Ejaculations induced by p-chloroamphetamine (PCA) in rats, hamsters and mice.

The ejaculatory response induced by p-chloroamphetamine (PCA) in male rats, hamsters and mice was observed during 2 hours after the injection. The animals were treated intraperitoneally with PCA at doses ranging from 0.78125 to 160 mg/kg. The ED50 (effective dose in 50% of animals) values of PCA for the initiation of ejaculation in rats and hamsters were 1.3397 (1.0732-1.6725) and 0.1105 (0.0802-0.1522) mg/kg, respectively. On the other hand, no ejaculation was observed in any mice at any doses examined. So we concluded that there are species differences in the ejaculatory response, induced by PCA, among rats, hamsters and mice.

HIV seroconversion in two homosexual men after receptive oral intercourse with ejaculation: implications for counseling concerning safe sexual practices.

Seroconversion for HIV antibody occurred in two homosexual men who reported no anal intercourse for greater than or equal to 5 years and multiple episodes of receptive oral intercourse with ejaculation. Neither man reported intravenous drug use or receipt of blood products. The last antibody-negative specimen was also negative by the polymerase chain reaction and p24 antigen assays. All sexually active persons should be clearly counselled that receptive oral intercourse with ejaculation carries a potential risk of HIV transmission.

Anhedonic ejaculation with desipramine.

Sexual dysfunction secondary to antidepressant drugs is an under-recognized clinical problem, and likely an important factor in noncompliance with treatment. We report two patients with anhedonic ejaculation (ejaculation without orgasm) associated with initiation of treatment with desipramine.

Secondary premature ejaculation.

Secondary premature ejaculation is a definite entity which can be due to a variety of causes--physical, pharmacological and psychological. The sparse literature on this subject is reviewed and a series of 25 cases of diverse and sometimes unusual aetiology reported. The fact that ejaculatory control can be adversely affected by a variety of physical and psychological conditions has implications for theories as to the cause of the much commoner entity of primary premature ejaculation, for which there is still no generally agreed-upon aetiological formulation.

Pregnancy-associated plasma protein-A (PAPP-A) and immunosuppressive activity in split ejaculates.

Pregnancy-associated plasma protein-A (PAPP-A) and phytohemagglutinin (PHA)-induced lymphocyte transformation were measured in split ejaculates of 17 normal volunteers. The concentration of PAPP-A was identical in both fractions of the ejaculate but the first fraction was twice as immunosuppressive as the second fraction. The origin of PAPP-A in men as well as its possibility of being an immunosuppressor is discussed.

Sensory evoked potential and effect of SS-cream in premature ejaculation.

The cause of premature ejaculation (PE) has been thought to be psychological in the majority of patients but we investigated penile hypersensitivity for an organic basis of PE. For another organic basis of PE, we have suggested hyperexcitability of the ejaculation center. SS-cream is a topical agent containing 9 oriental herbs for treating PE. Clinically SS-cream has been effective in the treatment of PE. Therefore, in order to implicate the organic basis of PE and realize the effect of SS-cream on PE, we investigated the somatosensory evoked potential (SEP) in patients with PE(16 cases) and the effects of SS-cream on SEP for treating PE. The latencies and amplitudes of the evoked responses were measured by two different places in stimuli, one was on the penile shaft with ring electrode and the other on the glans penis with a surface electrode. The latency of SEP stimulated at the glans penis was significantly longer than that stimulated at the penile shaft (p < 0.05). The latency stimulated at the glans penis after applying SS-cream was significantly longer than before applying SS-cream (p < 0.05), which was near the level of a normal potent man. But the latency stimulated at the penile shaft has no significant difference between before and after the application of SS-cream (P > 0.05). The amplitudes of the evoked responses stimulated at the glans penis were significantly higher than those stimulated at penile shaft (p < 0.05). And both these amplitudes were significantly reduced with the application of SS-cream (p < 0.05). With these result, we can suggest that the patients with PE have glans penile hyperexcitability and it provides further implications for an organic basis of PE, SEP stimulated at the glans penis can be a very useful method to evaluate PE, along with SEP stimulated a penile shaft and SS-cream prolongs the sensory conduction and reduces the penile hyperexcitability of the patient with PE.

[AIH in male infertility due to retrograde ejaculation after retroperitoneal lymph node dissection].

A patient with testicular cancer (non-seminoma, stage IIB) who had undergone resection for primary disease and retroperitoneal lymph node dissection as well as chemotherapy and radiotherapy 6 years before developed iatrogenic retrograde ejaculation. The infertility was treated by oral medications, including herbal medicine and injections of a hormone preparation as well as artificial insemination of husband (AIH). After 24 sessions of AIH for 4 years, his spouse gave birth to a healthy baby girl. We believe that for patients with iatrogenic retrograde ejaculation AIH is an effective method of increasing fertility to be employed more often.

[The experimental evaluation of the difference in closing dynamics of internal urethral ostium on the urinary continence and on the ejaculation induced by an administration of phenylephrine].

BACKGROUND: The internal urethral ostium is usually closed both on the ejaculation and on the urine collection, although the difference of the closing characteristics have not been known. METHODS: The difference in closing style of bladder neck (internal urethral ostium) between on the phenylephrine administration corresponding state to the ejaculation and on the filling of urinary bladder corresponding state to the urinary continence was evaluated by Stereo-UPP method using mongrel dogs. RESULTS: On phenylephrine administration, the length between verumontanum and bladder neck was lengthened, and a remarkable pressure increase was observed in the area. On the urinary continence filling physiological saline in the bladder, the length of the urethra on the UPP curve of the anterior wall of urethra was lengthened (10%) and the base of bladder was gradually become flat. The pressure in the bladder neck was not relatively increased. CONCLUSIONS: The internal urethral ostium was closed both on the phenylephrine administration and on the urine collection, although the closing characteristics of each was very different. It was demonstrated that on the phenylephrine administration the area between the verumontanum and the bladder neck was lengthened, together with the rise of pressure in the area, whereas on the urine collection bladder neck was closed in low pressure by flattening the base of bladder.

Risperidone-induced absence of ejaculation.

The absence of ejaculation in two patients treated with risperidone is described. This side-effect has rarely been reported. The mechanism responsible for ejaculation dysfunction in risperidone treated patients is unclear, but might be due to the alpha1-adrenergic antagonist action of the drug.

Characteristics of the successive jets of ejaculated semen of stallions.

The use of an 'open' artificial vagina made it possible to separate the individual jets in the ejaculate of the stallion. A total of 132 jets were collected in this way from twenty-one ejaculates. After macro- and microscopic examination and sperm counting, glycerylphosphorylcholine (GPC), ergothioneine and fructose levels were determined for every jet. The values obtained for the first three jets were similar and accounted for about 76% of all spermatozoa, and 70% of GPC. These jets also contained the highest concentration of ergothioneine and fructose.

[Vacuum-constriction therapy in association of erectile dysfunction and premature ejaculation].

Choice of treatment in combination of erectile dysfunction with premature ejaculation (PE) is not a simple task because of multifactor etiology of these diseases and physiologically antagonistic stimulating domination of parasympathetic autonomic nervous system for erectile function and sympathetic one for ejaculation. Therefore, some present-day effective treatments for each of the above diseases are contraindicated in their combination. 5 patients with the above combination gave an informed consent to vacuum-constrictor therapy (VCT) with application of ErecAid Classic Osbon unit. Control examination 6 months after VCT stated rehabilitation of the erectile function. Ejaculatory latency increased insignificantly, but erection could sustained coitus for 30 minutes.

[Comparison of the efficacy of ICSI using either spermatozoa aspired microsurgically from the epididymis (MESA-ICSI) or obtained from the ejaculation].

OBJECTIVE: Microsurgical epididymal sperm aspiration and intracytoplasmic sperm injection (MESA-ICSI) is a viable alternative to the treatment of couple sterility caused by obstructive azoospermia. Because there are structural differences between epididymal and ejaculated spermatozoa and it is not univocally stated, if this fact influences positively or negatively the results of ICSI, it was decided to compare the efficacies of ICSI using both the kinds of spermatozoa at the same setting. DESIGN: Retrospective comparative analysis. MATERIALS AND METHODS: The MESA group consisted of 27 couples and the control ICSI group of 154 couples--both matched in age, duration of sterility, health status, stimulation protocols and luteal supplementation. The trial was performed at the same period in one center, keeping the same laboratory and personal conditions in both the groups. RESULTS: In the MESA group the fertilization rate was 53.9%, the cleavage rate 87.6% and pregnancy rates 37.0% per microinjection and 43.5% per embryotransfer. In the control group the corresponding values were 50.1%, 90.0%, 28.0% and 31.7%. The differences between the groups were insignificant. CONCLUSIONS: It may be concluded, that the structural differences between epididymal and ejaculated spermatozoa do not significantly influence the efficacy of ICSI, however further investigations are still needed.

[Intracytoplasmic injection with sperm from ejaculation, epididymis and testis in treating different male infertility with oligo-asthenozoospermia or azoospermia ].

OBJECTIVE: To retrospectively study 400 intracytoplasmic sperm injection (ICSI) cycles by using three different sources of sperm. METHOD: Different male infertility with oligo-asthenozoospermia or azoospermia was treated by ICSI using sperm from ejaculation, epididymis and testis. RESULTS: In the ejaculation group (n = 277), the fertilization rate was 69.6% and the clinical pregnancy rate 28.9%. In the 104 cycles using epididymal sperm, the fertilization and clinical pregnancy rate was 65.9% and 37.5% respectively. And the fertilization and clinical pregnancy of testis sperm (n = 19) was 65.7% and 21.1%. The difference of fertilization and clinical pregnancy rate among the three groups were not significant. CONCLUSIONS: Male infertility of different causes can be treated by ICSI. The outcome has nothing to do with the common parameter of semen except that the sperm motility is 0, but the effectiveness of ICSI should be studied by long-term observation.

[Medication for premature ejaculation].

Drugs for the treatment of premature ejaculation (PE) are divided to two categories: oral drugs and local drugs. Oral drugs include antidepressive drugs, alpha-adrenoceptor blocking drugs, phosphodiesterase type V blocking drugs and Chinese herbal medicine. Local drugs include local surface drugs, intracavernosal injective drugs and local urethra drugs. Antidepressive drugs are extensively used, which have moderate efficacy, relatively more side effects and high recurrence rate; alpha-adrenoceptor blocking drugs are seldom used and are less effective than antidepressive drugs; phosphodiesterase type V blocking drugs like sildenafil have good efficacy and few side-effects and are worthy to be studied further. Local surface drugs like SS-Cream have good efficacy and few side-effects and are worthy to be applied and promoted; local urethral drugs like MUSE and Befar may become a new method to treat PE after being further studied. Medication for premature ejaculation shall be made specific and suitable as much as for each individual patient.

[Clinical study of sildenafil in the treatment of premature ejaculation complicated by erectile dysfunction].

OBJECTIVES: To evaluate the efficacy and safety of sildenafil citrate in the treatment of premature ejaculation (PE) complicated by erectile dysfunction (ED). METHODS: Forty-five patients of PE complicated by ED received flexible doses of sildenafil from 50 to 100 mg for 1 to 3 months. Intravaginal ejaculatory latency time (IELT) and sexual satisfaction ratio (SSR) of partner were recorded to evaluate the effect of PE treatment, as well as the general efficacy and satisfaction of ED treatment. And the difference of IIEF-5 before and after the treatment were compared. RESULTS: Twenty-seven patients had their PE improved and the effective rate was 60%. Forty patients reported the improvement in erection and the percentage of erectile improvement was 88.88%. All the 27 patients with improvement of PE achieved effective erection through the administration of 50 mg sildenafil and the satisfaction rate reached 81.48%. On the other hand, only 1 case (5.56%) reported satisfaction over the treatment in the 18 patients who did not obtain improvement of PE. Between the PE improvement group and non-improvement group, there were significant differences (P < 0.001) shown in IIEF-5 scores before and after the treatment. Mild or moderate side effects were reported in 9 patients(20%), who recovered without any treatment. CONCLUSIONS: To premature ejaculation patients with ED, sildenafil can safely and effectively improve their erectile function, the satisfaction over the ED treatment outcome means that their PE symptoms could be alleviated.

The complementary role of different neurophysiological methods to demonstrate organicity in male with premature ejaculation and erectile dysfunction.

Erectile dysfunction and premature ejaculation are the most common male sexual disorders. Generally, it is considered that erectile dysfunction is related to organic factors whilst premature ejaculation is related to psychological factors. A detailed history along with physical examination such as using penile doppler, penile tumesans and ultrasonography are insufficient for differential diagnosis in 15-20% of cases. Therefore the role of neurophysiological techniques are important. In this study urological and biochemical investigations were used in conjunction with other neurophysiological methods. According to our results, using a neurophysiological test study on its own was ineffective. As a conclusion our study has shown that neurophysiological methods are important differential diagnosis of revealing suspicious organicity--generally believed to be due to psychological factors- in male sexual disorders.

Apoptosis in human ejaculated spermatozoa.

After the introduction of assays determining apoptosis in human ejaculated spermatozoa, several studies have been published about the relationship between apoptosis in spermatozoa and semen quality. Apoptosis in spermatozoa is significantly correlated with conventional semen quality parameters, but also with the outcome of assisted reproductive techniques. The apoptotic process is probably set in motion before ejaculation. Determining apoptosis in spermatozoa can improve selection criteria in assisted reproduction.
