A new functional brace for the treatment of Colles' fractures.

A new prefabricated brace for the functional treatment of Colles' fractures has been developed. It is applied at fracture reduction and maintains fracture position by the application of three-point loading. In a prospective randomized clinical trial treating 85 displaced Colles' fractures, with blind independent follow-up, the brace gave better functional results than conventional plaster treatment. The improved function was apparent up to 6 months after injury. Finger function and pinch strength were also better in the brace-treated patients. Anatomical results were similar in the two groups.

Continuous blood-letting for congestion in replantation of the amputated finger.

Experimental and clinical studies have been made on the function of the venous system in the replantation of the contused severed finger. In experiments on rats replantation of severed limbs was more successful in a group treated by continuous blood-letting and anastomosis of the artery alone, compared with controls in which only the artery was anastomosed. Histologically, in the first group there was less congestion, thrombus-formation, oedema and exudative haemorrhage in the initial stage; in the reparative phase there was less granulation and the degree of muscular degeneration was very slight. Clinically, contused, severed fingers were successfully replanted when venous anastomosis was impossible or incomplete, by using the fish-mouth incision for continuous blood-letting. The resulting scar caused no problem aesthetically or functionally.

Venous occlusion plethysmography in patients with cold related symptoms after digital salvage procedures.

Fourteen patients who had undergone microsurgical salvage of one or more digits were interviewed and examined. An assessment of sensitivity to cold is defined as cold intolerance and correlated with nerve function and arterial inflow measured by venous occlusion plethysmography. No relationship between arterial inflow and cold intolerance nor nerve function was found. There was, however a tendency for worsening cold intolerance to be associated with poorer nerve function. Half the salvaged digits had arterial inflow greater than the contralateral control digit.

The relationship between cold intolerance and cutaneous blood flow in digital replantation patients.

Twenty-one patients with replanted digits were examined. An assessment of cold symptoms relating to their replants was made and the pulp two-point discrimination measured. Cutaneous blood flow was then monitored using a laser Doppler flowmeter. Recordings were taken at room temperature and following cooling. In all cases but one flow decreased with lowering of temperature in both replanted and normal digits. In eleven cases (52%) replant flow fell to noticeably lower levels compared to normal digits, but in the remainder this difference was not apparent. The presence or absence of cold intolerance did not correlate with flow levels or with two-point discrimination measurements.

Postburn nailfold retraction: a reconstructive technique.

Postburn nailfold retraction often results in nail deformity and loss of distal interphalangeal joint flexion. A technique of reconstruction of the nailfold using a proximally based transposition flap is described in a patient with postburn nailfold retraction of the left thumb, index and middle fingers.
