Understanding customer reactions to brokered ultimatums: applying negotiation and justice theory.

There has been little research examining customer reactions to brokered ultimatum game (BUG) contexts (i.e. exchanges in which 1 party offers an ultimatum price for a resource through an intermediary, and the ultimatum offer is accepted or rejected by the other party). In this study, the authors incorporated rational decision-making theory and justice theory to examine how customers' bids, recommendations, and repatronage behavior are affected by characteristics of BUG contexts (changing from an ultimatum to negotiation transaction, response timeliness, and offer acceptance or rejection). Results indicated that customers attempt to be economically efficient with their bidding behavior. However, negotiation structures, long waits for a response, and rejected bids create injustice perceptions (particularly informational and distributive injustice), negatively influencing customers' recommendations to others and their repatronage. The authors then discuss the practical and theoretical implications of their results.

The effect of a prophylactic dose of flurbiprofen on muscle soreness and sprinting performance in trained subjects.

The aim of this study was to examine the effects of a prophylactic dose of a local, transcutaneously administered, non-steroidal anti-inflammatory drug on muscle soreness, muscle damage and sprinting performance in young trained males. Twenty-five subjects aged 19+/-3 years, actively participating in rugby union and field hockey, were familiarized with the test procedure and then divided at random into an experimental group (n = 13) and a control group (n = 12). The experimental group received two patches, each containing 40 mg flurbiprofen (TransAct LAT), 12 h before an exercise bout designed to produce delayed-onset soreness (DOMS). The control group received identical non-medicated placebo patches at the same time. Delayed-onset muscle soreness was induced by an exercise protocol consisting of drop jumps (seven sets of 10 repetitions). Serum creatine kinase activity, muscle soreness, muscle girth and acceleration in a maximal sprint over 30 m were measured before the induction of DOMS and at 12, 24, 48 and 72 h thereafter. Plasma lactate concentration was measured 3 min after the 30-m sprint tests. Subjects in both groups had significantly more pain at 24 and 48 h compared with at 12 and 72 h (P < 0.05; Friedman two-way analysis of variance). Thigh girth and serum creatine kinase did not change throughout the experiment. Although plasma lactate concentrations were elevated after the 30-m sprint, there were no differences between groups or as a result of DOMS. The greatest acceleration occurred between 5 and 10 m. This was not affected by the anti-inflammatory drug or DOMS. In conclusion, the aetiology of the DOMS induced in the trained subjects in this study seems to be independent of inflammatory processes or, more specifically, of increases in prostaglandin synthesis in the muscles.

Economics and cigarettes.

Economic facts on cigarette consumption and production are summarized, and the health consequences of cigarette smoking are reviewed. The magnitude and distribution of these health consequences among the population are discussed in economic terms, that is, in an "accounting framework" comprising such disparate elements as lost lives, lost livelihoods, pain, fear, discomfort, medical costs, excise taxes, and the costs of regulating smoking behaviors. The importance of these factors and their potential influence on public policy and individual behavior are considered. Difficulties include assigning a monetary value to an expected extension of life, the "voluntary" nature of smoking (even though most smokers wish they could quit), deciding what to include as economic consequences of smoking, and the attribution to smoking of some share of the costs for diseases known to be affected by smoking. "Transfers," or purely financial transactions, in contrast to expenditures for goods and services, are explained as one assessment component of the economic impact of smoking-related diseases. The issue of the economic benefit to the United States as a whole and to the population engaged in the cigarette industry, because of the earnings and employment generated by cigarette purchases, is examined, as is the issue of cigarette purchases as a significant source of federal and state revenue.

[Health psychology, medical psychology, psychosomatic medicine and liaison psychiatry].

Health psychology is a new discipline which introduces transactional conceptions and biopsychosocial integrative models of health and disease. Its fields of research overlap the traditional domain of psychosomatic medicine (study and treatment of the mind--conscious or unconscious--of organic patients), and also, partially, the domain of psychological medicine (which in France is focused on the psychology of the practitioner and the patient-doctor relationship), behavioral medicine and liaison psychiatry (which is the use of modern diagnosis and treatment of psychiatry proper to medical patients). Rather than a potential competition, one must see a challenge and an opportunity for renewal of psychiatry in the field of medicine as well as collaborative perspectives. Health psychology uses new stringent methodology and instruments for research (dimensions) evolving from scientific psychology. Brief and critical analysis of this new discipline.

Inhibition of E-selectin gene transcription through a cAMP-dependent protein kinase pathway.

Cytokines induce the expression of E-selectin, VCAM-1, and ICAM-1 on human umbilical vein endothelial cells (HUVECs). We show that expression of these surface proteins is differently affected by cAMP. Increased cAMP levels decrease E-selectin and VCAM-1 but increase ICAM-1 expression. We demonstrate by mRNA half-life analysis and nuclear run-on assays that the cAMP repression of E-selectin occurs at the transcription level. This effect is abolished by protein kinase A inhibition, suggesting that repression is mediated by protein kinase A-driven phosphorylation. We found that a minimal E-selectin promoter sequence necessary to confer cytokine inducibility is also sufficient to mimic the cAMP effect in transfected HUVECs. Previously we characterized two regions (NF-kappa B and NF-ELAM1) of the minimal promoter that bind transcription factors necessary for E-selectin induction, Increased cAMP did not alter the binding of the complexes formed on either the NF-kappa B or NF-ELAM1 site. In contrast, in interleukin-1-treated HUVECs transactivity due to an NF-kappa B site is reduced by elevated cAMP. Increased cAMP in HUVECs appears to induce a protein kinase activity that reduces the cytokine signal for E-selectin and VCAM-1 expression. The reduction in signal may occur through an inhibitory phosphorylation of one or more of the factors responsible for regulating E-selectin expression.

Characterization of a TPA-response element in the 5'-flanking region of the androgen receptor gene.

The androgen receptor (AR) protein is an important transacting factor that is necessary for mediating gene expression of androgen-responsive genes. The expression of the AR gene is regulated by androgens and agents that utilize the calcium, protein kinase A, and protein kinase C pathways. Although the role of the calcium and protein kinase A pathways in the regulation of the AR gene has been investigated, the mechanism of regulation of AR through the protein kinase C pathway is not known. We have isolated the 5'-flanking region of the mouse AR gene and identified a consensus TPA (12-O-tetradecanoylphorbol 13-acetate)-response element (TRE). Transient transfection assays indicate that the TRE sequence is sufficient to confer TPA responsiveness to cells treated with TPA. Gel retardation assays and DNA footprint analysis demonstrated specific binding of the TRE and protection of the TRE sequence. Thus, these results describe a TRE in the 5'-flanking region of the AR gene and demonstrate that the TRE is responsive to TPA treatment.

Cloning and expression of mitochondrial and protoflagellar creatine kinases from a marine sponge: implications for the origin of intracellular energy transport systems.

Creatine kinase (CK) plays a central role in energy transactions in cells displaying high and variable rates of ATP turnover. Cytoplasmic and mitochondrial CK genes code for isoforms which are targeted to distinct intracellular compartments often in close physical proximity to sites of ATP hydrolysis or synthesis. In certain lower groups a third CK gene is present which codes for a flagellar CK isoform consisting of three complete, fused CK domains. Recent work has shown that cytoplasmic, mitochondrial, and flagellar CKs are present in protochordates and in deuterostome and protostome invertebrates. We report here that the marine sponge Tethya aurantia, a representative of the oldest of all multi-cellular animal groups, expresses three unique CK transcripts. One of these CK transcripts codes for protein that has a mitochondrial targeting sequence and in a phylogenetic analysis is positioned at the base of the cluster containing mitochondrial CK sequences from invertebrates, protochordates, and vertebrates; it is clearly a mitochondrial CK. When expressed in Escherichia coli the mitochondrial form from T. aurantia was found to be dimeric unlike all other mitochondrial CKs which are typically octameric. The other two T. aurantia transcripts code for proteins that appear to be more closely related to flagellar CKs. These protoflagellar CKs were found to be dimers when expressed in Escherichia coli. Sponges last shared a common ancestor with higher animals as long as one billion years ago. The antiquity of intracellular localization, as evidenced by the presence of a true mitochondrial CK and protoflagellar CKs in the sponge T. aurantia, indicates that physical constraints on cellular energy transport were key, early driving forces in the evolution of this key enzyme system.

[Coping with stress strategies among female patients suffering from a depression].

INTRODUCTION: Within the past few years there was an ever increasing number of scientific publications focusing on the question of stress, new theories were created as well as efforts were taken to classify them. According to stress researchers, finding out how to manage with stress and with difficulties (the strategy of coping) plays an even more important role than the phenomenon of stress itself. This is due to the fact that the former decides about the quality of one's life. Stressful transactions in the aspect of a disease seem to differ from everyday stressful problems. The intensity of stress and the capability of taking control over stress itself are different in the case of a danger of disease compared to an experience of disease, which influences applying certain coping strategies (task-aimed strategies). AIM: The purpose the study was to determine the relationship between the social functioning of female patients suffering from depression with their coping style. METHOD: 60 female patients with a diagnosed depression and 60 mentally ordered female patients were tested with the use of Endler's and Parker's Coping Inventory for Stressful Situations Questionnaire (CISS). RESULTS: The study presents and describes the result of the research concerning female patients suffering from depression. The analysis of the results indicated the existence of differences in choosing coping strategies (task-aimed strategies) in both the studied groups of women. CONCLUSIONS: The study shows that female patients with depression cope with stress mostly using emotional strategies, and the avoidance strategies are used the least often. Mentally ordered women however, most commonly use problem (task)-focused strategies.

Wireless health care service system for elderly with dementia.

The purpose of this paper is to integrate the technologies of radio frequency identification, global positioning system, global system for mobile communications, and geographic information system (GIS) to construct a stray prevention system for elderly persons suffering from dementia without interfering with their activities of daily livings. We also aim to improve the passive and manpowered way of searching the missing patient with the help of the information technology. Our system provides four monitoring schemes, including indoor residence monitoring, outdoor activity area monitoring, emergency rescue, and remote monitoring modes, and we have developed a service platform to implement these monitoring schemes. The platform consists of a web service server, a database server, a message controller server, and a health-GIS (H-GIS) server. Family members or volunteer workers can identify the real-time positions of missing elderly using mobile phone, PDA, Notebook PC, and various mobile devices through the service platform. System performance and reliability is analyzed. Experiments performed on four different time slots, from three locations, through three mobile telecommunication companies show that the overall transaction time is 34 s and the average deviation of the geographical location is about 8 m. A questionnaire surveyed by 11 users show that eight users are satisfied with the system stability and 10 users would like to carry the locating device themselves, or recommend it to their family members.

Developmental theories of parental contributors to antisocial behavior.

In view of the increased interest in a developmental approach to psychopathology, and mounting evidence of the importance of parent-child interactions in the etiology of early antisocial behavior, the following questions were posed for this review. What theories of parent-child relationships and family management techniques are available? How developmental are they, how specific and transactional are they relative to parent and child behaviors involved? And how well do they cover the period in which antisocial behavior develops? Six theories have some developmental features but the attachment theories (by L. A. Sroufe, B. Egeland, and M. T. Greenberg) and two social learning theories (by G. R. Patterson and J. Martin) are most clearly developmental. They postulate reciprocal interactions of parent and child, and transformations in the form of normative changes in the child or changes in family processes. The social learning theories of Patterson and Martin are most specific, microanalytic in fact, as to the interaction processes involved, and the attachment theories at least specify kinds of behavior involved and also do not rely on traits or types of influence as their units of analysis. Conceptualization is most weak and overly general between late infancy and the preschool years. This gap makes it difficult to link attachment and social learning theories, both of which have driven a large number of studies. A bridging theory is offered to link the two sets of theories in the critical period involved.

Identification of a methyl jasmonate-responsive region in the promoter of a lipoxygenase 1 gene expressed in barley grain.

A genomic DNA fragment was isolated containing 5' upstream sequences and part of the open reading frame corresponding to the lipoxygenase 1 cDNA (LoxA) expressed in barley grains during development and germination. Lox1 transcription was shown to be methyl jasmonate (MeJA)- and wound-inducible in leaves, but Lox1 transcripts were not detected in mildew-infected leaves, although this is a commonly observed response to pathogenic attack in various plants. Transient gene expression assays were used to identify a promoter region involved in MeJA-responsive expression. Analysis of 5' and 3' promoter deletions indicated that sequences between -363 and -294 conferred MeJA-responsive expression. Deletions/replacements covering this part of the promoter further defined a MeJA-responsive region between -331 and -291. Insertion of the region -328 to -293 into the constitutive CaMV 35S promoter conferred MeJA-responsive expression. The 36 bp fragment contains the motif TGACG as inverted repeats, which has been previously identified as a binding site for bZIP transactivating factors. Site-directed mutagenesis on these TGACG motifs abolished MeJA-responsive expression, clearly identifying them as MeJA-responsive elements. Sequence comparisons found no similar motif in other characterized promoters of MeJA-inducible genes, but suggested a common spatial structure which may serve as a binding site for transacting factors involved in the MeJA signal transduction pathway.

[Maxillary swing approach in the management of tumors in the central and lateral cranial base].

OBJECTIVE: A retrospective review of seventeen patients who were operated through the maxillary swing approach was carried out to assess the efficacy of this approach in the management of tumors of the central and lateral cranial base. METHODS: From May 2000 to January 2005, 17 patients with primary or recurrent neoplasms involving the central cranial or lateral base underwent surgical resection via maxillary swing approach. Ten patients were male, and other seven patients were female, and age range was 7 to 58 years, with a mean age of 42. 6 years. Eight patients had tumors originally involving lateral cranial base, and nine patients had tumors originated from central cranial base. The pathology spectrum was very wide. Among them, five suffered from chordoma, two had rhabdomyosarcoma, two had squamous cell carcinoma, one had malignant fibrous histiocytoma, one had malignant melanoma, one had esthesioneuroblastoma, one had invaded hypophysoma, two had schwannoma, one had pleomorphic adenoma, and one had angiofibroma. Three patients had received previous surgery, two patients had previous radiation therapy and nine patients received postoperative radiotherapy. RESULTS: Sixteen of all seventeen patients had oncologically complete resection, one had near-total resection. This group patients was followed up from 10 to 60 months, with a median follow-up time of 28 months. Two patients died 14 and 26 months after surgery respectively, as a result of local recurrence and metastasis. One patient defaulted follow-up at 12 months after operation, and the other 14 patients were alive at the time of analysis. Of the 12 malignant tumors, the 1-and 2-year survival rate were 91.67% and 72.92%, respectively. The facial wounds of all patients healed primarily, and there were no necrosis of the maxilla, damage of the temporal branch of the facial nerve, lower-lid ectropion, and facial deformity. Epiphora and facial hypoesthesia were detected in all patients. Four patients (23.5%) developed palatal fistula, ten patients developed serous otitis media (58.8%), and four patients developed a certain degree of trismus (23.5%). Cerebrospinal fluid leak occurred in two patients. They subsequently healed with conservative management. CONCLUSIONS: The maxillary swing approach is a proven method for access to the central and lateral skull base with good exposure and acceptable morbidity. Complications and sequelae associated with this approach include facial scarring, transaction of the infraorbital nerve, impaired lacrimal drainage, eustachian tube dysfunction and serous otitis, palatal fistula, trismus etc. Some procedures should be performed for reducing the incidence and severity of complications in the maxillary swing approach.

A longitudinal study of job stress in the nursing profession: causes and consequences.

AIM AND BACKGROUND: This study examines the influence of changes in work conditions on stress outcomes as well as influence of changes in stress outcomes on work conditions. As such, it answers questions still open in the literature regarding causality of work environmental characteristics and the health of nurses. METHOD: A complete, two wave panel design was used with a time interval of 3 years. The sample consisted of 381 hospital nurses in different functions, working in different wards. RESULTS: Changes in work conditions are predictive of the outcomes, especially of job satisfaction and emotional exhaustion. The strongest predictors of job satisfaction were social support from supervisor, reward and control over work. The strongest predictors of emotional exhaustion were work and time pressure and physical demands. Reversed relationships were also found for these outcomes. CONCLUSION: The results of this study are consistent with transactional models of stress that indicate that stressors and stress outcomes mutually influence each other. To prevent nurses from a negative spiral, it seems of importance to intervene early in the process.

Market power and hospital pricing: are nonprofits different?

Dramatic changes in hospitals' operating environments are leading to major restructuring of hospital organizations. Hospital mergers and acquisitions are increasing each year, and conversions by hospitals to different forms of ownership also are continuing apace. Such changes require policymakers and regulators to develop and implement policies to ensure that consumers' interests are protected. An important consideration in this process is the impact on the price of hospital care following such transactions. This paper reviews empirical evidence that mergers that reduce competition will lead to price increases at both merging hospitals and their competitors, regardless of ownership status. We show that nonprofit and government hospitals have steadily become more willing to raise prices to exploit market power and discuss the implications for antitrust regulators and agencies that must approve nonprofit conversions.

Training older adults to use automatic teller machines.

The present study assessed the success of several instructional programs in teaching the use of automatic teller machines (ATMs). Fifty-six older adults (aged 61 to 81) participated in the study, randomly assigned to each of four, 14-member groups. The description group received only a general overview of an ATM; the text guide group received written instructions for performing various transactions on an ATM; the pictorial guide group received written instructions accompanied by pictures of corresponding ATM screens; and the online tutorial group completed a step-by-step tutorial on a simulated ATM. Participants practiced on an ATM simulator. They were tested after a 24-h interval on their ability to perform familiar transactions on an unfamiliar ATM simulator and to perform completely novel transactions. Accuracy was best for the online tutorial group, intermediate for the text and pictorial guide groups, and worst for the description group. These data demonstrate both the importance of providing older adults with ATM training and the fact that the type of training influences the level of performance. The online tutorial, which provided specific practice on the task components, best facilitated acquisition and transfer performance.

Coping with stress at work.

This paper considers the quality-of-life concept from the point of view of harmony between fundamental human needs and environmental conditions. It is argued that the knowledge gained by research in psychobiology can aid in directing technological applications to suit human needs and abilities. Examples are given from a multidisciplinary research program concerned with the dynamics of stressful person-environment transactions, viewed from psychological and biological perspectives. Emphasis is placed on coping and adaptation in workers exposed to conditions characterized by underload, overload, and lack of control. On the basis of empirical results, it is argued that a moderately varied flow of stimuli and events, opportunities to engage in psychologically meaningful work, and to exercise personal control over situational factors, may be considered key components in the quality-of life concept.

The payers' automation challenge.

Despite some success stories, most of the nation's 300 commercial insurers and hundreds of managed care plans are in the earliest stages of using electronic data interchange for health care transactions. However, there are some encouraging signs of progress in the effort to shift to EDI, as more payers offer providers incentives to automate. Payers blame the relatively slow progress on cost, the absence of universally accepted standard electronic formats and uncertainties over the political debate on health reform. But providers say many commercial payers are moving too slowly to automate and are expecting providers to bear too much of the financial burden.

Efficiency and complexity in neural coding.

Neural coding in the retina and lamina of fly compound eyes is amenable to detailed anatomical, physiological and theoretical analysis. This approach shows how identified cell signalling systems are optimized to maximize the transmission of information. Optimization reveals three familiar constraints, noise, saturation and bandwidth, and shows how coding can minimize their effects. Experiments reveal a fourth constraint, metabolic cost, whose properties favour the distribution of information among multiple pathways. The advantages of distributed codes will be offset by increasing complexity and the build up of noise. The optimization of coding in fly retina suggests that both noise and complexity will be reduced by matching each step in the system's operations to the input signal, and to the logical requirements of the network's ultimate function, pattern processing. This line of argument suggests tightly organized networks, laid out that information flows freely and independently, yet patterned so that the necessary contacts and transactions are made quickly and efficiently.
