Evaluation of the effects of implant materials and designs on thermal necrosis of bone in cemented hip arthroplasty.

The exothermic polymerization of bone cement may induce thermal necrosis of bone in cemented hip arthroplasty. A finite element formulation was developed to predict the evolution of the temperature with time in the cemented hip replacement system. The developed method is capable of taking into account both the chemical reaction that generates heat during bone cement polymerization (through a kinetic model) and the physical process of heat conduction (with an energy balance equation). The possibility of thermal necrosis of bone was then evaluated based on the temperature history in the bone and an appropriate damage criterion. Specifically, we evaluate the role of implant materials and designs on the thermal response of the system. Results indicated that the peak temperature at the bone/cement interface with a metal prosthesis was lower than that with a polymer or a composite prosthesis in hip replacement systems. Necrosis of bone was predicted to occur with a polymer or a composite prosthesis while no necrosis was predicted with a metal prosthesis in the simulated conditions. When reinforcing osteoporotic hips with injected bone cement in the cancellous core of the femur, the volume of bone cement implanted is increased which may increase the risk of thermal necrosis of bone. We evaluate whether this risk can be decreased through the use of an insulator to contain the bone cement. No thermal necrosis of bone was predicted with a 3 mm thick polyurethane insulator while more damage is predicted for the use of bone cement without the insulator. This method provides a numerical tool for the quantitative simulation of the thermal behavior of bone-cement-prosthesis designs and for examining and refining new designs computationally.

Detection of aquatic colloids in drinking water during its distribution via a water pipeline network.

Laser-induced Breakdown Detection (LIBD) is a highly sensitive method for the direct detection of nano-particles (colloids). During the detection process plasmas are generated on single particles by a focused laser beam, the resulting plasma light emissions are detected optically. The method is based on the difference in breakdown thresholds of liquid and solid matter, it is lower for solid material. The laser pulse energy is adjusted precisely so that in the pure liquid no breakdown events occur, and only in the presence of colloids is the breakdown threshold in the focal volume exceeded. The spatial distribution of several thousand recorded plasma flashes within the focal volume reveals the mean particle diameter. The evaluation of the number of breakdown events per number of laser pulses results in a breakdown probability, together with the particle size the concentration is calculated using specially-designed computer software. Compared to conventional laser light scattering methods the LIBD is approximately 6 orders of magnitude more sensitive for particles smaller than ca. 0.05 pm. Together with laser light obscuration the LIBD technique has been used successfully for the quantification of aquatic nano-particles during drinking water processing and its distribution via a pipeline network of nearly 1,700 km total length. In addition, the particulate content of several brands of mineral water has been investigated.

Esthetic restoration of infra-occluded retained primary mandibular incisors with all-ceramic crowns in adult dentition.

The prevalence of hypodontia is reported to be between 1.5% to 10% in the permanent dentition. In the anterior teeth, maxillary lateral incisors and mandibular central incisors are the most frequently involved teeth. This causes esthetic problems for the patient. Several reports have focused on restoration of retained maxillary primary anterior teeth, but none have described restoration of retained mandibular primary incisors. This clinical report describes the restoration of infra-occluded retained primary mandibular central incisors of a 17 year-old girl diagnosed with hypodontia. All-ceramic crowns made with computer-aided design/ computer-aided manufacturing technology were used to restore the teeth incisally and interproximally. Due to a relatively short root length and inadequate crown-root ratio, the primary mandibular central incisors were splinted and adjusted to distribute the protrusive force evenly across the maxillary and mandibular incisors. Functional and esthetic results were achieved.

[Recognition of actual periods of nocturnal rest from 24-hour heart rate profile].

UNLABELLED: Inability to delineate exactly periods of nocturnal sleep and diurnal wakefulness during 24-hour blood pressure (BP) monitoring causes reporting of erroneous BP values for these periods. We suggested a simple mathematical algorithm for detection of periods of nocturnal rest determined as period of low values of cardiac rhythm using data of BP monitoring itself. AIM: To compare novel method of recognition of periods of sleep with 2 standard techniques: use of fixed time interval between 23 and 7 hours (1), or period of sleep according to patient's diaries (2). Reproducibility of nocturnal BP lowering between two 24-hour intervals during 48-hour blood pressure monitoring was used as a measure of precision of determination of diurnal/nocturnal BP. METHODS: Ambulatory 48-hour BP monitoring was carried out in 33 patients with uncomplicated stage II hypertensive disease. Automatic analysis of BP monitoring data was performed with the use of specially designed computer application. Standard deviation (SD) of differences (SDD) between pairs of nocturnal BP lowering during 48 hours was used as a measure of reproducibility. RESULTS: Reproducibility of values obtained with novel algorithm (SDD for systolic/diastolic BP 6.7/8.2 mm Hg) was substantially better than those obtained with standard methods (1) and (2) (SDD 13.0/14.8 and 13.5/18.3 mm Hg, respectively). CONCLUSION: The proposed method of recognition of the period of nocturnal rest substantially improved precision of automatic analysis of 24-hour BP monitoring.

["Indicators of Reha-Status (IRES)"--a patient questionnaire for assessing need and success of rehabilitation].

A basic instrument is presented which enables essential parameters of rehabilitation patients' somatic, functional and psychosocial status to be brought out and data collected at a "middle level of detail" in a standardized manner, i.e. a patient questionnaire which can be used to complement medical assessment or, for example in follow-up, as a stand-alone tool. Development and testing of the instrument has been supported over the past three years by the LVA Württemberg pension insurance fund, both financially and conceptually. In the present article, questionnaire contents and structure are described, and validation findings reported, standardization on a representative sample of the resident population (n = 1848) is set out. The validation studies performed showed the instrument to be reliable, valid and sensitive, and meeting with good acceptance on the part of patients. Finally, a specifically designed computer programme is set out which facilitates questionnaire data input and basic analyses. Potential applications for the IRES questionnaire are for example seen in describing rehabilitation course and outcome, in documenting the effectiveness of rehabilitation measures (quality control), or in the framework of rehabilitation diagnosis, in particular relative to functional and psychosocial dimensions.

Constructing the eukaryotic expression vector to study preliminarily the functions of hammerhead ribozyme targeting base excision repair gene HOGG1.

OBJECTIVE: Adriamycin is widely used as an effective anti-tumor drug clinically treating a number of human cancers, but the effect of adriamycin is limited by drug resistance. The various kinds of investigations indicated that the anti-tumor activity of adriamycin resulted from drug-induced free radical formation. The free radicals could lead to oxidative DNA damage, and the lesion would be repaired by base excision repair (BER) pathway. Human 8-oxoguanine DNA glycosylase 1 (HOGG1) is a key enzyme on BER pathway. To study the influence and biological mechanism of the HOGG1 to adriamycin drug-sensitivity, the eukaryotic expression vector with gene of hammerhead ribozyme targeting HOGG1 mRNA would be constructed and identified, and then the change of drug-sensitivity in lung cancer A549 cells would be investigated. METHODS: According to computer design, two specific restriction site BamH I and EcoR I were added to both ends of the ribozyme gene, then the modified ribozyme gene was synthesized and cloned into the eukaryotic expression vector pcDNA3.1 (+). The positive recombinants were screened by ampicillin resistance, and plasmids were extracted from the positive recombinants and digested by BamH I and EcoR I , and then were analyzed by agarose gel electrophoresis and DNA sequencing. The recombinants were transiently transfected into A549 cells. The positive recombinants were identified by reverse transcription-polymerase chain reaction (RT-PCR) targeting to NEO gene, which was a neomycin resistance gene for selection of stable cell lines and only existed in vectors. The changes of HOGG1 mRNA in A549 cells were detected by RT-PCR. Then the cellular sensitivity to adriamycin was tested by comparison between untransfected cells and transfected cells by MTT assay. The adriamycin-induced DNA damage was investigated by comet assay or single cell gel electrophoresis (SCGE) between untransfected and transfected cells. RESULTS: The recombinants containing the ribozyme gene were successfully selected by restriction endonuclease digestion and agarose gel electrophoresis, and were further proved by DNA automatic sequencing. A549 cells containing the recombinants were identified by RT-PCR, because NEO genes were amplified only in cells transfected successfully. The expression of HOGG1 mRNA in A549 transfected with ribozyme gene was 36% significantly less than in control cells (P < 0.05). MTT assay showed that the sensitivity of transfected cells to adriamycin were significantly increased in comparison with untransfected cells (P < 0.05). The comet assay showed that the extent of DNA damage induced by adriamycin was worse in transfected cells than unrtransfected cells (P < 0.05), but there was no time-dependent reaction correlation observed in cells. CONCLUSION: The eukaryotic expression vector with gene of hammerhead ribozyme targeting HOGG1 mRNA was constructed successfully, and effectively inhibited the expression of HOGG1 gene in lung cancer A549 cells, and increased the cellular sensitivity to adriamycin, and will help to study deeply the functions of base excision repair genes-HOGG1.

Virtual reality applications for the remapping of space in neglect patients.

PURPOSE: The aims of the present article were the following: (i) to provide some evidence of the potential of virtual reality (VR) for the assessment, training and recovery of hemispatial neglect; (ii) to present data from our laboratory which seem to confirm that the clinical manifestation of neglect can be improved by using VR techniques; and (iii) to ascertain the neural bases of this improvement. METHODS: We used a VR device (DataGlove) interfaced with a specially designed computer program which allowed neglect patients to reach and grasp a real object while simultaneously observing the grasping of a virtual object located within a virtual environment by a virtual hand. The virtual hand was commanded in real time by their real hand. RESULTS: After a period of training, hemispatial neglect patients coded the visual stimuli within the neglected space in an identical fashion as those presented within the preserved portions of space. However it was also found that only patients with lesions that spared the inferior parietal/superior temporal regions were able to benefit from the virtual reality training. CONCLUSIONS: It was concluded that using VR it is possible to re-create links between the affected and the nonaffected space in neglect patients. Furthermore, that specific regions may play a crucial role in the recovery of space that underlies the improvement of neglect patients when trained with virtual reality. The implications of these results for determining the neural bases of a higher order attentional and/or spatial representation, and for the treatment of patients with unilateral neglect are discussed.

Information management in ambulatory care: the nurse and computerized records.

The ambulatory office setting is increasingly becoming a central focus of patient care. The nursing staff are integral to patient care management in this environment. They are frequently the heaviest users of medical records and are generally early advocates of well designed computer records systems. As our implementation of a Computer Stored Ambulatory Record (COSTAR V) has grown in complexity and utility, reorganization of the record and development of new features to support nursing has become critical. This demonstration will show how nursing information management has changed using a variety of computer record tools, including features of exam room data entry, specialized nursing displays and problem based patient summaries. Specific items for demonstration will include: nursing check-in module, rule driven nursing history program, prescription refill functions, documentation and billing for nursing procedures, nursing telephone management functions, a module for management of anticoagulation and document retrieval and display utilities.

Patient management in the ICU: the PDB System.

The Intensive Care Unit is the area in patient care where the amount of patient data from a variety of sources is particularly large. The problem for clinicians lies in the ability to gather, and use these data in the decision making process. A well designed computer based patient data management system, incorporating a variety of data analysis tools, would have a dramatic impact in patient care in an environment such as this. The PDB System has been in continuous use at the Montreal General Hospital's Surgical and Trauma Intensive Care Unit since Jan. 88. Its initial implementation in two beds in our SICU has allowed the complete replacement of the conventional patient paper record. It is used by all ICU staff, including nurses, physicians, and ward clerks for the recording/viewing of all patient vital data, laboratory data, medications, and optionally chart notes. In addition, medical staff has the option to use the entered data to perform a variety of data analysis procedures.

Expert systems in treating substance abuse.

Computer programs can assist humans in solving complex problems that cannot be solved by traditional computational techniques using mathematic formulas. These programs, or "expert systems," are commonly used in finance, engineering, and computer design. Although not routinely used in medicine at present, medical expert systems have been developed to assist physicians in solving many kinds of medical problems that traditionally require consultation from a physician specialist. No expert systems are available specifically for drug abuse treatment, but at least one is under development. Where access to a physician specialist in substance abuse is not available for consultation, this expert system will extend specialized substance abuse treatment expertise to nonspecialists. Medical expert systems are a developing technologic tool that can assist physicians in practicing better medicine.

[Clinical application of asphero-iseikonic posterior chamber intraocular lens].

The asphero-iseikonic posterior chamber intraocular lens has the advantages of an aberration and iseikonia. The clinical study of 63 eyes in 62 patients showed that 98.4% of the eyes achieved a visual acuity of 0.5 or better, 84.1% of 0.8 or better, and 79.4% of 1.0 or better. With the distance correction, 90.6% of the eyes achieved a near vision of 0.3 or better and the highest was 0.8. The mean apparent accommodative power in the pseudophakic eyes was 3.11 +/- 0.85 D, and the mean spherical aberration 0.21 +/- 0.21 D. The distance from the anterior apex of cornea to the anterior surface of the implant lens corresponded to the theoretical design. Computer image analysis system confirmed the iseikonic precision. The visual field of all cases was normal and elevation of intraocular pressure occurred in none.

Behavioral profiles of dog breeds.

Breed behavior profiles were obtained by a method that was quantitative and free of personal biases. The profiles concerned 13 traits, eg, excitability, snapping at children, watchdog barking, and affection demand, which are of interest to people wanting dogs as pets. Authorities for the development of the profiles were 48 small animal veterinarians and 48 obedience judges, randomly selected from directories so as to represent equally men and women, and eastern, central, and western geographic regions of the United States. Each authority was asked to rank on each of the behavioral traits a list of 7 breeds chosen randomly from a list of 56 breeds. The data were analyzed in a custom-designed computer program that pooled the data and then ranked all 56 breeds on the basis of the 13 traits. The results indicated that some behavioral traits discriminate between breeds better than others. An examination of sample profiles indicated the feasibility of developing a statistically meaningful behavioral profile that integrates comparative rankings of several authorities balanced as to representation of geographic location, sex, and type of experience with dogs.

Normal values of two-dimensional echocardiographic evaluation of left and right ventricular geometry in children.

Normal values for left and right ventricular geometry in childhood were evaluated from two-dimensional echocardiograms using a specially-designed computer program package. Based on the normal values for ventricular cross-sectional areas, ventricular volumes were calculated applying the algorithm of "sphere volumes". For normal subjects, these "sphere volumes" were found to be linearly related to bodyweight which also can be seen in angiographic volume determinations. Thus, these echocardiographic "sphere volumes" could be transferred to actual ventricular volumes by using a factor which results from the different slopes of angio- and echocardiographic "sphere volumes" versus bodyweight. The mean relative error of ventricular volumes thus obtained from echocardiograms is comparable with that of angiocardiographic determinations except for right ventricular end-diastolic volumes. Parameters of wall thickness and myocardial volume are favourably calculated as myocardial area and related to cavity area.

Effect of wall shear stress on intimal thickening of arterially transplanted autogenous veins in dogs.

To determine whether or not changes in wall shear stress play a determinant role in the induction of hyperplasia of intimal tissue of arterially transplanted vein grafts, we developed two models of canine femoral arteries. Wall shear stress was defined by variation of wall shear stress (tau-variation) in the cardiac cycle, with the use of a newly designed computational flow waveform analyzer. In the group I model autogenous vein grafts were implanted under flow conditions of 79.7 +/- 3.2 ml/min of the normally high flow rate with 33.1 +/- 1.9 dynes/cm2 of low tau-variation. In the group II model grafts were implanted under conditions of 2.9 +/- 1.8 ml/min of low flow rate with 178.8 +/- 11.0 dynes/cm2 of normally high value of tau-variation. The intimal thickness of 259 +/- 36 microns 4 weeks after implantation in group I was statistically significant compared with that of 31 +/- 14 microns in group II (p less than 0.005). Our study revealed that change in wall shear stress and not the rate of blood flow is the essential hemodynamic factor related to intimal hyperplasia.

[Polarcardiographic diagnosis of an infarct of the anterior wall of the heart in its subacute and chronic stages].

One hundred and ten patients, with clinically confirmed acute myocardial infarction of the anterior wall in subacute and/or chronic stage were polarcardiographically examined. The polarcardiograms were recorded with e velocity of the record 100 mm/s by an original little analogue computer, designed at RIMT, Ministry of Public Health, connected to a 6-channel ECG apparatus, programmed for orthogonal Frank leads. The recorded curves of space magnitude (M), transversal magnitude (tm) and length were quantitatively analyzed for amplitudes and time intervals. The results obtained were compared with the data from the polarcardiographic studies on a control group of 114 healthy subjects. Positive ECG criteria for the diagnosis of myocardial infarction were found in 82 patients (74.54 per cent). Polarcardiographic criteria for myocardial infarction of the anterior wall (11-16) were found in another 28 patients (25.45%). Thus, myocardial infarction was polarcardiographically diagnosed in 108 patients (98.18%). Infarction criteria were polarcardiographically established in three healthy subjects, i.e. 2.63 per cent false positive diagnoses. The polarcardiography is a valuable additional method to the conventional ECG in making the diagnosis of myocardial infarction of the anterior wall in subacute and/or chronic stage, easily applicable in the cardiological practice.
