Colorimetric quantification of erythema--a comparison of two colorimeters (Lange Micro Color and Minolta Chroma Meter CR-200) with a clinical scoring scheme and laser-Doppler flowmetry.

Erythema elicited by the local irritant sodium lauryl sulphate was studied in comparison with unexposed skin. Colour coordinates of the CIE system were recorded using two commercially available colorimeters. With both colorimeters a positive movement of the alpha-axis (toward red, P less than 0.001) and a negative movement on the L-axis (toward dark, P less than 0.01) were registered. Technical and in-vivo experimentation showed the two pieces of equipment to be accurate. However, a simple conversion factor cannot be established. Both pieces of equipment correlated positively with clinical scoring of erythema and with measurement of cutaneous blood flow by laser-Doppler flowmetry. In conclusion, the two colorimeters appeared comparable. Colorimetry by the CIE system, which takes into account the non-linear colour perception of the human eye, was found useful with potential value in both experimental and clinical dermatology. The techniques appeared suitable for routine testing.

Bleomycin-induced flagellate erythema.

A 56-year-old woman who developed widespread pruritus and flagellate erythema after attempted pleuredesis with bleomycin is described. The raised linear lesions of flagellate erythema could not be reproduced by scratching, and histopathological examination revealed a lymphocytic vasculitis. The rash faded spontaneously over several weeks to leave streaks of post-inflammatory melanoderma which remained for 6 months. The role of scratching and dermographism in the pathogenesis of the bleomycin-specific eruption is discussed.

Acute febrile neutrophilic dermatosis in association with erythema nodosum and sarcoidosis.

We report two cases of simultaneous Sweet's syndrome and erythema nodosum. We believe this to be a real association rather than the extension of the neutrophilic infiltrate of Sweet's syndrome into the subcutaneous fat. In one of our cases, concurrent erythema nodosum and Sweet's syndrome was a presentation of sarcoidosis with bilateral hilar lymphadenopathy. To our knowledge, this is the first report of Sweet's syndrome with sarcoidosis.

Erythema nodosum-like eruption in sarcoidosis.

Two cases of sarcoidosis with tender, erythematous nodules on the legs are reported. The cutaneous lesions were clinically similar to those of erythema nodosum, but histologically showed non-caseating epithelioid granulomas. A review of 14 cases of this particular sarcoid eruption reported in Japan showed that 13 had ocular involvement as in our cases. In the light of the high frequency of ocular involvement, a skin biopsy should be considered in patients presenting with erythema nodosum-like eruptions situated on the legs.

Oral terbinafine and erythema multiforme.

Two patients developed classical erythema multiforme while taking oral terbinafine. A case of Stevens-Johnson syndrome occurring after terbinafine therapy has recently been described, but there have been no published reports of an association with erythema multiforme until now.

Sarcoidosis presenting as erythema annulare centrifugum.

Erythema annulare centrifugum (EAC), a disease belonging to the poorly characterized category of 'figurate erythemas', has been associated with a variety of conditions, such as connective tissue diseases, infections, neoplasms and drug reactions. Here we report a case of EAC associated with sarcoidosis, the first case in the literature to our knowledge. EAC was the sole sign of the granulomatous disease process, which was diagnosed by means of appropriate investigations only after the patient reported the sudden resolution of a long-standing sensitization to perfumes and parabens. Steroid treatment for sarcoidosis improved the patient's condition, and restored the allergic response to these substances.

Exfoliative erythema of malnutrition with zinc and essential amino acid deficiency.

We present a patient with a desquamating predominantly flexural erythema and glossitis due to a combination of alcoholism, zinc deficiency and amino acid deficiency. A similar clinical picture to necrolytic migratory erythema can be seen with zinc deficiency or protein malnutrition, often in patients with alcoholic liver disease, in the absence of glucagonoma. The speed of clinical improvement following zinc replacement therapy, usually within days to weeks, is striking, confirming the clinical diagnosis.

Erythema multiforme infantum atrophicans.

A new case of erythema multiforme is described, characterized by multiforme eruption of erythematous papules, plaques and occasional bullous lesions. The acute stage lasts several seeks, then atrophy is noted. The face appears senile. Histopathologically, the lesions show erythema multiforme in the acute phase; in the atrophic stage, a disappearance of the elastic fibers was observed.

Erythema nodosum leprosum in Nigeria.

The occurrence of erythema nodosum leprosum among leprosy patients treated with diamino diphenyl sulfone in an endemic area is not rare, unlike in the leprosy patients treated at Lagos University Teaching Hospital. Two Nigerian patients with this disease are described, in whom this rarity was seen. Recurrent monthly fever with painful skin swellings in lepromatous patients from an edemic area is highly suggestive of erythema nodosum leprosum. The occurrence of this reaction in lepromatous leprosy gives a relatively poor prognosis.

The effect of antihistamines on ultraviolet-light-induced erythema.

Because of the possible role of histamine as an early mediator of ultraviolet light induced erythema, we studied a group of volunteers treated with both H1 and H2 blockers, alone and in combination, before and at various time intervals after, UVL irradiation. No difference was found in the degree of erythema produced by any of the antihistamines used, whether alone or in combination, when compared with the saline control.

Epidemiology of an outbreak of erythema infectiosum in Tokyo.

Endemic outbreaks of erythema infectiosum were observed in various areas of Japan from 1977 to 1981. Even in a limited district of Tokyo, we recorded 395 cases until June 1981, so this might be the largest epidemic in Japan. Most cases occurred from January to June. The peak incidence was at the age of 7 years and the average age was 8.9 years. The outbreak also occurred among our hospital nurses, but no severe case was observed in hospitalized patients. Furthermore, the symptoms in adults were as mild as in children, suggesting the little participation of immunity in this disease. By epidemiologic survey, erythema infectiosum was distinct from rubella.

Linear IgA bullous dermatosis mimicking erythema multiforme in adult.

This report describes a 49-year-old woman with an erythema multiforme--like rash and direct immunofluorescence showing linear IgA deposits at the dermoepidermal junction. Light microscopy revealed features of bullous pemphigoid, dermatitis herpetiformis, and erythema multiforme; immunoelectron microscopy demonstrated IgA deposition beneath the lamina densa about anchoring fibrils. These data provide additional information about the variable clinical and histologic manifestations of the adult linear IgA bullous dermatosis and emphasize the diagnostic dilemmas of light microscopy, which are resolved by immunohistochemical methods.

Necrolytic migratory erythema: clinicopathologic study of 13 cases.

BACKGROUND: The clinical mucocutaneous manifestations of glucagonoma syndrome are recognized easily when they occur in the classic pattern of acral or periorificial lesions evolving in recurrent crops, with an annular and migratory distribution, in a patient with diabetes mellitus who has had recent weight loss and anemia. Not infrequently, noncharacteristic clinical and histopathologic features are observed and, in these cases, the diagnosis of pancreatic neoplasm may be delayed. AIM: To review the clinical and histopathologic features of cutaneous manifestations of glucagonoma syndrome. METHODS: The clinicopathologic features of 13 patients (eight women) with widespread or localized cutaneous eruption as a manifestation of islet cell pancreatic carcinoma with marked glucagon secretion (glucagonoma) were reviewed. RESULTS: The definitive diagnosis of the cutaneous eruption was established at the time of diagnosis of the pancreatic neoplasm (three patients) or afterwards (10 patients). In nine patients, the mucocutaneous manifestations preceded the diagnosis of the pancreatic neoplasm by 1 month to 3 years (mean, 12 months). In only eight biopsy specimens were the histopathologic features considered to be suggestive or characteristic of necrolytic migratory erythema. Diffuse parakeratosis, that occasionally arose abruptly from normal epidermis, was observed in 12 biopsy specimens. By the time necrolytic migratory erythema was diagnosed, the pancreatic carcinoma had metastasized to the liver, regional lymph nodes, or bone in 12 patients. CONCLUSION: Increased awareness of the polymorphic mucocutaneous and nonspecific histopathologic features of glucagonoma syndrome is needed to avoid unnecessary delay in the diagnosis of this syndrome.

Necrolytic acral erythema in Egyptian patients with hepatitis C virus infection.

BACKGROUND: Necrolytic acral erythema (NAE) is a distinctive skin lesion that was first described in 1996 with only few cases being reported, mostly from Egypt. It is unique in its acral distribution and exclusive association with hepatitis C virus (HCV) infection. METHODS: Twenty-three patients (mean age 41.7 +/- 11.5 years; M:F 10:13) with clinical features consistent with NAE were enrolled in a 3-year period. Five of those were known HCV-infected individuals and 18 were referred by the dermatologist for evaluation and HCV screening. Liver function tests, serum zinc, hepatitis B markers, HCV antibodies and HCV-RNA were tested. All patients were subjected to skin biopsy examination; five lesional biopsies were selected for electron microscopic examination and capillary endothelium was scanned for hepatitis C viral particles. An additional five patients were subjected to detection of HCV-RNA in their skin biopsies by polymerase chain reaction. All patients received oral zinc sulfate supplementation while interferon-alpha therapy combined with ribavirin was available for four patients. RESULTS: Most NAE patients were adults (91.3%) and the skin lesions were predominantly chronic (78.3%), with affection of the dorsa of toes and/or feet in all cases. Skin biopsies showed hyperkeratosis, psoriasiform epidermis and upper epidermal necrosis. Electron microscope examination demonstrated clumped tonofilaments in the keratinocytes, yet HCV-RNA could not be detected in the skin lesions of examined cases. Interferon-alpha combined with ribavirin caused regression of skin lesions in three patients and complete clearance in one patient. Some improvement was induced by oral zinc administration. CONCLUSION: Necrolytic acral erythema is considered to be a cutaneous marker for HCV infection. The majority of patients are diagnosed by dermatologists. Therefore, improved awareness of this cutaneous lesion should prompt early diagnosis and treatment of HCV, which should in turn cure the lesion and prevent progression of liver disease.

Appearance of membranocystic lesion (Nasu)-like changes in Behçet's syndrome. An electron microscopic study of erythema nodosum-like lesions.

Membranocystic lesion (Nasu)-like changes were seen in the subcutaneous fat tissues of erythema nodosum-like lesions in three of 18 patients with Behçet's syndrome. The membranocystic structures, which were derived from degenerative fat cells with a loss of the cell membrane, were observed in the edematous connective tissues accompanied by fibrin deposition and diapedesis of erythrocytes. Electron microscopic evaluation revealed two different patterns of peculiar membranous structures in the cytoplasm of the degenerative fat cells. One was proliferating membrane structures such as well developed Golgi-endoplasmic reticulum, and the other was minute tubular structures such as intracytoplasmic microvesicles. These degenerative fat cells are considered to be derived from abnormal fat cells which lack a cell membrane, but whose cellular and nuclear configurations appear to be normal morphologically, because the cytoplasm of these abnormal cells showed the same tubular structures as those seen in the characteristic architecture of membranocystic lesion-like change. The relation between the appearance of such abnormal cells and the pathologic conditions of erythema nodosum-like lesions in Behçet's syndrome was discussed.

Cutaneous reactive angiomatosis occurring in erythema ab igne can cause atypia in endothelial cells: potential mimic of malignant vascular neoplasm.

Erythema ab igne (EAI), an old and rare disease, is an erythematous, often pigmented, reticular, macular dermatosis that occurs at the site of repeated exposure to moderate heat. Reported herein is an unusual case of EAI occurring in a 33-year-old woman with a very broad lesion of reticular erythema and pigmentation on the lower extremities. The patient frequently put her lower extremities close to a heater in the wintertime to alleviate chill. The lesion started a decade ago, and it gradually became conspicuous. Microscopic findings showed a proliferation of small blood vessels in a thickened papillary dermis, not as typical as seen in EAI, but as seen in cutaneous reactive angiomatosis. They were arranged as small lobules and associated with hyalinization, edema and delicate fibroplasia. Many vessels were lined by plump endothelial cells, some of which had enlarged hyperchromatic nuclei. Many of these cells were multinucleated. Similar-appearing cells were associated with concentric foci of hyalinization without vascular lumina. A few atypical mitoses were observed. The lesion became much less conspicuous after the patient started avoiding close exposure to a heater, without any other special treatments. The aforementioned changes may be confused with malignant vascular neoplasm because of unusual cytological atypia and atypical mitoses in the endothelial cells.

Erythema after laser skin resurfacing.

Erythema after laser skin resurfacing is seen by many as a necessary evil in order to get good results from the procedure. A critical review of widely accepted concepts may lead to diminishing the length of postoperative erythema in patients undergoing laser resurfacing. The authors report on two previously unrecognized factors in the causation of this problem.
