Log files analysis to assess the use and workload of a dynamic web server dedicated to end-stage renal disease.

A Multi-Source Information System (MSIS), has been designed for the Renal Epidemiology and Information Network (REIN) dedicated to End-Stage Renal Disease (ESRD). MSIS aims at providing reliable follow-up data for ESRD patients. It is based on an n-tier architecture, made out of a universal client, a dynamic Web server connected to a production database and to a data warehouse. MSIS is operational since 2002 and progressively deployed in 9 regions in France. It includes 16,677 patients. We show that the analysis of MSIS web log files allows evaluating the use of the system and the workload in a public-health perspective.

[Management of the adult population in ten psychiatric sectors. A new active file from a single year].

In 1983, a study was conducted during one year on the active file of patients of ten public psychiatric sectors. In the first part of this study, the social characteristics of the patients were compared to those of their population of reference. This comparison defined a profile of the psychiatric patients. It also showed that males had a socio-demographic profile differing significantly from the general population by age, marital and occupational status. For females, differences were found by age, marital status and nationality. Considering professional status, a remarkable similarity exists between the females and the general population.

[Setting up a CMV negative donor file].

Cytomegalovirus (CMV) infection is serious to immunocompromized individuals such as recipients of bone marrow transplantation, preterm infants and other immunosuppressed patients. Since there is no effective treatment as yet, prevention is the most effective way so far to avoid CMV infection. Blood transfusion is one of the dangerous sources and screening for CMV negative donors is mandatory, therefore, to provide blood components for immunocompromized patients. We have screened 2,015 donors at various ages for CMV antibody by using passive latex agglutination test (PLA). The results showed that the CMV antibody positive rate was 91.7% in Taiwan. It differs from those of the western countries (40-79%) and the countries of Africa and Asia (96-100%). It has also shown that the positivity rate is closely related to the age of donors. The seronegative rates of ages among 18-20, 21-30, 31-40, 41-50 and 51-60 years old were 10.7%, 9.8%, 3.9%, 1.2% and 0% respectively. It has suggested thus that the older the donor is, the higher the seropositive rate will be. Therefore, we should choose the donors of younger age as candidates for making CMV negative donor file.

An update on endodontics: 2. File manufacture and obturation techniques.

In recent years manufacturers have responded to an increase in endodontic treatments by developing a number of new products. New techniques have also been introduced. In this two-part article the author critically reviews these developments. In Part 1 he looked at diagnosis of endodontic problems and at root canal preparation techniques. In Part 2 he now examines file designs together with techniques for root canal obturation.

[The use of ultrasonic files in canal preparation].

The continuous high volume of irrigating solution delivered by the ultrasonic system facilitates the root canal debridement. An excellent cleaning of dentin wall is obtained even on surfaces unreached by the mechanical instrumentation. In order to obtain an efficacious preparation, the use of ultrasonic files were combined with instrumentation. This new technique is described in this article.

[Study of the section of different brands of K files and broaches].

The authors of this article observed the cross section of different brands of K file and reamer, in order to compare the reality to the square or triangular symbol. On an other hand, they defined the ideal cross section of the endodontic instrument according to the user's requirements, and studied the most approaching one.

Single-file diffusion of uncharged particles.

An equation for unidirectional fluxes of nonelectrolytes and for the total flux in the single-file transport through a narrow pore has been derived. The equation obtained accounts for the correlations of the population of the pore in the coordinate of transport. The problem has been solved using superposition approximation and unidirectional fluxes has been found. The population profile in the pore was shown to have nonlinear shape; this is principally different from the results of the classical diffusion approach.

Using linked birth and infant death files for program planning and evaluation: NIMS workshop lessons.

Health planners should base program decisions on the best information available. Combining information from different sources can be valuable in identifying problems--the essential first step in program planning. To facilitate this process, a workshop was conducted during the National Infant Mortality Surveillance Conference in Atlanta, GA. Maternal and child health directors explored the use of linked birth and infant death data for program planning and evaluation. Linked birth and infant death certificate files permit evaluation of infant mortality by birth weight and other infant and maternal characteristics, thus providing more detailed information than birth or death certificates alone. An assessment of the birth weight distribution of live births, birth weight specific-mortality risks, distribution of deaths by birth weight, and birth weight-specific causes of death can help identify problems in the childbearing population and with the delivery of health services. Once the infant health problems are defined clearly, the selection and delivery of services can be better targeted and evaluated for the reduction of these problems.

[Setting-up a national file of bone marrow volunteer donors].

The lack of HLA matched sibling donors has become a problem. Recent improvements suggest the feasibility of marrow transplantation from unrelated donors. A program to obtain volunteer bone marrow donors has been developed. A total of 3,853 HLA A B typings were performed. A search for 83 patients was initiated; 51 of them had one or more HLA A and B (61%), 8 HLA A, B and DR antigen identical donors, 2 did not react in mixed lymphocyte culture. This experience confirms the necessity of increasing the donor file but the size remains to be established.

[Statistical analysis of a file of angiologic data on a micro- computer. Discriminant analysis of occlusive impedance rheoplethysmography data].

The statistical analysis by multidimensional analysis of the data of a computerized file should lead to the improvement of the quality of the examinations. A discriminant analysis of the data of occlusive impedance rheoplethysmography allows a global analysis of all of the indices, by taking into account both their absolute values and their relative values. This results in a better compromise between the sensitivity and the specificity. The graphic representation gives an immediate conclusion and allows the functional outcome to be monitored.

Storing summary patient data as a microcomputer file.

The time taken to summarize 1000 patient records and store this data as a hard disc file on a microcomputer was evaluated in a health centre with about 7300 patients. On average it took 25 minutes to prepare or update summary sheets for each patient and between three to five minutes to input the data to the computer. About one in 10 of the records were long and complicated and took considerably more time to summarize. An error rate of 9% over six months was determined for the age-sex register, mostly owing to patients who had left the practice. In about 5% of cases important information was missing from the A4 folders and was only obtainable from the health visitor records.

[The 2nd saccoradiculography: changes in the post-radiculographic and postoperative radiological morphology (340 cases apropos of 2,292 files)].

A study was made of the files 2292 patients examined by sacco-radiculography with watersoluble contrast medium, of whom 340 had been subjected to a second examination. 284 of these had undergone surgery with good results during the interval between the two examinations. In 56 cases, repeat sacco-radiculography was carried out but no operation performed. In both groups of patients a semiological study was made of the radiological appearances to determine which were the effects of the contrast medium and which could be attributed to the operation. The paper concludes with a discussion of the problem of genuine and false albuminoachies of sacco-radiculography using meglumine locarmate (dimer-X).

Cases from the Aerospace Medicine Residents' Teaching File.

"From the aerospace medicine residents' teaching file: toxic peripheral neuropathy, sacroiliitis, and mitral valve prolapse." The clinical presentation, evaluation and diagnosis of an aviator exposed to N-Hexane and Butanone are discussed. The aeromedical disposition of this patient, who also had mitral valve prolapse and subclinical sacroiliitis, is also presented.

[On the meaningful filing period for x-ray films (author's transl)].

A total of 19,577 x-ray films made from 5,092 patients was examined, subdivided into 94 different kinds of examination. The following observations were made: The time since the last x-ray examination of the patient up to Jan. 6 1973 was less than 2 years in 30.1% and less than 5 years in 63.8% of the patients. These figures are due to the fact that the hospital is still very young. The period between two successive x-ray examinations was less than 2 years in 88.2% and less than 5 years in 95.8% of the patients. It is obviously not necessary to file roentgenographs for more than 5 years, since after that time only 4.2% of the patients return for a follow-up examination, and only a fraction of these can probably be regarded as really remarkable (Brolin).

Access to social security microdata files for research and statistical purposes.

This article focuses on the characteristics of SSA microdata files and on the development of a disclosure policy aimed at serving the public interest while protecting the privacy of individuals and the confidentiality of research and statistical information. Several dimensions of the disclosure question are explored: A description of the persons (both living and dead) and other entities that are the data subjects; the ability of users to associate known data subjects with information about them; the sources of data; expectations as to recontact of data subjects; and the terms and conditions under which microdata are released to users outside SSA. The factors controlling the decision whether or not to release microdata are discussed. The factors range from those intended to protect the data subject--the criteria specified by law for maintaining confidentiality for example and the principles applied by SSA in assessment of disclosure risk--to those more concerned with agency function, such as financial cost to the agency, and interference with its primary mission. Some particular practices are described to illustrate application of present policy principles. Brief attention is given to future implications of certain current developments such as the Privacy Act, the Sunshine Act, and the Tax Reform Act of 1976.
