Constructive thinking inventory: evidence of validity among Iranian managers.

This study examined the validity and incremental validity of the Constructive Thinking Inventory in a sample of Iranian managers. These 159 men were 39.9 yr. old (SD=2.5) and volunteered to participate in a project in which they responded to the Constructive Thinking Inventory, the Big Five Factors, the Costello and Comrey Depression and Anxiety Scales, and the Perceived Stress Scale. Numerous findings confirmed the validity of the Constructive Thinking Inventory, and the Global Constructive Thinking subscale displayed incremental validity. These data supported the validity of the Constructive Thinking Inventory and its associated theoretical assumptions in a sample of Iranian managers.

[Sexual function in men with lower urinary tract symptoms].

PURPOSE: Recently there have been several alternatives not only to improve symptoms but to retain an acceptable quality of life as well as to reduce the complications. Therefore the objective of this study was to evaluate quantitatively and qualitatively the degree of erectile dysfunction in the population of men with lower urinary tract symptoms (LUTS). MATERIALS AND METHODS: Total 252 men with LUTS were investigated using the International Prostate Symptom Score (I-PSS) and sexual function inventory (SFI) consisting of sexual drive, erection and ejaculation. Spearman's rank order correlation was used to determine the degree of any correlation between age, the total I-PSS, the individual I-PSS questions and the various sexual function scores. RESULTS: 208 patient data were available for analysis. There were poor function patients in 67.8% for sexual drive, in 46.2% for erection and in 47.1% for ejaculation. On the other hand, 24% considered their sexual drive to be a big or medium problem, 20.7% for erection and 18.3% for ejaculation. Overall, 27.4% of the men were mostly or very dissatisfied with their sex life. There was a significant correlation between a patient's age and his score for each of the three sexual variables (p < 0.05). Furthermore, a significant correlation was noticed between the total I-PSS and the SFI score (p < 0.05). It might be suggested that the more symptomatic a patient for LUTS, the poorer his sexual function will be. CONCLUSIONS: About 20% men with LUTS are bothered by their sexual symptoms. Erectile dysfunction in dependent of age and the extent of LUTS. Consideration of the high population of erectile dysfunction in men with LUTS is necessary to the treatment of their urinary symptoms.

The generation of big-endothelin (1-22) (endothelin-valine) from big-endothelin in the soluble fraction of porcine lung.

The degradation of big-endothelin (big-ET) in the soluble fraction of porcine lung was investigated. The degradation in the presence of p-chloromercuribenzoate (PCMB), pepstatin A, and EDTA resulted in the accumulation of two newly-formed fragments, big-ET (23-39) and big-ET (1-22), the latter called endothelin-valine (ET-Val). The generation of the two fragments was inhibited by diisopropylfluorophosphate (DFP). The enzyme responsible, called ET-Val-generating endopeptidase, was isolated from porcine lung by a procedure including chromatographies on columns of DEAE-cellulose, hydroxylapatite, Mono Q, p-mercuribenzoate-Sepharose, and Superose 6. The molecular weight of the enzyme was 140,000 and the pH optimum of the activity was 7.0. The activity was strongly inhibited by DFP, but scarcely inhibited by PCMB, EDTA, and pepstatin A. Thus, the isolated enzyme was classified as a serine protease cleaving big-ET at the Val22-Asn23 bond.

[The organization of work in the pediatric department of an urban ophthalmologic hospital].

The paper analyses the work of a children ophthalmological department and of a centre of microsurgery in children created on its basis, that are attributed to the structure of the town ophthalmological clinical hospital in Kharkov. The paper presents data on the structure of eye pathology, surgical activity (it makes up 60%), suggestions for optimizing the work of the department. A positive role in optimization of work in children department is its belonging to a big ophthalmological hospital. This allows to widely use for children the whole store of diagnostic means, different surgical equipment, other technical means. Of importance is a permanent contact with the chair of children ophthalmology, enrollment of physicians into research work, constant improvement of their skill.

[Osteoscintigraphy in the diagnosis of rheumatoid arthritis and Bechterew's diseases].

The paper is concerned with an analysis of the results of clinical and radionuclide examination of 36 patients with ankylosing spondylitis (AS) and 63 patients with rheumatoid arthritis (RA). The period of observation lasted from several week to 5 yrs. Osteoscintigraphy was performed using labeled agents. The data obtained coincided with pain sensations whereas the correspondence of painful zones and x-ray findings was noted in 39% of the patients. In RA hyperfixation of the drug was determined mainly in small and big joints (130-250%) in 92% of the patients coinciding with the pain syndrome and with X-ray findings in 38% of the cases only. Radionuclide signs of therapeutic efficacy were noted in 56% of the patients with AS and in 82% of the patients with RA. Radiation exposure of the bone marrow in osteoscintigraphy and x-ray investigation was defined (4.1 and 14.4 microGy). A radioimmunoassay showed elevated content of PTH in 67% of the patients with AS up to 4.0 +/- 0.5 iu/ml.

[The load-bearing function of a patellar tendon bearing cast].

An experiment was performed to study the load bearing function of a patellar tendon bearing (PTB) cast and the optimal effect of walking heel position and contra-lateral heel height. Miniature load cells were placed on the patellar tendon area, front and back area of the lower leg, anterior sole (MP joint of big toe), and posterior sole of the foot. The examinee walked on a force plate with the PTB cast. Vertical load bearing at each area in the PTB cast was evaluated by multiple regression analysis of data from the force plate and the load cells. The extent of decrease of skeletal loading was thirty per cent of body weight when the walking heel was placed slightly distal to the navicular bone. The effect of the contralateral heel height was not clear.

[Splenic lymphoma with circulating hairy lymphocytes lymphoma. Clinical cytological study of 4 patients].

PURPOSE: The splenic lymphoma with circulating villous lymphocytes (SLCVL) is an infrequent disease included within the low grade non Hodgkin's lymphoma, B-cell type. The results of the study of four patients are reported. PATIENTS AND METHODS: Clinical, cytological, immunophenotypic, ultrastructural, evolutive and therapeutic data have been revised in all the cases. RESULTS: Two males and 2 females of 76, 66, 68 and 62 years, respectively were diagnosed as having SLCVL. The initial symptoms were scarce, basically asthenia, and a big spleen without significant lymphadenopaty was the most relevant physical finding in each of them. In peripheral blood leukocyte count was normal with a slight lymphocytosis and a variable percentage of villous circulating lymphocytes. The immunophenotype of peripheral blood obtained by flow cytometry was according with a mature B-cell lymphocyte population, CD 5 and CD 25 negative. The cells were positive to acid phosphatase with a diffuse pattern of variable intensity; the reaction was inhibited by tartaric acid. All the patients had BM infiltration, studied with aspiration and biopsy. One case (M,66) had an IgM monoclonal gammopathy. The ultrastructural study, performed in 3 cases, showed thin and short villous prolongations. After splenectomy, a low degree lymphoma therapy has been employed in all the cases. The follow-up ranges between, 4 years and 4 months, all the patients being alive. CONCLUSIONS: The SLCVL is a definitive entity regarding the clinical, morphologic and immunophenotype features. A long clinical evolution and a good prognosis after splenectomy are common.

[The outlook for Spanish primary care starting with a bibliometric analysis of its scientific production (1971-1994)].

OBJECTIVE: To establish the relative weight of the various kinds of primary health care (PHC) research collected in the IME (Spanish medical index), In order to determine their possible relationships with Spain's PHC model. DESIGN: Bibliometric analysis. PARTICIPANTS: PHC documents (1971-1994) from the IME data base (CD-ROM), subdivided by years, journals, themes and Autonomous Communities (AC). RESULTS: 3,015 studies were published, with a first phase (1970s) of under 10 documents per year, a second (1980s) with a big increase and a third (1990s) of stagnation. Of the 117 journals containing studies, Atención Primaria gave a home to almost 58% (60% after 1984). CONCLUSIONS: PHC research production has stagnated recently, though the journal Atención Primaria has maintained its undisputed leadership position. The clinical model predominant in Spanish medicine is generally followed.

[Studies on the development of fetus scapula].

Extract and measure the scapular every part from the fetus that was carried into Saga medical college, the data was processed statistically. Also, the morphological characteristic of scapula of the fetus was clarified by image processing that a computer is used. The result was as follows. A scapular morphological length and also height become big in proportion to the height of fetus. Index of height-length of a fetus was big compared with adult, this trend is remarkable especially in a early stage of development of a fetus. Spinal axis angle of a fetus was also bigger than that of adult and it remarkable differ from an anthropoid very small ane. In general, the scapular shape of a fetus was short length with wide-spreading side and this form resembles to scapula of Jomon period people. The width of cartilage part tend to decrease as ossification progress fast width-wise with growth. However, area of cartilage part tend to become big, because the growth of a cartilage part is going fast lengthwise.

An examination of cetacean brain structure with a novel hypothesis correlating thermogenesis to the evolution of a big brain.

This review examines aspects of cetacean brain structure related to behaviour and evolution. Major considerations include cetacean brain-body allometry, structure of the cerebral cortex, the hippocampal formation, specialisations of the cetacean brain related to vocalisations and sleep phenomenology, paleoneurology, and brain-body allometry during cetacean evolution. These data are assimilated to demonstrate that there is no neural basis for the often-asserted high intellectual abilities of cetaceans. Despite this, the cetaceans do have volumetrically large brains. A novel hypothesis regarding the evolution of large brain size in cetaceans is put forward. It is shown that a combination of an unusually high number of glial cells and unihemispheric sleep phenomenology make the cetacean brain an efficient thermogenetic organ, which is needed to counteract heat loss to the water. It is demonstrated that water temperature is the major selection pressure driving an altered scaling of brain and body size and an increased actual brain size in cetaceans. A point in the evolutionary history of cetaceans is identified as the moment in which water temperature became a significant selection pressure in cetacean brain evolution. This occurred at the Archaeoceti - modern cetacean faunal transition. The size, structure and scaling of the cetacean brain continues to be shaped by water temperature in extant cetaceans. The alterations in cetacean brain structure, function and scaling, combined with the imperative of producing offspring that can withstand the rate of heat loss experienced in water, within the genetic confines of eutherian mammal reproductive constraints, provides an explanation for the evolution of the large size of the cetacean brain. These observations provide an alternative to the widely held belief of a correlation between brain size and intelligence in cetaceans.

Src and multiple MAP kinase activation in cardiac hypertrophy and congestive heart failure under chronic pressure-overload: comparison with acute mechanical stretch.

Activation of members of the mitogen-activated protein (MAP) kinase family and their downstream effectors has been proposed to play a key role in the pathogenesis of cell survival, ischaemic preconditioning, cardiac hypertrophy and heart failure. This study investigated the responses of Src kinase and multiple MAP kinases during the transition from compensated pressure-overload hypertrophy to decompensated congestive heart failure. Extracellular signal-regulated protein kinase (ERK) 1/2, p38, and Src were activated by chronic pressure-overload and their activity was sustained for 8 weeks after aortic banding. In contrast, while p90 ribosomal S6 kinase (90RSK) and big MAP kinase 1 (BMK1) were activated in compensated hypertrophy, their activities were significantly decreased in hearts with heart failure. No changes were found in C-Jun NH2 terminal kinase (JNK) activity after aortic banding. These data suggest that differential activation of MAP kinase family members may contribute to the transition from compensated to decompensated hypertrophy. We also examined acute effects of mechanical stretch on the activation of these kinases in normal and hypertrophied hearts. In the isolated coronary-perfused heart, a balloon in the left ventricle was inflated to achieve minimum end-diastolic pressure of 25 mmHg for 10-20 min. In normal guinea pig hearts, stretch activated ERK1/2, p90RSK, p38, Src, and BMK1 but not JNK. However in hypertrophied hearts, further activation of these kinases was not observed by acute mechanical stretch. Mechanical stretch-induced activation of ERK1/2 and p38 kinase in normal hearts was attenuated significantly by a protein kinase C inhibitor, chelerythrine. We demonstrate that ERK1/2, p90RSK, p38, Src, and BMK1 are activated by chronic pressure-overload and by acute mechanical stretch. These data suggest that Src, BMK1 and p90RSK play a role as novel signal transduction pathways leading to cardiac hypertrophy. In addition, the differential inhibition of p90RSK and BMK1 in hearts with congestive heart failure suggests the specific role of these two kinases to maintain cardiac function under chronic pressure-overload.

[BIG-H3 protein: mutation of codon 124 and corneal amyloidosis].

In 1997, a group of hereditary corneal dystrophies was related to mutations in the TGFBI (BIGH3) gene. Within this group, some corneal dystrophies present particular biochemical features in that they are characterized by corneal amyloid deposition. Contrary to clinical and genetic knowledge, the biochemical characteristics of the encoded protein (Big-h3) and the mechanisms of its amyloid conversion remain unclear. We review the current knowledge on the Big-h3 protein and focus on the behavior of the codon 124 region. We discuss this protein's mechanisms of amyloid conversion from our results and previous reports as well as from other types of amyloidosis. These data provide a better understanding of the putative processes leading to the phenotypic variations linked with their respective codon 124 mutation.

Dynamics of family size and composition: a computer simulation study with reference to rural India.

"This study attempts to understand the dynamics that produce the persistent observation of a strong positive correlation between family size and extent of landholdings in predominantly agrarian economies [in India]. Such a correlation can arise from different types of demographic configurations including the rules of family formation. For example, big landholdings may be associated with large families, despite the lack of differentials across holdings of different size in fertility and mortality, simply because these families may remain undivided for long periods. In the absence of conclusive data to analyse this relationship in the Indian case, this study sets up a computer simulation model for studying the results of alternative demographic configurations."

Regulation of endothelin-1 production in cultured rat vascular smooth muscle cells.

Endothelin-1 (ET-1) is secreted from all rat vascular smooth muscle cells (VSMCs) examined, in addition to endothelial cells (ECs). An average secretion rate of ET-1 from rat VSMCs was determined to be 10% that excreted from ECs. We examined the effects of 22 substances on ET-1 secretion from VSMCs and compared them with those from ECs. Transforming growth factor-beta1 (TGF-beta), acidic and basic fibroblast growth factors, epidermal growth factor, angiotensin II, and adrenaline stimulated ET-1 secretion from VSMCs, whereas forskolin, thrombin, and platelet-derived growth factor-BB reduced it. Only TGF-beta and phorbol ester elicited consistent effects on ET-1 secretion from VSMCs and ECs. Regulation of ET-1 and adrenomedullin secretion from VSMCs was distinctly different. These data suggest that ET- 1 production in VSMCs is regulated by a mechanism separate from that in ECs and from adrenomedullin production in VSMCs. Chromatographic analysis showed immunoreactive ET-1 secreted from VSMCs was mainly composed of big ET- 1, whereas approximately 90% of that from ECs was ET-1. By TGF-beta stimulation of VSMCs, the ratio of big ET-1 to ET-1 was further increased. Because big ET-1 is converted into ET-1 only on the surface of the ECs in the culture system, big ET-1 secreted from the VSMCs may function as a mediator transmitting a signal from VSMCs to ECs in vivo.

[Combination of bilateral pelvic lymphadenectomy, permanent iodine-125 implantation and percutaneous irradiation of localized prostate carcinoma. 2: Discussion and conclusions].

Severe complications (urethral stricture, ulcer at the anterior rectum wall, prostato-rectal fistula) have been observed in 28%(5/18) of patients treated by combined percutaneous and interstitial therapy for locally confined prostatic cancer. These complications were caused above all by charging the big prostatic volumes with an excessive number of iodine-125 seeds applied through cannulas arranged too closely and by an insufficient distance between the mucous membrane of the rectum and the first seed. The evaluation of our data showed insignificant complications up to a total activity of 28 mCi, slight or medium complications between 28 and 35 mCi, and severe complications between 35 and 40 mCi. It is therefore necessary in case of an intended combination of interstitial and percutaneous irradiation to take precautionary measures already during the implantation in order to avoid critical accumulated doses: the spatial distribution of the seeds may be not so close and the total activity has to be lower (25 to 30 mCi). The MPD (minimal peripheral dose) can be by 30 to 40% below that of implantation alone and the dose in the centre of the implant should not exceed the MPD value by more than 100%. If a considerable volume (greater than 10 cm3) is irradiated by the implant with more than 240 Gy, the percutaneous boost generally performed up to 40 Gy has to be reduced, or the centre of the implant must be shielded by lead satellites after 20 Gy at the latest. The distance between the seeds and the mucous membrane of the rectum should be at least 1 cm, the interval between interstitial and percutaneous irradiation at least 8, better 12 weeks. A percutaneous boost is only performed in case of a very inhomogeneous interstitial dose distribution and a too low MPD (in form of a rotating irradiation up to 30 Gy: 16 Gy by open radiation and 14 Gy with H absorber) or in case of microscopic manifestations in the pelvic lymph nodes or suspected beginning manifestations in the seminal vesicles (in form of a four field irradiation up to 50 Gy using individual secondary collimators in order to shield sound tissues as well as the prostate which is already sufficiently irradiated by the seeds.

[Connections of field 17 of the visual cortex with the mammillary nuclei in cats].

Using optical techniques by Nauta--Gygax, Wiitanen and Eager, degenerating nerve fibres and terminals were demonstrated to be present in the hypothalamic mammillary nuclei 9 days after a part of field 17 of the brain cortex was extirpated. Electron microscopic examination revealed different changes in large and small terminals of the boutons 5, 7 and 11 days after similar operations. The data represented demonstrate direct monosynaptic bilateral connections in field 17 of the optic cortex with the hypothalamic mammillary nuclei in cats. They are realized by fine fibrillae terminating mainly in large terminal boutons which form synapses on big and small dendritic branches. Thus, there is a structural base for the immediate influence of the optic cortex on the posterior hypothalamus.

Changes in the clinical and laboratory features of lupus patients after the big earthquake in Taiwan.

In order to understand the impact of the massive earthquake on disease activity, 60 patients with systemic lupus erythematosus (SLE) living in the earthquake area (group 1) and 31 patients living away from the area (group 2) were enrolled in this study. A questionnaire was used to record demographic data and clinical manifestations before and 6 months after the earthquake. Laboratory tests, including serum C3, C4, creatinine, anti-DNA antibody, CBC and 24 h urine protein were taken. The results showed that nine patients in group 1 had obvious disease flare-up as evidenced by fever, arthralgia, malar rash and photosensitivity, increased protein loss or edema, and other symptoms. However, there were no significant changes in either group 1 or 2 patients. In contrast, the comparison of laboratory parameters in group 1 patients before and after the earthquake showed a significant decrease of C4 from 17.3 +/- 15.3 to 14.1 +/- 7.33 and a significant increase of creatinine from 1.20 +/- 1.54 to 1.27 +/- 1.70 (P < 0.05), respectively. No significant difference for either C4 or creatinine was found in group 2 patients. In conclusion, there was no striking change in the clinical aspects for lupus patients. However, some laboratory results became abnormal in group 1 patients after the earthquake, indicating that their lupus status may have worsened.

Sexual division of labor: energetic and evolutionary scenarios.

This article examines comparative energetic data on hunter-gatherers in the context of evolutionary scenarios of the sexual division of labor, with respect to both specific task allocation and overall levels of daily physical activity. The division of labor between men and women, well marked in contemporary foraging societies, was once posited as the "true watershed" for the evolution of the genus Homo. Some research on brain-wiring even links sex differences in cognitive and spatial abilities to sex-specific foraging activities. Most recent evolutionary arguments posit that men focus on hunting and women on gathering activities to realize potentially conflicting mating and parenting goals. A range of cooperative strategies (male/female and female/female) for child provisioning is also under investigation. Attention to energetic and reproductive trade-offs has usefully challenged the proposition that women are excluded from big-game hunting due to constraints of foraging ecology and reproduction. Simplistic assumptions about gender roles are thus increasingly questioned in anthropology, as well as in archaeology. Current models in behavioral ecology explore ways in which foraging practices vary with ecological circumstances, aiming to derive testable hypotheses from fine-grained data on the behavior of contemporary hunter-gatherers. Data on overall physical activity levels (PAL) can also serve to evaluate relative male/female workloads in modern groups, reconstruct hominid energy requirements and activity profiles, and examine changes with subsistence intensification. Male/female PAL ratios show that a task-specific division of labor does not readily extrapolate to 24-hour energy expenditure and that male/female differences in workloads were not necessarily reduced with the transition to agriculture. With respect to gender roles and PAL, a shift away from facile stereotypes of human behavior is evident. The challenge is to incorporate a range of behavioral responses to ecological circumstances in reconstructions of our evolutionary past.
