authors discuss both conventional and modern methods. Although the method of double contrast barium meal is still the most important in stomach diagnostics, the modern methods such as EUSG, CT and MRI are becoming more and more meaningful not only in evaluating gastric neoplastic pathological changes. All applied diagnostic procedures should not be competitive but complementary and constitute an excellent means in hands of an experienced clinicist and radiologist.

[Theoretical and practical aspects of modern vitaminology].

General subjects and achievement of modern vitaminology are discussed. The most impressive success in fundamental vitaminology during last three-four decades was achieved in following areas: elucidation of vitamins metabolic roles and molecular mechanisms in their action; development and improvement of high sensitive modern methods for analytical vitamins determination in biological objects and food; establishment of reliable criteria and methods for human vitamin status assessment; examination of physiological human vitamin requirements and establishment of Recommended Dietary Allowances (RDA) or Adequate Levels (AL) of their intake; elaboration of scientifical grounds for reasonable vitamins application in diseases prevention and treatment. In the area of applied vitaminology the most attention during last decades was paid to next subjects: regular broad vitamin status examination of representative groups of population; reasoning, development, realization and evaluation of broad scale measures for prevention of vitamin insufficiency and improvement of vitamin status of population; development and industrial production of vitamins, multivitamin-mineral preparations and various vitamin enriched food stuffs for the same purpose. Decrease of state financial support for fundamental studies and cessation of such a support for applied investigation make it necessary to organize nonstate commercial scientific-technologic center functioning on the requests of vitamin producers and consumers.

[Effects of acupuncture combined with modern rehabilitation technique at early stage on life self-care ability and mental state of the patient of stroke].

OBJECTIVE: To observe effects of acupuncture combined with modern rehabilitation at early stage on life self-care ability and mental state of the patient of stroke. METHODS: Two hundred and twelve cases with stable life signs and no progress of neurological signs for 48 hours were randomly divided into an acupuncture plus rehabilitation group (group I, n = 113) and a simple acupuncture group (group II, n = 99). The group I were treated by acupuncture plus psychological therapy and movement function rehabilitation therapy, and the group II by simple acupuncture. Their life self-care ability and mental state were evaluated. RESULTS: The score of life self-care ability in the group I was up-regulated by 17.57 points (P < 0.05) and the mental state was down-regulated by 5.86 points (P < 0.05) in average as compared with that in the group II. CONCLUSION: Acupuncture combined with modern rehabilitation technique at early stage of stroke can significantly improve life self-care ability and mental state of the patient.

[Max Josef von Pettenkofer--founder of modern hygiene (1818-1901)].

Max Josef von Pettenkofer was one of the leading personalities in the world of medicine in the 19th century. He was the founder of the modern science of hygiene. In his experimental work, he was involved in the research of problems dealing with the relationship between human beings and the environment, including such topics as soil and air pollution, water supply, sewage water management, room ventilation and heating, as well as the function of clothing and the cleanliness of homes and streets. Pettenkofer also studied the onset, the course, and the consequences of infectious diseases, such as cholera and typhus. He realised the great economic value of public health and emphasised that personal preventive measures should be supplemented with the improvement of factors in communal and work environments. His efforts lead to hygiene becoming a part of medical studies in 1865. The Institute for Hygiene at the School of Medicine in Munich was established in 1879. It was constructed according to his drawings and was considered to be the most modern institute for hygiene in the world. Since hygiene was a subject on the school curriculum in the German Empire in 1882, Pettenkofer became the Chairman of Hygiene in Berlin in 1885. Research institutions established by Pettenkofer and the fact that many of his students became professors of hygiene speak about the importance of his work. One of his students was professor Milan Jovanović Batut, founder of the Institute for Hygiene at the School of Medicine in Belgrade.

Pharmaceuticals: Medicare Modernization Act--2005. End of Year Issue Brief.

The enactment of the landmark Medicare Modernization Act of 2003 (MMA) marked the first structural change to Medicare since its inception in 1965. The MMA established the Medicare prescription drug benefit with an intermediate Medicare-approved drug discount card program to be implemented six months after enactment and full implementation in January 2006. The MMA marks the first time the federal government would be providing access to prescription drugs for seniors. Though the merits and cost of the MMA continue to be debated, the impending change to how seniors and the low-income access prescription drugs is on the horizon. Historically, state lawmakers believed it was appropriate to provide assistance to the elderly and, in some states, the disabled and indigent, for the purchase of much needed prescription drugs. Since 1975, states have been creating, terminating and redesigning state pharmaceutical assistance programs (SPAPs) that either provides a prescription drug subsidy, discount card program or full benefit design. Currently, 33 states operate pharmaceutical assistance programs that provide coverage for seniors. Now, due to the MMA, the states' role as provider of prescription drug benefits to seniors is being re-evaluated. The MMA is intended to save costs for the states. Medicare beneficiaries, the bulk of enrollees in state pharmaceutical assistance programs, will now access prescription drugs through the new Medicare prescription drug benefit. The MMA mandates a state to federal government cost shift when the financial burden of those who are eligible for both Medicaid and Medicare (dual eligibles) switch to receiving prescription drug coverage through Medicare, with Medicaid only being an option for drugs not covered by Medicare. Employers, many of which have been cutting prescription drug benefits to retirees because of rising health care costs, will now receive subsidies from the federal government to continue providing prescription drug benefits to retirees. Initially, the MMA appeared to be a fiscal reprieve for states and the need for SPAPs limited. As state policymakers began to analyze the MMA's benefit design and regulations, the cost benefit to the states became questionable. There is a great deal of uncertainty for the states about how they should respond in implementing the MMA. As states review final regulations and full implementation draws closer, the financial impact of the Medicare prescription drug benefit and actual benefits offered by Medicare prescription drug plans are unknown. The states' ability to cut costs as a result of the enactment of the Medicare Modernization Act is questionable. This issue brief describes challenges of MMA implementation for the states and provides an overview of current state pharmaceutical assistance programs and 2005 state legislative activity.

On the Ottoman consent documents for medical interventions and the modern concept of informed consent.

Information for patients prior to medical intervention is one of the principles of modern medical practice. In this study, we looked at an earlier practice of this principle. Ottoman judges had record books called sicil. One of the categories in sicils was the consent documents called riza senedi, which was a patient-physician contract approved by the courts. These contracts were especially for the protection of physicians from punishment if the patient dies. It is not clear whether patients were informed properly or not. Consent for minors was obtained from parents. However, a situation where an adult does not have the capacity to consent, was not clear in these documents. Any sign of free withdrawal of consent was not found in these records. Due to the legal system of Ottoman State, these contracts were related to Islamic law rather than modern civil law. We aim, in this paper, to present a legal practice, which is possible to consider as an early example of the informed consent practice.

Modern brachytherapy for localized prostate cancers: the northwest hospital (Seattle) experience.

Modern ultrasound-guided prostate brachytherapy is rapidly changing the way localized prostate cancer is managed. With routine use of prostate-specific antigen screening, prostate cancer is being diagnosed in younger men, who are understandably concerned about the morbidity of radical treatments that may significantly decrease their quality of life. Numerous studies of prostate brachytherapy have shown the excellent disease control rates achieved while maintaining low levels of urinary and erectile difficulties. This report examines a modern implant method of brachytherapy; describes patient selection for brachytherapy, alone and in combination with external beam therapy; and presents results from a series of men followed for 12 years.

[Modern biological basis of Chinese medical theory that "kidney nourishes marrow and brain is sea of marrow"].

The theory that "the kidney nourishes marrow and brain is the sea of marrow" has been instructing traditional Chinese medical doctors in preventing and treating dementia in aged people for thousands of years. However, the modern biological basis of this theory has not been systemically studied. In this review, we summarized our serial pharmacological studies on the treatment of Alzheimer's disease (AD) with Chinese medicinal herbs, using multiple kinds of AD-like animal and cell models. The results show that "kidney-reinforcing" herbs of traditional Chinese medicine play a very important role in the anti-AD effects; and different "kidney-reinforcing" herbs have different effects in brain functions. The common effects of "kidney-reinforcing" herbs are improving cellular energy metabolism, increasing neurotrophic factors and the number of cholinergic neurons, and decreasing neurotoxin production. Based on above results, we propose that the essence of "brain marrow" is the neurons and neurotrophic factors in the brain; "the deficiency of brain marrow" is induced by the decrease in neurotrophic factors and the atrophy and loss of neurons in the brain, thus resulting in cognitive impairment and dementia. The modern biological basis of "reinforcing kidney to replenish marrow" by traditional Chinese medicine includes improving cellular energy metabolism and utilization, enhancing endogenous neurotrophic effects and decreasing neurotoxin production, thus reducing the cell death and increasing the survival and regeneration of neurons.

[Modern labor and stress on skeletal muscles].

Introductorily the actual state in analysis and evaluation of physical work is characterized as a problem of changing chemical into mechanical energy in the skeletal muscle. Modern work is characterized in the view of the physiologist. Considering experimental results and epidemiological studies, problems of the K+ homeostasis during long lasting low level muscle activity are discussed as reasons of limiting performance. Changes in the potassium distribution between intra- and extracellular space have enormous consequences for excitability, contractili, metabolism, and arterial blood pressure. Finally references are given for evaluation and designing of modern work.

[Advances of modern studies of acupuncture and moxibustion for treatment of ovulation disorders].

OBJECTIVE: To introduce the modern advances of studies on acupuncture and moxibustion for treatment of ovulation disorders. METHODS: The recent years' literature of acupuncture and moxibustion for treatment of ovulation disorders were reviewed from clinical studies and treatment mechanisms. RESULTS: Acupuncture and moxibustion can regulate the hypothalamus-pituitary-gonadal axis, the nervous system and immune system of the organism. CONCLUSION: Acupuncture and moxibustion has very good clinical effects on ovulation disorders, and it is a safe and effective adjuvant way for modern assisted reproductive technique.

[Aversion to spectacles and the diffusion of optical knowledge among early modern medical practitioners].

In this essay it is shown that eyeglasses were not highly recommended in medical treatises of the Early Modern times. They were often not mentioned amongst the therapeutical advices for eye disorders, or, if they were, not without a certain reluctance. It is shown how this can be understood in view of commonly held opinions about the diseased body, and the way vision takes place. Eyeglasses did not fit within the holistic remedies in use. Oculists were ambiguous too, but on different grounds: whereas the conceptual background seemed not such an issue, they considered eyeglasses as a threat towards their profession. The patient himself seemed to be seduced by the effect of eyeglasses, though some doubts about its working would also push him to try other popular remedies. The attitude of physicians gradually changed during the seventeenth century. It is argued that this can be linked to the evolution in ocular anatomy, strongly influenced by optical insights, whereby the eye is no longer considered in relation to the body, but to the world outside. Eyeglasses are thus on the edge of different discourses. It is the dynamic interaction between all of them that makes Early Modern ophthalmology so interesting and complex.

[The life and works of Han Shin Gwang: a midwife and nurse of Korean modern times].

Han Shin Gwang, born in an early Christian family in Korea in 1902, could get western education different from the ordinary Korean girls in that period. She participated in the 1919 Samil Independence Movement in her teens, and got nursing and midwifery education in a missionary hospital. She got a midwife license and worked as a member in an early mother-and-child health center. She organized 'Korean Nurses' Association (see text)' in 1924 and focused on public health movement as the chairwoman. She actively participated in women's movement organizations, and Gwangjoo Student's Movement. She was known to be a representative of leading working women, and wrote articles on woman's right, the needs and works of nurses and midwives. From late Japanese colonial period, she opened her own clinic and devoted herself to midwifery. After the Korean Liberation in 1945, she began political movement and went in for a senate election. During the Korean War, she founded a shelter for mothers and children in help. After the War, she reopened a midwifery clinic and devoted to the works of Korean Midwives' Association. Han Shin Gwang's life and works belong to the first generation of Korean working women in modern times. She actively participated in women's movement, nurses' and midwives professional movement, Korea liberation movement, and mother-and-child health movement for 60 years. Her life is truly exemplary as one of the first generation of working women in modern Korea, distinguished of devotion and calling.

[Modernization of health regulations in Mexico].

This paper discusses the main points of a proposal for the modernization of the sanitary regulation in Mexico. The basic goal of the Program for the Modernization of the Sanitary Regulation is to promote the free and easy circulation of goods and services guaranteeing the protection and promotion of the health of the population.

[The concept of "ganglionitis" in the modern science of the autonomic nervous system].

The authors review the concept of vegetative "ganglionitis" formed in the Soviet literature in the 40-60s. Consider the etiological, clinical and diagnostic aspects thereof. Provide the modern concepts of the clinical manifestations related to the lesions of the paravertebral sympathetic ganglia. Relate some hypotheses concerning the origin of painful phenomena within the framework of the syndrome of reflex sympathetic dystrophy. Indicate that verified lesions of the sympathetic ganglia are associated with well-defined clinical symptoms which absolutely differ in their appearance from the reported descriptions of "ganglionitis", which are unjustifiedly widely used in the Soviet literature over the recent decades. The conclusion is made about the necessity of reviewing the outdated concepts of the pathology of the vegetative sympathetic ganglia from the standpoint of the modern concept of peripheral vegetative insufficiency.

[The current physiopathological, diagnostic, clinical and therapeutic considerations of peptic ulcer. A comparison between traditional and modern therapeutic methods].

The authors, keeping in mind their clinical, diagnostic and therapeutic experience on ulcerous gastric and duodenal pathology, underline the effectiveness of the traditional antacids as well as that of the modern up-to-date antisecretory and cytoprotective agents. After having put into evidence the causes that can provoke peptic ulcer not excluding that of iatrogenic and their probable aetiopathogenetic mechanism and having hinted at another pathology that uses the same therapeutic remedies, the gastro-oesophagus reflux, they discuss in detail the active principles used in the gastric or duodenal pathology, the possible side and/or secondary effects that in the long run are determined when using only the modern therapy with the usual antacids. Therefore, they hope that these therapies are used after carefully considering the general clinical and anatomicopathologic situation of the lesion that must be created.
