[The occurrence of Ixodes ricinus in the select recreative areas in the Province of Szczecin. Part I].

Among Polish ticks species the most common Ixodes ricinus has the biggest medical importance. Within the last few years, the incidence of disease transmitted by ticks has rapidly increased. We have made a thorough analysis of the quantative and rate per cent of occurrence of various stages Ixodes ricinus in the forest areas of some places in Szczecin province and in the parks of Szczecin, that are known as highly recreative and frequently visted by many people. A total of 426 (68% numphs) specimens collected there show that ticks frequently occupy habitats closely associated with man.

Hematuria in a young recreational runner.

A 27-year-old recreational jogger developed gross hematuria following a 2-mile run. Evaluation revealed a transitional cell carcinoma of the bladder. Etiologies for pigmenturia and the pathogenesis of exertional hematuria are reviewed. The need to exclude other causes of hematuria before attributing the finding to exercise is emphasized.

Effects of recreational drugs on physical activity.

The literature relating to the effects of recreational drugs on physical work and endurance is reviewed. Interactions between muscular coordination and cognitive function make it difficult to formulate overall conclusions about the effect of these drugs on physical performance.

Cocaine-induced psychosis and sudden death in recreational cocaine users.

Fatal cocaine intoxication presenting as an excited delirium is described in seven recreational cocaine users. Symptoms began with the acute onset of an intense paranoia, followed by bizarre and violent behavior necessitating forcible restraint. The symptoms were frequently accompanied by unexpected strength and hyperthermia. Fatal respiratory collapse occurred suddenly and without warning, generally within a few minutes to an hour after the victim was restrained. Five of the seven died while in police custody. Blood concentration of cocaine averaged 0.6 mg/L, about ten times lower than that seen in fatal cocaine overdoses. Police, rescue personnel, and emergency room physicians should be aware that excited delirium may be the result of a potentially fatal cocaine intoxication; its appearance should prompt immediate transport of the victim to a medical facility. Continuous monitoring, administration of appropriate cocaine antagonists, and respiratory support will hopefully avert a fatal outcome.

Acute recreational injuries.

The injuries which occur in recreation have the effect of eliminating participation for a period and also in many cases lead to a cessation of physical activity. The loss of well being as a result of idleness is a price that today's inactive world cannot afford to pay.

Allergogenous mites (Acari: Pyroglyphidae) in private recreation houses.

The bed fauna of periodically inhabited dwellings markedly differs from the bed fauna in town apartments, both in quantity--a higher number of mites and quality--an important dominance of the species Euroglyphus maynei which makes up 70.4% of mites in country cottages, but only 28% in town apratments used by the owners of such recreation houses. In other apartments it is very rare. There differences are explained by the specific regime of humidity and temperature in the periodically inhabited rooms. The patients' worsening condition is apparently caused by quantitative changes.

[Correlations between the components of the test for the evaluation of physical performance II of the Canadian Association of Health, Physical Education and Recreation].

The Fitness-Performance Tests developed in 1965 by the Canadian Association for Health, Physical Education and Recreation have been criticized with regard to the validity of the 300 yard run as an endurance measure and with regard to the redundance of some items involving lower limb performance. The purposes of this study were to identify a more appropriate endurance performance test item and to analyze the inter-dependence of the items which utilize leg power in their performance. A sample of 9000 young Canadians aged 6 to 17 years, was randomly selected from the 10 provinces and the 2 territories. Correlation coefficients were calculated between: standing long jump, the 50 m. run, and the endurance run (800 m. for 6-9 years, 1600 m. for 10-12 years and 2400 m. for 13-17 years). Among the different age and sex groups, the correlations between the endurance run and the 50 m. run varied from 0.40 to 0.49. The correlations between standing long jump, the agility run (4 x 10 m.) and the 50 m. run were from 0.69 to 0.83 for all the boys and girls. The correlations between these three items are even lower when performances are analyzed by age and sex groups. The highest variations are mainly in the preadolescent period for both sexes.

Fatal methemoglobinemia resulting from ingestion of isobutyl nitrite, a "room odorizer" widely used for recreational purposes.

The alkyl nitrites, specifically isobutyl nitrite, have taken a prominent place among those substances enjoying widespread recreational use, primarily in discotheques and in any toxicologically verified deaths, the chemical may cause fatal toxic methemoglobinemia if ingested. A case with a fatal outcome is presented, and the chemistry and toxicology of the substance are discussed.

Effect of recreational exercise on pregnancy weight gain and subcutaneous fat deposition.

This study was designed to test the hypothesis that continuing a regular regimen of recreational endurance exercise alters the time-specific rate of maternal weight gain and subcutaneous fat deposition during pregnancy. Serial measurements of body mass and 5-site skinfold thickness were obtained from 44 women before and during pregnancy who continued their preconceptional exercise regimen throughout pregnancy and from women who voluntarily stopped their preconceptional exercise regimen either before conception (N = 31) or reduced it below baseline fitness levels in very early pregnancy (N = 4). In the first and second trimester, the rate of weight gain and change in skinfold thicknesses were unrelated to exercise performance. However, those who continued exercise had a reduced rate of weight gain and change in skinfold thickness at specific sites in the last trimester of pregnancy. Overall weight gains were (mean +/- SEM) 13.0 +/- 0.5 kg and 16.3 +/- 0.7 kg in the exercise and control groups, respectively, and the increases in the sum of skinfolds were 22 +/- 2 mm and 31 +/- 2 mm, respectively. We conclude that continuing a regular exercise regimen throughout pregnancy does not influence the rate of early pregnancy weight gain or subcutaneous fat deposition but decreases both in late pregnancy. However, overall pregnancy weight gain remains well within the normal range.

Syndesmotic ankle sprain in a recreational hockey player.

OBJECTIVE: To present a case of distal tibiofibular sprain, a rare injury among ubiquitous lateral ligament sprains of the ankle. CLINICAL FEATURES: A 45-yr-old recreational hockey player was treated for an ankle sprain demonstrating increased translation on the drawer test and pain with inversion stress, but also pain with external rotation and dorsiflexion. X-rays demonstrated no fractures. INTERVENTION AND OUTCOME: The conventional protocol of protect, rest, ice, compress and elevate (PRICE), employed for the more common lateral ankle sprains, was followed by early mobilization and proprioceptive training with a tilt board. Recovery was prolonged, and ossification of the ankle syndesmosis was seen in follow-up radiographs. CONCLUSION: Because management implications and outcome sequelae of the uncommon syndesmotic sprain are more severe than the commonly seen lateral ankle sprain, simple physical examination procedures useful in identifying syndesmotic sprains in the routine examination of the injured ankle are emphasized. These include dorsiflexion and external rotation stress and the "squeeze test."

The normalization of recreational drug use amongst young people in north-west England.

An anonymous self-report survey of drug use among a cohort of 776 14-15 year olds in North-West England was conducted at the end of 1991, aiming to estimate prevalence and profile users. Six in 10 reported being offered drugs, and 36 per cent to using drugs (20 per cent in the past month). Over half of those offered drugs had tried them. Lifetime prevalence was highest for cannabis (32 per cent), with five other drugs each being used by 10-14 per cent. Three groups identified were cannabis smokers, solvents sniffers, and stimulant/psychedelic (or dance drug) users. The population parameters of drug use were estimated at 33-40 per cent, suggesting up to 200,000 local young adults have used drugs. Most significantly, the proportion of young women being offered and using drugs and the prevalence of drug trying amongst young people in 'middle-class' schools, suggests a substantial social transformation is underway in respect of recreational drug use. The results of this survey confirm a general trend apparent in other British studies. As this cohort is tracked into the mid 1990s, the sociological implications will be significant.

Medical coverage of recreational cycling events.

Our goal is to optimize the level of medical care delivered to riders on the tour in a comprehensive way. Participation of local EMS squads and advance planning is essential for providing acute, prompt medical care at the scene and to provide transport to local or distant hospitals to evaluate or treat more serious conditions. Fortunately the majority of injuries is minor, but when EMS personnel and physicians work closely together, riders and tour staff feel more comfortable with concerns about their health during the event. Based on our experience, we have made suggestions for preventing injuries and have provided a framework for the physician interested in effectively delivering medical care at any recreational cycling event.

Muscle hernia in a recreational athlete.

Muscle hernia is not a new entity but has received little attention in the medical literature. With the public's increased interest in fitness and physical conditioning, it is likely that the orthopaedic surgeon will encounter these injuries. We present the case of a recreational athlete with a peroneus longus muscle hernia that was successfully treated by fasciotomy.

Constraints on the participation of individuals with mental retardation in mainstream recreation.

A questionnaire was designed to investigate the influence of skill level, behavioral characteristics, and labeling upon attitudes toward the participation of individuals with mental retardation in a recreation activity. The respondents were 320 members of 12 private tennis clubs. High multiple correlations between the predictor variables, attitude toward the behavior, and subjective norm, with behavioral intention supported the model of the theory of reasoned action. Attitudes, subjective norms, and behavioral intentions toward individuals with mental retardation were favorable regardless of the label assigned and perceived skill level. Perceived behavior of the target person, rather than skill level and label, was the more salient influence upon attitudes, subjective norms, and behavioral intentions.

Surveillance of serious recreational injuries: a capture-recapture approach.

Serious injury from sport and recreation is a leading cause of morbidity and mortality in the United States. Historically, occurrences of diseases with substantial public health impact have been monitored via surveillance systems in order to obtain information concerning the frequency with which the diseases occur. Surveillance leads to efforts that identify risk factors, and eventually, control measures to reduce the incidence of disease. Currently, the surveillance of sports injury represents only limited coverage in the U.S. It is important to begin discussions regarding approaches toward the development of surveillance of these injuries. Methods based upon the communicable disease surveillance model could potentially be used to monitor serious sports injuries. One method of surveillance, using the statistical approach of capture-mark-recapture, is presented as an example by which a national system of surveillance of serious sports injury could be established.

Influence of recreational activity and muscle strength on ulnar bending stiffness in men.

Bone bending stiffness (modulus of elasticity [E] x moment of inertia [I]), a measure of bone strength, is related to its mineral content (BMC) and geometry and may be influenced by exercise. We evaluated the relationship of habitual recreational exercise and muscle strength to ulnar EI, width, and BMC in 51 healthy men, 28-61 yr of age. BMC and width were measured by single photon absorptiometry and EI by mechanical resistance tissue analysis. Maximum biceps strength was determined dynamically (1-RM) and grip strength isometrically. Subjects were classified as sedentary (S) (N = 13), moderately (M) (N = 18), or highly active (H) (N = 20) and exercised 0.2 +/- 0.2; 2.2 +/- 1.3; and 6.8 +/- 2.3 h.wk-1 (P < 0.001). H had greater biceps (P < 0.0005) and grip strength (P < 0.05), ulnar BMC (P < 0.05), and ulnar EI (P = 0.01) than M or S, who were similar. Amount of activity correlated with grip and biceps strength (r = 0.47 and 0.49; P < 0.001), but not with bone measurements, whereas muscle strength correlated with both EI and BMC (r = 0.40-0.52, P < 0.005). EI also correlated significantly with both BMC and ulnar width (P < 0.0001). Ulnar width and biceps strength were the only independent predictors of EI (r2 = 0.67, P < 0.0001). We conclude that levels of physical activity sufficient to increase arm strength influence ulnar bending stiffness.
