[Characteristics of human sleep, circadian periodicity of physiological functions and of work capacity indices in the 1st 24 hours after the transition from an altered to a usual sleep and wakefulness cycle].

Twelve male test subjects, aged 28 to 40, took part in the studies, in which a normal work-rest cycle was preceded by three types of an altered cycle, to investigate their adaptation-readaptation reactions. The return to the normal work-rest cycle was accompanied by slight changes in physiological functions, work changes was correlated with the type of altered work-rest cycles and with the degree of men's adaptation to them. The return to the normal work-rest cycle proceeded more readily, if adaptation to an altered cycle was incomplete. It is concluded that the required level of work capacity of men that have to vary their work-rest cycles frequently can be maintained by short-term (for 1 to 3 days) cycle alterations.

Sickness certification in general practice: a review.

Sickness certification is one of the most common tasks performed in general practice. This review describes and discusses concepts and terms used in earlier studies. 'Sickness certification' is defined, and related to the issues of 'absence from work' and 'sickness absence'. The use of measurements and results reported are emphasized according to patient- and doctor-related variables. Great variations are found, and some of the reasons may be differences in morbidity patterns, diagnostic procedures or sickness benefit acts. However, in studies from general practice, the number of sickness certificates is related to different denominators without describing the real population at risk, that is those of the patients who were employed or entitled to sickness benefits. Further studies are needed on the epidemiology of sickness certification, and the duration of the episodes. Analysis of the basis for the doctors' decisions, the patients' viewpoint, inter-doctor variations and doctors' attitudes should also be emphasized in the future. There is a need to discuss the reliability and validity of the measurements used, and theoretical considerations of the doctor's sickness certification practice are called for.

[Functional capacity evaluation of nursing professionals].

OBJECTIVE: To evaluate the functional capacity of nursing staff in a hospital complex and its association with individual and work characteristics. METHODS: A cross-sectional comprising 885 subjects was carried out. A standardized self-administered questionnaire was used to calculate a work capacity index based on information regarding occupation, morbidity, demographic and socioeconomic data. Odds ratios and their respective 95% confidence intervals were estimated using the Chi-square test. A multiple logistic model was used for testing the associations and adjusted odds ratios were obtained. RESULTS: The study population included primarily women (87.3%), aged between 35 and 68 years (43+/-6.3 on average), who worked as nursing aides, technical-level professionals and nurses in seven hospital units that integrate the hospital complex. Work capacity was considered good in over 80% of the staff. Statistical analyses showed that those with higher schooling (OR=0.4; CI=0.2-1.0; p=0.05 and adjOR= 0.4; CI: 0.2-1.0; p=0.04) and who exercise and have some physical activity (OR=0.5; CI: 0.3-0.9; p=0.02 and adjOR: 0.5; CI: 0.3-0.9, p=0.02) are more likely to have good work capacity. Those with reduced work capacity were found to have a high prevalence of musculoskeletal diseases. CONCLUSIONS: The company's worker health program should include guidance and follow-up of nursing staff regarding their physical and leisure activities aiming at promoting good work capacity as well as implementing preventive measures against musculoskeletal diseases.

Three frames of reference in work-related occupational therapy programs.

This paper discusses the relationship of a theoretical frame of reference to the implementation of a work-oriented program. Three frames of reference in occupational therapy are reviewed, and work-related theories and approaches available to therapists are categorized under each. Examples of the application of various frames of reference are given through an analysis of work programs described in the occupational therapy literature. We recommend that therapists select and apply frames of reference systematically when developing work programs to facilitate the development of a body of knowledge in this area.
