Why nut? The aspiration of hazelnuts has become a public health problem among small children in the central and eastern Black Sea regions of Turkey.

Our objective was to determine the significance of hazelnuts as the causative agent in pediatric foreign body aspirations in the main hazelnut production region of the world. We conducted a 10-year retrospective review of patients admitted to the pediatric surgery department in a 900-bed university hospital. Of 135 children with foreign body aspiration, hazelnut aspiration was identified in 35 (26%). All patients were younger than 5 years of age, and 32% of the hazelnut aspirations occurred during late summer, coinciding with the hazelnut harvest. We conclude that hazelnut aspiration is a public health problem in our region in children younger than 5 years of age, particularly during harvest-time. Hazelnut aspiration can be prevented by a well-defined public education program and precautionary measures taken by government and civil organizations.

Endoscopic ultrasound-guided transrectal aspiration of pelvic fluid collections.

BACKGROUND: The aim of this study was to evaluate the feasibility, safety, and diagnostic accuracy of endorectal ultrasound-guided aspiration and drainage of pelvic fluid collections. METHODS: All patients who underwent endorectal ultrasound-guided aspiration and/or drainage of pelvic fluid collections were included in this prospective study. Patients were prepared using bowel lavage and a single-dose antibiotic combination. The procedure was carried out in the lithotomy position usually without sedation. All aspirates were examined microbiologically and/or cytologically. RESULTS: Twenty-nine patients [18 females and 11 males; median age, 67 years (range, 18-79)] underwent 33 endosonographic controlled aspirations. No procedure-related complications were encountered. In 22 cases (76%) the lesions were encountered following a surgical procedure. Fluid amounts varied between 5 and 750 ml. Fluids were sterile in 14 cases (42%). These fluid collections were hematomas, seromas, peritoneal cysts, and a mucocele. Microorganisms were found in the remaining 19 aspirations (58%), i.e., abscesses (n = 16) and infected hematomas (n = 3). A transrectal (n = 14) or transvaginal (n = 2) drainage catheter was placed in 16 patients under endosonographic guidance. Only two patients required a subsequent laparotomy for definitive treatment of their septic focus, whereas all other patients could be treated successfully by this conservative approach. CONCLUSION: Endoscopic ultrasound-guided transrectal aspiration and drainage of pelvic fluid collections is a safe method with a favorable outcome because it avoids unnecessary operations in selected patients.

Foreign body aspiration in children.

BACKGROUND: There is still considerable controversy about the diagnostic procedure, the endoscopic approach, and the complication rate with foreign body aspiration in children. METHODS: Review of our data for 98 children suspected for foreign body aspiration between January 1990 and December 1998 was performed. RESULTS: In this data review, 78% of the children studied were younger than 2 years. A foreign body aspiration was identified in 70%, and 67% had a definite history of aspiration. Predominant clinical features were fever (46%), pneumonia (39%), and coughing (29%). Pathologic chest radiographs were found in 84% of the children. Sixty-two percent of the foreign bodies were trapped in the right lung, and 87% were of organic in origin. In 93%, a single endoscopic procedure was successful in removing the foreign bodies. The mean time between aspiration and bronchoscopic extraction was 5.4 days (range, 1 h to 36 weeks). The procedure-related morbidity rate was 0.96% and the mortality 0. CONCLUSIONS: Outcome and complications were found to depend mainly on the time the foreign body stayed in the tracheobronchial system. Early bronchoscopy is paramount in any case of suspected foreign body aspiration, and it is mandatory to increase the awareness of the population and medical professionals.

Diagnostic potential and pitfalls of ultrasound-guided fine-needle aspiration cytology for breast lesions.

Ultrasound (US)-guided fine-needle aspiration cytology (FNAC) is now widely accepted as a diagnostic procedure for breast lesions. Along with its advantages, US-guided FNAC also has some pitfalls. The recognition of these pitfalls for this procedure is extremely important for the strict management of the disease. We retrospectively investigated the diagnostic potential and pitfalls of US-guided FNAC in the diagnosis of breast lesions. This study consisted of 348 aspirated samples from 274 breast tumors. The rate of sufficient aspirates was 74% after a single aspiration, while sufficient materials were finally obtained from 93% of the tumors by repeated aspirations. The rate was lower in tumors measuring less than 10 mm in diameter (62%), and in sclerosing adenosis (25%). The sensitivity of FNAC was 65%, the specificity was 75%, the border diagnosis rate was 18%, and the positive predictive value was 92%. The false-negative rate was higher in noninvasive carcinoma (45%). The border diagnosis rate was also higher in scirrhous carcinoma (29%). There were also five false-positive cases. Limited to nonpalpable lesions, the sufficient aspirates rate was 70% and the accuracy was 67%.

Large deformation finite element analysis of micropipette aspiration to determine the mechanical properties of the chondrocyte.

Chondrocytes, the cells in articular cartilage, exhibit solid-like viscoelastic behavior in response to mechanical stress. In modeling the creep response of these cells during micropipette aspiration, previous studies have attributed the viscoelastic behavior of chondrocytes to either intrinsic viscoelasticity of the cytoplasm or to biphasic effects arising from fluid-solid interactions within the cell. However, the mechanisms responsible for the viscoelastic behavior of chondrocytes are not fully understood and may involve one or both of these phenomena. In this study, the micropipette aspiration experiment was modeled using a large strain finite element simulation that incorporated contact boundary conditions. The cell was modeled using finite strain incompressible and compressible elastic models, a two-mode compressible viscoelastic model, or a biphasic elastic or viscoelastic model. Comparison of the model to the experimentally measured response of chondrocytes to a step increase in aspiration pressure showed that a two-mode compressible viscoelastic formulation accurately captured the creep response of chondrocytes during micropipette aspiration. Similarly, a biphasic two-mode viscoelastic analysis could predict all aspects of the cell's creep response to a step aspiration. In contrast, a biphasic elastic formulation was not capable of predicting the complete creep response, suggesting that the creep response of the chondrocytes under micropipette aspiration is predominantly due to intrinsic viscoelastic phenomena and is not due to the biphasic behavior.

Diagnosis of amyloidosis by abdominal fat aspiration. Analysis of four years' experience.

Abdominal fat aspiration samples from 443 consecutive patients were examined for amyloid after Congo red and hematoxylin staining. Of the aspirates from 83 patients known to have systemic amyloid disease prior to the biopsy, 70 (84 percent) were found to yield positive results. The results for four aspirates from patients with localized amyloid disease were negative. Of the aspirates from 356 patients of unknown clinical status referred for analysis by outside physicians, 26 (7 percent) yielded positive results for amyloid. On review of the clinical records of these 26 patients, 11 had proved systemic amyloidosis demonstrated on biopsy of another site; all had a clinical course consistent with amyloid disease. In no case was amyloid found in a fat aspiration sample from a patient without clinical evidence suggestive of systemic amyloid disease. This study supports the proposal that abdominal fat aspiration is the diagnostic procedure of choice in the evaluation of amyloidosis since it requires no specialty consultation or technical expertise, causes minimal patient discomfort, and is accompanied by virtually no risk of morbid complication. A positive result has a high predictive value of amyloid disease in patients of unknown clinical status.

Fine needle aspiration of the thyroid. Use in an average health care facility.

Fine needle aspiration of the thyroid is suggested as an initial diagnostic procedure in the evaluation of thyroid nodules. Some proponents of the procedure, however, believe that fine needle aspiration should be restricted to centers in which each operator obtains at least 500 biopsy specimens annually. Reviewed were 155 biopsy specimens obtained over three and one-half years, and the sensitivity of fine needle aspiration was determined to be 100 percent, the specificity to be 47.4 percent, and the accuracy to be 73 percent. A review of the literature revealed the sensitivity in larger series to range between 92 and 98 percent, specificity to range between 52 and 99.5 percent, and accuracy to range between 56.1 and 91.3 percent. The surgical yield of carcinoma was 64 percent in patients evaluated with fine needle aspiration, whereas the yield was 26 percent in those who underwent surgery without fine needle aspiration. These results appear to justify the use of fine needle aspiration in the evaluation of nodular thyroid disease at average-sized health care centers.

Breast aspiration cytology: a neglected gynecologic procedure.

Presented is a retrospective review of 1,196 breast aspiration cytologic procedures done during 1973 through 1981, at the Straub Clinic and Hospital, Inc., Honolulu, Hawaii. Aspirations which yielded clear fluid are not included. Of all the patients who had breast aspirations, 204 subsequently underwent open biopsy, and a definitive histologic diagnosis was made. Of the 204 open biopsies, 23% proved the existence of malignancy. Thus, 4% of the 1,196 breast aspirations proved the presence of carcinoma. Office aspiration cytologic examination of breast lesions is an efficient and cost-effective technique which is available to every gynecologist, and for which there are no medical contraindications. Complications are rare and usually not significant. If clear fluid is obtained, a clinical diagnosis of benign cystic disease can be made with a high degree of reliability. If the findings of breast aspiration cytologic examination are reported to be suspicious or to disclose malignancy, the patient should be referred as soon as possible for a definitive open biopsy and appropriate surgical treatment.

Quantitation of tumor seeding from fine needle aspiration of ocular melanomas.

Twenty-two fine needle (30 gauge) aspirations were performed in eyes enucleated for the clinical diagnosis of melanoma. Cytologic preparations were evaluated for adequacy of material, and needle tracts were evaluated for tumor implantation. A scleral marking method was used to identify all needle tracts. The number of tumor cells in tracts of direct transscleral aspirates was compared to those in tracts of indirect aspirates that traversed the anterior chamber or vitreous. Cellular material obtained with 30-gauge needles was sufficient for the diagnosis of malignant melanoma in all but one case. While 14 of 21 (67%) of all fine needle aspiration tracts and eight of 15 (53%) of indirect tracts contained tumor cells, the number of tumor cells was less than that associated with tumor growth in experimental models. Indirect aspirate tracts contained significantly fewer cells than tracts of direct aspirates (P less than .001).

Abdominal decompression: increased efficency by esophageal aspiration utilizing a new nasogastric tube.

Gastric aspiration alone utilizing either a Levin-type nasogastric tube or a gastrostomy tube is inefficient. The esophagus proved to be a more efficient supplemental site for aspiration of a swallowed bolus. For thirty-one patients, esophagogastric aspiration proved to be approximately twelve times as efficient as aspiration via a Levin-type tube for twenty-four patients or a gastrostomy tube in five patients (residual activity, of 3,35, and 42 per cent, respectively). Radiographic studies of a volunteer swallowing barium with each type of nasogastric tube in place showed efficient removal of the contrast agent by esophageal aspiration. With the Levin-type tube, the bolus promptly traversed the stomach and entered the duodenum along parallel channels remote from the x-ray -visualized gastric tube. Efficient postoperative exclusion of swallowed air clinically and experimentally by esophageal aspiration permits more rapid return of gastrointestinal function and full nutrition and perhaps shortened hospitalization.

Human colonic aspirates containing immunoglobulin A antibody to Clostridium difficile toxin A inhibit toxin A-receptor binding.

Clostridium difficile toxin A, a 308-kilodalton protein exotoxin, is the principal causative agent of antibiotic-associated, C. difficile-induced colitis. In the current study, the prevalence of specific human serum and secretory antibody to toxin A and the possible protective effect of secretory, intestinal anti-toxin A antibody are examined. Serum (n = 35), colonic aspirates (n = 35), and duodenal aspirates (n = 20) were collected from adults at diagnostic endoscopy. Patients with evidence of colitis or a history of recent antibiotic use were excluded from the study. Specific serum immunoglobulin (Ig) A and IgG antitoxin A antibodies were detected in 60% and 57% of subjects, respectively, by enzyme-linked immunosorbent assay. Fifty-seven percent of colonic aspirates contained IgA antitoxin, whereas only 10% of duodenal aspirates were positive (P = 0.002). Binding of toxin A to its intestinal receptor was studied using [3H]toxin A and purified rabbit ileal brush border membranes. Toxin A binding was significantly inhibited by colonic aspirates with high IgA anti-toxin A antibody levels (0.503 +/- 0.055 pmol toxin A bound per milligram of brush border membrane protein, mean +/- SE) in comparison with antitoxin A-negative aspirates (0.778 +/- 0.089 pmol; P = 0.02) and control (0.766 +/- 0.004 pmol; P = 0.03). In the current study, a specific intestinal secretory IgA antibody response to C. difficile toxin A in humans is reported. This antibody response is more evident in the colon, the site of C. difficile infection, than in the upper intestinal tract. Our data suggest that human colonic IgA antitoxin may protect against C. difficile colitis by inhibiting the binding of toxin A to its intestinal epithelial cell receptor.

In vitro fertilization and development of bovine oocytes recovered from the ovaries of individual donors: A comparison between the cutting and aspiration method.

Bovine oocytes were recovered from ovaries by either the cutting or the aspiration method, after which the oocytes were fertilized and cultured in vitro to investigate their developmental ability. In the cutting method, the surface and the interior of ovaries were cut with a set of 10 razors stacked at 2-mm intervals in modified TCM-199 medium supplemented with 5% fetal bovine serum; the liberated oocytes were then collected. In the aspiration method all visible follicles (2 to 5 mm in diameter) at the ovarian surface were aspirated with a syringe and an 18-gauge needle. Significantly more oocytes were recovered by the cutting than the aspiration method (mean: 63.3 vs 22.1), and the proportion of Rank A oocytes was also higher for the cutting method (84.6 vs 41.3%). Although no significant differences were observed between the 2 methods in the proportion of fertilized oocytes developing to the blastocyst stage in culture, the average number of blastocysts obtained by the cutting method was about 3.6-fold higher than by aspiration. The blastocysts were transferred nonsurgically to 37 (cutting method) and 36 (aspiration method) recipients, and 22 (59.0%) and 19 (52.8%), respectively, became pregnant.

In vitro maturation of equine oocytes collected by follicle aspiration and by the slicing of ovaries.

The aim of this study was to examine 2 techniques for oocyte recovery from equine ovaries at slaughter: by aspiration of follicles and by additional slicing of ovaries. The morphology and nuclear configuration of oocytes recovered with either technique, and the time course of nuclear maturation during in vitro maturation were evaluated. Recovery rates were 1.75 and 4.14 oocytes per ovary for aspiration and slicing (total 145 and 344 oocytes from 83 ovaries), respectively. The oocytes were classified according to their cumulus/ooplasm morphology into 4 groups: compact/circular(A), compact/semicircular(B), expanded(C) and others(D). The percentages of oocytes in Groups A, B, C and D were 34, 38, 25 and 3% (aspiration) and 55, 26, 17 and 3% (slicing), respectively. The proportions of oocytes with a germinal vesicle in Groups A, B, C and D were 28 29 (97%), 23 35 (66%), 11 23 (48%) and 2 4 (50%) in oocytes from aspiration and 91 100 (91%), 52 65 (80%), 15 29 (52%) and 1 2 (50%) in oocytes from slicing, respectively. Group A and B oocytes recovered by aspiration (n=212) and slicing (n=312) were cultured in TCM199 supplemented with 10% fetal bovine serum, 1 mug/ml estradiol-17beta, and 0.02 AU/ml FSH at 38.5 degrees C in 5% CO(2) in air (5 to 10 oocytes per 50- mu l microdrop). At 8, 16, 24, 32 and 40 h of culture, the oocytes were fixed and stained. There were no significant differences in the percentages of Metaphase II stage (MII)-oocytes between recovery techniques at any time points examined. The proportions of MII-oocytes were 1 42 (2%), 4 43 (9%), 21 42 (50%), 28 45 (62%), and 28 40 (70%) at the respective time point in oocytes from aspiration and 0 51 (0%), 3 54 (6%), 22 59 (37%), 43 72 (60%), and 51 76 (67%) in oocytes from slicing, respectively. In most of the oocytes, resumption of meiosis occurred between 8 and 16 h of culture. The proportions of MII-oocytes increased significantly between 16 and 24 h and between 24 and 32 h of culture.

A comparison of the effects of two fimbria-fornix lesion techniques on beam-walking performance in the rat: aspiration versus electrolysis.

This experiment was aimed at comparing the sensorimotor correlates of fimbria-fornix lesions made with either a classical aspiration technique that also removes part of the overlying cortical structures, or an electrolytic one that does not encroach upon these cortical structures. About 4 months after lesion surgery, Long-Evans female rats which had sustained an aspiration or an electrolytic fimbria-fornix lesion at the age of 90 days were tested to measure their beam-walking performance as an index for their sensorimotor capabilities. We found that after an aspiration lesion, the rats presented sensorimotor deficits which did not occur after an electrolytic lesion. After having found that electrolytic lesions of the fimbria and the fornix produced neurochemical deficits (in the dorsal hippocampus) and cognitive alterations close to those resulting from aspiration lesions, it is concluded from the present experiment that the electrolytic lesion technique is an interesting alternative to an aspiration technique, essentially because the former does not induce the sensorimotor deficits due to the partial damage that an aspiration technique produces in the medial parietal cortex. As the electrolytic lesion technique may minimize the risk of introducing a sensorimotor bias in the accuracy of cognitive evaluations, the present result might be of interest to neuroscientist using a fimbria-fornix lesion paradigm in order to investigate the efficacy of drugs, grafts or other treatments on the recovery from cognitive deficits.

Short intervals between ultrasonographically guided follicle aspiration improve oocyte quality but do not prevent establishment of dominant follicles in the Gir breed (Bos indicus) of cattle.

The aim of the present study was to evaluate oocyte recovery and embryo yield using two different ovarian follicular aspiration schedules in donor cows of the Gir breed. Pluriparous, non-lactating Gir cows (n = 14) were randomly allocated to one of two groups, one of which had aspirations of ovarian follicular contents conducted once a week (TVFA-1x) and the other twice weekly (TVFA-2x), for nine consecutive weeks. Before follicle aspiration, follicular population was recorded in three classes according to size (> 6 mm, 6-9 mm and > 9 mm). The cumulus-oocyte complexes (COCs) recovered were identified, morphologically classified and in vitro matured, fertilized with Gir sperm and cultured in CR2 medium for 7 days. There was no difference (P > 0.05) in the size of the largest follicle, number of follicles identified or follicular content aspirations between TVFA-1x and TVFA-2x groups. Large follicles (> 9 mm) were observed for all the aspiration intervals considered (3, 4 or 7 days). More oocytes were recovered per session in TVFA-1x as compared with TVFA-2x (8.9 +/- 0.8 versus 7.0 +/- 0.7, P < 0.01), resulting in a greater recovery rate in this group (74.3% versus 58.7%, P < 0.01). More COCs of Grade I were recovered from TVFA-2x (22.6% versus 13.3%, P < 0.01). There was no difference in cleavage rate between groups, but the percentage of embryos that reached the blastocyst stage was greater in TVFA-2x as compared with the TVFA-1x (31.8% versus 21.6%, P < 0.01). The greater in vitro performance qualities of TVFA-2x oocytes compensates for the greater oocyte recovery rate in TVFA-1x, demonstrating a greater embryo production potential. Despite showing uncommon follicular dynamics characteristics when subjected to follicular aspiration, Gir cows can be successfully used as oocyte donors for in vitro embryo production.

Pulse oximetry does not reliably detect aspiration on videofluoroscopic swallowing study.

OBJECTIVE: To examine the reliability of pulse oximetry for identifying aspiration by comparing it with the videofluoroscopic swallowing study (VFSS). DESIGN: Nonrandomized, prospective, double-blind study. SETTING: VFSS laboratory in a teaching hospital. PARTICIPANTS: Sixty patients from among 130 patients with clinically diagnosed dysphagia between September and December 2002. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Simultaneously monitoring the arterial oxygen saturation (Sp o 2 ) by pulse oximetry while patients were performing VFSS. A decrease in Sp o 2 exceeding 3% was considered as significant desaturation. Bolus or portion of bolus passing through the vocal cords and entering the subglottic space was defined as aspiration on VFSS. The results of pulse oximetry and VFSS were compared. RESULTS: No significant correlation existed between desaturation measured by pulse oximetry and aspiration on VFSS (chi 2 test, P =.87). The positive predictive rate of pulse oximetry in detecting aspiration on VFSS was 39.1%, and the negative predictive rate was 59.4%. CONCLUSIONS: Aspiration occurring on VFSS cannot be predicted based on decrease in Sp o 2 in pulse oximetry. The application of pulse oximetry to detect aspiration during regular meals requires further investigation.

Accuracy of joint aspiration for the preoperative diagnosis of infection in total hip arthroplasty.

The aim of this study was to assess the diagnostic accuracy of radiology department-based hip aspiration for the diagnosis of infection after total hip arthroplasty. Patient selection criteria were devised so that only patients with moderate to high risk of infection were selected for hip aspiration. Seventy-three patients are included in this study, all of whom underwent revision hip surgery after aspiration. Culture results of hip aspirates were compared with the intraoperative tissue cultures at the time of revision surgery, which was used as the gold standard for diagnosing infection. Seventeen patients (23%) had infected hip joints at operative culture. The sensitivity and specificity, positive and negative predictive values, and accuracy of the hip aspiration were 0.82 and 0.91, 0.74 and 0.94, and 0.89, respectively. We conclude that hip aspiration carried out in the radiology department is reliable, but appropriate patient selection is important to obtain accurate results.

Usefulness of intraovarian artery pulsatility and resistance indices measurement on the day of follicle aspiration for the assessment of oocyte quality.

OBJECTIVE: To examine the correlation between the pulsatility and resistance indices (PI and RI) of the intraovarian artery on the day of follicle aspiration and the oocyte quality, intrafollicular oxidative stress, and luteinization. Pulsatility index and RI on the day of hCG administration also were measured. DESIGN: Prospective study. SETTING: Obstetrics and gynecology department of a university medical school in Japan. PATIENT(S): Thirty-five patients in an IVF-ET program. INTERVENTION(S): The PI and RI of the intraovarian artery were measured by transvaginal color ultrasonographic pulsed wave Doppler on the day of hCG administration and the day of follicle aspiration. Follicular fluids and the granulosa cells were collected at follicle aspiration. MAIN OUTCOME MEASURE(S): The PI and RI of the intraovarian artery blood flow on the day of hCG administration and of follicle aspiration, as well as the rate of development of mature oocytes, follicular fluid steroid levels, and the incidence of apoptosis in the granulosa cells. RESULT(S): The PI and RI on the day of follicle aspiration were correlated positively with the rate of mature oocytes retrieved (PI: r = 0.429; RI: r = 0.348), were correlated negatively with the incidence of apoptotic mural (PI: r = -0.383; RI: r = -0.459) and cumulus (PI: r = -0.378; RI: r = -0.469) granulosa cells, and were negatively correlated with the concentration of P in the follicular fluid (PI: r = -0.429; RI: r = -0.359). The PI and RI on the day of hCG administration were negatively correlated only with the total number of retrieved oocytes (PI: r = -0.393; RI: r = -0.374). CONCLUSION(S): The PI and RI of the intraovarian artery blood flow measured on the day of follicle aspiration may be good indicators of the follicle luteinization and oxidation as well as of oocyte quality.
