Laparoscopic surgery in ectopic pregnancy. A randomized trial versus laparotomy.

A randomized, prospective clinical trial was conducted to compare the efficacy of laparoscopic treatment with conventional conservative abdominal surgery for tubal pregnancy. Entry criteria were: size of the ectopic gestation less than 4 cm, hemodynamic stability, accessibility for laparoscopic treatment and a trained laparoscopist on duty. There was no difference between the groups regarding gestational duration, size and location of the ectopic gestation, or the mean preoperative hCG values. The groups differed with respect to total operation time (73 min for the laparoscopy group vs. 88 min for the laparotomy group), hospital stay (2.2 vs. 5.4 days) and convalescence period (11 vs. 24 days). The rates of elimination of hCG was similar in the groups, and there were no statistical difference in the rate of second intervention.

Transmission of epidemic dengue hemorrhagic fever in easternmost Indonesia.

In April 2001, a second suspected outbreak of dengue hemorrhagic fever in the easternmost region of Indonesia was investigated in Merauke, a town located in the southeastern corner of Papua, by the Indonesian Ministry of Health and the U.S. Naval Medical Research Unit No. 2. Principal case criteria of hemorrhagic disease provided for a study enrollment of 15 clinically acute and 37 convalescing subjects. Additionally, 32 comparable age/sex controls were selected from neighboring households. Laboratory diagnosis involved three testing methodologies: virus isolation by cell culture, a reverse transcriptase-polymerase chain reaction (RT-PCR) assay, and serologic assays. Antibody (IgM) to dengue virus was detected in 27% of the acute clinical cases, 30% of the convalescing cases, and only 3% of the matched controls. Dengue 3 was the only viral serotype detected from acute serum samples by the RT-PCR. The mean +/- SD age of the acute and convalescing cases was 7.8 +/- 5.4 years. Overall hospital records accounted for 172 suspected outbreak cases, all urban residents of Merauke with no recent travel history outside the area. The estimated outbreak-associated case fatality rate among all suspected dengue cases was 1.2%. A seven-year retrospective review of hospital records in Merauke showed negligible disease reporting involving hemorrhagic disease prior to the outbreak.

Crusted (Norwegian) scabies in two old-age home residents.

Scabies is commonly seen in hospitals, where it frequently affects geriatric and convalescent patients. The clinical features of the classic form of scabies are well recognised. Crusted (Norwegian) scabies, which is a hyperinfestation variant of scabies, is very contagious and can present as other dermatoses, thus delaying the correct diagnosis and management. Two residents of different old-age homes presented with hyperkeratotic skin eruptions, which later proved to be crusted scabies. In both cases, the scabies was initially misdiagnosed as eczema. The delay in making a correct diagnosis led to an outbreak of scabies in the old-age home in which one of the patients was residing.

Hand assisted laparoscopic bilateral nephroureterectomy in 1 session without repositioning patients is facilitated by alternating inflation cuffs.

PURPOSE: Transitional cell carcinoma is common and often multifocal in patients with uremia. We report a novel approach of simultaneous laparoscopic bilateral nephroureterectomy without the need to reposition patients. MATERIALS AND METHODS: Seven patients with uremia and organ confined transitional cell carcinoma of the upper urinary tract underwent simultaneous laparoscopic bilateral nephroureterectomy. Inflatable cuffs of an air tourniquet device were placed beneath each side of the back. Alternatively inflating the cuffs and rotating the operation table facilitated position changes during surgery. The incision for the hand assisted device during laparoscopy was also used for resecting the distal ureters in an open manner. Data were compared with those on 7 patients treated with simultaneous open bilateral nephroureterectomy. RESULTS: All laparoscopic bilateral nephroureterectomies were accomplished successfully without complications except for heart failure and atlectasis in a patient with a history of myocardial ischemia. Mean blood loss (218 versus 457 ml.), parenteral narcotic requirement (25 versus 60 mg. morphine equivalent), solid food oral intake (39 versus 83 hours), postoperative hospital stay (9 versus 14 days) and convalescence to normal nonstrenuous activity (3.5 versus 5.4 weeks) were superior in the laparoscopic bilateral nephroureterectomy group. Mean operative time was about 5 hours in each group. CONCLUSION: Our data show that laparoscopic bilateral nephroureterectomy facilitated by inflatable cuffs can be performed at 1 stage with a clear operative field and no repositioning problems.

[Transcatheter closure of atrial septal defects in the oval fossa].

BACKGROUND: We report five years' experience with transcatheter closure of defects in the oval fossa at Rikshospitalet University Hospital in Oslo, Norway. MATERIAL AND METHODS: We reviewed the results in 69 patients between 1.3 and 74 years of age (median 5.3), weighing median 20 kg (7.8-109), with pulmonary blood flow 2.40.74 (1.3-4.8) times systemic flow, and median diameter of the defect 18 mm (9-34). The patients were followed clinically for one year. RESULTS: In 69/77 (90%) the defect could be closed. None had a significant residual shunt. One patient showed a very small residual leak after one year. Two children, not yet seen after one year, had a small shunt after one month. The complication rate has been low, and we have not been able to demonstrate any sequelae. The hospital stay is two nights without any convalescence period. The results compare favourably to surgery. INTERPRETATION: The method was efficient and safe in all age and weight groups. The very limited total volume of such procedures indicates they should be centralised and only carried out by a small number of physicians.

Factors affecting glycosaminoglycan concentration in normal human plasma.

Acid Glycosaminoglycans (GAGs) were measured, in terms of hexuronic acid, following alkaline treatment and Ecteola chromatography, in plasma obtained from healthy volunteers, blood-donors, amateur soccer players and university students, and from hospitalized subjects at the end of their convalescence. Diurnal variations of plasma GAG concentration, with significant decrease during the morning, were obtained in students and patients, suggesting hormonal influences. Furthermore, moderate modifications of plasma GAG concentration were observed in students following cyclo ergometer exercise which were consistent in each subject with cortisol mediated changes. However, the absolute value of plasma GAG concentration appears to be depending on the physical training of the subject, being significantly higher in the soccer players and in the blood-donors than in the other groups of subjects, chiefly composed of sedentary individuals. The intramuscular connective tissue is then suggested to represent a main site of origin of plasma GAGs.

Treatment options for stress urinary incontinence.

Treatment options for stress urinary incontinence (SUI) in women are designed to prevent the involuntary loss of urine from the urethra during increases in intraabdominal pressure that occur during physical activity, coughing, or sneezing. Effective nonsurgical therapies include behavioral therapy (eg, bladder training, fluid and dietary modification) and drug therapy. Surgical therapy for this condition has existed for well over 100 years. Currently, approximately 200 different surgical procedures have been described. Because of the physiologic risks inherent in surgical procedures, the cost of hospitalization, and the loss of productivity during convalescence, surgeons continue to modify their techniques to improve efficacy, safety, and cost-effectiveness, and to minimize invasiveness. No single procedure or intervention is optimal for all patients. Having a variety of treatment options offers the possibility of tailoring therapy to the desires and needs of the individual patient. The key to an optimal therapeutic outcome is an accurate diagnosis combined with the selection of an appropriate intervention that is acceptable to the patient after balancing multiple factors.

[Catheter ablation in eight patients with Wolff-Parkinson-White syndrome].

After a follow-up study of 13-39 months, the clinical effect of catheter ablation in 8 patients with WPW syndrome and recurrence of SVT was reported. Five cases of left free-wall accessory pathway underwent catheter ablation attempts by direct current shocks delivered in coronary sinus. 3 of them were free from SVT without any medications after ablation, 1 case was improved. The other one was unsuccessful. Catheter ablation was attempted by delivery of shocks at coronary sinus orifice in 3 patients with posteroseptal accessory pathway. Arrhythmias were controlled without the need of drug therapy in 2 cases and the clinical symptoms were relieved in 1 case after shocks. We consider that the catheter ablation attempted to block the conduction of left free-wall and posteroseptal accessory pathway induces much less damages than surgical ablation and offers the advantages of shorter hospitalization, lower cost and a much shorter convalescence. The authors stated that it is a viable alternative to surgical ablation for those patients who are not responsive or allergic to drugs or those who are not willing to take a life-long period of medications. Some concerns for increasing the success rate and reducing the complications of ablation were also discussed.

Chlorpropamide-induced pure white cell aplasia.

We investigated the mechanism for isolated agranulocytosis and marrow pure white cell aplasia in an elderly man receiving 0.5 to 1.0 g per day of chlorpropamide (Chl) without other toxic drug exposure or overt systemic illness. Patient marrow revealed an absence of recognizable granulocytic precursors; megakaryocytes and erythroid precursors were normal. The WBC count was 1800/mm3 on admission with only 2% neutrophils; the absolute neutrophil count first exceeded 500/mm3 on the 17th day following cessation of Chl. A serum Chl level on admission was 100 micrograms/mL (acute phase, AP); no Chl was detected in serum (convalescent phase, CP) assessed on the 22nd hospital day. Antineutrophil antibodies were not detected, and T cell depletion failed to augment patient in vitro granulopoiesis. Patient AP serum produced potent complement-mediated inhibition (87% +/- 7%) of autologous granulocyte progenitors (CFU-GM) with minimal inhibition of erythroid (11% +/- 5%) or multipotent (5% +/- 4%) progenitor cells. Selective inhibition by patient AP serum of CFU-GM (74% +/- 11%) was also seen against two allogeneic marrows. Patient CP serum no longer inhibited (6% +/- 4%) autologous CFU-GM. Addition of Chl (5 to 120 micrograms/mL) to CP serum but not to control serum resulted in potent drug concentration-dependent complement-mediated inhibition of autologous and allogeneic CFU-GM. Inhibition of CFU-GM in the presence of Chl was no longer demonstrable following immunoabsorbent removal of IgG from patient serum. Patient serum in the presence of Chl had limited activity against morphologically recognizable marrow granulocytic precursors in a microimmunofluorescence assay. These results are most consistent with the development of Chl-dependent, selective antibody-mediated immune inhibition of granulopoiesis.

Antileukoprotease in patients with maxillary sinusitis.

Antileukoprotease, an inhibitor of leukocyte elastase, was studied in paired sera from 12 patients with maxillary sinusitis. The serum concentration of antileukoprotease was increased at the day of admission to hospital, compared with the serum concentration in convalescence sera. In purulent maxillary sinus secretions antileukoprotease was found in complex with leukocyte elastase, as shown by gel filtration. The findings suggest a local protective function of antileukoprotease in maxillary sinus.

[Laparoscopic appendectomy at the Department of Surgery in Jablonec nad Nisou].

The paper deals with experience regarding laparoscopic appendectomy at the Department of Surgery in Jablonec nad Nisou. Between May, 1992 and Jan. 1, 1994 the authors performed 82 operations. They are listed in a table according to pathological findings and the number of complications according to a described classification. In the author's opinion the advantage of the new method is, as compared with the classical open one, a shorter time of hospitalization and a substantially shorter period of convalescence.

[Radiographic evaluation of the clinical course and outcome in acute pneumonias].

The authors review the data of x-ray functional examinations of 525 patients with acute parenchymatoys pneumonia, 393 convalescents after pneumonia 3-6 months after the disease and analyze 480 case histories of patients, hospitalized for acute pneumonia 10 years ago. Development, x-ray picture, and resolution of the process are described. Besides traditional x-ray examination, roentgenography in oblique projections and pulmonary sonography are recommended in order to specify the anatomic changes in the lungs. Functional peculiarities of the involved and intact lungs were studied. Altogether 92.4% of convalescents are discharged from hospital with changed pulmonary pattern and, rather often, with infiltration of the parenchyma in the subpieural portions of the involved lobes and segments, as well as with disordered respiratory function of the lungs. Unfavorable outcomes in remote period after disease (pneumosclerosis, pneumocirrhosis, emphysema, chronic pneumonia) are observed mainly in subjects discharged from hospital before complete clinical cure.

[Evaluation of the direct cost of trochanteric fractures in the elderly].

PURPOSE OF THE STUDY: The cost effectiveness of trochanteric hip fractures in 1995 at Pitié-Salpétrière Hospital in Paris has been thoroughly analysed. The aim of this retrospective study was to identify the factors responsible for the variation in the treatment cost of those fractures. MATERIAL AND METHODS: Cost, Hospital stay, functional status, ASA score, mental status and surgical treatment were analysed in 74 patients aged over 60 years old. RESULTS: The mean cost per patient was 23,901 FF divided as follows: 8.5 per cent for preoperative care, 40.5 per cent for surgical procedures, 51 per cent for post-operative care. The mean hospital stay was 18 days. The cost of hospital personnel (44 per cent) and medical materiel (26 per cent) were the two main sources of hospital expenses beside medical investigations (11 per cent), hostelry (8 per cent), blood transfusion (6 per cent) and drugs (5 per cent). DISCUSSION: The duration of hospital stay was the only factor that affected statistically the mean cost per patient. Furthermore, factors related to the patient as age, sex, place of residence prior to admission, functional status, ASA score, mental status, had no influence on cost variation. CONCLUSION: Therefore, the best way to reduce the cost of trochanteric fractures treatment is to develop convalescence structures to avoid a lengthy and costly hospital stay and to minimize the abuse utilization of medical materials.

Short convalescence after inguinal herniorrhaphy with standardised recommendations: duration and reasons for delayed return to work.

OBJECTIVE: To provide a detailed description of post-herniorrhaphy convalescence. DESIGN: Prospective, descriptive, consecutive questionnaire case series. SETTING: Public university hospital, Denmark. PATIENTS: 100 consecutive patients treated for inguinal hernia. INTERVENTION: Elective open inguinal herniorrhaphy under local anaesthesia. One day convalescence for light/moderate and three weeks for strenuous physical activity was recommended. MAIN OUTCOME MEASURE: Duration of absence from work or main recreational activity. RESULTS: Overall median absence (including the day of operation) was 6 days (interquartile range 1-16). For unemployed patients it was 1 day (0-7), for patients with a light or moderate workload 6 days (3-12), and for those with a heavy workload 25 days (21-37). Among the 64 patients, who did not follow the recommendations, pain was contributory in 33 and advice from the general practitioner in 12. Pain was the main cause of impairment of activities of daily living. CONCLUSION: Well-defined recommendations for convalescence may, together with improved management of postoperative pain, shorten convalescence; they are essential in the evaluation of effects of different surgical techniques of herniorrhaphy on convalescence.

Salmonella outbreak in a nursing home.

We performed a retrospective review of an outbreak of Salmonella gastroenteritis that occurred in a community nursing home in 1987. Forty-four of 199 residents had a diarrheal illness; Salmonella heidelberg was isolated from the stool in 19 cases. Although the distribution of cases suggested a common source for the outbreak, no common source of infection could be demonstrated, despite extensive investigation. The clinical presentation of symptomatic individuals ranged from mild diarrhea to a severe gastrointestinal illness, and 26% of symptomatic, culture-positive patients required hospitalization. The median duration of pathogen excretion during convalescence in untreated residents was six weeks, but six patients who were treated with antibiotics shed S. heidelberg for a median duration of 14.5 weeks. We conclude that (1) the clinical spectrum of Salmonella gastroenteritis in nursing-home patients is variable, ranging from mild to severe illness; and (2) nursing-home Salmonella outbreaks impose a high economic burden because of expense of epidemiologic investigation, prolonged isolation measures, hospitalization for severely ill residents, and potential institutional closure.

Potassium retention in acute diarrhoeal disease.

Most children suffering from severe acute diarrhoeal disease had a low total body potassium (TBK) or admission to hospital. TBK fell even lower during the first 5 - 9 days in hospital, and returned to normal levels during the next 8 - 12 days. There was good correlation between potassium retention and stool weight. An increase in the potassium intake had little effect on the rate of potassium repletion.
