[Non-Hodgkin's lymphoma with primary osseous manifestation, from the files of the Bone Tumor Registry of Westphalia].

30 cases of non-Hodgkin-lymphoma with primary bone manifestation were selected from the files of the Bone Tumor Registry of Westfalia from 1975 until 1992. Clinical data and radiologic aspects were evaluated. 60% of the lesions were localized within the long bones (femur 10, humerus 6, tibia 2). The lymphomas were classified based on paraffin-embedded material in combination with immunohistochemical findings according to the updated Kiel-classification. 29 B-cell lymphomas and one T-cell lymphoma of medium-sized pleomorphic type were found. 5 B-cell lymphomas were of low and 23 of high malignancy. Almost 50% of the tumors were of centroblastic type. The Kiel-classification was not applicable for one lymphoma, 3 others could not be subclassified because of shrinking artefacts. Prognosis in patients with localized disease (9 cases) was favorable, whereas only 25% of patients with generalized disease (12/cases) survived for 3 years or longer.

Jamaica: petition for antiretroviral drugs filed.

In February 2003, five Jamaican women with HIV/AIDS filed petitions with the Inter-American Commission on Human Rights seeking an order that the government provide them with antiretroviral drugs, in fulfillment of its obligations under the American Convention on Human Rights to respect the right to health and the right to enjoy the benefits of scientific progress.

[Future development of the Japanese chemicals regulation data search system and the effective use of the IRPTC legal file].

After ten years of international cooperation in developing the IRPTC Legal File, its usefulness in searching for chemicals regulation data from abroad and in Japan was studied. The superiority of the database in offering summaries of the legal texts from 50 countries and 7 international organizations was obvious, but the need for improvements in the coverage and the accuracy of the data was found. The development of a Japanese database of legal information on chemicals based on international information exchange is desirable.

At-risk providers hit again. Like NCFE before it, DVI files for bankruptcy, threatening access to capital for struggling and start-up healthcare providers.

Healthcare lenders National Century Financial Enterprises and DVI have fallen on hard times. One NCFE official pleaded guilty to fraud and DVI filed for bankruptcy last week. However, many say DVI's collapse won't reverberate through the industry like the fall of NCFE did. The president of Cypress Partners, Joseph Paul, left, says DVI's bankruptcy won't affect profitable, well-run companies.

Adequacy of representation of the National Drug File Reference Terminology Physiologic Effects reference hierarchy for commonly prescribed medications.

The National Drug File Reference Terminology contains a novel reference hierarchy to describe physiologic effects (PE) of drugs. The PE reference hierarchy contains 1697 concepts arranged into two broad categories; organ specific and generalized systemic effects. This investigation evaluated the appropriateness of the PE concepts for classifying a random selection of commonly prescribed medications. Ten physician reviewers classified the physiologic effects of ten drugs and rated the accuracy of the selected term. Inter reviewer agreement, overall confidence, and concept frequencies were assessed and were correlated with the complexity of the drug's known physiologic effects. In general, agreement between reviewers was fair to moderate (kappa 0.08-0.49). The physiologic effects modeled became more disperse with drugs having and inducing multiple physiologic processes. Complete modeling of all physiologic effects was limited by reviewers focusing on different physiologic processes. The reviewers were generally comfortable with the accuracy of the concepts selected. Overall, the PE reference hierarchy was useful for physician reviewers classifying the physiologic effects of drugs. Ongoing evolution of the PE reference hierarchy as it evolves should take into account the experiences of our reviewers.

A guide for tooth resection with a used reamer or file.

Tooth resection is an important procedure, frequently encountered in dental treatment. A surgical guide is essential to providing the direction for tooth resection. A used (dulled) endodontic reamer or file can be utilized as directional guide for tooth resection, because it is economical, autoclavable, and safe. The clinical use of this surgical guide is described.

Public Interest Litigation (PIL) filed by Centre for Health and Allied Themes (CEHAT) and others in the Supreme Court of India in 2000.

The Pre-Natal Diagnostic Techniques (Regulation and Prevention of Misuse) Act, 1994 [now renamed as Pre-conception and Pre-natal Diagnostic Techniques (Prohibition of Sex Selection) Act, 1994] came into operation with effect from 01.01.1996. The implementation of the Act rests with the States and UTs. However, nothing much happened till there was Public Interest Litigation (PIL) filed by Centre for Health and Allied Themes (CEHAT) and others in the Supreme Court of India in 2000. The Supreme Court intervened in response to this PIL and taking cognizance to the situation and prayers of the petitioners, ordered strict enforcement of the Act and gave number of directions in its several hearings. The Court heard the matter about 20 times since 2001. The PIL was disposed of in its last hearing on 5.9.2003. The final judgment dated 10.09.2003 reiterates all the earlier directions of the Supreme Court issued to Central Government, States/UTs for proper implementation of the Act.

Dental aspects of 1248 cases of child maltreatment on file at a major county hospital.

The frequency of head, face, mouth, and neck injuries due to child maltreatment was investigated in a review of 1248 cases on file in the pediatrics office at Hennepin County Medical Center in Minneapolis, Minnesota, from January, 1985, to December, 1989. There were 485 boys (38.8%) and 763 girls (61.2%) examined. Physical abuse cases (41.0%) outnumbered sexual abuse (35.4%) and neglect episodes (23.6%). The gender ratio was distributed equally except in sexual abuse, where there were 4.7 girls for each boy examined. More than half of the children (52.9%) were in the 0-to-4 year age group. Considering all cases together, 37.5% presented with injuries to the head, face, mouth, and neck. However, that percentage doubled (75.5%) when physical abuse episodes were reviewed alone. The intraoral injuries seen were five tooth fractures, three tongue and tongue frenulum lacerations, two lip frenulum lacerations, 11 injuries to the oral mucosa, eight palatal lesions, two fractures of the mandible or maxilla, loose and missing teeth, and dental neglect. Despite the high frequency of injuries, no dentists examined children at the time of the hospital intake or referred children for suspicion of maltreatment in the population studied. The findings suggest the importance of involving dental professionals in identifying, reporting, and preventing child abuse and neglect.

VA National Drug File Reference Terminology: a cross-institutional content coverage study.

BACKGROUND: Content coverage studies provide valuable information to potential users of terminologies. We detail the VA National Drug File Reference Terminology's (NDF-RT) ability to represent dictated medication list phrases from the Mayo Clinic. NDF-RT is a description logic-based resource created to support clinical operations at one of the largest healthcare providers in the US. METHODS: Medication list phrases were extracted from dictated patient notes from the Mayo Clinic. Algorithmic mappings to NDF-RT using the SmartAccess Vocabulary Server (SAVS) were presented to two non-VA physicians. The physicians used a terminology browser to determine the accuracy of the algorithmic mapping and the content coverage of NDF-RT. RESULTS: The 509 extracted documents on 300 patients contained 847 medication concepts in medication lists. NDF-RT covered 97.8% of concepts. Of the 18 phrases that NDF-RT did not represent, 10 were for OTC's and food supplements, 5 were for prescription medications, and 3 were missing synonyms. The SAVS engine properly mapped 773 of 810 phrases with an overall sensitivity (precision) was 95.4% and positive predictive value (recall) of 99.9%. CONCLUSIONS: This study demonstrates that NDF-RT has more general utility than its initial design parameters dictated

[Analysis of genetic diversity of preserved population of native chicken breeds by microsatellites and file foundation of markers].

The genetic diversity (allele frequency, mean heterozygosity, mean polymorphic information content(PIC), and Nei genetic distances) of preserved population of 11 native chicken breeds was analyzed by 20 microsatellite markers with high polymorphism. The clustering dendrogram was obtained eventually based on Nei genetic distances of 20 microsatellite. The results showed that 176 alleles were detected of 20 microsatellite loci in 11 native chicken breeds, the average number of alleles of each loci was 8.8 and the range of allele frequencies was 0.013-0.838, and 11 native chicken breeds shared 45 alleles. The average heterozygosity of 20 microsatellite loci was 0.6800-0.7432, the heterozygosity of Zang chicken was the highest (0.7432), and that of Langshang chicken was the lowest (0.6800). The range of PIC of 20 microsatellite loci were 0.6329-0.7023, which is higher than 0.5 and highly polymorphic. By the results of UPGMA tree, four groups were formed from 11 chicken breeds, the first group was Silky, Chahua, Xianju, Zang and Xiaoshan chicken; the second group was Luyuan and Langshan chicken; the third group was Gushi, Fatty and Dagu chicken; and the fourth group was Henan game chicken. The effect of preserved breeds and the differences among generations can be tested by the file foundation of molecular markers, which contained allele frequency, mean heterozygosity, and mean PIC of 20 microsatellite loci in different generation populations.

[Exclusivity of data in drug registration files and Israel's international status with regard to intellectual property rights protection].

Protecting intellectual property rights encourages creativity and innovation, which are key measures for research and development of new drugs and also promotes the introduction of drugs to new markets. Over the past years, many countries have enacted laws implementing the protection of data in drug registration files. Until recently, Israel has not provided any form of data protection. Current moves focus on new legislation granting exclusivity in the marketing of innovative drugs. Although controversial, this legislation aims to secure balance in Israel between the innovative pharmaceutical companies and the public's interest in competitive generic drugs.

The EpiLink record linkage software: presentation and results of linkage test on cancer registry files.

OBJECTIVES: Record linkage, the process of bringing together separately compiled but related records from different databases, is essential in many areas of biomedical research. We developed a record linkage program (EpiLink), which employs a simple mathematical approach. We describe the program and present results obtained testing it in a linkage task. METHODS: EpiLink was designed to be flexible with user-friendly settings to tailor linkage and operating parameters to specific linkage tasks, and employ deterministic, probabilistic or sequential deterministic-probabilistic linkage strategies as required. The user can also standardize data format, examine linkage results and accept or discard them. We used EpiLink to link a subset of cases of the Lombardy Cancer Registry (20,724 records) with the Social Security file of the population (1,021,846 records) covered by the registry. The linkage strategy was deterministic, followed by several probabilistic linkage steps. RESULTS: Manual inspection of the results showed that EpiLink achieved 98.8% specificity and 96.5% sensitivity. CONCLUSIONS: EpiLink is a practical and accurate means of linking records from different databases that can be used by non-statisticians and is efficient in terms of human and financial resources.

[Infanticide in the light of post-mortem findings and court files from the period 1990-2000 (selected problems)].

Drawing upon 28 court files of the District Court in Poznań and 30 post-mortem protocols--from the Department of Forensic Medicine at Poznań Medical Academy. This article tackles the issue of infanticide in the period from 1990 to 2000. The aim of this paper was to find answers to the following questions: what was the social background and mental state of female offenders? How was infanticide committed? In order to solve certain research problems, a document examination technique was employed to analyse the contents of the documents available. Female offenders were aged between 17 to 42 years. In the majority of cases (56.7%), perpetrators were occupationally active, single young women with a low level of education and having a working-class background. In the majority of cases (80%), active infanticide was committed. Most frequently, infanticide was committed by shutting a child into a tight space, and tamponade of throat and larynx. Passive infanticide was committed in 20% of cases, with infants left without care at the place of birth. No case of psychosis was determined in the examined material.

United States: challenges filed to anti-prostitution pledge requirement.

Two separate lawsuits were filed recently in US federal courts challenging a provision of US law requiring that non-governmental organizations have a policy "explicitly opposing prostitution" as a condition of receiving funding under the United States Leadership against HIV/AIDS, Tuberculosis, and Malaria Act of 2003 (US Global AIDS Act). US-based plaintiffs in both cases argue that the anti-prostitution pledge requirement in the Act violates US Constitutional guarantees of free speech and due process, and undermines proven, effective efforts to fight HIV/AIDS among sex workers.

Filing of applications and requirements for widow's and widower's benefits. Final rule.

We are revising one of our regulations to clarify that we will protect a title II claimant's filing date as of the date the claimant or other proper applicant on the claimant's behalf completes and transmits to the Social Security Administration (SSA) the Personal Information Identification data on the Internet Social Security Benefit Application (ISBA). This revision addresses an aspect of implementing the ISBA to provide certain rights to Internet filers that we afford to other filers. In addition, we are correcting one word in a different title II regulation. The revision is necessary to correctly reflect the circumstances under which a claimant for widow's or widower's benefits as the insured person's surviving divorced spouse would be considered "unmarried."

Overcoming diverse security protocols across institutions to enable convenient file-sharing and videoconferencing among collaborators.

The use of videoconferencing and file sharing tools among colleagues at large institutions has traditionally been confounded by differing security policies in place at each location. Building an electronic collaboration infrastructure based upon the lowest-common-denominator security configuration expected to be encountered will yield an accessible collaboration environment previously unavailable to geographically diverse researchers and clinicians.

Nuclear magnetic resonance in target profiling and compound file enhancement.

Nuclear magnetic resonance (NMR) has matured as an important tool in drug discovery and development, with firm establishment of its roles in lead generation and optimization through application of NMR-based fragment screening and structure-based drug design. Besides these applications, NMR technology has expanded to make contributions both earlier and later in the drug discovery process. This review will focus on the impact of NMR in the early stages of drug discovery, in particular in characterizing the viability of targets for further discovery exercises and improving high-throughput screening through compound file-enhancement initiatives.
