[Exposure of surgical/orthopedic operating room personnel to monomer vapors during the use of bone cements--review of the literature and report of experiences].

PMMA bone cements are used for the fixation of artificial joint replacements. During their application in the operating theater, a small amount of the monomer evaporates. Operating room staff and surgeons are often not adequately informed about the negative properties and the effects on health of these vapours. Basic information about monomers and their toxicity, and safety regulations have been compiled. On the basis of present knowledge, the authors do not regard the repeated use of bone cement throughout the course of a day to represent a hazard to health.

New study surveys data on operating room efficiency.

The folks doing knee and hip replacement surgery at Bryn Mawr (PA) Hospital must be doing something right: For the second year in a row, the 330-bed hospital has been named Best Performer for operating room effectiveness and efficiency by OR Benchmarks, a Santa Fe, NM-based benchmarking company. The hospital was one of a dozen studied that provided data to OR Benchmarks on 51 cases of total knee replacements. Thus far, the company has collected data on 880 cases over the past five years. Data collected ranged from cost of supplies to use of clinical pathways.

Case-mix management in the operating room.

Faced with stiff competition from freestanding surgical centers, hospitals can increase revenue by knowing product-line costing and closely monitoring productivity. This article describes three applications of an acuity-based case-mix management system in the operating room of Primary Children's Medical Center, Salt Lake City.

Outcome of trauma patients who present to the operating room with hypotension.

The case records of 101 patients with trauma who presented to the operating room (OR) for emergency surgery and had a systolic blood pressure (sBP) less than 90 mm Hg over a period of 3 years were reviewed. The sBP was 70-89 mm Hg in 47 patients, 50-69 mm Hg in 19 patients, and unobtainable in 35 patients. The mortality rates for these three groups were 6 per cent, 79 per cent, and 86 per cent, respectively. The increase in mortality rate with a sBP less than 70 mm Hg was highly significant (p less than 0.001). The duration of initial shock in the OR and emergency department (ED) exceeded 30 minutes in 56 patients, and 35 patients (62%) died. In contrast, of 45 patients who had less than 30 minutes of shock, only 13 patients (29%) died (p less than 0.001). Of the 54 patients who presented to the OR with a sBP less than 70 mm Hg, 42 patients had early cross-clamping of the thoracic aorta. This was performed as a prelaparatomy thoracotomy in 25 patients with abdominal injuries and as part of the thoracotomy in 17 patients with chest trauma. Nineteen (45%) of these 42 patients responded favorably to aortic cross-clamping with a sustained increase in sBP to greater than 90 mm Hg within 5 minutes. Of these 19 patients, 42 per cent survived. The 23 patients who did not respond to aortic cross-clamping died in the OR.(ABSTRACT TRUNCATED AT 250 WORDS)

Activity-based costing in the operating room at Valley View Hospital.

This article presents an example of how one hospital reports the results of activity-based costing (ABC). It examines the composition and supporting assumptions of an ABC report for a particular procedure in the operating room (OR). It describes management uses of the information generated. It comments upon how the continuous quality improvement (CQI) is synchronized with the ABC reporting.

[The anesthetist and operating room hygiene].

Environmental contamination with bacteria in the course of extubation was shown to be minimal as compared to contamination of the hands and gowns. 7 out of 21 anaesthesiologists were carriers of staphylococcus aureus. Two persons spread staphylococci to their patients and the operating room environment in spite of strict adherence to conventional hygienic policies.
