Prenatally diagnosed pelviureteric junction obstruction: a benign condition?

Renal function in neonates with antenatally diagnosed pelviureteric obstruction is usually well preserved and rarely deteriorates in the short term. To assess if function is maintained in the medium term, and, where not, if any factors are predictive of deterioration, the authors reviewed their cases (1985 to 1992) of antenatally diagnosed unilateral pelviureteric obstruction with renographic confirmation (type II and IIIb curves). Initial renographic assessment was performed at 1 to 3 months. Among the 160 cases, initial differential function exceeded 40% in 140 (87.5%), and these were managed expectantly with renographic follow-up, usually at 24, 48, and 72 months of age. The 20 patients with reduced differential function underwent early intervention (pyeloplasty, or trial of nephrostomy drainage in those with very poor function). Follow-up in those managed expectantly showed good differential function in 135 (96.5%), despite persistence of renographic obstruction in two thirds of the cases, more than half of which have had renographic follow-up beyond 4 years. Differential function decreased to below 40% in five patients (3.5%); this was not related to the type of renographic curve or severity of the hydronephrosis. The natural history of antenatally diagnosed pelviureteric obstruction continues to appear benign, and it may differ from that in older children who present clinically.

Gender differences on negative affectivity: an IRT study of differential item functioning on the Multidimensional Personality Questionnaire Stress Reaction Scale.

Item response theory methods were used to study differential item functioning (DIF) between gender groups on a measure of stress reaction. Results revealed that women were more likely to endorse items describing emotional vulnerability and sensitivity, whereas men were more likely to endorse items describing tension, irritability, and being easily upset. Item factor analysis yielded 5 correlated factors, and the DIF analysis, in turn, revealed differential gender mean differences on these factors. This finding illustrates how even in an essentially unidimensional scale, comparison of group mean differences can be affected by multidimensionality caused by item clusters that share similar content. Results do not support arguments that measures of negative affective dispositions "artificially" produce gender mean differences by focusing on specific selected content areas.

Regulation of adipocyte differentiation and function by polyunsaturated fatty acids.

A diet enriched in PUFAs, in particular of the n-3 family, decreases adipose tissue mass and suppresses development of obesity in rodents. Although several nuclear hormone receptors are identified as PUFA targets, the precise molecular mechanisms underlying the effects of PUFAs still remain to be elucidated. Here we review research aimed at elucidating molecular mechanisms governing the effects of PUFAs on the differentiation and function of white fat cells. This review focuses on dietary PUFAs as signaling molecules, with special emphasis on agonistic and antagonistic effects on transcription factors currently implicated as key players in adipocyte differentiation and function, including peroxisome proliferator activated receptors (PPARs) (alpha, beta and gamma), sterol regulatory element binding proteins (SREBPs) and liver X receptors (LXRs). We review evidence that dietary n-3 PUFAs decrease adipose tissue mass and suppress the development of obesity in rodents by targeting a set of key regulatory transcription factors involved in both adipogensis and lipid homeostasis in mature adipocytes. The same set of factors are targeted by PUFAs of the n-6 family, but the cellular/physiological responses are dependent on the experimental setting as n-6 PUFAs may exert either an anti- or a proadipogenic effect. Feeding status and hormonal background may therefore be of particular importance in determining the physiological effects of PUFAs of the n-6 family.

The effects of growth factors and serum on DNA synthesis and differentiation in thyroid cells in culture.

The effects of three putative growth factors and serum on [Me-3H]thymidine and Na125I incorporation into thyroid cell cultures have been examined. We found that serum and EGF could stimulate radioactively labelled thymidine incorporation into confluent cultures. However, both factors completely inhibited iodine uptake and organification at low concentrations. Insulin also stimulated [Me-3H]thymidine incorporation but had no adverse effect on thyroid differentiated function. TSH examined under the same conditions was not a growth factor but was essential to maintain differentiated functions. We conclude that thyroid growth and differentiation are not mutually exclusive processes. However, EGF and serum inhibit thyroid differentiated function at very low concentrations. Elucidation of the physiological role of these factors and their mechanism of action may lead to a greater understanding of thyroid hormone biosynthesis.

Regulation of lymphoid development, differentiation, and function by the Notch pathway.

The Notch pathway is gaining increasing recognition as a key regulator of developmental choices, differentiation, and function throughout the hematolymphoid system. Notch controls the generation of hematopoietic stem cells during embryonic development and may affect their subsequent homeostasis. Commitment to the T cell lineage and subsequent stages of early thymopoiesis is critically regulated by Notch. Recent data indicate that Notch can also direct the differentiation and activity of peripheral T and B cells. Thus, the full spectrum of Notch effects is just beginning to be understood. In this review, we discuss this explosion of knowledge as well as current controversies and challenges in the field.

Derepression of mouse beta-major-globin gene transcription during erythroid differentiation.

Functional analysis of the mouse beta-major-globin gene promoter has revealed a negative regulatory element (-100 to -250 bp) which represses promoter activity in mouse erythroleukemia (MEL) cells. Promoter activity is induced 14-fold during terminal differentiation of MEL cells. Three major in vitro binding sites for NF1 (-250 bp), GATA-1 (-212 bp), and a sequence at -165 bp (BB1) have been defined in this region. Site-directed mutagenesis of any one of the three sites resulted in a five- to sixfold up-regulation of promoter activity in uninduced MEL cells, but only three- to fourfold stimulation was observed from the mutant promoters during MEL cell terminal differentiation. This finding suggests that all three sites are required for repressor activity in uninduced MEL cells and that derepression occurs during MEL cell differentiation. BB1 DNA-binding activity decreases during MEL cell differentiation, suggesting a central role for this factor in modulating the effects of the repressor element. The BB1-binding factor also competes with the CCAAT-binding protein for binding the CCAAT motif. The fact that a reduced but significant stimulation of promoter activity during differentiation is observed in the absence of the repressor element raises the possibility that the BB1 factor also down-regulates transcription in undifferentiated MEL cells by displacing binding of CCAAT-binding protein to the proximal CCAAT motif.

DIFFERENTIAL RENAL FUNCTION STUDY: AN AID IN PREOPERATIVE EVALUATION OF HYPERTENSIVE PATIENTS.

Non-occlusive ureteral catheters were placed bilaterally in the renal pelves of 30 patients known to have hypertension. Samples of urine were collected under conditions of normal hydration, of urea-PAH-saline diuresis and, in some cases, mannitol diuresis. The samples were analyzed for indications of impaired flow of blood to the kidneys. Aortograms were obtained in all cases. This placement of non-occlusive catheters up into the renal pelves was felt to have caused only minimal disturbance of renal function, and was not accompanied by ureteral edema with the concomitant complications caused by occlusive catheters. Under conditions of normal hydration, leakage was insignificant. Of the 20 patients in whom urea-PAH-saline infusion revealed an ischemic pattern, 19 had an ischemic pattern under conditions of normal hydration. Since in the one exception an aortographic examination did not show a surgically reparable renal lesion, it may be inferred that the use of urea-PAH-saline diuresis is not essential in the preoperative evaluation of hypertensive renal disease. Correlation of the results of differential renal function studies and aortographic findings was possible in 19 of the 30 patients. Lack of correlation in the remaining 11 patients emphasized the importance of obtaining both types of study. Aortographic examination combined with differential renal function studies, using small ureteral catheters under conditions of normal hydration, should give the urologist a practical and yet accurate method of determining differential renal blood flow. If desired, further verification could be obtained by administering contrast medium and performing serial measurements of urine density.

Differential item functioning related to education and age in the Italian version of the Mini-mental State Examination.

BACKGROUND: Differential item functioning (DIF) exists when test item responses by members of different demographic groups are statistically different when controlling for ability. DIF may indicate item bias. Our objective was to determine whether items from the Italian Mini-mental State Examination (MMSE) had DIF related to educational attainment, age, gender and occupation. We were also interested in exploring the significance of DIF in screening tests. METHODS: In a two-stage study from Granarolo, Italy, residents over age 61 (n = 495) were evaluated with the Italian MMSE. Those with MMSE scores of 28 or lower were further evaluated for dementia. MMSE results were coded in 10 item bundles. We used ordinal logistic regression to determine whether item bundles had DIF. RESULTS: Six of the 10 MMSE item bundles had DIF in educational attainment subgroups. Four of these six bundles also had DIF related to age. Items that required literacy were much harder for those with lower educational attainment. CONCLUSIONS: DIF related to education appeared at as few as 3 years of formal schooling. These findings suggest cautious interpretation of data from studies using the Italian MMSE in populations with heterogeneous educational backgrounds. DIF is especially troublesome for two-stage studies that use screening tests.

Increased nephron volume is not a cause of supranormal renographic differential renal function in patients with ureteropelvic junction obstruction.

PURPOSE: Increasing clinical importance is being placed on the role of differential renal function (DRF) in the management of congenital ureteropelvic junction obstruction. Supranormal DRF of the hydronephrotic kidney on renal scan is a puzzling phenomenon and is hypothesized to be due to an increase in single nephron filtration or nephron volume without sound evidence. We studied the histopathological changes of hydronephrotic kidneys to determine whether glomerular hypertrophy underlies supranormal DRF. MATERIALS AND METHODS: We retrospectively evaluated the records of 3 females and 32 males with unilateral congenital hydronephrosis who underwent pyeloplasty. Mean patient age at operation was 12.6 months (range 0.1 to 144). Needle biopsies from 3 different sites at the lower pole of the kidney were performed during surgery. To evaluate the presence of glomerular hypertrophy, the maximal planar area of glomeruli was measured under light microscopy using an image analyzer. Tissue samples obtained from kidneys without a history of urinary tract disease at autopsy were used as controls. The mean glomerular areas of the patient and control groups were evaluated according to DRF and age. RESULTS: The mean glomerular area values of the patient group were smaller than those of the control group, except for 4 patients. The glomerular areas of the hydronephrotic kidneys with supranormal DRF were not significantly different from those of the control group. Instead, the probability of larger renal glomeruli increased with decreasing DRF (p = 0.1155). CONCLUSIONS: Increased nephron volume can be discounted as a cause of supranormal DRF.

Overview of quantitative measurement methods. Equivalence, invariance, and differential item functioning in health applications.

BACKGROUND: Reviewed in this article are issues relating to the study of invariance and differential item functioning (DIF). The aim of factor analyses and DIF, in the context of invariance testing, is the examination of group differences in item response conditional on an estimate of disability. Discussed are parameters and statistics that are not invariant and cannot be compared validly in crosscultural studies with varying distributions of disability in contrast to those that can be compared (if the model assumptions are met) because they are produced by models such as linear and nonlinear regression. OBJECTIVES: The purpose of this overview is to provide an integrated approach to the quantitative methods used in this special issue to examine measurement equivalence. The methods include classical test theory (CTT), factor analytic, and parametric and nonparametric approaches to DIF detection. Also included in the quantitative section is a discussion of item banking and computerized adaptive testing (CAT). METHODS: Factorial invariance and the articles discussing this topic are introduced. A brief overview of the DIF methods presented in the quantitative section of the special issue is provided together with a discussion of ways in which DIF analyses and examination of invariance using factor models may be complementary. CONCLUSIONS: Although factor analytic and DIF detection methods share features, they provide unique information and can be viewed as complementary in informing about measurement equivalence.

Role of Cbfa1 in osteoblast differentiation and function.

Among the multiple cell lineages whose differentiation is affected by a runt-related gene the osteoblast is a relative newcomer. Molecular biology, developmental biology and mouse and human genetic studies have demonstrated that Cbfa1 is a critical regulator of osteoblast differentiation in vertebrates. Cbfa1 is not only a differentiation factor but also a regulator of bone formation by differentiated osteoblasts beyond development. Thus, Cbfa1 controls osteogenesis at multiple stages.

[Effect of soluble anti-CD47 monoclonal antibody on the differentiation and function of human dendritic cells].

OBJECTIVE: To explore the influences of anti-CD47 monoclonal antibody (mAb) B6H12 on the differentiation and function of cultured dendritic cells (DCs). METHODS: Human peripheral monocyte derived DCs were propagated with granulocyte-macrophage colony-stimulating factor (GM-CSF) and lipopolysaccharide (LPS) plus interleukin-4 (IL-4) in the presence or absence of soluble B6H12. Flow cytometry was used to analyze the immunophenotypes of cells, semi-quantitative RT-PCR and ELISA methods to analyse the mRNA and protein expression levels of interleukin-12 (IL-12). The antigen-presenting function of DCs was determined in one-way mixed leukocyte reaction by Brdu-ELISA technique. RESULTS: There was a high expression rate (94% approximately 98%) of CD47 molecules in DCs. The cell immunophenotypes in B6H12 mAb treated and untreated DC groups were as follows: CD80(+) (68.14 +/- 7.41)% vs (89.17 +/- 8.59)%; CD86(+) (67.33 +/- 4.71)% vs (87.27 +/- 3.56)%; CD83 (40.08 +/- 14.80)% vs (72.77 +/- 8.68)%; CD1a(+) (66.45 +/- 4.06)% vs (95.93 +/- 3.03)%; and HLA-DR (40.67 +/- 13.48) vs (98.97 +/- 1.01)%, respectively. The expression levels of mRNA and protein of IL-12 were strongly inhibited in B6H12 mAb treated DC (P < 0.01). The quantity of Brdu was also lower in B6H12 mAb treated DC than that in untreated DCs (P < 0.01). CONCLUSION: The anti-CD47 McAb exerts a negative effect on the maturation and functions of cultured DCs.

Test bias in a cognitive test: differential item functioning in the CASI.

Assessment of test bias is important to establish the construct validity of tests. Assessment of differential item functioning (DIF) is an important first step in this process. DIF is present when examinees from different groups have differing probabilities of success on an item, after controlling for overall ability level. Here, we present analysis of DIF in the Cognitive Assessment Screening Instrument (CASI) using data from a large cohort study of elderly adults. We developed an ordinal logistic regression modelling technique to assess test items for DIF. Estimates of cognitive ability were obtained in two ways based on responses to CASI items: using traditional CASI scoring according to the original test instructions as well as using item response theory (IRT) scoring. Several demographic characteristics were examined for potential DIF, including ethnicity and gender (entered into the model as dichotomous variables), and years of education and age (entered as continuous variables). We found that a disappointingly large number of items had DIF with respect to at least one of these demographic variables. More items were found to have DIF with traditional CASI scoring than with IRT scoring. This study demonstrates a powerful technique for the evaluation of DIF in psychometric tests. The finding that so many CASI items had DIF suggests that previous findings of differences between groups in cognitive functioning as measured by the CASI may be due to biased test items rather than true differences between groups. The finding that IRT scoring diminished the impact of DIF is discussed. Some preliminary suggestions for how to deal with items found to have DIF in cognitive tests are made. The advantages of the DIF detection techniques we developed are discussed in relation to other techniques for the evaluation of DIF.

[Effects of vascular endothelial growth factor on differentiation and function of dendritic cells generated from CD34+ hematopoietic progenitor cells in vitro].

OBJECTIVE: To investigate the effects of vascular endothelial growth factor (VEGF) on differentiation and function of dendritic cells derived from CD34+ hematopoietic progenitor cells. METHODS: After isolation from umbilical cord blood with a high-gradient magnetic cell sorting system (MACS), the CD34+ cells were cultured with a cocktail cytokines for differentiating into dendritic cells (DC). The cells were stimulated by VEGF (25 ng/ml) either at the beginning or at day 9 of culture. Kinetics analysis of cell proliferation was performed during the process of cell culture, and the expression of DC differentiation antigens including CD1alpha, CD83, CD80, CD54 and HLA-DR was examined by flow cytometry. DC function was evaluated by the ability to induce proliferation of allogeneic T cells in mixed lymphocyte reaction (MLR) assay, and the production of IL-12 by ELISA. RESULTS: VEGF added at day 1 of culture induced an increase of total cell numbers by (1.51 +/- 0.23)-folds (P = 0.001). VEGF added at the initial but not the late stage of culture could dramatically down-regulate the expression of CD1a [(33.00 +/- 2.12)% vs (81.20 +/- 6.93)%], CD83 [(42.23 +/- 1.15)% vs (87.98 +/- 7.97)%], CD80 (42.93 +/- 1.32)% vs (94.53 +/- 0.87)%], and HLA-DR [(37.93 +/- 5.30)% vs (74.15 +/- 3.74)%], while obviously up-regulate the expression of CD14. Moreover, the inhibitory effect of VEGF on DC function was confirmed by a reduced ability to induce proliferation of allogeneic T cells and production of IL-12 (P < 0.01). CONCLUSIONS: VEGF could induce the expansion of hematopoietic progenitor cells and inhibit at the early stage their differentiation into mature DC.

Recursive graphical construction of feynman diagrams in straight phi(4) theory: asymmetric case and effective energy

The free energy of a multicomponent scalar field theory is considered as a functional W[G,J] of the free correlation function G and an external current J. It obeys nonlinear functional differential equations which are turned into recursion relations for the connected Green's functions in a loop expansion. These relations amount to a simple proof that W[G,J] generates only connected graphs and can be used to find all such graphs with their combinatoric weights. A Legendre transformation with respect to the external current converts the functional differential equations for the free energy into those for the effective energy Gamma[G,Phi], which is considered as a functional of the free correlation function G and the field expectation Phi. These equations are turned into recursion relations for the one-particle irreducible Green's functions. These relations amount to a simple proof that Gamma[G,J] generates only one-particle irreducible graphs and can be used to find all such graphs with their combinatoric weights. The techniques used also allow for a systematic investigation into resummations of classes of graphs. Examples are given for resumming one-loop and multiloop tadpoles, both through all orders of perturbation theory. Since the functional differential equations derived are nonperturbative, they constitute also a convenient starting point for other expansions than those in numbers of loops or powers of coupling constants. We work with general interactions through four powers in the field.

Pelviureteral junction obstruction: correlation of renal cell apoptosis and differential renal function.

PURPOSE: We evaluated the relationship between renal biopsy changes and preoperative and postoperative renal scans in 29 male and 14 female infants with prenatal severe hydronephrosis and unilateral ureteropelvic junction obstruction. We also verified on immunohistochemical studies glomerular changes, degeneration of the epithelium of the proximal tubules, interstitial fibrosis and inflammation and apoptotic nuclei. MATERIALS AND METHODS: In the children, all with prenatal diagnosis of hydronephrosis, ureteropelvic junction obstruction was diagnosed with 99mtechnetium mercaptoacetyltriglycine renal scan performed in all patients at ages 4 to 6 weeks to establish baseline differential renal function. All patients underwent renal biopsies at the time of pyeloplasty. The biopsy samples were examined with histological and immunohistochemical methods for antigens associated with apoptosis, such as clusterin, CD95, TDAG51 and bcl-2. Renograms were performed 3 months after surgical repair. RESULTS: The male-to-female ratio was 2.1:1. There was no difference between males and females in regard to baseline differential renal function of the affected kidney. All biopsy samples confirmed degeneration of the epithelium of the proximal tubules, interstitial focal fibrosis was found in 4 samples, mild chronic inflammation with lymphoid aggregates in 4 and focal Bowman's space dilatation in 1. No specimen demonstrated apoptotic nuclei as confirmed by immunohistochemical study which showed the presence of bcl-2 and absence of CD95, TDAG51 and clusterin, probably because there was no dysplasia in the samples examined. CONCLUSIONS: These results indicate absence of apoptosis in the kidney with preoperative uptake less than 40% and minimal histological changes.

Is furosemide administration effective in improving the accuracy of determination of differential renal function by means of technetium-99m DMSA in patients with hydronephrosis.

It has been suggested that determination of differential renal function (DRF)using technetium-99m dimercaptosuccinic acid (DMSA) may lead to overestimation of the function of an obstructed kidney in patients with excretion abnormalities owing to pelvic retention of DMSA. Recently published guidelines have recommended use of furosemide injection when calculating DRF in these particular patients. The aim of this study was to evaluate the effect of diuretic administration on the determination of DRF using DMSA scintigraphy. For this purpose, 19 patients, aged from 1 month to 69 years (19.4+/-24.8 years, 15 males, 4 females), in whom pelvic retention had been documented by diuresis scintigraphy were included in the study. DMSA scintigraphy was performed in all patients 2-4 h after injection and six planar images were obtained. Immediately after the standard study, furosemide was injected in all patients, and 30 min later the same number of images was obtained. DRF was calculated for each patient and from each DMSA study by using the arithmetic mean method. The difference between two studies (DMSA scintigraphy with or without furosemide administration and diuresis scintigraphy) was expressed as a percentage of the mean value of the two studies (the DRF value of the affected kidney was thus taken into account). The mean of the differences represented the systemic bias and the SD of the mean of the differences represented the precision of the technique. In seven patients, diuresis renography revealed an obstructive curve pattern. We did not observe any significant difference between the DRF values obtained before and after diuretic administration ( P>0.5). When we compared DRF values obtained from standard and from diuretic DMSA studies, the mean of the differences was only 0.3% and the SD was only 1.2%. There was also no significant difference in DRF between patients with the obstructive curve pattern and those with a dilated renogram curve pattern (with washout of activity during the second phase of the study) ( P>0.1). In conclusion, we did not observe interference from pelvicalyceal activity in patients with documented pelvic retention and infer that diuretic administration may be a useless intervention for improving the accuracy of determination of DRF.
