[Self-monitoring and self-management in the treatment of asthma].

Self-monitoring and self-medication are the key contents of an efficient asthma education program, which contribute not only to a reduction of morbidity and costs, but also to an enhancement of quality of life. Within the framework of action plans, the patient himself/herself can vary the medication administered in accordance with the actual momentary severity of the disease. In this way the patient overcomes his/her fear of an attack and is enabled better to deal with any worsening situation. With regard to the individual stage-oriented development of action plans,further research is needed.

[Some performance tests among athletes: their importance according to sport, their interests and interpretation].

In the multiple performance tests that actually exist, the choice of the most representative evaluation of the physical activity is crucial. For the general or field practitioner who wants to give his or her athlete the most useful informations for the follow-up, this choice may be very difficult. Nevertheless, the results of those tests, once a correct interpretation have been done, will give opportunity to built an efficient training's program, whatever the type of sports or the athlete's level. This change in the management of the sportsman is an answer to the request of the athlete or his trainer; from now on, they both want a more scientific training's planning, built on physiological parameters.

Development of a specific serological test and an efficient subunit vaccine to control bovine leukemia virus infection.

Study of the antigenic structure of the Bovine Leukemia Virus (BLV) envelope glycoprotein gp51 with a panel of mouse monoclonal antibodies (MAbs) has allowed the identification of biologically important determinants directly involved in the infectivity of BLV. Considering the various facts reported in this paper, it follows that diagnostic and vaccination procedures that make use of gp51 in a native configuration constitute a prerequisite for the design of an efficient BLV eradication program. To improve the efficacy of a serological detection test, MAbs have been selected as reagents of choice to develop a competition enzyme-linked immunosorbent assay (cELISA). Recombinant vaccinia virus expressing gp51 and gp30 was indicated as a very promising protective vaccine against BLV infection.

Intervention policy in endemic goitre areas.

The multifactorial nature and complex interactions of regionspecific environmental conditions with host factors in the pathogenesis of endemic goitre constitute a major challenge to the understanding and control of the problem in endemic areas. However, to control and prevent this important public health problem, the most obvious but difficult initial step requires substantial socioeconomic improvements in the affected areas of less developed countries, including, first, provision of efficient iodine prophylaxis programs, second, diversification of dietary constituents with adequate daily protein-calorie intake, and third, institution of proper sanitary conditions with effective water treatment to eliminate organic and bacterial pollutants. This last intervention is also a requirement to control and prevent goitre in the iodine-sufficient more developed countries. In this regard, more research is needed to provide effective ways of water treatment that can be applied in individual households or at the community level.

[The optimization of combined physiotherapy in the system of rehabilitating victims with complicated fractures of the extremities].

The schemes were worked out for optimization of complex physiotherapy in rehabilitation system of patients with complicated fractures of extremities. This work was carried out on the basis of 178 dynamic clinic observations with application of biomechanical and roentgenological methods, as well as methods of mathematical modelling and prognostication. That made it possible to increase the efficiency of rehabilitation programs by 3.2--4.1 times. The article deals with the perspective trends in the development of this problem.

[Results of 16-year use of the 06-Paris-LA-66 program in the treatment of children with acute lymphoblastic leukemia].

The treatment of children with acute lymphoblastic leukemia was conducted with the use of 06-Paris-LA-66 program modified by the authors. Complete remission was obtained in 91.8% of cases. Recovery was achieved and the therapy was discontinued in 16 out of 34 patients who had strictly adhered the program treatment, the treatment of 7 patients in complete remission is continued. The authors consider the program as high efficient and recommend its use.

Fully automated measurements by light microscopy of tissue sections using a cellular array computer.

Software was developed for the acquisition, segmentation and analysis of microscopic OD-images on a VICOM digital image processor, extended with a VISIOMORPH morphoprocessor board. The delineation algorithms for peroxisomes, lysosomes, and nuclei in liver, kidney, and adrenal gland sections start by thresholding the difference between the original image and a low pass filtered version. The resulting binary mask is then processed by morphological operations in order to produce an object overlay. The efficiency of the programs is evaluated by comparing delineated objects at different OD-levels, created by varying the stain or by multiplying the original pixel values with constant factors. Manual delineation on some images is also used as a reference. More complex algorithms are used for the delineation of muscle fibres in ATP-ase-stained sections and immunocytochemically labelled cells in monolayer preparations. Muscle images from parallel sections with different stainings are matched with a coordinate transform, enabling the transfer of the object mask from a single delineated image to the unprocessed images and thus obtain all necessary information for fibre classification. After segmentation, the OD-images and their object overlays are fed into a data extraction program, measuring for each delineated object user-selected features. Data are sent to a VAX for statistical interpretation.

[Teaching-inservice integration in primary care. Experience of the Facultad de Enfermería de la Universidad Autónoma de Nuevo León, Mexico].

This paper presents the reasons which justify the need to integrate teaching in-service and health care delivery in nursing, and the mechanisms to achieve efficiency in health programs. In 1976 the Autonomous University of Nuevo Leon (Universidad Autónoma de Nuevo León), as an educational and training center and as a means for scientific and humanistic endeavor to foster the development of the community it serves, began a joint experimental project with the health sector, by means of the Guadalupe Health Program and the Nursing Development Program, with the participation of the Schools of Medicine, Nursing and Dentistry. Special importance is attached to the objectives and organization of these nursing programs, the teaching in-service methodology, and the functions and responsibilities of teachers, students and support staff. The achievements of eleven years of work are reported.

Biologic, pathologic, and radiologic studies of cadaver kidneys preserved in Collins' solution.

Collins' solution is one of the world-wide methods used for kidney preservation. A long-term survey of transplanted kidneys thus preserved has never been reported. Clinical, biologic, radiologic, and pathologic data collected from 225 cadaver kidney transplantations were analyzed according to the total ischemic time (group 1: TIT less than or equal to 12 hr; group 2: TIT 12-30 hr). No differences could be demonstrated between the two groups, whether considering the percentage of immediate function, the serum creatinine level that was normal up to 2 years, the incidence of abnormal blood pressure, the pathologic changes on routine biopsies performed at 6 months and 2 years, or arteriographic features at 2 years. A better graft survival rate was consistently found at 6 months, 1 year, and 2 years when TIT was between 12 and 30 hr. This could be related to the higher incidence of well-matched kidneys in this group of patients, thereby emphasizing the efficiency of exchange programs for compatible kidneys.

Collins solution is one of the world methods used for kidney preservation. A long term survey of transplanted kidneys thus preserved has never been reported. Clinical, biological, radiological and pathological data collected from 225 cadaver kidney tranplantations were analysed according to the total ischemic time (T.I.T) groupe I: TIT less than or equal to 12 hours, groupe II: TIT from 12 to 30 hours). No difference could be demonstrated between the two groups whether considering the percentage of immediate function, the serum creatinine level which was normal up to two years, the incidence of abnormal blood pressure, the pathological changes on routine biopsies performed at 6 months and 2 years and arteriographic features at 2 years. Development and organization of outpatient surgery units: the hospital's perspective.

Many indicators suggest continued growth in the ambulatory surgery arena. Economic pressures, technology, and third-party payers are contributing to this trend. Hospital admissions and inpatient stays are declining. The patient base is shifting to more economical settings. The impact of these factors has been considered throughout this discussion. These occurrences have prompted hospitals to diversify and to create organizational structures that provide opportunities for a total health care business approach. The success of ambulatory surgery programs developed by hospitals has been a major contributor to the continuation of these reorganization efforts. Acceptance on the part of patients and physicians is the major contributing factor to the continued growth of outpatient surgery units. Marketing techniques and consumer-oriented programs, coupled with efficiency and reduced costs, also assure that the ambulatory surgery phenomenon is here to stay.

Population screening for breast cancer by mammography in The Netherlands. Expectations, early results, negative effects and conditions for large-scale screening.

Recent publications have shown clearly that screening by mammography is the only method of reducing breast cancer mortality substantially. This can only be achieved, however, by regular annual screening of all women over 40 years of age. An efficiently organized screening program that guarantees optimal mammographic quality should be developed in order to avoid negative effects of population screening such as false-positive and false-negative reports.

Influenza vaccination in the elderly: 2. The economics of sending reminder letters.

Reminder letters and follow-up telephone calls were used to increase influenza vaccination acceptance by 273 well elderly registered at an urban community health centre. The net effect of the reminder letters was to increase overall coverage to 43%, from 17% in the previous year. Follow-up telephone calls to patients who had not responded to the letters increased coverage to only 55%. Calculation of costs per additional vaccination given revealed that the use of reminder letters alone was much more cost-effective than follow-up telephone calls in increasing coverage. However, with the current fee-for-service reimbursement by medical care insurance in Ontario, neither means of improving vaccination coverage would result in net practice earnings. The implications for an effective and efficient annual influenza program in Canada are discussed.

A simple nutrition screening procedure for hospital patients.

Nutrition screening is the process of identifying hospital patients with a high risk for nutrition problems who may require comprehensive nutrition assessment. Dietitians at a 700-bed teaching hospital recently implemented a nutrition screening program remarkable for efficient use of existing personnel resources. The three-step procedure includes a nutrition questionnaire completed by the patient in the hospital admissions office, measurement of each patient's height and weight by an admissions nurse, and addition of the patient's serum albumin concentration plus summary and recommendations for nutrition intervention by a registered dietitian. The procedure reduces the time needed for individual evaluation from 25 to 5 minutes and results in 1 1/2-hour time saving per day per clinical dietitian. Patients designated as "high risk" by this method appear to be more seriously ill, as shown by significantly longer hospitalization. The nutrition screening procedure described is simple, efficient, and applicable to a wide variety of institutional settings.

Rapid acetylcholinesterase screening test for neural tube defect.

A 5-min optimized automated assay for measuring acetylcholinesterase (AChE) and total cholinesterase (ChE) in amniotic fluids is described in which a centrifugal analyzer is used. Total ChE was measured by using acetylthiocholine iodide and monitoring the color developed after 5,5'-dithiobis(2-nitrobenzoic acid) is added. We evaluated four direct and indirect procedures for measuring AChE in which thiocholine is generated. These methods involve either the specific enzyme inhibitors ethopropazine and BW284C51 or the specific substrates S-butyrylthiocholine and acetyl-beta-methylthiocholine. We found the last method distinguishes normal pregnancies most efficiently from those with neural tube defect. Filtering samples through 0.22-micron membrane filters before analysis decreased the incidence of false positives. Significantly increased AChE concentrations were noted in all abnormal pregnancies (n = 45) and in 4.8% of normal pregnancies (n = 310). The rapid AChE assay involving acetyl-beta-methylthiocholine, when applied alongside a rapid alpha-fetoprotein assay, increases the efficiency of screening programs for pregnancies with neural tube defect to 99.4%. It also identifies samples with negative results for alpha-fetoprotein that require further examination by the sensitive gel-electrophoretic test for AChE.
