Serum level of immunoreactive gastrin: influence of kidney function.

Serum gastrin levels have been studied in 70 patients with chronically reduced glomerular filtration rate (GFR) as estimated by 51Cr-EDTA clearance, creatinine, and beta2-microglobulin values. A strong dependence upon GFR was found, although the correlation between gastrin levels and GFR was not as high as that between beta2-microglobulin and GFR, indicating the existence of extrarenal factors regulating the levels of circulating gastrin. In a separate group of 31 patients on maintenance dialysis the mean gastrin level was 65.9 pmol/l--that is, a fourfold increase compared to healthy subjects. Three of the uraemic patients had pronounced rises in serum gastrin in the range 800-1800 pmol/l. Finally, the influence of acute alterations of kidney function on serum gastrin was studied in 11 patients undergoing renal transplantation. In addition to a GFR dependence the results indicate the existence of feedback mechanisms in gastrin homeostasis. Although the clinical importance of the increased gastrin levels in renal failure is unknown, hypergastrinaemia occurs with sufficient frequency to be involved in upper gastrointestinal complications of uraemic patients.

Type IV procollagen missense mutations associated with defects of the eye, vascular stability, the brain, kidney function and embryonic or postnatal viability in the mouse, Mus musculus: an extension of the Col4a1 allelic series and the identification of the first two Col4a2 mutant alleles.

The basement membrane is important for proper tissue development, stability, and physiology. Major components of the basement membrane include laminins and type IV collagens. The type IV procollagens Col4a1 and Col4a2 form the heterotrimer [alpha1(IV)]2[alpha2(IV)], which is ubiquitously expressed in basement membranes during early developmental stages. We present the genetic, molecular, and phenotypic characterization of nine Col4a1 and three Col4a2 missense mutations recovered in random mutagenesis experiments in the mouse. Heterozygous carriers express defects in the eye, the brain, kidney function, vascular stability, and viability. Homozygotes do not survive beyond the second trimester. Ten mutations result in amino acid substitutions at nine conserved Gly sites within the collagenous domain, one mutation is in the carboxy-terminal noncollagenous domain, and one mutation is in the signal peptide sequence and is predicted to disrupt the signal peptide cleavage site. Patients with COL4A2 mutations have still not been identified. We suggest that the spontaneous intraorbital hemorrhages observed in the mouse are a clinically relevant phenotype with a relatively high predictive value to identify carriers of COL4A1 or COL4A2 mutations.

Evaluation of sulprostone for second trimester abortions and its effects on liver and kidney function.

Sulprostone was given intramuscularly in two dose schedules of 0.5 mg at 4 hourly intervals for 8 doses and 1.0 mg at 8-hour intervals for 4 doses to 56 women in the 12th-20th week of gestation coming for termination of pregnancy. Forty-nine of them aborted within the 30-hour observation period, giving a success rate of 87.5%. Within the observation period, the induction-abortion interval with the 0.5-mg schedule was 16.6 hours while the interval with the 1.0-mg schedule was 15.4 hours. Side effects caused by the drug were minimal and it was observed that patients receiving 1.0 mg Sulprostone had slightly more side effects per person than in those cases receiving 0.5 mg Sulprostone. Serum parameters assessing liver and kidney functions were carried out in 48 patients prior to, 24 hours later and two weeks after the abortion. There were significant changes in the levels of uric acid, potassium, serum proteins and SGOT, though these changes were within the normal range.

Active and inactive renin and kidney function in young subjects with different predisposition to develop essential hypertension.

Renal and cardiac function were measured in 65 offspring of hypertensive parents (OHP) and in 56 offspring of normotensive parents (ONP). In two additional groups of 24 OHP and 42 ONP plasma renin activity (PRA) and plasma active (PRAC), inactive (PRIC) and total (PRTC) concentration were measured. OHP had significantly higher renal plasma flow (p less than 0.01), glomerular filtration rate (p less than 0.02) and 24-hour urinary output than ONP, while PRA was lower (p less than 0.01). The measurements of the different forms of renin gave the following results: PRIC and PRTC were lower in OHP than in ONP, but the only statistically significant difference concerns PRIC (p less than 0.025). A possible interpretation of these findings is that a primary increased tubular ion and water reabsorption might be the cause of the kidney function pattern seen in OHP.
