[Kleine-Levin syndrome: contribution of brain SPECT in diagnosis].

INTRODUCTION: Kleine Levin syndrome is an infrequent disorder of uncertain aetiopathogenesis that usually affects adolescent males, producing drowsiness and alterations in behaviour, appetite and sexuality. We report a clinically typical case in which the brain SPECT showed right frontal hypoperfusion. CASE REPORT: Male aged 17, right handed, who presented drowsiness, apathy, alterations in his sexual behaviour (masturbations in public and attempted assault of women) and hyperphagia, which coincided with his undergoing an emotionally stressful period in his life. Brain SPECT revealed reduced flow in the right frontal lobe, although MRI and CSF study were normal. The polysomnographic study revealed a destructured pattern of sleep, with a reduction in phases III and IV and in REM sleep. He was treated with lithium and evolved favourably without any relapses during the 18 month follow up. CONCLUSIONS: Cases of Kleine Levin syndrome with structural lesions have been reported, but most of them are idiopathic. A disorder in the hypothalamus and the limbic system is suggested. In our case, the findings from the brain SPECT confirmed a non dominant hypoperfusion of the frontal lobe, which could result from a phenomenon of diaschisis brought about by a diencephalic dysfunction.

[The sexual life of 60-year-old Danish men].

INTRODUCTION: The sexuality of men is influenced by many factors, and there are changes throughout life. In a literature search only 35 fairly representative cross-sectional studies of elderly men were found. There is no population study of elderly Danish men's sexuality. The aim of our study was to describe the sexual life of a group of 60-year-old men from the Copenhagen area. MATERIAL AND METHODS: The material is a random sample of 66 men from a cohort found to be fairly representative of 60-year-old men in the County of Copenhagen. The men were interviewed using structured guidelines, which also included open questions. RESULTS: Around half of the men felt sexual desire once a week or more. Around one third had had fewer than four sexual partners throughout their lives, and six per cent did not have a partner at the time of the interview. Slightly more than half had intercourse once a month or more. Two thirds masturbated. One tenth had problems with the erection. Two thirds were satisfied with their sexual life. DISCUSSION: This study reveals a large variation in the sexual life of elderly Danish men.

[Influence of dibutyryl cyclic adenosine monophosphate on motility of sperm].

OBJECTIVES: To investigate the influence of cAMP/PKA signal transduction on human sperm motility, and to study the effect of dibutyryl cyclic adenosine monophosphate dibutyryl cyclic adenosine monophosphate (dbcAMP) on human sperm motility in vitro. METHODS: Sperm aseptically obtained by masturbation and prepared by swim-up technique from 10 healthy fertile men were incubated with different concentrations of dbcAMP. Measurement of mobility were carried out at 20, 30, and 60 min in all specimens. RESULTS: The sperm treated with dbcAMP showed a significant increase in sperm progressive motility and the percentage of motile cells. The effect seemed enhanced with the increasing of dbcAMP concentration. VSL and VCL were not affected by dbcAMP. CONCLUSIONS: dbcAMP can activate the mobility of human sperm in vitro.

Measurement of steroid levels in saliva in a population-based survey of lifestyle, medical conditions, marriage, sex life and hormone status in aging men: a feasibility study.

Some population-based studies on male aging measure testosterone and cortisol in saliva instead of serum, but very few measure estradiol and dehydroepiandrosterone sulfate (DHEA-S), suggesting further testing is needed for reliability and comparative validity. In addition, the effects of interview stress and circadian hormone secretion need to be checked. In a pilot study on the overall sexual capacity of aging men, 48 randomly selected, healthy, heterosexual, cohabiting men aged 50-80 years, from Mannheim, Germany, and 50 from the State College, Pennsylvania, USA, were administered a standardized interview covering medical biography, present and previous life and work, marriage and emotional status. Two saliva samples were collected from each subject for measurement of testosterone, cortisol, estradiol and DHEA-S levels before and after the interview, and each subject completed a confidential self-administered questionnaire on intercourse, masturbation, orgasm, fantasies, libido and arousal. Questionnaires, hormone measurement techniques and the survey protocol had been extensively pretested. Prior to the pilot study, the kits for measuring testosterone and DHEA-S in saliva were checked for comparative validity against established measuring techniques in serum in 31 cases for testosterone and in 24 different cases for DHEA-S. These 55 cases underwent clinical diagnosis and were not otherwise involved in this study. The cases had been referred to the Andrology Unit of the University Hospital, Marburg, for reasons unrelated to this study. Given the biological differences for both steroids between their presence in blood and in saliva, a perfect correspondence between the two values was not expected and was not observed. The correlations obtained, however, support the assumption that all statistical relationships between testosterone and DHEA-S values in serum and clinical, as well as behavioral, variables reported to date may be replicated for testosterone and DHEA-S values in saliva.

Influences of dibutyryl cyclic adenosine monophosphate and forskolin on human sperm motility in vitro.

AIM: To study the influences of dibutyryl cyclic adenosine monophosphate (dbcAMP) and forskolin on human sperm motility in vitro. METHODS: Semen samples, aseptically obtained by masturbation and prepared by swim-up technique from 20 fertile men, were incubated with different concentrations of dbcAMP and forskolin at 37 deg. Measurements were carried out after 10 min, 20 min, 30 min and 60 min incubation. Motility parameters were estimated by using an automatic analyzing system. RESULTS: Treatment with dbcAMP or forskolin resulted in a significant increase in sperm motility and progressive motility. The larger the concentrations of dbcAMP or forskolin, the greater the effect appeared. The straight linear velocity and curvilinear velocity were not affected by both agents. CONCLUSION: dbcAMP and forskolin increase the motility and progressive motility of human sperm in vitro.

[Seminal profile in workers exposed to cholinesterase inhibitor insecticides].

The objective of this study was to determine the physical, chemical and cytomorphologic characteristics of semen obtained from workers exposed to cholinesterase inhibitor insecticides and compare them with samples of the same nature obtained from unexposed subjects, and in addition, to correlate those findings with blood levels of cholinesterase. The study group consisted of 29 adult males, age range 20-54, that were exposed to cholinesterase inhibitor insecticides during 4 hours per day for a variable lapse (one year minimum), whereas the control group consisted of 30 unexposed individuals of the same gender, and similar age range. A thorough medical examination was performed in every individual. It consisted of an occupational medical history and complete physical exam. A blood sample was obtained in all the subjects in order to determine total cholinesterase levels by the modified S. Hestrin Hydroxamate method. Semen analysis was done in specimens collected after recent masturbation following a 3-day abstinence period. The data was processed and analyzed by the SAS computerized statistics program. The results revealed significant differences between both groups (p < 0.05) for the following variables: sperm count per mL., percentage of fast, mobile and immobile, live and dead spermatozoids. Spermatozoa morphology was found to be within normal limits as established by the WHO. Nevertheless, the differences related to the percentage of big head and amorphous spermatozoids was significant (p < 0.05). The results indicate that cholinesterase inhibitor insecticides affect certain variables of the semen analysis and hence the quality of the semen.

Poor acceptance rate for semen donors to a private cryobank in Nigeria.

A retrospective analysis of sixty one prospective donors who visited the cryobank in person from August 1999 to December 2001 was done. The study evaluated the acceptance rate of prospective semen donors and semen characteristics in order to assess the fertility potential of the supposedly normal population of young men. The outcome of the various screening processes--history taking, infection screening, blood grouping and Hb genotype assessment, semen analyses (pre-freeze and post-thaw), and post-quarantine (six months after production of last sample) infection--was analysed. Out of the 61 enquiries, 9 (14.8%) donors were recruited, 7 (77.8%) of whom were less than 25 years old. Fourteen (22.9%) donors had sperm concentrations greater than 60' 10(6) ml, 31 (50.8%) had motility greater than 60% and 17 (27.8%) had normal forms greater than 30%. Six (9.8%) of the donors were unable to masturbate and thus were disqualified, while only one donor (6.1%) failed to show up for a re-test after the completion of the quarantine period. Twenty one (38.2%) donors had low semen values based on the WHO reference value. We conclude that a high proportion of prospective semen donors may have suboptimal semen parameters, and that using strict criteria, poor acceptance rates for donors is observed in a semen donation programme.

Free radical scavenger effect of rebamipide in sperm processing and cryopreservation.

AIM: To study the effect of rebamipide added to semen samples and cryoprotectant on reactive oxygen species (ROS) production. METHODS: Semen samples from 30 fertile and healthy volunteers were collected by masturbation after 2 days approximately 3 days of abstinence. After liquefaction, the specimens were diluted with sperm wash media to a uniform density of 20 x 10(6)/mL. Rebamipide was added to semen samples and cryoprotectant to a final concentration of 10 micromol/L, 30 micromol/L, 100 micromol/L or 300 micromol/L. Specimens were incubated at 37 degree C in a 0.5 % CO(2) incubator for 1 h or cryopreserved at -196 degree C LN(2) for 3 days. The sperm motility and viability and the levels of ROS and lipid peroxidation of sperm membrance were assessed before and after incubation and cryopreservation by means of computer assisted semen analyzer, eosin-nigrosin stain, chemiluminescence and thiobarbituric acid assay, respectively. RESULTS: The sperm motility was significantly increased after incubation with 100 micromol/L and 300 micromol/L rebamipide (P<0.05). After cryopreservation, the sperm motility was significantly decreased in all concentrations (P<0.05), but the decrease was less with 100 micromol/L and 300 micromol/L rebamipide than that with other concentrations. The sperm viability showed no significant difference before and after incubation (P>0.05). The levels of ROS and lipid peroxidation in semen were significantly decreased in proportion to the concentrations of rebamipide both after incubation and cryopreservation (P<0.05). CONCLUSION: Rebamipide is an effective free radical scavenger in semen in vitro.

Importance of controlled liquefaction periods for valid steroid assays in human seminal plasma.

Incubation of seminal plasma for different periods up to 24 hours at room temperature resulted in a marked gradual increase in the levels of unconjugated pregnenolone, dehydroepiandrosterone, and testosterone, whereas no alterations were detected in unconjugated dihydrotestosterone and estradiol. No significant difference was observed in the levels of the most sensitive indicators of artifactual changes, pregnenolone and dehydroepiandrosterone, when seminal plasma was frozen immediately or within 30 minutes of ejaculation. It is suggested that, at least when steroid analyses in seminal plasma are contemplated, the samples of ejaculate should be provided by masturbation on hospital premises so that the necessary liquefaction period can be kept short, approximately 30 minutes.

The child's sexual life; a consideration of clinical implications.

A healthy sexual life begins in childhood, and the groundwork for later difficulties also begins in childhood. For the child, sexuality is a rather general, pleasurable excitation, not a specific genital stimulation. Excessive attention to the perineal area, operation upon or injury to the genital area and injections are vicissitudes which may have an unhealthy influence on later sexual development. Healthy and informed parental attitudes are the key here, just as they are in the normal exhibitionism, curiosity and intense emotional attachments to parents. Parents infect children, healthily or unhealthily, with their attitudes. In healthy growth exhibitionism and peeping become transformed in socially acceptable ways. In unhealthy growth, either perversions or strong reactions like over-modesty or shame result. Masturbation is common and transitory in most children. Parents, and especially pediatricians to whom parents turn, have a golden opportunity to direct healthy growth by being well informed about the infant and child's sexual growth and thus be enabled to advise upon or manage the common developmental phenomena with good commonsense and patience. Infants and children do not enter the world possessing the morals, standards or inhibitions of adults.

Video EEG analysis of non-ictal events in children.

Over a 3 year period 186 children aged 3 weeks to 17 years were studied by telemetry (prolonged video and EEG monitoring) at the Prince of Wales Children's Hospital: 74 had events considered at referral to possibly represent seizures but which were shown by clinical analysis and telemetry to be non-ictal. Nine such patients were developmentally delayed, one was neurologically impaired and 16 were both developmentally delayed and neurologically impaired. A specific diagnosis of the non-ictal events was reached in 24 subjects-postures of spasticity in children with neurological impairment (6), Münchausen-by-proxy (5), pseudoseizures (3), breathholding (2), masturbation (2), reflux (2), shudder (1), movement disorder (1), motor tic (1) and pertussis (1). Specific descriptive patterns were assigned to the remaining 51 events. These included staring (20) and jerks (16) or unusual behaviour (15). 49 inter-event EEGs were normal; 7 displayed abnormal background rhythms and 19 showed epileptiform activity. We discourage use of the term 'pseudoseizure' for the majority of the events described and prefer that a specific diagnosis be made or a descriptive analysis be provided. The events seen illustrate the wide spectrum of childhood behaviour and on occasions suggest the need for telemetry to determine their true nature.

Trends in the incidence, clinical presentation, and management of traumatic rupture of the corpus cavernosum.

BACKGROUND: Recent observations in our hospital of apparent increase in frequency prompted a revisit to the subject of fracture of penis. MATERIALS AND METHODS: In a retrospective review, patients' age, marital status, and causal activities; clinical presentations; methods of management; operative findings; and postoperative complications were analyzed for changing trends. Literature was also reviewed briefly. RESULTS: Nineteen episodes of penile fracture in 18 patients exceed the previous incidence by more than 58%. Most were due to noncoital causes (73.7%); more unmarried people (31.6%) were affected. Most presented with the classic symptoms/signs, and tears were repaired by degloving through subcoronal incisions; one was treated conservatively. The higher morbidity observed was attributed to higher rates of hematoma and wound infections, probably enhanced by poor hemostasis and early persistent, postoperative erections. Painful erections, painful coitus, and deformities, however subsided within weeks, with no long-term ill effects. CONCLUSION: The incidence of penile fracture, postoperative hematoma, and infections has increased; about 32% of the patients were unmarried. The prominence of masturbation as a cause of penile fracture and increased ratio of noncoital to coital causes are highlighted. Degloving through subcoronal incisions remains an acceptable method of approach for repair in line with cultural practices in Saudi Arabia.

Conflicted gender. The case of a woman who believed she was half male and half female.

The author presents the case of a middle-aged woman who had a fantasy that she was half male and half female, which did not emerge until she was well into her analysis. Initially, the analytic work focused on issues of attachment and trust, as the patient was mute and despairing, and unable to verbalize affects or put together a coherent story of her life. Gradually over the course of her analysis, a history of severe emotional neglect and trauma was pieced together--she had been left at the hospital as a newborn because her mother had not wanted a girl. It was only after the establishment of more comfortable and stable connection to the analyst that the conflicts around gender and the boy/girl fantasy came into focus. Two masturbation fantasies, a primitive, non-visualized sensation of sexual insatiability with abdominal throbbing, and a more traditional beating fantasy were also analyzed. The fantasy of being both male and female, which was conscious since childhood, was both a result of and a solution to more basic issues. It was a sign of problems in integration of internal parental identifications, a shaky maternal attachment, and difficulties in dealing with affect. The fantasy reflected a profound sense of being unwanted and unloved and became a narcissistic solution for these troubling feelings. The author details the interplay of constitutional, familial, and psychological factors that contributed to this solution. Because issues of gender were highlighted in this case, their complex relationship with other underlying factors could be appreciated.

Sexuality in healthy postmenopausal women.

OBJECTIVE: To assess the influence of psychosocial factors, behavior and hormones on postmenopausal sexuality. METHODS: Nine hundred and ninety-nine women (age range 41-60 years) underwent physical and supplementary tests and answered questionnaires regarding sexual behavior. Sixty healthy women with 1 or more years of amenorrhea, without hormone replacement therapy and with a partner capable of intercourse were chosen from this group. Logistic regression models with dependent variables (sexual satisfaction and orgasmic capacity) and independent variables (sexual initiation, psychosocial factors, behavior, relationship, menopause and hormones) were developed. RESULTS: Important variables for sexual satisfaction were: good self-esteem (p< 0.01), first orgasm obtained by masturbation (p = 0.004), major personal income (p = 0.007), sexual initiation in adulthood (p = 0.008), value physical contact with partner (p = 0.021) and major orgasmic capacity p = 0.040). The following contributed (towards orgasmic capacity with the partner: sexual initiation in adulthood (p = 0.012), regular physical activity (p = 0.040) and higher testosterone levels (p = 0.050). CONCLUSIONS: The importance of relationship, psychological, hormonal, economic and behavioral factors confirm the complexity of sexuality, and we note that current as well as prior events seem to affect the sexual satisfaction and orgasmic capacity of healthy postmenopausal women.

[Influence of the metabolite produced by Trichomonas vaginalis on human sperm motility in vitro].

OBJECTIVE: To investigate the effects of the metabolite produced by Trichomonas vaginalis on human sperm motility in vitro. METHODS: Trichomonas vaginalis having been cultured, the culture solution containing metabolite was obtained by removing the protozoa, then diluted into 3 kinds of concentration. Sperm was obtained from 10 healthy fertile men by masturbation and prepared by swim-up technique to produce a spermatozoon solution of high motility. Every sperm sample was divided into 4 groups (A, B, C, D). Unused culture solution was added to Group A as control, and the other 3 groups (B, C, D) were respectively incubated with the above used culture solution at 3 kinds of concentration (1.2 x 10(9)/L, 6 x 10(8)/L, 1.2 x 10(8)/L). Measurements were carried out at 30 s, 1 h, 2 h, 4 h, 6 h by CASA. RESULTS: Sperm motility decreased in both Group B and C markedly, and the effects displayed a concentration- and time-dependent manner. CONCLUSION: The metabolite of Trichomonas vaginalis can reduce human sperm motility in vitro, and may be one of the causes of infertility.

[Behavioral features of men who have sex with men].

OBJECTIVE: This behavioral surveillance survey in Sichuan province was aimed to gain an insight into the behavioral features related to HIV prevalence among men who have sex with men (MSM). METHODS: The pilot survey was initiated in the year from September to December, 2003. Two cities, Chengdu and Nanchong, were selected as the surveillance sites. All behavioral data were collected by a special questionnaire. Convenience sampling and snowball sampling techniques were applied to recruit participants in 5 types of places where MSM appeared more often. RESULTS: Most of the participants sexed only with male partners, and the most common ways of intercourse for MSM were anal sex, mouth sex and masturbation. The cases of multiple sex partners existed generally in MSM. The median partner numbers for anal sex and mouth sex were 4.4 and 4.2 respectively. There were 491 MSM (84.7%) who had sexed with male partners 6 months before, in which 68.6% of them sexed with non-commercial regular male partners, 66.2% with non-commercial non-regular male partners, and 20.4% with commercial male partners. Condom use with different male partners varied significantly. The proportions of consistent condom use with non-commercial regular, non-commercial non-regular, and commercial male partners were 15.8%, 16.3% and 32.3% respectively. 1.9% of participants reported they had had the experience of injecting drug and 18.3% of participants reported they had been tested for HIV antibody 12 months before. CONCLUSION: High risk behaviors such as multiple sex partners, unprotected anal sex, commercial sex, and injecting drug use among the MSM population in the two cities were unveiled broadly.

[Experimental study of the effect of rhTNF-alpha on human sperm mitochondrial function and motility in vitro].

OBJECTIVE: To study the effect of rhTNF-alpha on human sperm mitochondrial function and motility in vitro. METHODS: Fifty-six semen samples collected by masturbation were analyzed according to WHO protocols. Semen samples from 40 healthy men were prepared using Percoll centrifugation. Sperm suspension was diluted to a concentration of 10 x 10(6)/ml in Ham's F10 medium. Sperm samples were incubated with rhTNF-alpha solution (final concentration 0.03 microg/L, 0.06 microg/L, 0.09 microg/L and 0.27 microg/L, respectively) for 0.5 h, 1 h, 2 h, 3 h and 4 h at 37 degrees C in 5% CO2, and comparative studies were made with a control group. Ten microl sperm samples were examined with CASA technique, 250 microl stained in the presence of 10 microg/ml Rh123 and PI, and mitochondrial function analyzed by flow cytometry. RESULTS: Significant differences were found between the experimental groups (final concentration 0.06 microg/L, 0.09 microg/L and 0.27 microg/L) and the control group in viability, straight line velocity, curvilinear velocity, average path velocity, progressive motility of human sperm and the number of spermatozoa with normal mitochondrial function (P < 0.01) except the final concentration 0.03 microg/L group (P > 0.05). Motility of human sperm lowered with the increase of rhTNF-alpha concentration and incubation time, and r values were 0.675, 0.691, 0.762, 0.693, 0.724 and 0.571, 0.594, 0.752, 0.791, 0.816, respectively (P < 0.01). The number of spermatozoa with normal mitochondrial function decreased with the increased rhTNF-alpha concentration and incubation time, and r values were 0.615, 0.643, 0.752, 0.691, 0.754 and 0.532, 0.567, 0.782, 0.692, 0.854, respectively (P < 0.01). CONCLUSION: rhTNF-alpha can reduce human sperm motility function in vitro, possibly by interfering with human sperm mitochondrial function.

[A case of vesico-urethral foreign body with urinary fistula].

We report a rare case of vesico-urethral foreign body with urinary fistula. On March 12, 2002, a 53-year-old single men inserted a 3 m vinyl tube 6 mm in diameter into his urethra by himself for the purpose of masturbation, but he could not remove the tube. He cut his urethra by himself and tried to remove the tube. The next day he was admitted to our hospital, with complaints of urinary fistula and fever. Open surgical removal of foreign body, cystostomy and repair of bulbous portion of the urethra were performed. At 24 days postoperatively the cystostomy and urethral catheter were removed and urination became smooth.
