Toward an alcohol use disorder continuum using item response theory: results from the National Epidemiologic Survey on Alcohol and Related Conditions.

BACKGROUND: Item response theory (IRT) was used to determine whether the DSM-IV diagnostic criteria for alcohol abuse and dependence are arrayed along a continuum of severity. METHOD: Data came from a large nationally representative sample of the US population, 18 years and older. A two-parameter logistic IRT model was used to determine the severity and discrimination of each DSM-IV criterion. Differential criterion functioning (DCF) was also assessed across subgroups of the population defined by sex, age and race-ethnicity. RESULTS: All DSM-IV alcohol abuse and dependence criteria, except alcohol-related legal problems, formed a continuum of alcohol use disorder severity. Abuse and dependence criteria did not consistently tap the mildest or more severe end of the continuum respectively, and several criteria were identified as potentially redundant. The drinking in larger amounts or for longer than intended dependence criterion had the greatest discrimination and lowest severity than any other criterion. Although several criteria were found to function differentially between subgroups defined in terms of sex and age, there was evidence that the generalizability and validity of the criterion forming the continuum remained intact at the test score level. CONCLUSIONS: DSM-IV diagnostic criteria for alcohol abuse and dependence form a continuum of severity, calling into question the abuse-dependence distinction in the DSM-IV and the interpretation of abuse as a milder disorder than dependence. The criteria tapped the more severe end of the alcohol use disorder continuum, highlighting the need to identify other criteria capturing the mild to intermediate range of the severity. The drinking larger amounts or longer than intended dependence criterion may be a bridging criterion between drinking patterns that incur risk of alcohol use disorder at the milder end of the continuum, with tolerance, withdrawal, impaired control and serious social and occupational dysfunction at the more severe end of the alcohol use disorder continuum. Future IRT and other dimensional analyses hold great promise in informing revisions to categorical classifications and constructing new dimensional classifications of alcohol use disorders based on the DSM and the ICD.

Impact of differential item functioning on age and gender differences in functional disability.

OBJECTIVES: Estimates of group differences in functional disability may be biased if items exhibit differential item functioning (DIF). For a given item, DIF exists if persons in different groups do not have the same probability of responding, given their level of disability. This study examines the extent to which DIF affects estimates of age and gender group differences in disability severity among adults with some functional disability. METHODS: Data came from the 1994/1995 National Health Interview Survey Disability Supplement. Analyses focused on 5,750 adult respondents who received help or supervision with at least one of 11 activities of daily living/instrumental activities of daily living tasks. We estimated gender and age group (18-39, 40-69, and 70+) differences in disability, using multiple-indicator/multiple-cause models, which treat functional disability as a latent trait. RESULTS: Nine items manifested significant DIF by age or gender; DIF was especially large for "shopping" and "money management." Without adjusting for DIF, middle-aged persons were less disabled than elderly men, and women were less disabled than men among nonelderly persons. After adjusting for DIF, middle-aged persons did not differ from elderly persons, and gender differences within age groups were not significant. DISCUSSION: Comparisons of disability across sociodemographic groups need to take DIF into account. Future research should examine the causes of DIF and develop alternative question wordings that reduce DIF effects.

Nuclear retinoid receptor expression in normal human endometrium throughout the menstrual cycle.

Previous work has shown that retinoic acid receptors (RARs) and retinoid X receptors (RXRs) are expressed in human endometrial epithelial and stromal cells. These nuclear receptors mediate the biological effects of retinoic acid, a vitamin A derivative which may have an important, though poorly characterized role in the functional differentiation of secretory epithelia. The aim of this study was to find out whether the expression of RAR and RXR mRNA in endometrial epithelial and stromal cells varies in relation to the menstrual cycle. The expression of RARs and RXRs was investigated by Northern blotting and, for stromal cells, there were no differences in expression of RAR-alpha, RAR-beta, RAR-gamma and RXR-alpha between the proliferative and secretory phases of the menstrual cycle. Similarly, for epithelial tissue, there were no significant differences between the proliferative and secretory phases with respect to the expression of RAR-alpha, RAR-gamma and RXR-alpha. However, RAR-beta was expressed at a 1.7-fold higher level in epithelial samples from the proliferative phase compared to the secretory phase. Overall, the levels of expression of RAR-alpha, RAR-beta and RAR-gamma were 1.7- to 4-fold higher in stromal cells compared to epithelial cells whereas RXR-alpha was expressed at a similar level in both cell types. We have previously suggested that retinoic acid has a role in endometrial differentiation or function which may be reflected by cyclical changes in intracellular retinoic acid levels. These data indicate that RARs and RXRs are expressed at a similar level throughout the menstrual cycle, with the possible exception of RAR-beta, implying that any menstrual cycle-related function of RARs in controlled by ligand availability rather than by changes in expression of the receptors.

Satisfactory cross cultural equivalence of the Dutch WOMAC in patients with hip osteoarthritis waiting for arthroplasty.

BACKGROUND: Cross cultural validity is of vital importance for international comparisons. OBJECTIVE: To investigate the validity of international Dutch-English comparisons when using the Dutch translation of the Western Ontario and McMaster Universities osteoarthritis index (WOMAC). PATIENTS AND METHODS: The dimensionality, reliability, construct validity, and cross cultural equivalence of the Dutch WOMAC in Dutch and Canadian patients waiting for primary total hip arthroplasty was investigated. Unidimensionality and cross cultural equivalence was quantified by principal component and Rasch analysis. Intratest reliability was quantified with Cronbach's alpha, and test-retest reliability with the intraclass correlation coefficient. Construct validity was quantified by correlating sum scores of the Dutch WOMAC, Arthritis Impact Measurement Scales (Dutch AIMS2), Health Assessment Questionnaire (Dutch HAQ), and Harris Hip Score (Dutch HHS). RESULTS: The WOMAC was completed by 180 Dutch and 244 English speaking Canadian patients. Unidimensionality of the Dutch WOMAC was confirmed by principal component and Rasch analysis (good fit for 20/22 items). The intratest reliability of the Dutch WOMAC for pain and physical functioning was 0.88 and 0.96, whereas the test-retest reliability was 0.77 and 0.92, respectively. Dutch WOMAC pain sum score correlated 0.69 with Dutch HAQ pain, and 0.39 with Dutch HHS pain. Dutch WOMAC physical functioning sum score correlated 0.46 with Dutch AIMS2 mobility, 0.62 with Dutch AIMS2 walking and bending, 0.67 with Dutch HAQ disability, and 0.49 with Dutch HHS function. Differential item functioning (DIF) was shown for 6/22 Dutch items. CONCLUSIONS: The Dutch WOMAC permits valid international Dutch-English comparisons after correction for DIF.

Role of stromal-epithelial interactions in hormonal responses.

Steroid sex hormones (17beta-estradiol, testosterone, dihydrotestosterone, and progesterone) and aryl hydrocarbons such as the dioxins regulate epithelial proliferation and secretory protein production and differentiation in their respective target organs in male and female urogenital tracts and mammary glands. Recent evidence has demonstrated that stromal-epithelial interactions are critical for mediating the effects of these molecules on epithelial cells. Our results have indicated that estradiol, testosterone, progesterone, and dioxin regulate epithelial proliferation (stimulation or inhibition) via paracrine mechanisms requiring the appropriate receptor in the stroma. The androgen receptor (AR), estrogen receptor alpha (ERalpha), progesterone receptor (PR), or aryl hydrocarbon receptor (AhR) in the epithelium are neither necessary nor sufficient for the regulation of epithelial proliferation. Moreover, during prostatic development, signaling through the stromal AR is required to induce prostatic epithelial identity, ductal morphogenesis and glandular differentiation. Epithelial functional differentiation is regulated in the prostate, uterus, and vagina via AR (prostate) and ERalpha(uterus and vagina). In these organs both epithelial and stromal steroid receptors are required for steroidal regulation of certain aspects of epithelial differentiation such as epithelial secretory protein production in the uterus and epithelial cornification in the vagina and prostate (squamous metaplasia). The mechanistic basis of these stromal-epithelial interactions is poorly understood, but growth factors appear to be mediators of these cell-cell interactions.

Classification of osteosarcoma by 85-Sr scintimetry.

The histologic type of a classical osteosarcoma may be difficult to determine because of insufficient biopsy material and polymorphism of the tumour. Since osteoformation is directly related to the differentiation and functional capacity of the tumour cells, 85-Sr scintimetry was used to evaluate osteoid formation in 90 patients with classical osteosarcoma. Measurements over an 8-day period from isotope injection revealed a separation of the lesions into three groups, with high, medium and low activity. Compared to histologic classification into osteoblastic, chondroblastic/fibroblastic, the 41 tumours classified as osteoblastic had a high 85-Sr uptake. The anaplastic lesions belonged to the group with low 85-Sr uptake. Scintimetric determination of the functional differentiation of tumoral cells may possibly be of prognostic value in osteosarcoma.

Distractors--can they be biased too?

Numerous work has been done on item bias and differential item functioning. Although there is some research on distractor analysis, no detailed study has been attempted to examine the way distractors in an item function, with regards to comparing distractor performance. This paper examines how distractors function differentially and compares various methods for identifying this. The Pearson chi-square, likelihood ratio chi-square and Neyman weighted least squares chi-square tests are some of these methods. Possible causes of distractor bias are discussed with illustrations from a physics problem-solving scale.

Rasch techniques for detecting bias in performance assessments: an example comparing the performance of native and non-native speakers on a test of academic English.

The use of common tasks and rating procedures when assessing the communicative skills of students from highly diverse linguistic and cultural backgrounds poses particular measurement challenges, which have thus far received little research attention. If assessment tasks or criteria are found to function differentially for particular subpopulations within a test candidature with the same or a similar level of criterion ability, then the test is open to charges of bias in favour of one or other group. While there have been numerous studies involving dichotomous language test items (see e.g. Chen and Henning, 1985 and more recently Elder, 1996) few studies have considered the issue of bias in relation to performance based tasks which are assessed subjectively, via analytic and holistic rating scales. The paper demonstrates how Rasch analytic procedures can be applied to the investigation of item bias or differential item functioning (DIF) in both dichotomous and scalar items on a test of English for academic purposes. The data were gathered from a pilot English language test administered to a representative sample of undergraduate students (N= 139) enrolled in their first year of study at an English-medium university. The sample included native speakers of English who had completed up to 12 years of secondary schooling in their first language (L1) and immigrant students, mainly from Asian language backgrounds, with varying degrees of prior English language instruction and exposure. The purpose of the test was to diagnose the academic English needs of incoming undergraduates so that additional support could be offered to those deemed at risk of failure in their university study. Some of the tasks included in the assessment procedure involved objectively-scored items (measuring vocabulary knowledge, text-editing skills and reading and listening comprehension) whereas others (i.e. a report and an argumentative writing task) were subjectively-scored. The study models a methodology for estimating bias with both dichotomous and scalar items using the programs Quest (Adams and Khoo, 1993) for the former and ConQuest (Wu, Adams and Wilson, 1998) for the latter. It also offers answers to the practical questions of whether a common set of assessment criteria can, in an academic context such as this one, be meaningfully applied to all subgroups within the candidature and whether analytic criteria are more susceptible to biased ratings than holistic ones. Implications for test fairness and test validity are discussed.

Anti-osteoactivin antibody inhibits osteoblast differentiation and function in vitro.

Osteoactivin (OA) is a novel protein identified by mRNA differential display using bone from osteopetrotic versus normal rats. Bioinformatic analysis showed that OA cDNA has an open reading frame of 1716 bp encoding a protein of 572 aa, the first 21 aa constitute a signal peptide. OA sequence analysis also demonstrated 13 putative N-glycosylation sites suggestive of a heavily glycosylated protein. In this study, we localized OA protein in primary osteoblast culture by immunofluorescent staining and Western blot analysis. Primary osteoblast cultures pass through three stages: proliferation from day 1 to 7, matrix formation from day 7 to 14, and matrix mineralization from day 14 to 21. OA protein was detected at all stages examined, with maximal expression at 3 weeks when osteoblasts are terminally differentiated. Using the Chariot transfection reagent as a vehicle to deliver anti-OA antibody into the cells, we demonstrated that anti-OA antibody significantly inhibited osteoblast differentiation markers, including alkaline phosphatase activity, nodule formation, osteocalcin production, and calcium deposition, without affecting cell proliferation or viability. These data suggest that OA is an osteoblast-related protein that plays an important role in the regulation of osteoblast differentiation and function.

Tissue-transglutaminase contributes to neutrophil granulocyte differentiation and functions.

Promyelocytic NB4 leukemia cells undergo differentiation to granulocytes following retinoic acid treatment. Here we report that tissue transglutaminase (TG2), a protein cross-linking enzyme, was induced, then partially translocated into the nucleus, and became strongly associated with the chromatin during the differentiation process. The transglutaminase-catalyzed cross-link content of both the cytosolic and the nuclear protein fractions increased while NB4 cells underwent cellular maturation. Inhibition of cross-linking activity of TG2 by monodansylcadaverin in these cells led to diminished nitroblue tetrazolium (NBT) positivity, production of less superoxide anion, and decreased expression of GP91PHOX, the membrane-associated subunit of NADPH oxidase. Neutrophils isolated from TG2(-/-) mice showed diminished NBT reduction capacity, reduced superoxide anion formation, and down-regulation of the gp91phox subunit of NADPH oxidase, compared with wild-type cells. It was also observed that TG2(-/-) mice exhibited increased neutrophil phagocytic activity, but had attenuated neutrophil chemotaxis and impaired neutrophil extravasation with higher neutrophil counts in their circulation during yeast extract-induced peritonitis. These results clearly suggest that TG2 may modulate the expression of genes related to neutrophil functions and is involved in several intracellular and extracellular functions of extravasating neutrophil.

Effects of a purified cecropin D from a Chinese silk moth on growth, function and differentiation of murine hemopoietic cells.

The effects of cecropin D, a small basic peptide isolated from a Chinese oak silk moth, on the functions or differentiation of mammalian hemopoietic cells are described in the present paper. This peptide suppressed lectin-induced DNA synthesis of murine splenocytes in a dose-dependent manner without any significant cytotoxic effects. It also exhibited inhibitory effects on antibody production in lipopolysaccharide-stimulated lymphocytes and on colony formation of hemopoietic progenitor cells in plasma clots culture. These results indicate that cecropin D can regulate growth, function and differentiation of murine hemopoietic cells. The biological significance of this finding is discussed from the comparative immunological point of view.

Differential item functioning in the Danish translation of the SF-36.

Statistical analyses of Differential Item Functioning (DIF) can be used for rigorous translation evaluations. DIF techniques test whether each item functions in the same way, irrespective of the country, language, or culture of the respondents. For a given level of health, the score on any item should be independent of nationality. This requirement can be tested through contingency-table methods, which are efficient for analyzing all types of items. We investigated DIF in the Danish translation of the SF-36 Health Survey, using two general population samples (USA, n = 1,506; Denmark, n = 3,950). DIF was identified for 12 out of 35 items. These results agreed with independent ratings of translation quality, but the statistical techniques were more sensitive. When included in scales, the items exhibiting DIF had only a little impact on conclusions about cross-national differences in health in the general population. However, if used as single items, the DIF items could seriously bias results from cross-national comparisons. Also, the DIF items might have larger impact on cross-national comparison of groups with poorer health status. We conclude that analysis of DIF is useful for evaluating questionnaire translations.

Comparison of effective renal plasma flow (ERPF) and endogenous creatinine clearance (Ccr) in evaluation of the differential kidney function: an in vivo study.

Eleven patients with obstructive uropathy were inserted with a pigtail catheter either for urinary diversion, endopyelotomy or percutaneous nephrolithotomy. Differential kidney functions were obtained by single-sample technique with I-131 orthoiodohippurate (OIH) and separate endogenous creatinine clearance (Ccr). Good linear correlation was noted between effective renal plasma flow (ERPF) and Ccr (r = 0.84, p less than 0.005), but there was still some discrepancy between these two examinations, chiefly found in two severely obstructed kidneys. Determination of ERPF by I-131-OIH single-sample technique is a simple, rapid, inexpensive and relative accurate test to measure the differential kidney function. But if the ERPF is at extremely low level, further investigations such as sonography of kidney and separate endogenous creatinine clearance should be done to avoid unnecessary nephrectomy.

Inhibition of macrophage differentiation and function by cortisol.

Neuroendocrine mechanisms are involved in modulation of the immune system, but the mode of action of the complex interplay between hormones and the immune system is only partially understood. This study examines the role of cortisol in monocyte differentiation and function, with regard to interleukin-1 beta (IL-1 beta) expression. The differentiation of the human histiocytic lymphoma cell line U 937 into macrophage-like cells by phorbol ester [phorbol myristate acetate (PMA)] is inhibited by cortisol. Some cells remain in suspension and continue to divide; others stop proliferation, but do not undergo full morphological differentiation. When cells are washed after 3 days to remove PMA and cortisol, all cells stop dividing and become fully differentiated. The PMA, therefore, commits the cells to differentiate even after its removal, while cortisol is only suppressive when present. Differentiated cells are shown to produce IL-1 beta mRNA when stimulated with lipopolysaccharide. This effect is inhibited by cortisol in a dose-dependent manner. After removal of cortisol, the least differentiated cells that remained in suspension were found to be overproducers of IL-1 beta mRNA after stimulation with lipopolysaccharide. This suggests that PMA induces a buildup of transcription-activating factors that were suppressed in the presence of cortisol. We conclude that the adrenal glucocorticoids that are elevated in acute stress conditions or major depression attenuate the differentiation and function of monocytes.

An analysis of test bias and differential item functioning due to race on the Mattis Dementia Rating Scale.

The Mattis Dementia Rating Scale (MDRS) is a commonly used cognitive measure designed to assess the course of decline in progressive dementias. However, little information is available about possible systematic racial bias on the items presented in this test. We investigated race as a potential source of test bias and differential item functioning in 40 pairs of African American and Caucasian dementia patients (N = 80), matched on age, education, and gender. Principal component analysis revealed similar patterns and magnitudes across component loadings for each racial group, indicating no clear evidence of test bias on account of race. Results of an item analysis of the MDRS revealed differential item functioning across groups on only 4 of 36 items, which may potentially be dropped to produce a modified MDRS that may be less sensitive to cultural factors. Given the absence of test bias because of race, the observed racial differences on the total MDRS score are most likely associated with group differences in dementia severity. We conclude that the MDRS shows no appreciable evidence of test bias and minimal differential item functioning (item bias) because of race, suggesting that the MDRS may be used in both African American and Caucasian dementia patients to assess dementia severity.

[The effects of intermittent hydromechanics on the differentiation and function of bone marrow stromal derived-osteoblasts in porous calcium phosphate ceramics].

OBJECTIVE: To observe the effects of the intermittent hydromechanical stimulation on the differentiation and function of the bone marrow stromal derived-osteoblasts in the porous calcium phosphate ceramics. METHODS: Rat bone marrow stromal derived-osteoblasts were seeded into porous calcium phosphate ceramic scaffolds at cells density 1 x 10(6) cells/cm(3). The cells-ceramics contrusts were cultured under rotary condition for 1 hour at 4 hours interval. After 4, 7 and 14 days cultivation, the osteoblastic phenotype markers (ALP Activity, Type I Collagen, Osteocalcin, Osteopontin, Osteonectin, and Bone Sialoprotein mRNA expression levels) were analyzed by biochemistry measurement and quantitative RT-PCR technique. Static cell culture as control. RESULTS: Under rotary cell culture condition, the ALP activities and expressions of Type I collagen mRNA increased markedly and reached the peak levels at 7 days. The expressions of other four markers mRNA occurred at 4 days and reached the peak levels at 7 days, then the expressions were down-regulated at 14 days. Under static cell culture as control, the ALP activities and expressions of Type I collagen mRNA increased gradually and reached the peak levels at 14 days. The expressions of other four markers mRNA occurred at 7 days and reached the peak levels at 14 days. CONCLUSION: The intermittent hydromechanical stimulation could promote the bone marrow stromal derived-osteoblasts differentiation and function which cultured in the porous calcium phosphate ceramics in vitro.

99mTechnetium penicillamine scans: an index of differential renal function.

The relative renal accumulation of 99mtechnetium penicillamine was evaluated as an index of differential renal function. Regions of interest were assigned over each kidney with a computer and the renal uptake of 99mtechnetium penicillamine was determined. A simple background correction was made by subtracting the renal activity at 10 minutes from that at 25 minutes. The percentage of 99mtechnetium penicillamine accumulation in the left kidney was correlated with the percentage of the total effective renal plasma flow to the left kidney. Effective renal plasma flow was determined after the intravenous injection of 131 I hippuran using a highly sophisticated computerized technique that has been validated in patients by comparison with traditional split renal function studies. The correlation coefficient between the 2 measurements was 0.97. In addition to its use as an index of differential effective renal plasma flow 99mtechnetium penicillamine is an excellent cortical imaging agent and the relative distribution of 99mtechnetium penicillamine between the 2 kidneys and within a single kidney can be assessed qualitatively by visual inspection of the scan.

[Benefits of pyeloplasty in children with pyeloureteral stenosis: study of somatic development and differential renal function].

INTRODUCTION: Under the actual controversy about the management of hydronephrosis for ureteropelvic junction obstruction, our objective has been to evaluate the indications and the effects of pyeloplasty as therapeutic option. MATERIAL AND METHODS: We have reviewed the charts of patients who underwent pyeloplasty, in special concerns at the preoperatory study and the postoperatory control by means of differential renal function (DRF) and half-time drainage (T1/2) on curves of Tc99m MAG3 diuretic renogram, and the effects in the body growth. RESULTS: Between 1995-1999 a total of 16 patients (17 kidneys) underwent pyeloplasty (range: 2 months to 12 years), selecting for the surgical treatment the symptomatic cases and the asymptomatics with alteration of renographic curve and/or DRF. The left side was involved in 12 cases, the right side in 3, and one case was bilateral. The malformation was detected by prenatual ultrasonography on 56.25% of cases and by several symptoms (abdominal pain, infection) in remaIns. The preoperatory DRF was normal (> or = 45%) in 53.3% and deteriorated in 46.7%. Postoperatively, global differential renal function improved significantly, with normal values in 66.7% of kidneys at first control and 75% at the second one. All the cases presented before surgery an obstructed renographic curve, with a half-time superior to 20 minutes; after pyeloplasty the excretory pattern in the diuresis renogram improved significantly, with a normal pattern (T1/2 < 10') in 54%, indeterminate (T1/2 between 11 and 20') in 26.7% and obstructive (T1/2 > 20') in 20% of cases. At the age of surgery, 29.4% of children was below the 50th percentile in weight; postoperatively, only 5.9% was, demonstrating a significant increment in all the cases. CONCLUSIONS: Pyeloplasty is an effective technique for the treatment of ureteropelvic junction obstruction in the selected cases, demonstrating a significant of differential renal function and body growth.
