The effects of Hochu-ekki-to in patients with atopic dermatitis resistant to conventional treatment.

Hochu-ekki-to is one of Kampo formulas containing Astragalus root, liquorice, jujube, ginseng, white Atractylodes rhizome, fresh ginger and Chinese angelica root. This formula has been identified as an effective drug to improve the function of digestive systems and to strengthen defensive systems against many kinds of infections. We examined serum IgE levels and eosinophils before and after the administration of Hochu-ekki-to in patients with recalcitrant atopic dermatitis. The increased numbers of eosinophils was statistically decreased after 3 months' use of this formula. Serum IgE levels showed a tendency to decrease after the administration of this substance.

Diet and Japanese herbal medicine for recalcitrant atopic dermatitis: efficacy and safety.

We have been utilizing Kampo, a Japanese herbal medicine, together with lifestyle advice, for recalcitrant atopic dermatitis. To estimate the safety and efficacy of the treatment, we administered Kampo formulas to patients in whom conventional treatment failed to improve symptoms, along with dietary advice recommending traditional Japanese food. The therapeutic effects of Kampo formulas were assessed in 95 patients with recalcitrant atopic dermatitis who consulted our clinic from January to June, 2000. The overall result was 'markedly effective" in 19 patients (20%), "moderately effective" in 33 (35%), "slightly effective" in 36 (38%) and "ineffective" in four (4%). Three patients dropped out of the study. No adverse reactions in laboratory data were noted in examined patients. The most commonly used formula was Hochu-ekki-to containing Astragalus root, liquorice, jujube, ginseng, white Atractylodes rhizome, fresh ginger and Chinese Angelica root. Diet and Japanese herbal medicine are thought to be useful as an alternative therapy of intractable atopic dermatitis.

[Effects of extracts of Chinese medicines on Ganoderma lucidum in submerged culture].

Effects of water and ethanol extracts of 10 Chinese medicines, such as Astragalus membranaceus, Coix lachryma-jobi, etc., on biomass and exopolysaccharide of Ganoderma lucidum were studied by submerged culture. The results showed: water extracts of all medicines can improve the culture of G. lucidum except of A. membranaceus, ethanol extracts of C. lachryma-jobi, Dioscorea opposita, Codonopsis pilosula, and Achyranthes bidentata( < 187.5g Medicine/L substrate) can also increase the biomass of G. lucidum, but the ethanol extracts of Angelica sinensis, Dendrobium nobile check the growth of G. lucidum. The production of exopolysa-ccharide can be improved by all the Chinese medicines and their dosage used in this experiment, Although A. sinensis, D. Nobile check the growth of G. lucidum, they could stimulate the secretion of exopolysaccharide in lower dosage. It is concluded that some Chinese medicines, such as C. lachryma-jobi, D. opposita, C. pilosula, etc. can be processed by the fermentation of G. lucidum, and bio-active compound can be produced by adding appropriate Chinese medicine in the substrate to culture G. lucidum.

[Relationship between different renal functional state with therapeutic effect on diabetic foot ulcers].

OBJECTIVE: To study the relationship between renal functional state and the therapeutic effect and prognosis of foot ulcers in the patients with diabetic mellitus. METHODS: The renal function was evaluated in term of glomerular filtration rate, microalbuminuria, proteinuria, blood urea nitrogen (BUN) and creatinine (Cr) levels in 126 patients with I-V class diabetic foot ulcers (according to Wagner classic standard) before the treatment, and then these patients were divided into 1-5 classes (according to Mogenson standard) and given systemic treatment and local debridement, with astragalus for topical application. The time of growth of granulation tissue (GT), the time of healing (HT), the amputation rate and mortality were observed. RESULTS: GT and HT of ulcer prolonged with worsening of diabetic nephropathy regardless of the disease phase of foot ulcers, especially the GT and HT of foot ulcers were significantly longer in IV and V phases of diabetic nephropathy than those of III phase diabetic nephropathy though the conditions of their foot ulcers were about the same. GT and HT in all the patients with the foot ulcers in the similar condition exhibited significantly positive linear correlation with the severity of diabetic nephropathy (r(1)=2.344 and r(2)=2.563, respectively, both P<0.05). The mortality of I-III phase diabetic nephropathy was significantly lower than that of IV and V phase diabetic nephropathy when the foot ulcers of these patients were of the same extent (P<0.05). CONCLUSION: A worsening of renal function would affect the treatment effect and prognosis of foot ulcers in the patients with diabetic foot ulcers, implicating that it is very important to improve the renal function in the treatment of patients with diabetic foot ulcers.

[Angiogenesis promoting effects of Chinese herbal medicine for activating blood circulation to remove stasis on chick embryo chorio-allantoic membrane].

OBJECTIVE: To observe the angiogenesis promoting effects of clinical common used Chinese herbal medicines (CHM) for activating blood circulation to remove stasis on chick embryo chorio-allantoic membrane (CAM). METHODS: Chicken CAM model was established and mice blood serum containing different kinds of medicines, including Radix Peaoniae rubra, Radix Angelicae sinensis, Flos Carthami, Rhizoma Chuanxiong, Radix Salviae miltiorrhizae, Astragalus membranaceus, and their complex prescriptions, Danggui Buxue Decoction, Xuefu Zhuyu Decoction, Xiongshao Capsule, was applied on it respectively to observe the condition of angiogenesis 72 hrs after incubation. Besides, the normal saline group, blank serum group, blank group and basic fibroblast growth factor (bFGF) group were set up for control. RESULTS: All the CHM applied and bFGF had the CAM angiogenetic promoting effect, among them, Radix Salviae Miltiorrhizae and the three complex prescriptions showed better effects than the three negative control groups in capillary formation and count, with the efficacy similar to that of bFGF. The effect of complex prescriptions was superior to that of single herb except Radix Salviae miltiorrhizae. CONCLUSION: Radix Salviae miltiorrhizae, Danggui Buxue Decoction, Xuefu Zhuyu Decoction and Xiongshao Capsule have good angiogenesis promoting effect on CAM. This study elucidated, from a certain aspect, the mechanism of action of CHM on ischemic diseases, and unfolded the scientific evidence of applying complex prescription.

[Effects of Ailing Granule on immuno-reconstruction in HIV/AIDS patients].

OBJECTIVE: To explore the possible mechanism of Ailing Granule (AG, a Chinese herbal preparation for nourishing Qi and activating blood circulation) in intervening HIV/AIDS. METHODS: Twenty-one HIV/AIDS patients were orally administered with AG (mainly composed of Fructus Ligustri lucidi, Radix Scutellariae, Radix Astragalus and Eupolyphaga et polyphage) by 20g, twice a day for 4 months. Their symptoms and signs were scored, T cell subgroup (CD3, CD4, CD8) and interleukin-2 (IL-2) were detected with flow-cytometry, and viral load determined with RT-PCR, as well as some indexes for safety evaluation, including ALT, Cr and BW were observed before and after treatment. RESULTS: After treatment, the symptom and sign score of patients lowered, among them the scores of fatigue, anorexia, spontaneous sweating and skin rash reduced with difference statistically (P < 0.05); the total effective rate was 61.9%; CD4 count and IL-2 level increased from 308.29 +/- 150.66/microl and 13.19 +/- 5.93 ng/L to 336.50 +/- 148.94/micro1 and 15.14 +/- 5.14 ng/L respectively( P < 0.05); while the viral load lowered but showed no significant difference. All the indexes of safety measured kept unwavering during treatment(P > 0.05). CONCLUSION: AG could significantly alleviate the symptoms of HIV/AIDS patients, improve their immune function, inhibit HIV reproduction to a certain extent or keep it stable. No obvious toxic or adverse reaction was seen.

Swainsonine-induced high mountain disease in calves.

Swainsonine, an indolizidine alkaloid in the locoweeds (certain species of the Astragalus and Oxytropis genera), was fed to young Holstein bull calves in their milk at high elevation (3090 m), and the incidence of high mountain disease (HMD) was compared with locoweed-fed and control calves. Five of 5 calves fed swainsonine and 5 of 5 calves fed fresh Oxytropis sericea showed outward signs of HMD, which included edema under the jaws, throat area and brisket and gross and microscopic lesions of HMD and locoweed poisoning. Grossly there were HMD lesions, including congestion of the liver, right ventricular hypertrophy, and dilatation and excessive fluid in the thoracic and abdominal cavities. Microscopically, the severe centrilobular lesions in the liver, edema of the pulmonary artery, severe edema and/or fibrosis of the roof of the right atrium were suggestive of HMD. The mild to moderate neurovisceral cytoplasmic foamy vacuolation of selected tissues and cerebellar neuroaxonal dystrophy in all calves fed swainsonine and locoweed were indicative of locoweed poisoning. In control calves, 1 of 6 showed equally severe outward, gross, and microscopic lesions of HMD, but none had any lesions indicative of locoweed poisoning. The ratio of right ventricle to left ventricle wall weights were significantly higher (P = 0.033) for the swainsonine-fed calves (1.4) and the locoweed-fed calves (1.3) compared to the controls (0.9). Scores indicating the severity of HMD from observations prior to necropsy were significantly higher for the swainsonine and locoweed-fed calves compared to controls (P = 0.032).(ABSTRACT TRUNCATED AT 250 WORDS)

[Reconstructive surgery of malunited joint fractures of the foot].

Innovative diagnostic methods such as the CT scan, MRI and 3rd-dimensional models of the foot and free-flap surgery to improve the soft tissues in the foot have all introduced new dimensions in reconstructive foot surgery. Of the foot bones only intra-articular fractures of the astragalus should be corrected because of its key position and the involvement of three joints. In double and triple arthrodesis, the axes and lengths of the medial and lateral columns should be corrected to prevent arthritis at the higher or lower joint level. Pan-talar arthrodesis can be worse than a well-done amputation below the knee. Cannulated 6.5 cancellous screws in double and triple arthrodesis and 3.5 corticalis screws in Lisfranc fusions allow stable fixation and function early after treatment.

A study on the mutagenicity of 102 raw pharmaceuticals used in Chinese traditional medicine.

The mutagenic potential of traditional Chinese drugs was studied. After extraction with boiling water and frozen vacuum drying, preparations from 102 raw drugs were tested with the Ames test, and the micronucleus and chromosomal aberration assays in mice in vivo. Astragalus mongholicus Bunge (+S9), Sophora japonica L. (+/- S9) and Eucommia ulmoides Oliv. (+/- S9) were found to significantly induce His+ revertants in Salmonella typhimurium TA98 and/or TA100. Besides these 3, 10 other extracts were positive in the chromosomal aberration and micronucleus assays in mice. They are Datura metal L., Artemisia capillaris Thunb., Carthamus tinctorius L., Forsythia suspensa Thumb., Rehmannia glutinosa f. Hueichingensis (fermented), Paeonia suffruticosa Andr., Platycodon grandiflorum Jacq. DC., Cinnamomum mairei Levl., Notopterygium incisium Ting., and Sophora flavescens Ait. The relevance of these findings is discussed.

[Prevention of hypoxic pulmonary hypertension with "qi-xue" injection].

Effects of "Qi-Xue" injection (Panax ginseng, Astragalus monogholicus, Angelica sinensis), verapamil (Vp, calcium antagonist) and their combination on pulmonary arterial pressure (PAP) and heart function were studied in rats exposed to chronic hypoxia. Male Wistar rats were divided into sea level (n = 9), hypoxic (n = 8). Chinese herbs (n = 15), Vp (n = 20) and herbs + Vp (n = 18) groups. All the rats, except the SL group, were placed in a hypobaric chamber simulating an altitude of 5000 m above sea level for 15 days. During this time, the aforementioned drugs were given i.p. bid every day. It was found that apparent increase of PAP and decrease of IC-R (index of contractility of right ventricle) were induced by hypoxia without significant change of CAP, HR or left ventricular function. Both "Qi-Xue" injection and Vp minimized the increase in PAP due to hypoxia, with the former being more effective than the latter. "Qi-Xue" injection also attenuated IC-R decrease and kept it in the normal range. No appreciable differences were observed between the effect of "Qi-Xue" and its combination with Vp. It is suggested that "Qi-Xue" injection might prevent the development of severe hypoxic pulmonary hypertension by improving heart function.

Study into wild Egyptian plants of potential medicinal activity. Ninth communication: hypoglycaemic activity of some selected plants in normal fasting and alloxanised rats.

31 desert plants belonging to 17 families were collected from different Egyptian localities. 21 plants extracts were orally given to normal rats, and 15 were tested on fasted and to alloxanised rats. The results were compared with a standard oral hypoglycaemic drug (Daonil, Hoechst) used as a positive control. The following findings were obtained: 8 plants exhibited persistent hypoglycaemic effects, Lycium shawii, Salvia (S.) aegyptiaca, Pergularia tomentosa, Convolvulus (C.) althaeoides, Haloxylon salicornicum, Ephedra alata, Scrophularia deserti, and Crotalaria aegyptiaca. Transient hypoglycaemic effects appeared only 1 hour after administration in response to 4 plants, Silena succulenta, Lygos raetam, C. lanatus, and Pulicaria incisa. In the cases of Ochradenus baccatus and Zygophyllum album, slow hypoglycaemic activity was produced and appeared 3 hours after administration. 5 plants showed hypoglycaemic effects viz, Thymus capitatus, Launaea nudicaulis, Conyza dioscorides, Nitraria retusa, and Limonium tubiflorum. Among the 15 plant extracts tested on alloxanised diabetic rats only 4 showed hypoglycaemic effects more potent than those of the administered dose of Daonil. These were Matthiola livida, S. aegyptiaca, Astragalus species, and Arthrocnemum glaucum. The hypoglycaemic effect of S. aegyptiaca in fasting rats has been confirmed also in alloxanised diabetic animals. This emphasises the importance of conducting both experiments in order to obtain a reliable conclusion.

[The structure of astrachrysosid A and the study of 2D-NMR on astrasieversianin XV and 7,2'-dihydroxy-3',4'-dimethoxy-isoflavane-7-O-beta-D-glycoside].

Six compounds were isolated from the root of Astragalus chrysopterus Bunge. One of them was identified as a new triterpenoid glycoside, named astrachrysoside A. Its structure was elucidated as 3-O-[alpha-L-rhamnopyranosyl (1----2) beta-D-xylopyranosyl]-cycloastragenol on the basis of physicochemical evidence and chemical reactions. The others were shown to be known compounds, astrasieversianin XV (II), 7,2'-dihydroxy-3',4'-dimethoxy-isoflavane-7-O-beta-D-glucoside (III), soyasaponin I, daucosterol and beta-sitosterol. Full assignment of the chemical shift signals of II and III were established by various 2D-NMR techniques.

[Cerebral infarction in 141 cases treated by traditional Chinese medicine and Western medicine].

Acute cerebral infarction of 141 cases with treatment combining TCM and WM was reported. The significant effective rate and the effective rate were 65% and 84% in the LT group (low molecular dextran + TCM) respectively; in the LN group (low molecular dextran + Nicotinic acid) were 29% and 41% respectively; in the T group (Salvia miltiorrhiza + Astragalus + Puraria) were 40% and 81.8% respectively. Among them the therapeutic effect in the LT group was the best. The LT group was also superior to the other two groups in serious cases. Cerebral edema developed following cerebrovascular dilator in a week after cerebral infarction; in the early stage, the therapeutic effect could be improved by giving mannitol first and then cerebrovascular dilator. Heart failure could be easily induced by low molecular dextran in the cerebral infarction following rheumatic heart disease. The therapeutic effect was poor in those cases with recurrent cerebral infarction.

The structures of oligosaccharides excreted by sheep with swainsonine toxicosis.

Eleven oligosaccharides were purified form the urine of sheep with swainsonine toxicosis induced by the feeding of Astragalus lentiginosus. Oligosaccharides were extracted by charcoal adsorption, chromatographed on Bio-Gel P-2, and partially fractionated by preparative-layer chromatography. Separation into individual compounds was completed by semi-preparative high pressure liquid chromatography. Structures were determined by a combination of high pressure liquid chromatography and exo- and endo- glycosidase action, methanolysis followed by gas-liquid chromatography, methylation analysis, and high resolution nuclear magnetic resonance spectroscopy. Two homologous series of oligosaccharides were identified: (a) alpha-D-Manp-(1----6)-beta-D-Manp-(1----4)-D-GlcpNAc, alpha-D-Manp(1----3)-[alpha-D-Manp-(1----6)]-beta-D-Manp+ ++-(1----4)-D-GlcpNAc, alpha-D-Manp-(1----2)-alpha-D-Manp(1----3)-[alpha-D-Manp+ ++-(1----6)]-beta-D-Manp-(1----4)-D-GlcpNAc, and alpha-D-Manp-(1----2)-alpha-D-Manp-(1----2)-alpha-D-Manp+ ++-(1----3)-[alpha- D-Manp-(1----6)]-beta-D-Manp-(1----4)-D-GlcpNAc (minor series); (b) alpha-D-Manp-(1----6)-beta-D-Manp-(1----4)-beta-D-GlcpNAc- (1----4)-D-GlcpNAc, alpha-D-Manp-(1----3)-[alpha-D-Manp-(1----6)]-beta-D-Manp -(1----4)-beta-D-GlcpNAc-(1----4)-D-GlcpNAc, alpha-D-Manp(1----3)-alpha-D-Manp-(1----6)-beta-D-Manp -(1----4)-beta-D-GlcpNAc- (1----4)-D-GlcpNAc, alpha-D-Manp-(1----6)-alpha-D-Manp-(1----6)-beta-D-Manp++ +-(1----4)-beta-D-GlcpNAc - (1----4)-D-GlcpNAc, alpha-D-Manp-(1----3)-alpha-D-Manp-(1----6)-[alpha-D-Manp -(1----3)]-beta-D- Manp-(1----4)-beta-D-GlcpNAc-(1----4)-D-GlcpNAc, alpha-D-Manp-(1----3)-[alpha-D-Manp-(1----6)]-alpha-D-Man p-(1----6)-beta-D- Manp-(1----4)-beta-D-GlcpNAc-(1----4)-D-GlcpNAc, and alpha-D-Manp-(1----3)-[alpha-D-Manp-(1----6)]-alpha-D-Man p-(1----6)- [alpha-D-Manp-(1----3)]-beta-D-Manp-(1----4)-beta-D-GlcpNAc- (1----4)-D- GlcpNAc (major series).

[Mechanism of the pro-oxidant activity of tetracycline].

It was shown on male rats that like ethanol tetracycline increased lipid peroxidation (LPO) in the hepatocyte membranes, evident from increased levels of diene conjugates and malone dialdehyde in the liver homogenates, especially on their combination. The signal amplitude of the EPR-forms of cytochrome P-450 and Cu-, Mo- and Fe-containing proteins did not change, while the content of the EPR-forms of the free radicals increased. High efficiency of antioxidants, such as tocopherol acetate, sodium selenite and tincture of Astragalus L. is indirect evidence of the role of the free radicals in initiation of LPO in tetracycline affections of the liver.

Effects of herbicides on the concentration of poisonous compounds in plants: a review.

Herbicides may raise, lower, or not affect the concentration of poisonous compounds in plants. Herbicides, 2,4,5-T and silvex controlled Wasatch milkvetch (Astragalus miser var oblongifolius) and markedly reduced the concentration of miserotoxin, a poisonous aliphatic nitro compound. The concentration of phototoxic furocoumarins in the leaves of spring parsley (Cymopterus watsonii) decreased to nontoxic concentrations 4 to 5 weeks after treatment with 2,4-D. The hydrocyanic acid content of wild cherry (Prunus spp) decreased after treatment with 2,4-D and 2,4,5-T, but increased in Sudangrass (Sorghum halapense) after treatment with 2,4-D. The alkaloid concentration in Barbey larkspur (Delphinium barbeyi) increased after treatment with 2,4,5-T or silvex, decreased in horsetail (Equisetum palustre) after treatment with MCPA, and was unaffected in Datura after treatment with several herbicides. The effect of herbicides on nitrate concentration in plants was dependent upon the species of plant and the herbicide used.

[Vitamin E and selenium-containing preparations in the prevention treatment of tetracycline-induced lesions of the liver].

Studies on albino rats showed that high doses of tetracycline-induced damages of the liver evident from increased activity of serum enzymes (alanine aminotransferase, aspartate aminotransferase and alkaline phosphatase) and inhibition of bile secretion, synthesis and secretion of bile acids and cholesterol excretion. Administration of vitamin E, sodium selenite, infusion of Astragalus L. and especially vitamin E combinations with sodium selenite markedly or completely arrested the occurrence of hepatotoxic properties of tetracycline. It is suggested that the use of vitamin E combinations with selenium-containing preparations is advisable in the prophylaxis and treatment of tetracycline-induced damages of the liver.
