Reversibility of clip sterilization.

A spring clip designed to be applied to the fallopian tube for sterilization without electrocoagulation was applied to 10 pigs. After documenting the prevention of pregnancy for 3 months, the clips were removed and successful reanastomosis was performed on eight pigs. Six of these pigs became pregnant within 2 months of the reanastomosis. This paper describes the technique of the reanastomosis and documents the potential reversibility of spring clip sterilization.

A clip applicator for laparoscopic sterilization.

Laparoscopic sterilization using spring-loaded clips promises to reduce complications encountered with other sterilization techniques. A second-puncture clip applicator for laparoscopic sterilization has been developed which has several advantages over the single-puncture method, allowing a full visual field and more accurate placement of the clips. A description of the applicator, its assembly, operative procedure, and preliminary results are reported.

Endoscopic clip application for closure of esophageal perforations caused by EMR.

BACKGROUND: With increasing use of EMR for early stage esophageal carcinoma, the number of cases of iatrogenic esophageal perforation is likely to increase. This study evaluated the results of endoscopic clip application for treatment of perforations caused by EMR in patients with esophageal carcinoma. METHODS: Among 185 patients who underwent EMR for esophageal carcinoma, esophageal perforation occurred in 3 patients (1.6%). Metallic clips were immediately applied endoscopically to close the perforations. OBSERVATIONS: All 3 patients were observed closely and were managed conservatively (intravenous hyperalimentation, antibiotics) after closure of the perforation. They were discharged without any further serious complication. CONCLUSIONS: When esophageal perforation caused by EMR is immediately recognized, endoscopic application of metallic clips is appropriate therapy. However, patients must be carefully monitored for the development of generalized mediastinitis.

Clinical and six-month angiographic evaluation of coronary arterial graft interrupted anastomoses by use of a self-closing clip device: a multicenter prospective clinical trial.

OBJECTIVES: To evaluate the safety and effectiveness of a self-closing surgical clip with an interrupted technique in left internal thoracic artery to left anterior descending artery bypass grafting. METHODS: Eighty-two patients were enrolled and treated (February 2000 through August 2001) in a prospective, nonrandomized, multicenter trial. Left internal thoracic artery to left anterior descending artery anastomoses were performed in 60 off-pump coronary artery bypasses (73%), 12 conventional coronary artery bypass grafting (15%), and 10 minimally invasive direct coronary artery bypass (12%) procedures. Angiograms (64 to 383 days, mean 200 days) were obtained on 63 patients (77%). Qualitative and quantitative angiographic assessment was performed by an independent core laboratory. RESULTS: The self-closing surgical clip was used for 82 left internal thoracic artery to left anterior descending artery interrupted anastomoses without the requirement for knot tying or primary suture management. Minimum left internal thoracic artery to left anterior descending artery anastomosis time was 3 minutes. There was one perioperative and one late death (both not heart related) and one reexploration for bleeding unrelated to the anastomotic site. FitzGibbon grades were as follows: A (n = 60, 95.2%), B (n = 3, 4.8%) including one kinked left internal thoracic artery, and O (n = 0, 0%). Quantitative analysis (n = 57) showed mean lumen diameters of left internal thoracic artery proximal to the anastomosis of 2.1 mm, at anastomosis of 2.0 mm, and in the left anterior descending artery distal to the anastomosis of 1.9 mm. The average ratio of the anastomosis to the left anterior descending artery diameter was 1.14 (0.45 to 1.93). Anastomotic stenosis as a percentage of average left internal thoracic artery to left anterior descending artery diameter was -2.3%, comparing favorably with results (23% to 24%) reported from the Patency, Outcomes, Economics, Minimally invasive direct coronary artery (POEM) bypass study. CONCLUSIONS: The interrupted technique, facilitated by a self-closing anastomotic clip, yields favorable 6-month angiographic results when compared with other published studies.

Normalization of transcranial Doppler middle cerebral artery velocities after aneurysm clipping.

BACKGROUND: Factors that promote normalization of middle cerebral artery (MCA) velocities after cerebral aneurysm surgery may be important to identify in order to optimize management. Survival analysis of serial transcranial Doppler (TCD) MCA velocities in the setting of aneurysm surgery offers an additional means of analyzing outcome. METHODS: We retrospectively studied serial MCA velocities obtained via TCD in 113 patients who underwent cerebral aneurysm clipping to analyze which factors promoted normalization. The presence of bleeding, aneurysm location, age, gender, ethnicity, initial blood pressure, hemoglobin, and clinical condition were examined. RESULTS: Patients who did not bleed but were clipped still tended to have transient periods of abnormally elevated velocities. Of those patients who bled and were clipped, older patients normalized faster. Women showed a trend toward slower normalization only in the subarachnoid hemorrhage (SAH) absent group. Other parameters listed above did not effect normalization time. CONCLUSIONS: Our data suggest aneurysm surgery in and of itself may promote transient rises in MCA velocities. Age appears to be a significant factor in predicting normalization of MCA velocities after SAH.

Local anaesthetic on Filshie clips for pain relief after tubal sterilisation: a randomised double-blind controlled trial.

Pain during tubal sterilisation is thought to be due to either ischaemia or pressure at the site of impact of sterilising devices on the fallopian tubes. We have evaluated the effectiveness of an application of 2% lignocaine gel to Filshie clips to relieve postoperative pain. In a randomised double-blind placebo-controlled study, 80 healthy women undergoing tubal sterilisation under general anaesthesia at the County Hospital, Lincoln, UK, were allocated to be sterilised by Flishie clips covered with 2% lignocaine gel or K-Y gel as placebo. Pelvic pain was assessed, with a 100 mm visual analogue scale, at 1 hour, at hospital discharge, and time of first analgesia or any other time analgesia was demanded. The lignocaine-treated group had significantly longer time to first analgesia, less pain at 1 hour, less nausea and vomiting, and shorter recovery time. Fewer lignocaine-treated patients needed additional analgesia and they required fewer opioids. There was no case of failed sterilisation or adverse reaction to lignocaine. The application of local anaesthetic gel to Filshie clips is a safe, non-invasive, and effective method of relieving postoperative pain during laparoscopic tubal sterilisation.

Differential usage of two 5' splice sites in a complex exon generates additional protein sequence complexity in chicken CLIP-170 isoforms.

Reverse transcription-polymerase chain reaction amplification and cloning of cDNA encoding the chicken CLIP-170(11) isoform of the Cytoplasmic Linker Protein 170 gene revealed an unusual source of protein sequence variation. In addition to differential combinatorial splicing of two cassette exons to yield four CLIP-170 protein isoforms, we found differential usage of alternative 5'-splice junctions in a single exon. Splicing at the downstream site yields message containing 18 bp of nucleotide sequence that is missing from message spliced at the more 5' site. This 18 bp sequence encodes a segment of 6 amino acids that fills a gap in the alignment of chicken and human CLIP-170 homologue sequences. Differential usage of the 5'-splice junctions in this complex exon appears to be tissue- rather than isoform-specific.

Sutureless vascular anastomosis: the VCS clip.

Non-penetrating, arcuate-legged clips facilitate vascular reconstructions and significantly reduce the incidence of anastomotic neointimal hyperplasia. Though originally developed as a microvascular-approximating instrument, by far the largest application has been for hemodialysis vascular access. Long-term studies comparing suture to clip for access creation have demonstrated the clear technical and biological superiority of the clip for this procedure. This "sutureless" vascular anastomotic technique is more demanding than conventional suturing, requiring precise vessel preparation and supervised training.

Percutaneous ureteral clipping: long-term results and complications.

PURPOSE: The authors present their experience with ureteral clipping in patients with urinary fistulas. MATERIALS AND METHODS: The authors retrospectively reviewed records from eight patients in whom 14 ureters were ligated with use of a percutaneously applied stainless steel clip as treatment for urinary fistulas. All patients were female and had undergone pelvic radiation for treatment of cervical carcinoma (n = 7) or endometrial carcinoma (n = 1). Permanent urinary diversion was via percutaneous nephrostomy, which required routine change, at which time recurrence of symptoms and ureteral leakage were determined. Duration of follow-up (patient survival) ranged from 2 weeks to 17 months (mean, 7.1 months). RESULTS: All patients had significant improvement in their symptoms, with seven patients achieving complete perineal dryness. Two patients had recurrence of perineal symptoms due to nephrostomy tube blockage, which was successfully managed by means of interventional techniques. CONCLUSIONS: The authors believe that ureteral clipping is a safe and effective technique in the management of ureteral fistulas due to pelvic neoplasm and its treatment, despite the short life expectancy of these patients.

Ketamine-induced changes in metabolic and endocrine parameters of normal and 2-kidney 1-clip rats.

BACKGROUND AND OBJECTIVE: The aim of this study was to investigate the effect of ketamine on the endocrine and lipid metabolic status of the renal-banded animals. METHODS: Forty male rats were randomly divided into four groups. Group A served as control, Group B animals received ketamine intraperitoneally at a dose of 100 mg kg(-1), Group C was submitted to 2-kidney 1-clip experimental hypertension and Group D received ketamine as above, as well as being submitted to renal artery clipping. Atrial natriuretic peptide, angiotensin II and free fatty acid concentrations were measured in serum. In addition, adipose tissue lipoprotein lipase activity and angiotensin II content were determined, while the left ventricular weight relative to body weight was used as a cardiac hypertrophy index. RESULTS: In renal-banded rats (Groups C and D) serum atrial natriuretic peptide, free fatty acid and angiotensin II concentrations as well as ventricular weight were increased, while adipose tissue lipoprotein lipase activity was lower than in control animals (Groups A and B). Ketamine administration did not influence angiotensin II concentrations either in normal (Group B) or banded rats (Group D). Ketamine increased serum atrial natriuretic peptide and free fatty acid concentrations only in normal animals (Group B). It had no influence on adipose tissue lipoprotein lipase activity either in normal (Group B) or banded animals (Group D). Adipose angiotensin II content did not differ between the four groups. CONCLUSION: Ketamine increased the atrial natriuretic peptide and free fatty acid concentration in normal rats. In 2-kidney 1-clip animals, ketamine did not elicit an additional response of serum atrial natriuretic peptide or free fatty acids levels. Its contribution to these factors was not significant.

Targeting the DNA cleavage activity of copper phenanthroline and clip-phen to A.T tracts via linkage to a poly-N-methylpyrrole.

To target DNA A.T tracts, a three-ring polyamide containing an N-methylpyrrole amino acid has been linked, on solid support, to carboxylic derivatives of phenanthroline and dimers of phenanthroline: 2-Clip-Phen, 3-Clip-Phen, or 2-Clip-Phen containing a long tether. After metalation by CuCl(2), the DNA cleavage activities of the different conjugates were compared on a restriction fragment. Cleavage patterns showed that the polyamide moiety of conjugates directs the cleavage activity in the vicinity of A.T tracts but the precise cleavage selectivity of these conjugates was dependent on the type of phenanthroline residue linked to the poly-N-methylpyrrole entity.

Acridine conjugates of 3-clip-phen: influence of the linker on the synthesis and the DNA cleavage activity of their copper complexes.

To increase the DNA cleavage activity and the cell delivery of the bis(phenanthroline) DNA cleaver "3-Clip-Phen", conjugates between 3-Clip-Phen and the intercalators acridine and 6-chloro-2-methoxyacridine, through amino acid linkers of various length, were prepared. After complexation with CuCl(2), the ability of these conjugates to cleave phiX 174 DNA in the presence of a reductant and air was compared. The results indicated that (i) the coupling of 3-Clip-Phen to an acridine derivative increased the DNA cleavage efficiency of the copper complexes, (ii) the acridine derivatives were more active than 6-chloro-2-methoxyacridine derivatives, (iii) the linker length influenced cleavage efficiency, the highest DNA cleavage activity being obtained for an aminocaproic spacer.

Self-assembly of rectangles and prisms via a molecular "clip".

Aromatic molecular "clips" bearing two symmetrically bound platinum moieties have been prepared. The molecular "clip" 4 readily self-assembled with linear linkers such as 4,4'-bipyridyl, 1,4-bis[2-(4-isocyano-3,5-diisopropylphenyl)ethynyl]benzene, and nicotinic acid to form molecular rectangles. The overall dimensions of the rectangle 7 were 7.3 Angstroms x 15.3 Angstroms. The molecular "clip" also self-assembled with tritopic pyridyl and isocyanide ligands to form trigonal prismatic frameworks. The characterization of the supramolecules by multinuclear NMR, electrospray mass spectrometry, and X-ray crystal structures is also reported.

The HETTAP approach: self-assembly and metal ion sensing of dumbbell-shaped molecules and clip molecules.

The pentacoordinated terpyridine-phenanthroline zinc(II) complex motif, conceived along the HETTAP concept, allows the preparation of a set of multicomponent supramolecular dumbbell and clip assemblies from various bisterpyridines. All assemblies show a notable luminescence at 350-400 nm. The formation of dumbbell [Zn2(1b)2(2b)]4+ is convincingly demonstrated from the X-ray crystal structure analysis. Both dumbbell [Zn2(1b)2(2b)]4+ and clip [Zn2(1d)(2b)]4+ allow the monitoring of Hg2+ ions due to highly selective quenching of the emission that is driven by a Zn2+ --> Hg2+ exchange process, while the more-strained clip [Zn2(1d)(2c)]4+ does not undergo such metal exchange and does not show quenching of the luminescence. Consequently, these assemblies exhibit a highly selective response due solely to supramolecular effects.

Substituent effects control the self-association of molecular clips in the crystalline state.

We report the X-ray crystal structure of 11 molecular clips and analyze the influence of substituents (e.g., OMe, Me, and NO2) and their location on the observed crystal packing. Molecular clips 3a and 3b form tapelike structures in the crystal due to pi-pi interactions between the aromatic walls. Compounds 3d, 3eC, and 3fC form dimers driven by critical C-H...O interactions and then form tapes driven by pi-pi interactions in the crystal. These two building motifs, pi-pi and C-H...O interactions, can be used to rationalize the enantio- and diastereoselectivity observed in the X-ray crystal structures of the remaining five molecular clips. For example, the C-H...O interactions are found to dictate the formation of homochiral dimers in the structures of (+/-)-3eT and (+/-)-3fT and to control the diastereoselective formation of 6a2-6c2 dimeric motifs with internal p-dimethoxy-o-xylylene walls. Overall, the results suggest that substituent effects that induce even weak intermolecular interactions (e.g., C-H...O) can be used to reliably control crystal packing within glycoluril-based systems.

Synthesis, Conformational Analysis, and Binding Properties of Molecular Clips with Two Different Side Walls.

A new general route toward the synthesis of diphenylglycoluril clips with two different side walls starting from the new precursor 6b and the known compound 2b is described. A variety of clips are accessible in this way, some of them containing metal binding ligand systems, viz. phenanthroline, pyridine, salophen, and porphyrin for future applications as supramolecular catalysts. The physical and binding properties of the new clips are presented and discussed.

Taxol content of stored fresh and dried Taxus clippings.

The taxol content of dried Taxus biomass was monitored monthly for 15 months. Intact and finely ground biomass was stored at room temperature (22 degrees-24 degrees) as well as under refrigeration (2 degrees-4 degrees). In addition, intact fresh clippings stored under refrigeration in sealed plastic bags for up to 10 weeks were evaluated for changes in taxol content. Analysis indicates that properly dried Taxus clippings can be stored either intact or powdered at room temperature or under refrigeration with no apparent loss of taxol content. The taxol content in fresh intact clippings was also stable for at least 10 weeks when stored under refrigeration.

CLIPs and CLASPs and cellular dynamics.

The dynamic properties of microtubules are regulated by plus-end tracking proteins (+TIPs), which associate with the distal ends of microtubules. Among the +TIPs are cytoplasmic linker proteins (CLIPs), which promote microtubule growth and regulate dynein-dynactin localization, and CLIP-associating proteins (CLASPs), which stabilize specific subsets of microtubules on reception of signalling cues. CLIPs and CLASPs interact and cooperate to direct the microtubule network, thereby regulating cellular asymmetry.
