Effect of repeated semen ejaculation on sperm quality.

The semen quality of 21 Medical students who produced sperm by masturbation has been assessed. The mean density of 21 samples examined on the first day was 64,4 millions per millilitre, while that on the third and fifth day of the study period were 52.2 and 50.7 millilitre respectively. This showed a statistically significant difference between the first and the third samples. Another statistically significant difference (P less than 0.5) was demonstrated in the semen volume of the first and third, and the third and fifth samples respectively. Significant differences were, however, not observed in the motility and morphology changes in the group samples. We therefore conclude that even though repeated semen ejaculations on alternate days do affect the density and volume of the semen, the most important overall effect is not such as to impair the potential fertility of the man who has normal initial sperm characteristics.

Potency disorder among Pathans.

In a working class industrial area of Karachi hundred consecutive Pathans presenting to a family physician with potency disorder were examined. After exclusion of those with structural or drug related conditions, a structured proforma was introduced. Their presentation, associated symptoms and background pointed to masked depression and lack of sex education. Symptoms of anxiety were noticed in 49% and depressive features in 43%. The guilt feelings were reinforced by Hakims and lay literature which stress more on masturbation (79%) and spermatorrhoea (60%) and not extra-marital intercourse (52%) or bestiality (39%).

Gender comparisons of college students' attitudes toward sexual behavior.

Questionnaires were distributed to 641 undergraduates at a large southeastern university to elicit opinions concerning various forms of premarital sexual behavior. Hypotheses were developed to determine whether there were differences between the attitudes of males and females. Significant differences were found between males' and females' attitudes toward premarital sex when the couple is casually acquainted, and attitudes toward extramarital sex, oral-genital sex, and anal sex. However, significant differences were not found between males' and females' attitudes toward premarital sex when the couple is in love, attitudes toward premarital sex when the couple is engaged, and attitudes toward masturbation, homosexuality, and sexual fantasizing. Males' attitudes toward various forms of sexual behavior were more liberal than those of females.

Sexual experience and sexual orientation among Ramathibodi medical students, Thailand.

This study aims at exploring the several sexual experiences and sexual orientation among medical students at Ramathibodi Hospital, Bangkok, Thailand. Questionnaires were given to all medical students during 1982-1988. Almost all, 93 per cent of the males (593/633) and 92 per cent (211/224) of the females completed the questionnaire. All males had had ejaculation. Eighty-four per cent of the males and 28.1 per cent of the females experienced nocturnal sex dreams. Ninety-eight per cent of the males and 47.8 per cent of the females masturbated. Most of the first events occurred incidentally among both sexes. Sixty-one per cent of the males and 1.5 per cent of the females had engaged in sexual intercourse. Most of the males had their first coital experience with prostitutes. Homosexual experience was admitted in 13 per cent of male and 3.4 per cent of female students. Almost 20 per cent of both sexes were either exclusively homosexual or bisexual.

Heterosexuals at risk for HIV: differences between private and commercial partners in sexual behaviour and condom use.

To study the prevalence of HIV and other sexually transmitted diseases (STDs), sexual behaviour and condom use among heterosexuals, 193 women and 157 men entered a longitudinal study. Participants were recruited through an STD clinic, and had had five or more heterosexual partners in the preceding 6 months but had no other AIDS risk factors. Participants were divided in groups with respect to private and/or commercial sexual partners. One hundred and thirty-six (71%) women worked as prostitutes and had, on average, 115 customers a month, and 99 (63%) men had visited, on the average, eight prostitutes in the past 4 months. One hundred and seventy-one (89%) women (114 prostitutes) had had, on average, four partners, and 112 (78%) men (of whom 64 had visited prostitutes) had had, on average, seven private partners in the past 4 months. Vaginal intercourse was often or always reported by all participants whilst anal contact was seldom reported. Non-high-risk sexual techniques (for example, oro-oral, masturbation) were practised more frequently with private than with commercial partners (P less than 0.001). Although no differences were found in the frequency of practising high-risk sexual techniques (for example, vaginal, ano-genital), differences were found for frequency of condom use. Condom use during vaginal intercourse was reported more frequently with commercial than with private partners (P less than 0.01). However, if the number of partners is taken into account, prostitutes had unprotected vaginal intercourse with an estimated average number of 160 partners in 4 months, which is far more than customers (seven) and men and women with private partners only (four).(ABSTRACT TRUNCATED AT 250 WORDS)

Correlation between the sperm motility and the adenylate cyclase activity in infertile men.

The relationship between the sperm motility and the adenylate cyclase activity in spermatozoa from patients with male infertility was investigated. Cyclic AMP contents were measured from 34 semen specimens and adenylate cyclase activity were assayed from 50 specimens. All semen specimens were collected from infertile men at our clinic by masturbation after 5 days sexual abstinence. Spermatozoa were washed and concentrated according to the method described by Harrison. The cAMP contents and the adenylate cyclase activity of spermatozoa were determined by radioimmunoassay. Positive correlations were found among the cAMP contents, the adenylate cyclase activity in human spermatozoa and the sperm motility. This result suggests that the sperm motility is regulated by the adenylate cyclase activity via cAMP and that poor sperm motility observed in infertile men is partially caused by the impairment of adenylate cyclase system.

Prevalence of child sex rings.

Within a geographically defined population of 710,000, 31 child sex rings were identified by the police during a 2-year period. A total of 47 male perpetrators aged 16 to 82 years and 334 children aged 4 to 15 years were involved. The perpetrators usually operated alone or in pairs. Three rings were expanding and developing into semicommercial enterprises, however and four or more men participated in them. Children acting as ringleaders recruited victims for the perpetrator in 22 rings, usually in exchange for money. The offenses reported included fondling and masturbation (30 rings), oral, anal, or vaginal intercourse (21 rings), and production of child pornography (two rings). In addition, two adolescent prostitution rings involving 12- to 18-year-old boys were investigated by the police. During the 2-year period these sex rings accounted for 4.6% of all cases of child sexual abuse reported to the police and 6.6% of the subsequent prosecutions.

Pharmacological manipulation of sexual behaviour in stallions.

Series of experiments and clinical trials were conducted to evaluate the effects of psychoneurotropic agents on sexual behaviour of stallions. The benzodiazepine derivative, diazepam (Valium), effectively reversed experimentally suppressed precopulatory arousal and response. Diazepam treatment also blocked the negative effect of novel environment on sexual response. The dibenzazepines imipramine and clomipramine induced erection, masturbation, and ejaculation in the absence of a sexual stimulus.

Prevalence of self-injury in institutionalised retarded children.

Twenty-three percent of the inpatient population of a psychopaedic hospital in New Zealand were found to have engaged in self-injury during a six month observation period. Of the 50 male and 34 female patients studied, 62 exhibited single self-injury, and 22 multiple self-injury. Self-injury consisted of head banging, face slapping, skin picking, hair pulling, self-biting, regurgitation/vomiting, and excessive painful masturbation. Twenty-five percent of these patients indulged in forms of self-injury which was potentially seriously harmful to themselves if not immediately treated. The overall percentage of such patients in this hospital was found to be much higher than that reported elsewhere.

Changes in seminal parameters of ejaculates after repeated ejaculation.

Seminal fluid parameters of ejaculates collected via masturbation successively after sixty minute interval were compared. Volumes of semen were in most cases significantly lower in the second ejaculations as compared with the first. Sperm density was higher in the second specimen only in 32 patients (31% of the group). The percentage of motile spermatozoa was comparable in both specimens. The velocity was in the second specimens significantly lower. The percentage of morphologically defective forms of spermatozoa was found to be approximately the same in both specimens. The use of successive ejaculation will have hardly any greater clinical importance in the treatment of hypofertile men.

[Andrologic appraisal in a court of law. Current problems in daily practice].

In a court of law the andrological expert is a servant of the court, not a judge. In the case of oligozoospermia manifested at the present time, where anamnestic data regarding any past sickness are lacking, it must be assumed that during the legal time of conception a condition similar to that now detected also existed. This means that the present finding was already in a state of effect in the past. Furthermore the presence of the expert at the time of masturbation by the patient controlling the semen production is discussed. The defense attorney asked the court for the presence of the expert. This presence does not cause any additional difficulty for the patient.

[The epidemiology of breast neoplasms].

In an epidemiologic study the risks of breast cancer were assessed. An evaluation of the distribution of different tumor stages revealed that more information, in a larger sense more civilization, workship and the life in town makes women more curious and brings them to go sooner to the doctor. A lack of information correlates with more advanced stage of tumor. Significantly more childless women suffer of breast cancer, but a high number of children is no protection. In women with 3 and more children yet more advanced stages were seen. This may be interpreted by isolation, lack of information and distress. A significantly high number of breast cancer patients lost one of their parents by death before the age of 20, which confirms the psychosomatic theory of cancerogenesis (overcoming of the "Objektverlust" in early childhood). The incidence of depressive states before the onset of the disease is significantly higher. A reduced capacity to communicate was also observed. The exploration of sexual behaviour showed more frequently a reduced sexual activity correlated to cohabitarche, number of partners, alibidimia, unwanted pregnancy and masturbation. The ability of adequate sexual life was deeply disturbed by disease and therapy. Following primary therapy intelligent guidance of the patient may avoid occurrence of disturbances of sexual communication.

Prolactin and fructose in human seminal fluid.

Two hundred thirty-six semen specimens were obtained in the sterility clinic by masturbation after 3 days of abstinence. In a radioimmunoassay for prolactin (NIH, batch no. 5) the dose-response curve obtained with seminal fluid prolactin was parallel to that obtained with serum prolactin. The average seminal fluid prolactin was 780 microunits/ml (35 ng/ml), three times higher than serum prolactin concentration in men. No relationship was found between seminal prolactin concentration and sperm count (per ml or per ejaculate) or sperm motility. There was a significant and positive correlation between seminal prolactin concentration and fructose concentration. The relationship was of an exponential type. Seminal fructose increased progressively with seminal prolactin; however, for prolactin values above 850 microunits/ml, there was no further increase in seminal fructose. A similar relationship was observed between seminal prolactin concentration and volume of the semen specimens. These data support the concept that prolactin may play a key role in male fertility, perhaps by influencing the composition of the milieu where spermatozoa survive rather than by direct effect on spermatogenesis.

[Clinical experience of bromazepam for psychogenic impotence patients].

A clinical trial was performed with the tranquilizer Bromazepam on 39 patients undergoing diagnosis of psychogenic impotence, and the drug effect was evaluated according to the criteria based on our protocol. The protocol specifies 8 tests (1 for libido, 4 for erection, 2 for ejaculation, 1 for orgasm) which are scored according to an arbitrary logarithmic scoring system. At the end of the study the points made in the 8 tests were added to obtain the total score as the basis for evaluation of the overall drug effect. After treatment all tests showed an improvement, and the improvement in erection during masturbation, reflective erection, and condition of ejaculation was statistically significant. The total score also improved from 16.77 +/- 2.62 (mean +/- S.D.) before treatment to 11.42 +/- 1.96 after treatment, and the change was again statistically significant (P less than 0.05). The rate of satisfaction as a subjective symptom of improvement also increased from 25.38 +/- 4.40% to 39.10 +/- 4.53%. The results of the present study provide evidence to indicate that Bromazepam is beneficial for psychogenic impotence.

Some effects of orchiectomy, oestrogen treatment and radiation therapy in patients with prostatic carcinoma.

In patients treated for prostatic carcinoma with oestrogen, orchiectomy or external radiation, serum concentrations of testosterone, LH, FSH, prolactin, TeBG and oestradiol-17 beta as well as changes in sexual behaviour and mental mood were studied. Oestrogen treatment as well as orchiectomy reduced serum testosterone concentration to similar values. Neither totally nor subcapsularly orchiectomized patients responded to HCG stimulation. The free testosterone was 68% lower in oestrogen treated than in orchiectomized patients, probably due to a high TeBG concentration induced by oestrogens. patients oestrogen treated for less than 3 years and in whom the treatment had been withdrawn had normal serum testosterone and LH at follow-up. In contrast, low serum testosterone concentration and normal LH were found after oestrogen cessation in patients oestrogen treated for more than 3 years indicating reduced Leydig cell, and/or hypothalamic-hypophyseal function. In patients oestrogen treated for more than 3 years the serum testosterone concentration neither increased after oestrogen cessation nor decreased after orchiectomy. Absorbed testes doses during radiation treatment were measured from a few to more than 10 Gy but were reduced by about 50% if the gonads were protected by lead shields during anterior and posterior treatment sessions. Radiation may affect gonadal function as decreased serum testosterone concentration and increased LH, FSH were found after treatment. Sexual function was altered after oestrogen, orchiectomy and radiation treatment. Sexual activity and capability were distinctly better maintained after radiation than after orchiectomy or oestrogen treatment. Sixty-seven percent of the patients had coitus or masturbated after radiation treatment, all experiencing orgasm. Patients on oestrogen treatment or after orchiectomy had coitus/masturbation less often (17% in both groups). They also experienced orgasm less often (8% and 17% respectively). The group of patients on oestrogen treatment had a higher average score for depression than those treated with orchiectomy or radiation treatment.

Some considerations arising from the treatment of a patient with necrophilic fantasies in late adolescence and young adulthood.

Some psychoanalytic theories relating to necrophilia are reviewed, supplemented by other relevant theoretical abstracts. A psychoanalytic case of a young man with necrophilic fantasies is described, emphasizing separation anxiety, sadism and anal masturbation, all compellingly manifest in the transference and rooted in infantile trauma. The material is compared with that of other analysts working with similar pathology, then discussed in relation to perversions generally. Finally, it is argued that if the prognosis depends on whether core fantasies are amenable to analysis in the transference, then there may be therapeutic justification for keeping adolescence open by blocking the developmental move forward, on the grounds that these fantasies would quickly be rendered inaccessible under the normal repressive processes of young adulthood.

[Fracture of the penis: report of 10 cases and a review of 231 cases in Japan].

Ten cases of fracture of the penis were experienced between 1961 and 1981 at our Department. In all cases, the penis was swollen and distorted by a subcutaneous hematoma. No hemorrhagic discharge from urethra was seen. The patient's age was between 19 and 57 years. The causes of these fracture were coitus in 3 cases, manipulation in 4 cases (including one masturbation case), inverting in 2 cases, and the last case resulted from the patient's rolling over in bed. Surgical repair was performed on all patients. The causes, diagnosis, therapy and prognosis of 231 cases of fracture of the penis reported in the Japanese literature between 1934 and 1982 are reviewed.
