[Modified surgical technique for treating hepatic echinococcosis].

The authors present modified operational technique in hepatic echinococcosis. Echinococcus in 14 patients was removed by either peri-cystic excision or hepatic lobe excision with the closure of blood inflow into the liver by construction the blood vessels in hepato-duodenal ligament for 20 minutes. All patients survived. Biliary fistulas lasting for a few days up to 3 weeks were the only complications encountered. Presented modified operational technique of surgery is safe; it shortens the duration of surgery and prevents major intraoperative haemorrhages.

[Hepatic alveolar echinococcosis].

Alveolar echinococcosis (AE) is an uncommon parasitic disease that is caused by the intrahepatic growth of Echinococcus multilocularis larvae. Eastern France is endemic for AE. A wild cycle allows the parasite to subsist in nature. The adult worm develops in the small intestine of foxes. The eggs of the taenia are dispersed on the ground by the foxes' stools. Rodents are infested by eating contaminated vegetables and larvae develop in the liver. Humans, accidental intermediate hosts, become infested due to either eating contaminated wild fruits, or by touching foxes or dogs. The human lesions of AE are often compared to a liver cancer which progress very slowly, causing necrosis and fibrosis. Complications invariably occurred during the follow-up. The most frequent are biliary complications, bleeding related to portal hypertension or Budd Chiari syndrome. During the last years, important progress has been made in many fields concerning AE, particularly in the immunology, epidemiology and therapy of this disease. The sole efficient treatment is partial hepatectomy. Recently, liver transplantation have been successfully performed for incurable AE.

[Intraoperative ultrasonic study in surgery of hepatic echinococcosis].

The authors analyse the results of intraoperative ultrasonic examination (IOUSE) of the liver in 31 patients with echinococcosis of the liver; 21 of them had multiple hydatid cysts in the liver. The authors came to the conclusion that IOUSE is a highly informative method for noninvasive diagnosis in surgery of the liver. IOUSE is indicated when information must be obtained during the operation concerning the location of invisible and nonpalpable pathological structures, the borderlines of their extension, the anatomical relations with the vascular-ductal structures of the liver. Exact echographic localization of these structures and their grading ensure safety of various types of operations in this disease.

[Echinococcosis of the spinal canal].

Among various forms of echinococcosis its location in the spinal canal and bone marrow affection are the rarest. Intravital diagnosis is complicated. As a rule, the disease manifests primarily with radicular syndrome followed by marrow compression, asymmetric compression of the roots of cauda equina and pelvic disfunction. The cerebrospinal fluid becomes xanthochromic with hyperalbuminosis and albuminocytologic dissociation. Myelography shows filling defect. Spinal x-ray examination detects specific changes of the vertebra and ossification foci. Unfavorable epidemiological situation may be indicative of probable infection and prompt a valid diagnosis. Surgery is advocated as the only adequate treatment.

[Echinococcosis of the heart].

From 1971 through 1987 six patients with echinococcus of the hear have been operated. In four of them the hydatid cyst was localized in the right portion of the heart and in two in the left ventricle. Left thoracotomy was used for the operation of the former four patients, whereas the hydatid cysts localized in the left ventricle were operated under bypass circulation. None of the operated patients died. The diagnosis was established by using computer tomography, which provided exact localization of the hydatid cyst and determined the operative approach. Echinococcosis of the heart is rare and creates diagnostic difficulties. This diagnosis is an absolute indication for operative intervention which under contemporary conditions should be performed by use of bypass circulation.

[Use of the CO2 laser in the surgical treatment of complicated echinococcosis of the liver].

The article discusses experience in the use of high-energy CO2 laser in the treatment of 108 patients with complicated forms of echinococcosis of the liver. The diagnostic efficacy of instrumental methods of examination is determined. The most informative are ultrasonic echolocation (96.8%) and computered tomography (100%). They allow exact localization of the cyst, which is important in the choice of the operative approach. The use of the laser for excising the fibrous capsule and treating the residual cavity makes is possible to: abandon the traditional methods for completing echinococcectomy (drainage of the residual cavity, marsupialization, capitonnage), exclude suppuration of the residual cavity, reduce markedly the period of the patient's hospitalization.

Purification of antigen for serodiagnosis of echinococcosis.

Antigen for serodiagnosis of Echinococcosis is purified by chromatography on DEAE-Cellulose and Sephadex G-200 from hydatid cyst fluid. The antigen is electrophoretically pure and found sensitive and specific for Echinococcus granulosus. The antigen is thermostable and is apparently a lipo protein.

[Reserves of pulmonary ventilation and gas exchange in patients with pulmonary echinococcosis].

An examination of 33 patients has shown that complicated course of Echinococcosis of the lungs and their multiple lesions results in certain exhaustion of reserves of the external ventilation system. A conclusion is made on preference of performing organ-preserving and organ-sparing operations, on necessity of dispensary observations and physical rehabilitation of patients operated upon for complicated forms and multiple lesions of the lungs.

[Costal echinococcosis. Report of one case and review of the literature (author's transl)].

Following the report of a case of costal echinococcosis, a review of the published literature showed that 32 identical cases have been the subject of publications in the Occident. A further 6 cases were found in the records of the University Hospitals in Teheran, which brings the total to 39 published cases, including our patient, known at the present time. It is important to make the diagnosis in these cases so that the surgeon can take the necessary precautions to avoid the anaphylactic shock which can follow dissemination of the parasite.

[Surgical treatment of pulmonary echinococcosis].

Operation with the use of a laser for resection of the external fibrous capsule with re-expansion of the remaining cavity in patients with echinococcosis of the lungs is more effective than operation by capitonnage of the cavity. The main advantages of laser operations were the absence of closed cavities, deformities of bronchi, atelectasic areas of tissue around the fibrous cavity, and disorders of capillary blood flow and bronchial patency. All this allowed poor results of the operation and fatal outcomes in the main group of patients to be avoided.

[An assessment of the clinical status of patients with alveolar echinococcosis by using statistical processing methods. 1. An assessment of the stage of the disease by cluster analysis].

The clinical condition was assessed in 44 patients with alveolar echinococcosis, using methods of mathematical analysis of 10 basic routine laboratory parameters reflecting protein-forming liver function and immune disturbances. The data were processed on a "NOVA-2" [correction of "HOBA-2"] computer using Fortran software on taxonomy 7 classes of conditions were determined, which reflect the progressing severity of the pathological process and correspond to three stages of the disease: uncomplicated, complicated and terminal. A high clinical value of the method has been demonstrated for the evaluation of the patient's condition and determination of the treatment tactics as well as its importance for the creation of a clinical data bank in the invasion focus.

[Changes in the level of immunoglobulins and other serum proteins in hepatic echinococcosis].

The humoral immunity and the protein-synthesizing function of the liver of 35 patients with liver echinococcosis were studied. No humoral immunity deficit was found. Statistically significant abnormalities in a number of proteins were reduced, with evidence of reduced protein-synthesizing liver function. Inferences are made on the anesthesia, resuscitation and operative interventions, in connection with the results of the study.

[Biological diagnosis of echinococcosis].

The authors review the laboratory techniques used for diagnosis of echinococcosis (IFL, IHA, IEP, ES, ELISA) with emphasis on the value and standardization of the antigens utilized. The specificity of these tests makes it possible to differentiate hydatidosis from alveolar hydatid disease.

IgE-dependent humoral immune response in Echinococcus multilocularis infection: circulating and basophil-bound specific IgE against Echinococcus antigens in patients with alveolar echinococcosis.

Clinical symptoms of immediate-type hypersensitivity (ITH) and specific IgE against Echinococcus granulosus antigens are frequently present in patients with hydatid cysts. In alveolar echinococcosis (AE) due to E. multilocularis, clinical manifestations related to ITH have never been reported. The IgE-dependent humoral immune response was evaluated in 30 patients with AE. Circulating specific IgE (sIgE) were determined with two different methods of radio-allergo-sorbent test. Serum sIgE were determined sequentially in 18 patients over 15 months. Specific IgE bound to circulating basophils were assessed with two tests in vitro, measuring specific degranulation and histamine release. The respective abilities of E. granulosus and E. multilocularis antigens to reveal bound and circulating IgE antibodies were also assayed. Despite the absence of clinical symptoms of ITH and the frequent lack of circulating sIgE, an immunological response involving IgE was always present in human AE: basophil-bound sIgE were revealed in every patient by histamine release and degranulation tests; these tests were constantly negative in control subjects. Echinococcus granulosus extracts were more effective for detecting circulating sIgE; however E. multilocularis antigenic preparation induced a histamine release significantly higher than E. granulosus extracts. These results suggest that IgE-dependent humoral immune response could play a role in the host-parasite relationship in AE. Moreover, the sensitivity of the tests used to detect basophil-bound sIgE was higher than that of the usual serological tests, and the basophil degranulation test could be used to confirm diagnosis of AE in endemic countries.
