Blocked- and integrated-content baccalaureate nursing programs: a comparative study.

This study evaluated the differences between blocked- and integrated-content baccalaureate nursing programs related to faculty satisfaction, senior students' critical thinking ability, and students' state board examination performance. Faculty satisfaction had 10 factor scores--7 directly teacher-related factors and 3 indirectly teacher-related factors. Analysis of 85 faculty members by a one-dimensional, nested multivariate analysis of variance design revealed that faculty satisfaction was not a function of the type of content program. Analysis of 104 senior students by a one-dimensional, nested univariate analysis of variance design revealed no significant difference in critical-thinking ability between the two types of content programs. When 230 students' SBE scores were subjected to a one-dimensional, nested multivariate analysis of variance, blocked-content program students scored significantly higher than integrated-content program students. Recommendations for further study are made.

Integrating human resources and program-planning strategies.

The integration of human resources management (HRM) strategies with long-term program-planning strategies in hospital pharmacy departments is described. HRM is a behaviorally based, comprehensive strategy for the effective management and use of people that seeks to achieve coordination and integration with overall planning strategies and other managerial functions. It encompasses forecasting of staffing requirements; determining work-related factors that are strong "motivators" and thus contribute to employee productivity and job satisfaction; conducting a departmental personnel and skills inventory; employee career planning and development, including training and education programs; strategies for promotion and succession, including routes of advancement that provide alternatives to the managerial route; and recruitment and selection of new personnel to meet changing departmental needs. Increased competitiveness among hospitals and a shortage of pharmacists make it imperative that hospital pharmacy managers create strategies to attract, develop, and retain the right individuals to enable the department--and the hospital as a whole--to grow and change in response to the changing health-care environment in the United States. Pharmacy managers would be greatly aided in this mission by the establishment of a well-defined, national strategic plan for pharmacy programs and services that includes an analysis of what education and training are necessary for their successful accomplishment. Creation of links between overall program objectives and people-planning strategies will aid hospital pharmacy departments in maximizing the long-term effectiveness of their practice.

Integrated school and community programs.

Collaboration and coordination among school nurses, health teachers, guidance counselors, food service directors, principals, administrators, parents, and community agencies have the potential to upgrade school health programs. Benefits derived from an integrated school and community program are documented. Three models that have effectively brought together schools and community agencies are described.

Use of manpower data in an integrated database for program efficiency analysis: an example from a statewide community mental health system.

This article describes a method for systematically utilizing manpower data to analyze program efficiency of community mental health services. Examples of how analyses of performance data can be substantially enhanced by integrating manpower staffing data with other key financial, client, and service volume variables are provided.

Significance of infectious bursal disease serology in an integrated quality control program under European epidemiologic conditions.

In this study performed between 1993 and 1997, infectious bursal disease virus (IBDV) antibody titers and performance data were recorded in a vertically integrated monitoring scheme in order to make a follow-up from day-old parents down to the broilers at slaughter. All measured data were used two by two in a simple correlation study to calculate the degree to which they were linearly correlated. It appeared that high and/or uniform antibody titers in the parents were correlated with increased daily weight gain and decreased mortality and slaughterhouse condemnation in the broilers. Antibody titers and their CVs were negatively correlated in broiler parents and their offspring at day-old and even at slaughter. Results indicate that high and uniform antibody titers against IBDV in broiler parents are important for good performance of the broiler offspring, at least under the epidemiologic conditions of this study, which included the presence of very virulent IBDV strains in the field and the sole use of live intermediate vaccines in broilers as well as broiler parents.

New JCAHO standards require integrated patient safety program.

The Joint Commission on Accreditation of Healthcare Organizations is getting mostly positive reviews on its recently approved standards focused on patient safety and medical error reduction in hospitals. But experts warn that integrating these new standards into existing safety protocols will be no easy task. The new standards augment the nearly 50% of current Joint Commission standards related to patient safety. Requirements for establishing ongoing patient safety programs in organizations accredited under the Comprehensive Accreditation Manual for Hospitals will be added in the areas of leadership, management of information, and other functions.

Comparison of sensory integrative therapy and motor programming.

The relative effects of individualized sensory integrative therapy vs. small group, gross-motor programs were examined with a group of preschool children with mild to moderate motor delays. Children in the sensory integrative therapy group received one-to-one treatment twice weekly. Children in the motor-programming group received intervention four times weekly in small groups of three to four. Both intervention programs were carried out simultaneously for 17 weeks. No significant between group differences were observed in gross-motor gains on the Peabody Developmental Motor Scales or in sensorimotor gains on the Assessment of Sensory Integration in Preschool Children. Results of this study compared to results of similar previous studies were discussed.

Interdisciplinary cooperation in an integrated psychiatric rehabilitation program.

The development of psychiatry is at present stormy with discussions dividing, polarising and partly paralysing. Current experience shows that psychotherapeutic approaches are not alternatives to psychopharmacological ones but necessary complements. An integrated therapy which treats the patient as a social, psychological and organic entity has not yet been spread to the poorly-equipped psychiatric periphery but is mainly restricted to the modern richly-endowed psychiatric centers. Modern psychiatric rehabilitation principles according to this integrated synoptic view seem however to be applicable even to custodial-treated patients at peripheral psychiatric institutions. In this article we present the special psycho-social, epidemiological, administrative and economical structure of the island of Gotland due to its geographical isolation. We describe the construction and forming of a rehabilitation program under greatly limited economical conditions. The program combines communication-oriented group therapy with consequent pharmacotherapy and a structured ergotherapeutic and ADL-therapeutic program and is applied to a primarily unselected sample of patients. We also describe the education and supervision of the staff as well as the components of our program. Positive results in spite of limited resources have been achieved. Problems are discussed.

Resident, faculty, and residency program development. An integrated approach through annual retreats.

Retreats during residency training have evolved as an escape from the daily routine. Recognizing that the retreat format could also be used as a foundation for program design and personal development, we have designed annual retreats for each of the 3 years of pediatric residency training. The sessions vary in length and agenda, but serve as the basis for an ongoing effort fostering growth and maturation of the department and its component participants. The effectiveness of the program (and the morale of the residents) is greatly influenced by this program.

Effects of an integrated physical education/music program in changing early childhood perceptual-motor performance.

Two approaches to facilitating perceptual-motor development in children, ages 4 to 6 yr., were investigated. The experimental group (n = 15) received 24 sessions of integrated physical education/music instruction based upon concepts of Kodaly and Dalcroze. The control group (n = 15) received 24 sessions of movement exploration and self-testing instruction. Analysis of covariance indicated that significant improvement occurred only in the experimental group, with discharges changes in the motor, auditory, and language aspects of perceptual-motor performance as well as total score.

Successful systems integration strategies: the access program for persons who are homeless and mentally ill.

In 1993, the Access to Community Care and Effective Services and Supports (ACCESS) federal demonstration program was initiated. Using a quasi-experimental design, the 5-year demonstration program sought to assess the impact of integrated systems of care on outcomes for homeless persons with mental illness. The authors report on which integration strategies were chosen and how their implementation is quantified. Data collected primarily through annual site visits revealed that only two strategies were used by all nine systems. The systems integration strategies employed remained relatively stable over the 5 years. Successful implementation appears to be related to the strategies selected.

Genomic expression programs and the integration of the CD28 costimulatory signal in T cell activation.

Optimal activation of T cells requires effective occupancy of both the antigen-specific T cell receptor and a second coreceptor such as CD28. We used cDNA microarrays to characterize the genomic expression program in human peripheral T cells responding to stimulation of these receptors. We found that CD28 agonists alone elicited few, but reproducible, changes in gene expression, whereas CD3 agonists elicited a multifaceted temporally choreographed gene expression program. The principal effect of simultaneous engagement of CD28 was to increase the amplitude of the CD3 transcriptional response. The induced genes whose expression was most enhanced by costimulation were significantly enriched for known targets of nuclear factor of activated T cells (NFAT) transcription factors. This enhancement was nearly abolished by blocking the nuclear translocation of NFATc by using the calcineurin inhibitor FK506. CD28 signaling promoted phosphorylation, and thus inactivation, of the NFAT nuclear export kinase glycogen synthase kinase-3 (GSK3), coincident with enhanced dephosphorylation of NFATc proteins. These results provide a detailed picture of the transcriptional program of T cell activation and suggest that enhancement of transcriptional activation by NFAT, through inhibition of its nuclear export, plays a key role in mediating the CD28 costimulatory signal.

Process improvement program evolves into compliance program at an integrated delivery system.

An integrated delivery system discovered questionable practices when it undertook a process-improvement initiative for its revenue-to-cash cycle. These discoveries served as a wake-up call to the organization that it needed to develop a comprehensive corporate compliance program. The organization engaged legal counsel to help it establish such a program. A corporate compliance officer was hired, and a compliance committee was set up. They worked with counsel to develop the structure and substance of the program and establish a corporate code of conduct that became a part of the organization's policies and procedures. Teams were formed in various areas of the organization to review compliance-related activities and suggest improvements. Clinical and nonclinical staff attended mandatory educational sessions about the program. By approaching compliance systematically, the organization has put itself in an excellent position to avoid fraudulent and abusive activities- and the government scrutiny they invite.

Analysis of progress curves for enzyme-catalysed reactions. Automatic construction of computer programs for fitting integrated rate equations.

The computer analysis of progress curves for enzyme-catalysed reactions involves a series of mathematical and computational tasks. The three most daunting of these are the derivation of an integrated rate equation, solving this equation so that the amount of product formed by the reaction at any time can be calculated, and incorporating this solution into a non-linear-regression computer program. This paper describes the basis of a computer program that greatly simplifies the problem. The proposed mechanism is specified in the familiar kinetic constant form, which is automatically translated into a program capable of fitting this mechanism to a series of experimental progress curves. The approach is illustrated for a reversible reaction with one substrate and one product, and tested with some data obtained for the fumarase reaction. [Pilot program of immunization against viral hepatitis B, integrated in the extended immunization program in Abancay (peru)].

INTRODUCTION: Abancay is located at 2,395 m.a.s.l. in the Peruvian Andean Region. It is a hyperendemic area for Hepatitis B virus (HBV) and hepatitis delta virus (HDV) infection, where 7% of the deaths are caused by acute and chronic hepatitis infections. In 1991, a pilot immunization program against HBV infection was started to control the incidence of VHB-VHD, and as a strategy to improve the EPI coverage in general. METHODS: Vaccination population was programmed to include 1,301 children under 1 year old and 3,488 children between 1-4 years old. A total of 3 doses of DNA recombinant vaccine against HBV per child were administered. The vaccination schedule fit within the EPI immunization schedule. For children under one year old, the program includes: Newborn: BCG, Polio 1, HBV1; 2-month old: Polio 2, DPT1, HBV2; 3-month old: DPT2, Polio 3; 4-month old: polio 4, HBV 3 and; 9 month old: Measles. For children aged between 1 and 4 years old, the program includes: HBV1, HBV2 one month after HBV1, and HBV six months after HBV1. RESULTS: One year after the immunization program started, 1,262 (97.0%) children under one year old and 2,529 (72.5%) between 1-4 years old were immunized. No relevant adverse effects have been reported. There were 8 deaths due to acute respiratory infection and acute diarrhea diseases, which are the main causes of death in this area. One of the objectives achieved was an improvement in the EPI vaccination coverage for children younger than 1 year old, with a DPT coverage of 97.0% in 1991 compared to 63.0% in 1988, 26.9% in 1989, and 61.9% in 1990. CONCLUSION: The inclusion of HVB vaccination in the EPI for a HBV and HDV endemic area improves the total EPI coverage and it is safe an effective.
