Development of a Web site for transplant patient education.

The Internet is a global communication network used by more than 17.6 million adults as a major source of current health information. Both the number of health-related Web sites and the number of Web users are increasing exponentially as well as reports indicating a growth in the number of persons who access the Internet specifically to retrieve information about organ transplantation. However, few are using this medium for posttransplant educational or psychosocial purposes. Armed with this information, as well as a commitment from the transplant team, we chose to develop a Web-based educational program to facilitate posttransplant care for our transplant recipients. The purpose of this article is to describe the planning, development, and implementation of a Web-based education program for transplant recipients.

A training pack. Field programme management: food and nutrition.

One of the main barriers to development is the lack of adequately trained field staff. It is now widely recognized that the field worker plays a crucial role in implementing community development programmes. National development plans may look promising on paper but without the staff in the field, capable of managing the programmes, there is little chance for substantial gain from development efforts. To help address the problem of training for field workers, the Food Policy and Nutrition Division has produced the Training Pack. Its aim is to help field workers develop simple management skills which can be used in their daily work. The material in the Pack stresses those management techniques required by all field workers, whether their primary responsibility is agriculture, health, nutrition or community development. The Pack uses aspects of food and nutrition to illustrate the basic management skills, without attempting to turn the field workers into nutritionists. The emphasis of the Course is on the planning, implementation and evaluation of community activities. The practical approach advocated by these training materials is described in detail here.

National action plan to reduce smoking during pregnancy: the National Partnership to Help Pregnant Smokers Quit.

Although there has been remarkable progress and momentum toward achieving smoke-free pregnancies in the United States since 1990, concerted action is needed to close the remaining gaps in treatment and prevention so that we can reach the Healthy People 2010 goal for pregnant smokers: a prevalence of 1% or less. This need for action led to the formation of the National Partnership to Help Pregnant Smokers Quit, a collaboration among more than 50 organizations and agencies, public and private, that have joined forces to help pregnant smokers quit by providing proven clinical and community-based interventions to every pregnant smoker. This article summarizes the action plan developed by the partnership, the strategies it outlines, and some of the actions taken by partners over the past year to put the plan into action. Action is planned and progress is being made in five strategic areas: offering help through the health care system; using the media effectively; harnessing community and worksite resources; promoting policies known to increase smoking cessation efforts and successes; and expanding national research, surveillance, and evaluation efforts.

The role of "go no-go" decisions in TB vaccine development.

"Go no-go" decisions play a critical role in the product development plan of any TB vaccine at several points. Go no-go decisions are designed to serve the fundamental maxim in vaccine development that killing a bad vaccine project as early as possible is the hallmark of a successful overall program; in development, unlike in basic research, costs and skilled manpower requirements rise exponentially as the program proceeds. The opportunity costs in resources, manpower and equipment utilized on bad vaccine projects are unavailable to other vaccine programs. The go no-go decision is a fundamental piece of the process that balances risk, time and resources. An example of a go no-go decision development program is presented.

Teaching pain management: how to make it work.

What are effective strategies for teaching and molding attitudes toward pain management? Nursing responsibility for pain management involves a complex decision-making process affected by many variables. Traditional pain management classes omit critical content and strategies that may affect implementation of the information. Techniques and resources for the educator planning pain management programs are presented in this article. Achieving positive changes in behavior related to pain management requires a well-developed plan that incorporates model-based instruction in both cognitive and affective domains.

Public health nutrition: a historical perspective.

This historical perspective describes important milestones and selected threads of concern in the development of public health nutrition services. Emphasis is given to nutrition service for mothers, children, and families from the mid-1800s through the 20th century. Efforts in the 1800s were primarily directed to building the foundation for public health nutrition services: organizing state health departments and voluntary health agencies, initiating early nutrition investigations, and establishing milk stations and school lunch programs in large cities to supplement the food of the poor and to combat the high rates of morbidity and mortality of infants and children. During the 20th century, demographic changes; advances in nutrition science and technology; and social, political, and economic changes influenced the growth and development of public health nutrition services. Such changes included the high rates of disease and deaths among mothers and children, foods shortages during the wars; an increasing number of children in day care as more mothers went to work; the development of new programs to serve special groups such as the mentally retarded, chronically ill and handicapped, recent immigrants, and the aged; the continuing presence of poverty and hunger; the rising prevalence of behavior-related problems (eg, adolescent pregnancy and substance abuse); and a greater recognition of the benefits of health promotion and disease prevention. The number of public health nutritionists employed at national, state, and local levels increased to meet the rising demand for nutrition services. Graduate-level training programs and continuing education opportunities were developed and a broad range of nutrition standards and guidelines evolved. Throughout the 20th century, continuing threads of concern included nutrition policy and planning; development and evaluation of public health nutrition programs; organization, administration, and management of programs; quality of public health nutrition services, and the transfer and application of research into public health nutrition practice.

Using fixed financial triggers for incentive plans.

Of course, some organizations are making no changes at all, because they don't believe that financial performance will justify paying even small awards. Healthcare organizations with executive incentive compensation plans need to review existing plans with their compensation committees. Merely leaving in place incentive plans developed prior to BBA may be doing a dis-service to the executive teams, the hospital, and the board. Using the incentive plan to focus management's attention on a few key areas is still relevant, maybe more so than ever. A complete review of incentive plans--participation, opportunity levels, financial triggers, performance measures, and other factors--is an important element of total executive compensation, probably now more than ever.

Treatment planning and site development for the implant-assisted periodontal reconstruction.

Implants can be used to recreate an edentulous dentition that is esthetic, comfortable, and functional by augmenting the total surface area of load carrying abutments. Traditionally, implants were placed in areas of the mouth where adequate amounts of bone were present. Today, clinicians are strategically planning and developing implant sites to optimize oral health. This article will describe methods of treatment planning and site development for the reconstruction of the implant-assisted periodontal restoration.

The food situation and the child -- an overview.

World food production has increased in this decade by an estimated 2.5% per annum, but it is probable that up to 200 million children -- a quarter of those in the developing countries -- suffer from undernutrition. Expansion of food production alone is not likely to remedy the situation -- improved distribution at international, national and even family level is also required. The latter can be assisted by suitable development planning and food policy measures.
