Compartmental multivariate analysis of exercise ECGs for accurate detection of myocardial ischaemia.

An accurate computer-assisted diagnostic method for detection of myocardial ischaemia, called MUSTA, is developed. MUSTA is based on compartmental multivariate analysis of variables available in the exercise ECGs, and is definitively implemented in Prolog. It is heuristically developed by determining diagnostic criteria, which interrelate a modified ST/HR-slope, ST-segment value and shape, and maximum heart rate, so that concordance with the TI-201 SPECT is maximised. In the learning group consisting of 47 patients, MUSTA provides a diagnostic accuracy of 98%, the detection of ischaemia being in absolute concordance with TI-201 SPECT. MUSTA is evaluated in a similar but independent group of 60 patients. Then, accuracy is 90%, and sensitivity is 94%. The performance characteristics are significantly better than those of the standard exercise ECG, whose diagnostic accuracy in these groups is 77% and 70%, respectively. This study suggests that MUSTA is a significant improvement for computerised assessment of myocardial ischaemia.

Further studies on the nature of fluid compartmentalization in chick calvaria.

Labeled mannitol and polyethylene glycol (mol. wt. 4000), PEG, were used as space markers to estimate the fluid compartmentalization of calvaria taken from 3-5-day-old chicks. The mannitol results indicated that about 80% of the water in these bones is extracellular. Only a small fraction of this extracellular space was available to the diffusion of polyehtylene glycol. Adsorption studies in vitro and prior incubation of the tissues with hydrolytic enzymes indicated that the phenomenon of exclusion of PEG was primarily physical in nature. The polyethylene polymer is excluded from the water of hydration of the mineral phase. Apparently much of the extracellular water (two-thirds) of bone is in the form of crystal hydration.

Nasal lymphoid tissue, intranasal immunization, and compartmentalization of the common mucosal immune system.

Mucosal application of vaccines with an appropriate adjuvant can induce immune responses at both systemic and mucosal sites, and therefore may prevent not only infectious disease, but also colonization of mucosal surfaces. Intranasal is more effective than intragastric immunization at generating earlier and stronger mucosal immune response. Nasal lymphoid tissue (NALT) and its local draining lymph nodes may retain long-term immune memory. IgA isotype switching, and the differentiation and maturation of IgA antibody-secreting cells (ASC) may occur before these cells migrate out of NALT, whereas IgG ASC responses require passage of the cells through draining lymph nodes of the NALT. Knowledge of whether immune memory cells can recirculate to and reside in the inductive sites other than their origin after encountering antigen will be helpful for understanding the compartmentalization of the common mucosal immune system as well as for determining the best route for delivering a mucosal vaccine against a particular pathogen.

Cryoprecipitate and Factor VIII commercial concentrates: in vitro characteristics and in vivo compartmental analysis.

In vitro and in vivo characteristics of cryoprecipitates and three commercial factor VIII concentrates (Kryobulin, Hemofil and Koate) were comparatively studied. Factor VIII:C/VIIIR:Ag ratio was very low in all commercial concentrates without differences among them. Conversely, the decrease of factor VIIIR:WFRCof was proportional to the degree of purity. Factor VIII:C/VIIIR:WFRCof ratio was shown to be a reliable index of factor VIII complex denaturation. Crossed electroimmunoassay showed a faster migration of factor VIIIR:Ag only in commercial concentrates. Using a two-compartmental open model that accounts for endogenous synthesis of factor VIII:C, single-dose kinetics of factor VIII:C were studied in 23 patients with classic hemophilia. Good agreement between measured and fitted values of factor VIII:C plasma concentration was observed. beta half-life was shorter in high purity concentrates and longer in intermediate purity concentrates and cryoprecipitates.

Compartmentation of acid-base balance in brain during complete ischemia.

During near complete hyperglycemic brain ischemia, brain lactate levels rise in excess of 16-18 mmol/K and are associated with severe brain infarction. Analyses of pHo, Pt(CO2), and total brain lactate under these circumstances suggest that H+, HCO3, and lactate become unequally distributed between cells and the interstitial space and, perhaps, even between different types of brain cells. In addition, to whatever physiological advantages it may generate, such compartmentalization may be a factor leading to cell death in brain ischemia.

Imaging and compartmental classification of solid pelvic tumours in children.

Thirty-five children aged from 1 day to 16 years (median 5 years) with solid pelvic tumours were investigated with US, CT and MR. All three methods gave similar estimates of tumour size. For defining location of the tumours, the pelvis was divided into three midline compartments (anterior, middle and posterior) and a right and left lateral compartment. CT and MR were accurate and equally reliable in determining the tumour location, US was less accurate. Evaluation of confinement to organ of origin was uncertain, regardless of imaging modality. Tissue characteristics with CT and MR did not contribute to the differentiation of the various tumour types, and contrast medium enhancement did not improve the discrimination. Compartmental localization was equally well assessed by CT and MR and, together with sex, was found to correlate with the tumour type.

A compartmental model for the uptake of chlorphentermine in isolated perfused rat lung.

Equilibrium binding studies in vitro have established that chlorphentermine (CP) can be accumulated in rat lung by both binding and partitioning. Kinetic analysis of CP uptake in the isolated organ assuming an exponential process also suggests two separate mechanisms, one being saturable binding or transport, and the other, non-saturable partitioning. The present study was concerned with an alternative compartmental representation of the uptake process, and the validity of conclusions based on an exponential model. A three compartment model was finally selected which explain ed the observed uptake over a wide range of CP perfusate concentrations (2.5 x 10(-7) - 2.5 x 10(-5) mol/L) to within experimental error; it consisted of a binding and partitioning compartment in addition to one associated with the vascular bed. At low perfusate concentrations (2.5 x 10(-7) mol/L) uptake was essentially due to binding, while at higher perfusate concentrations (2.5 x 10(-5) mol/L) most uptake was due to partitioning. The total number of kinetically estimated binding sites (0.109 mumol/g) was less than that found by equilibrium dialysis for lung homogenates in vitro (8.3 mumol/g), suggesting an accessibility limitation for binding sites in the isolated preparation receiving a brief (10 min) perfusion. The data also indicate that there are probably two classes of binding sites or regional concentration of CP in the lung. It is proposed that interpretation of exponential model parameters may be problematical.

Individualization of theophylline infusion rate on the basis of a nonlinear compartmental pharmacokinetic model.

In the present paper, a nonlinear compartmental model for theophylline pharmacokinetics is developed. The analytical solution of the model, in parametric form, is derived under plateau conditions for plasma metabolite concentration. The parameters are obtained from plasma and urine data using best fitting techniques and their values are used in order to calculate maintenance intravenous infusion. Numerical simulation is then performed in order to compare the drug concentration obtained by our approach with that of alternative intravenous regimens. The differences argue for individualized dosage regimens, since theophylline is a drug with a narrow therapeutic window and its concentration at the active sites strongly depends on characteristic parameters of the patient's response. Our results show that it is possible to estimate the patients' parameters during the first 8 h after intravenous administration of the drug and these parameters can be used to design an individualized dosage regimen in patients receiving theophylline intravenously.

Obesity, self-complexity, and compartmentalization: on the implications of obesity for self-concept organization.

OBJECTIVE: The relationship between obesity and structural aspects of the self-concept was examined in adult women. Participants were 119 adult women [age range: 18-73, M=26.9; body mass index (BMI) range: 16.2-54.7, M=27.3] who completed measures of self-esteem, self-complexity, and the spontaneous self-concept. BMI was associated with less complex and more compartmentalized self-knowledge and more frequent mention of weight-stereotypic traits as self-descriptive. The findings are discussed in the context of research on obesity- related stigma.

Effects of GABA-transaminase inhibition on brain metabolism and amino-acid compartmentation: an in vivo study by 2D 1H-NMR spectroscopy coupled with microdialysis.

The aim of this work was to study the neurochemical effects in the brain of GABA-transaminase inhibition by systemic administration of gabaculine (100 mg/kg, i.a.) in the rat. In order to investigate neurotransmitter and related amino-acid compartmentation and metabolism, we have developed an original tool: the coupling, in vivo, on the same animal, of 2D COSY 1H-NMR spectroscopy with intracerebral microdialysis. The main result is a continuous increase in GABA levels, both in the intracellular compartment (up to 3000+/-450%; P<0.001) and extracellular compartment (up to 808+/-82%; P<0.01) at the sixth hour. The intracellular increase in GABA level became significant at the first hour following gabaculine administration, whereas the extracellular level increased as of the second hour, probably indicating that accumulation of GABA in nerve endings precedes its release in synaptic clefts. Moreover, the levels of the excitatory amino acids, glutamate and aspartate, were decreased both in the intra- and extracellular compartments, thus enhancing sedative effects of the drug. We also observed a decrease in the global energetic creatine-phosphocreatine pool, which also could be related to the sedative properties of gabaculine, measurable by the diminution of cortical electrical activity and mean arterial blood pressure. Finally, the coupling between 2D 1H-NMR spectroscopy and intracerebral microdialysis appears to be an original tool for investigating the cerebral metabolic effects induced by pharmacological agents, in situ, in living animals.

Compartmental bone morphometry in the mouse femur: reproducibility and resolution dependence of microtomographic measurements.

Microcomputed tomography (microCT) is widely used for nondestructive bone phenotyping in small animals, especially in the mouse. Here, we investigated the reproducibility and resolution dependence of microCT analysis of microstructural parameters in three different compartments in the mouse femur. Reproducibility was assessed with respect to precision error (PE%CV) and intraclass correlation coefficient (ICC). We examined 14 left femurs isolated postmortem from two strains of mice (seven per group). Measurements and analyses were repeated five times on different days. In a second step, analysis was repeated again five times for a single measurement. Resolution dependence was assessed by high-resolution measurements (10 microm) in one strain and subsequent image degrading. Reproducibility was better in full bone compartment and in cortical bone compartment in the diaphysis (PE%CV = 0.06-2.16%) than in trabecular compartment in the distal metaphysis (PE(%CV) = 0.59-5.24%). Nevertheless, ICC (0.92-1.00) showed a very high reliability of the assessed parameters in all regions, indicating very small variances within repeated measurements compared to the population variances. Morphometric indices computed from lower- and higher-resolution images displayed in general only weak dependence and were highly correlated with each other (R2 = 0.91-0.99). The results show that parameters in the full and cortical compartments were very reproducible, whereas precision in the trabecular compartment was somewhat lower. Nevertheless, all compartmental analysis methods were very robust, as shown by the high ICC values, demonstrating high suitability for application in inbred strains, where highest precision is needed due to small population variances.

Comparison of total and compartmental gastric emptying and antral motility between healthy men and women.

There is increasing evidence of gender-related differences in gastric emptying. The purpose of this study was first, to confirm the difference in gastric emptying for both solid and liquid test meals between healthy men and women, and secondly, to investigate the origin of this difference by studying regional gastric emptying and antral motility. A standard gastric emptying test with additional compartmental (proximal and distal) evaluation and dynamic imaging of the antrum was performed in 20 healthy women studied during the first 10 days of the menstrual cycle, and in 31 healthy age-matched men. In concordance with previous reports, women had a longer half-emptying time for solids as compared to men (86. 2+/-5.1 vs 52.2+/-2.9 min, P<0.05). In our observations this seemed to be related to a significantly prolonged lag phase and a significant decrease in terminal slope. Dynamical antral scintigraphy did not show a significant difference. The distribution of the test meal within the stomach (proximal vs distal) showed more early proximal retention in women as compared to men. The terminal slope of the distal stomach was significantly lower in women. We did not observe a significant difference in gastric emptying of the liquid test meal between men and women. Gastric emptying of solids is significantly slower in healthy women as compared to men. These findings emphasise the importance of using different normal values for clinical and research purposes in gastric emptying scintigraphy in men and women. The difference could not be explained by antral motility alone. Increased proximal retention and a lower terminal emptying rate in women are observations to be further investigated.

Effect of saline acclimation on body water and sodium compartmentalization in Pekin ducks (Anas platyrhynchos).

The compartmentalization of body fluids was measured in individual Pekin ducks ( Anas platyrhynchos) drinking freshwater and after sequential acclimation to 300 mM NaCl and 400 mM NaCl. Total body water, extracellular fluid volume, plasma volume and exchangeable sodium pool were measured using (3)H(2)O, [(14)C]-polyethylene glycol, Evans Blue dye, and (22)Na dilution, respectively. Following acclimation to 300 mM NaCl, body mass decreased, but total body water and total exchangeable sodium pool were unaltered. Na and water were redistributed from the extracellular fluid (interstitial fluid) compartment into the intracellular fluid compartment. Following further acclimation to 400 mM NaCl, body mass, total body water and intracellular fluid volume decreased, but exchangeable sodium pool and extracellular fluid volume were unchanged. Our results suggested that, when Pekin ducks drink high but tolerable salinities, they maintain total body water, but redistribute Na(+) and water from interstitial fluid to the intracellular fluid compartment. When stressed beyond their ability to maintain total body water, they lose water from the intracellular fluid.

Effectiveness of neuroendoscopic procedures in the treatment of complex compartmentalized hydrocephalus in children.

OBJECTIVE: The main object of the work is to assess the suitability of neuroendoscopic techniques for the treatment of complex compartmentalized hydrocephalus. METHODS: For this purpose the authors compared two groups of children treated in the Research Institute of the Polish Mother's Memorial Hospital from March 1997 to January 2002. The first group of 47 children, treated using neuroendoscopic procedures, was compared with the second, which comprised 80 children treated conventionally by complicated shunt implantations. RESULTS: The children treated neuroendoscopically needed on average 1.7660 procedures during the entire therapy, and 1.0232 operations a year. The same parameters in the conventionally-treated group were as follows: the total number of procedures was 7.050, and the rate of reoperation was 3.949 procedures a year. The proportion of patients who suffered from complications connected with operative treatment and who had a bad outcome was higher in the conventionally-treated group than in the neuroendoscopic one.

Model for the behaviour of compartmental CO(2) stores during incremental exercise.

The respiratory exchange ratio (RER) is a valid method for determining fat and carbohydrate oxidation during exercise when the exchange of respiratory gas is in a state of steady flux between the tissue and fluid compartments and the alveoli. However, under incremental intensity or heavy exercise conditions, the movement of electrolytes, fluids, and CO(2) between body-fluid compartments is accentuated, leading to increased hydrogen-ion concentration ([H(+)]), decreased bicarbonate-ion concentration ([HCO(3) (-)]) and CO(2) stores, and the excretion of additional CO(2) at the alveoli (i.e. H(+)+HCO(3) (-) --> CO(2)+H(2)O) elevating the CO(2) minute volume. This non-respiratory CO(2) excretion can invalidate use of the RER for determination of fat and carbohydrate oxidation. Direct measurement of the labile CO(2) store and non-respiratory CO(2) excretion during exercise is difficult. Therefore, physicochemical models were derived to illustrate the likely behaviour of compartmental CO(2) stores during 8 W.min(-1) incremental cycling exercise to formulate correction factors to the RER for the non-respiratory CO(2) component. From these models, a polynomial regression equation was derived to describe the change in the total labile CO(2) store volume during incremental exercise from the relationship with blood HCO(3) (-) content: CO(2) volume (ml) = -17x(2)+464x+650, where x is the arterialised blood standard HCO(3) (-) concentration (mmol.l(-1)), relative to resting conditions. Non-respiratory CO(2) excretion (ml.min(-1)) was then determined from the rate of change in CO(2) volume. The modelling method could allow for straightforward calculation of the non-respiratory CO(2) excretion rate for future validation work.

Absence of turnover and futile cycling of sucrose in leaves of Lolium temulentum L.: implications for metabolic compartmentation.

To study the interdependence of sucrose accumulation and its hydrolyzing enzyme, soluble acid invertase (AI; EC 3.2.1.26), in fructan-accumulating temperate grasses and cereals, experiments were performed in which sucrose synthesis was abolished in leaves of Lolium temulentum by four independent inhibitory factors, each having a distinct mechanism of action. Trials in the light with mannose or vanadate and in the dark with anoxia or cyanide showed that previously accumulated sucrose was stable in the tissue over a 5- to 6-h period. Conversely, putatively vacuolar AI activity in tissue homogenates was sufficient to completely convert endogenous sucrose to monosaccharide within the same period. Continuous invertase-mediated breakdown of sucrose was thus not a feature of this tissue. It is concluded that AI and sucrose were not in metabolic contact in vivo, implying differential compartmentation. In darkness, in uninhibited leaves, sucrose concentrations fell linearly with respect to time at a rate of -0.6 mg g(-1) FW h(-1), over a 5- to 6-h period. This value is equivalent to rates of dark respiration measured by gas exchange. Dark-utilisation of sucrose was not accompanied by monosaccharide accumulation in the tissue. The rate of sucrose loss was 3-fold lower than rates of extractable AI activity. Hence, if AI was involved in dark-utilisation, then this implies at least a partial differential localisation of enzyme and substrate. However, the dark-consumption of sucrose was completely abolished by anoxia and by cyanide. It follows that dark-mobilisation (unlike invertase hydrolysis per se) was respiration-dependent and did not result from a simple co-localisation of sucrose and invertase. Taken together, the results show that sucrose and invertase do not share the same metabolic compartment in grass leaves. It is possible that invertase has no role in the mobilisation of stored sucrose in leaves of the fructan-accumulating grasses.

Dorsoventral compartmentalization of mesoderm in heart-forming area of chick embryo.

In early chick development (stages 5-8) the seemingly homogeneous mesoderm in the heart-forming area splits to somatic and splanchnic cardiogenic layers. Little is known about dorsoventral compartmentalization before splitting. Electron microscopic analysis shows the early dorsoventral polarization of precardiomyocytes. The dorsal compartment has epithelial and the ventral compartment mesenchymal features with numerous protrusions. At stage 5+-6 staining for wheat germ agglutinine (WGA) transiently demarcates the ventral part of mesoderm. The glycosomes (beta-glycogen) show a dorsoventral gradient in the mesoderm of the cardiogenic field during the initial step of the compaction. The differential expression of glycosomes depends on the activity of glycogen synthase kinase 3-beta, a component of the wnt-signaling pathway, and might in this spatiotemporal developmental window be involved in the commitment of presumptive cardiogenic and somatic cells. To verify this hypothesis simulation experiments with LiCl in vitro were carried out. The normal splitting of the mesoderm and the development of heart primordia were disturbed. Blocking the receptors of WGA by WGA in vitro at stage 5-5+ perturbs the migration of mesoderm to anterio-medial direction. It appears that early specification of dorsal and ventral compartments of the mesoderm in the heart-forming area correlates with the gradient of glycosomes. Our results suggest that the target of LiCl action (glycogen synthase kinase 3-beta) might be involved in the specification of heart primordia and that WGA receptors mediate the migration of mesoderm to the anteriomedial direction.

Differential interhemispheric cooling and ICP compartmentalization in a patient with left ICA occlusion.

We report a case of a 69-year-old white female who presented with a large left internal carotid artery occlusive stroke from a cardiogenic embolus. She was enrolled in an institutional study using a specially designed cooling helmet. Bilateral intracranial pressure (ICP) and temperature probes were placed to determine if there was any differential cooling and ICP compartmentalization between the two hemispheres. We demonstrated a significant temperature gradient between the infarcted and the non-infarcted hemisphere. A significant inter-hemispheric ICP gradient was also observed. We believe that this is the first demonstration of preferential cooling of the infarcted hemisphere over the non-infarcted hemisphere with regional surface hypothermia.
