Effect of loading on the development of nerve fibers around oral implants in the dog mandible.

Occlusal forces cause stress which morphologically affects the supporting tissues of implants. The aim of this study was to examine the effects of occlusal forces on the distribution of neurofilament protein (NFP)-positive nerve fibers in the tissue of peri-implant bone. The bilateral 2nd, 3rd and 4th mandibular premolars and the 1st molars were extracted from three mongrel dogs. After 4 months of healing, 4 screw-type implants were inserted in the oral cavity. Three months after insertion, the implants on the molar site were loaded by occlusal forces, while those on the premolar site were unloaded. After a further 3 months, the dogs were sacrificed, and specimens were prepared for immunohistochemical NFP-positive staining by the labeled-streptavidin-biotin method. Many NFP-positive nerve fibers were found in the tissues of the loaded site when compared with the unloaded site. These fibers were localized in both the bone marrow space and in the peri-implant fibrous tissue. They had two types of nerve endings: simple free nerve endings, and nerve endings with tree-like ramifications. The present results suggest that loading by occlusal force causes an increase in the number of NFP-positive nerve fibers, many of which have free nerve endings in the peri-implant tissue. The possible role of these NFP-positive nerve fibers is discussed.

Purification and properties of a new Brevibacterium sterolicum cholesterol oxidase produced by E. coli MM294/pnH10.

A gene encoding a cholesterol oxidase from Brevibacterium sterolicum nov. sp. ATCC21387 was isolated by an expression cloning method and highly expressed by a recombinant strain Escherichia coli MM294/pnH10. The purified cholesterol oxidase was a typical flavoprotein with a molecular mass of 46.5 kDa, absorption peaks at 280, 360, and 450 nm. Optimum pH and temperature were found at pH 6.5 and 55 degrees C, respectively. The enzyme acted on 3beta-hydroxysteroids such as cholesterol, pregnenolone, and beta-sitosterol at high rates, but on dehydro-epi-androsterone to a lesser degree. The molecular and catalytic properties were different from those of cholesterol oxidase I, which was initially discovered in B. sterolicum nov. sp. ATCC21387. The new enzyme, designated cholesterol oxidase II, was distinguished by its high affinity toward cholesterol (K(m)=30 microM).

[Adsorption of local anesthetic into disposable infusion balloon].

BACKGROUND: The aim of this study was to investigate the adsorption of local anesthetics lidocaine and ropivacaine, into disposable infusion balloons made from various kinds of plastics. METHODS: The concentration of local anesthetic that flows out of a balloon was measured. RESULTS: The concentration of both lidocaine and ropivacaine in clinical formula decreased only 4.5 percent regardless of infusion balloons. However, the concentration of lidocaine pH 7.4 decreased by 10 percent in the Syrinjector made from polypropylene and polyvinyl chloride, and that of 18-20 percent in other infusion balloons (Surefuser, Baxtor Infuser, DIB Catheter made from isoprene rubber and polyvinyl chloride, isoprene rubber and polyvinyl chloride, silicon and polyvinyl chloride, respectively). CONCLUSIONS: The adsorption of local anesthetic into infusion balloons has little effect in clinical situation. Whereas, in case of lidocaine pH 7.4, the adsorption depends on the specific type of plastics.

mRNA expression of cytokines and chemokines in the normal gastric surface mucous epithelial cell line GSM06 during bacterial infection with Helicobacter felis.

BACKGROUND AND AIM: A group of the proinflammatory and chemotactic cytokines (chemokines) has been considered as an important factor in the pathomechanism of different bacterial diseases, among them the common Helicobacter pylori infection. Experimental results obtained with gastric biopsy samples of H. pylori positive patients, and with H. pylori infected tumor originated gastric cell lines indicated that these cytokines have essential roles in the development and maintenance of the immune response and inflammation of the gastric mucosa during H. pylori infection. Although the mRNA expression was shown in these biopsy samples and cell lines, it is not yet proved that the normal gastric mucosal epithelial cells themselves express these cytokines. The establishment of a gastric surface mucous cell line with non-tumor origin (GSM06), and the usage of Helicobacter felis as a model of the classic H. pylori infection gave us the possibility to check this question. MATERIALS AND METHODS: in this study GSM06 cells were infected with different numbers (10(5), 10(6), 10(7), 10(8), 10(9) bacterium/ml medium) of H. felis for two different time periods (2, 4 h). Cells treated with medium only were used as control. Then the mRNA expression of the following cytokines was measured by RT-PCR method in the GSM06 cells: proinflammatory cytokine IL1-beta, and chemokine RANTES, eotaxin, MCP-1, MIP1-alpha and MIP1-beta. RESULTS: we found that neither mRNA of the investigated cytokines was expressed constitutively, however the GSM06 cells expressed the mRNA of each cytokine during H. felis infection. CONCLUSION: our results prove that normal gastric surface mucous epithelial cells express immunologically active peptides during H. felis infection. We may suppose that the epithelial cells of the gastric mucosa contribute to the immune response and inflammation by expressing proinflammatory (IL1-beta) and chemotactic (RANTES, eotaxin, MCP-1, MIP1-alpha and beta) cytokines during H. pylori infection in human.

Is the questionnaire for the assessment of gastroesophageal reflux useful for diabetic patients?

OBJECTIVE: It has been suggested that the questionnaire reported by Carlsson et al. has high sensitivity in diagnosing gastroesophageal reflux disease (GERD) and is considered to be one of the most useful diagnostic tools for GERD. The aim of the present study was to evaluate the ability of the questionnaire to identify gastroesophageal reflux (GER) diagnosed by 24-h pH monitoring in diabetic patients. MATERIAL AND METHODS: Fifty-three patients with type 2 diabetes were enrolled in the present study. GER was monitored by means of an antimony electrode. Patients completed the questionnaires immediately before undergoing pH monitoring. RESULTS: Diabetic patients were found to have few symptoms, and the average score on the questionnaire was extremely low. No significant difference in scores on the questionnaire was observed between patients with and those without GER. CONCLUSIONS: The questionnaire was shown not to be useful for diagnosing GER as a complication in diabetic patients. We, therefore, should not diagnose GER as a complication in diabetes mellitus on the basis of the questionnaire.

Percutaneous hot saline injection therapy for hepatic tumors: an alternative to percutaneous ethanol injection therapy.

PURPOSE: To evaluate the usefulness of percutaneous hot saline injection therapy (PSIT) in the treatment of hepatocellular carcinoma (HCC). MATERIALS AND METHODS: Twenty patients with HCC (23 nodules) underwent PSIT. Mean tumor diameter was < or = 30 mm. PSIT was performed by injecting hot saline into the HCC. The therapeutic effect of PSIT was evaluated with computed tomography, angiography, measurement of alpha-fetoprotein levels, and histopathologic examination of needle biopsy and resected specimens. RESULTS: No complications were encountered, and a therapeutic effect was obtained in all cases. When the injected material cools, it becomes physiologic saline making it possible to inject relatively large volumes in a single session. CONCLUSION: PSIT has promise as a form of curative local therapy for small HCCs and is expected to exert a safe and favorable therapeutic effect on large HCCs as well.

Involvement of CGRP, substance P and blood circulation in aggravating mechanism of absolute ethanol-induced antral lesions by capsaicin treatment in rats.

The effects of capsaicin-sensitive nerve degeneration (capsaicin-treatment) on the corpus and the antrum was investigated in the absolute ethanol-induced lesion model in rats. The gastric lesion in the antrum were significantly aggravated by the capsaicin-treatment, while those in the corpus were not affected. To clarify the different susceptibility between the antrum and the corpus, the effects on gastric mucosal blood flow (GMBF), mucus secretion and levels of calcitonin gene-related peptide (CGRP) or substance P (Sub P), were investigated by the hydrogen gas clearance method, histochemical methods and immunohistochemical methods, respectively. The GMBF in the antrum was significantly decreased by the capsaicin-treatment, but that in the corpus was not. Moreover, capsaicin-treatment increased the mucus secretion in the antrum, but not in the corpus. Capsaicin-treatment significantly decreased CGRP- and Sub P-immunoreactive substances in the vascular smooth muscle in the antrum, but not in the corpus. On the 4th day after absolute ethanol, antral ulcers were observed. From the above results, it was suggested that capsaicin-treatment decreased the gastroprotective ability in the antrum to a greater extent than in the corpus and this may be caused by the decrease of GMBF through the decrease of CGRP- and Sub P-immunoreactive substances.

In vitro testing of a denture cleaning method using ozone.

The purpose of this study was to compare the microbicidal effect of gaseous ozone with that of ozonated water in order to determine its usefulness as a method for disinfecting dentures. Although a large number of research studies have been done on the bactericidal effect of ozone, little is known about its microbicidal effects on oral microorganisms. Therefore, we tested the effect of ozone on three standard strains of oral microorganisms: Streptococcus mutans (strain IID 973), Staphylococcus aureus (strain 209-P), and Candida albicans (strain LAM 14322). When the gaseous ozone injection method was used, the numbers of cells of all three strains decreased to 1/10(5) at 1 min, and by 3 min they were below the detection limit. Thus, the microbicidal effect of gaseous ozone was ascertained in a short time. In contrast, when ozonated water at 1 ppm and 3 ppm was used, C. albicans decreased to 1/10. A 700 mg/h ozone production level was needed to prepare 1 ppm ozonated water, whereas 20 mg/h of ozone was required by the gaseous ozone generator. These findings indicate that direct exposure to gaseous ozone seems to be a more effective microbicide compared with ozonated water, and that gaseous ozone can be clinically useful for disinfection of dentures.

Chronic cyclosporin-induced nephropathy.

AIMS: There is still no consensus about the prognostic influence of chronic nephropathy induced by low-dose maintenance therapy with cyclosporin. Our aim was to investigate the prognostic effect of cyclosporin nephropathy in Japanese renal transplant recipients. MATERIAL: We retrospectively investigated the clinical records of 1,323 kidney transplant recipients who received cyclosporin at 65 institutions in Japan from 1982 to 1991. METHOD: Renal biopsy was performed in 461 patients. RESULTS: At 5 years and 9 years after transplantation, the patients who had cyclosporin nephropathy associated with immunological rejection, glomerulonephritis, or both showed a significantly worse prognosis than those with cyclosporin nephropathy alone (p < 0.01). There was no significant difference in the loss of renal function at 9 years after transplantation between patients showing no abnormalities and patients with cyclosporin nephropathy alone. Even when cyclosporin nephropathy was absent, the long-term prognosis was unfavorable in recipients with immunological rejection or glomerulonephritis. CONCLUSIONS: These results suggest that cyclosporin nephropathy does not influence the prognosis of renal transplantation in patients on low-dose maintenance therapy with cyclosporin.

[Xenon-enhanced CT CBF measurements in intracranial vascular malformations].

In the management of intracranial vascular malformations, it is important to know the regional cerebral blood flow in its surrounding structure. However, CT scan with contrast medium and angiography have only a limited ability to estimate the rCBF. In this study, stable xenon-computerized tomography scanning by means of the end-tidal gas-sampling method was performed in eleven patients with intracranial vascular malformations. Seven of the patients had arteriovenous malformations, three had venous angiomas and one had aneurysm of the vein of Galen. In two patients with large arteriovenous malformations, in two with "larger" venous angioma and in one with aneurysm of the vein of Galen, rCBF values were significantly reduced, particularly adjacent to the malformations. In contrast, there were no areas showing reduced rCBF in cases where the malformations were small. This indicates development of ischemia correlates with the size of malformations. From the xenon-enhanced CT scan and angiographic findings, the presence of steal phenomenon with venous congestion might be a cause of rCBF reduction in those cases where ischemia exists.

Characterization and potential application of purified aldehyde oxidase from Pseudomonas stutzeri IFO12695.

The molecular weight of purified aldehyde oxidase from Pseudomonas stutzeri IFO12695 was estimated to be 160 kDa by a gel filtration method. SDS-PAGE showed that the enzyme consisted of three non-identical subunits with molecular weights of 18, 38, and 83 kDa. The enzyme exhibited an absorption spectrum with maxima at 277, 325, 365, 415, 450, 480, and 550 nm and possessed molybdenum, CMP, iron, sulfur, and FAD as its cofactors, indicating that it belonged to the xanthine oxidase family. A variety of aliphatic and aromatic aldehydes were oxidized; and among them n-hexylaldehyde gave the most rapidly action. When 10 mM formaldehyde was treated with the aldehyde oxidase in the presence of catalase for 240 min, the formaldehyde concentration was reduced to 0.8 mM, suggesting this enzyme might be effective for the removal of formaldehyde contained in wastewater.

Expression of enamel proteins and LEF1 in adamantinomatous craniopharyngioma: evidence for its odontogenic epithelial differentiation.

AIMS: Adamantinomatous craniopharyngioma (ACP) resembles histologically some odontogenic tumours, such as ameloblastoma and calcifying odontogenic cyst. However, there has been no evidence that ACP differentiates also functionally as odontogenic epithelium. The aim of this study was to gain evidence of odontogenic epithelial differentiation in ACP by means of immunohistochemistry. Among normal human tissues, enamel proteins are expressed exclusively in teeth, and lymphoid enhancer factor 1 (LEF1), in co-operation with beta-catenin, play an important role in tooth development. The expression of these proteins is therefore indicative of odontogenic epithelial differentiation. METHODS AND RESULTS: The expression of enamel proteins and LEF1 was examined in 10 adamantinomatous and six papillary craniopharyngiomas. All the ACPs showed a variable degree of enamel protein expression, including amelogenin, enamelin and enamelysin, mainly in ghost cells. LEF1 was also heterogeneously expressed in ACPs; remarkably, its expression pattern was identical to that of nuclear beta-catenin accumulation. In contrast, none of the papillary craniopharyngiomas expressed enamel proteins or LEF1. CONCLUSIONS: These results suggest that ACP consistently shows odontogenic epithelial differentiation. Since ACPs harbour beta-catenin mutation, the inappropriate activation of beta-catenin/LEF1 complex-dependent transcription may play a critical role in ACP tumorigenesis.

Identification of T helper cell subsets in continuous ambulatory peritoneal dialysis patients.

BACKGROUND/AIM: End-stage renal failure induces a clinical state of immunodeficiency with a higher incidence of infections and a higher mortality due to infectious complications as compared with the normal population. A crucial element in this immune response is the development of a network of cytokines. CD4-positive T helper (Th) cells differentiate into either Th1 or Th2 cells in response to various stimuli. It is important to examine whether the impaired immune responses in patients with end-stage renal disease are due to an enhanced Th1 response or a reduced number of Th2 cells. METHOD: Using a newly developed immunofluorescence staining of intracellular cytokines for flow cytometric analysis, we studied Th subsets in 24 patients on continuous ambulatory peritoneal dialysis (CAPD). RESULTS: The proportions of Th1 cells did not differ between CAPD patients (18.7 +/- 1.8%) and controls (19.6 +/- 1.5%). Unlike Th1 cells, a significantly higher proportion of CD4 cells in CAPD patients is characterized by a Th2-type cytokine secretion pattern as compared with healthy controls: 2.9 +/- 0.8% versus 1.7 +/- 0.4%. CONCLUSION: These data demonstrate that Th cells from CAPD patients manifest a dysregulated differentiation profile characterized by a major increase in the percentage of Th2 cells and by a normal percentage of Th1 cells.

The passage of thiamine across the rat placenta and its uptake by the fetal organs. II. The uptake of thiamine by fetal organs.

Having injected 3H-thiamine into the maternal bloodstream of rats in their 15th day of pregnancy, the uptake by the fetal organs (liver and cardiac muscle) of the 3H-thiamine which had been transported across the placenta was studied by electron microscope autoradiography and by measuring the radio-activity of the tissues, using the same methods reported in the first paper of this series. In the fetal liver both developed silver grains and radioactivity were found in greates amounts in specimens collected 5 hrs after injection. Developed silver grains were abundant in the mitochrondria, Golgi apparatus, rough-surfaced endoplasmic reticulum, glycogen area, and nucleus, indicating that these cell organelles take up thiamine and utilize it for the cell metabolism. In the fetal cardiac muscle, the developed silver grains and radioactivity were found in greatest amounts 2 hrs after injection. The silver grains were abundant in the glycogen area, Golgi apparatus, and myofibrils, but they were especially abundant in the mitochondria. This indicates that thiamine is taken up by these organelles and employed in the cell metabolism.

[Effects of morphine, buprenorphine and butorphanol on airway dynamics of the rabbit].

An investigation was made on the effects of morphine, buprenorphine, and butorphanol on airway dynamics with special reference to the airway smooth muscle in rabbits. After tracheostomy under anesthesia, the animals were ventilated with a Servo 900B ventilator, and airway resistance (R) and total compliance (C) were measured according to the method of Norlander, and the effect of each drug was examined. Intravenous injection of morphine 0.2 mg.kg-1 increased R significantly at 10 and 20 minutes after its administration in nontreated animals and changed R insignificantly in animals pretreated with intravenous administration of chlorpheniramine 2.5 mg, an antihistaminic. It is considered that the increase in R after the administration of morphine in the nontreated animals is due to histamine release. Buprenorphine 0.8 micrograms.kg-1 and butorphanol 0.1 mg.kg-1 brought no significant changes in R in both nontreated and the chlorpheniramine pretreated animals. C decreased in all the groups including the control group which received mechanical ventilation alone without any drug administration.

Characterization of stem cells from exfoliated deciduous teeth from a patient with Alagille syndrome carrying a JAG1 mutation.

PURPOSE: Alagille syndrome (ALGS) is an autosomal dominantly inherited disorder primarily caused by mutations in the Jagged Canonical Notch Ligand 1 (JAG1) gene. Although many pluripotent stem cells are well established, no patient-derived stem cells from exfoliated deciduous teeth (SHED) have been developed. In this study, we aimed to establish SHED from an ALGS patient carrying a heterozygous JAG1mut mutation. METHODS: We isolated SHED from a deciduous tooth of an ALGS patient with a heterozygous JAG1 mutation (ALGS-SHED) by the colony-forming unit-fibroblast (CFU-F) method. We then compared the characteristics of ALGS-SHED and healthy donor-derived control SHED (CONT-SHED). RESULTS: ALGS-SHED displayed mesenchymal stem cell features as indicated by CFU-F formation, immunophenotype, and mesenchymal multipotency into adipocytes, chondrocytes, and osteoblasts. ALGS-SHED showed reduced population doubling capacity and exhibited induced chondrogenic potency and diminished osteogenic potency, but similar hepatic potency. ALGS-SHED damaged in situ potency to form bile duct-like tubular structures in the livers of chronically CCl4-injured mice. CONCLUSIONS: We successfully established ALGS-SHED from an ALGS patient carrying a heterozygous JAG1 mutation. Our established ALGS-SHED may represent a potential model for studying ALGS involving a JAG1 mutation.

Jugular compression and radionuclide cisternographic patterns in patients with chronic headache.

OBJECTIVE: We investigated the value of the jugular compression test (JCT) in screening patients with chronic headache attributable to persistent cerebrospinal fluid (CSF) leakage. METHODS: Clinical records of 35 consecutive patients who underwent both 111In-diethylenetriamine pentaacetic acid radioisotope (RI) cisternography and JCT were retrospectively analyzed. RESULTS: A strong correlation was seen between JCT and RI cisternographic findings. Most patients who reported a feeling of fullness in the ear, hearing loss or headache during JCT had positive findings on RI cisternograms indicative of CSF leakage. In contrast, no RI study abnormalities were seen in patients reporting no symptoms in JCT. CONCLUSIONS: Among patients complaining of refractory headache and other miscellaneous symptoms, JCT may represent a simple, economic, and reliable technique in the screening of candidates for RI cisternography to evaluate CSF leakage. A subgroup of patients with chronic headache may have persistent CSF leakage.

Direct projections from the cerebellar nuclei to the superior colliculus in the rabbit: an HRP study.

Cerebellar projections to the superior colliculus in the rabbit were studied by the anterograde and retrograde HRP methods. Cerebellotectal fibers arise mainly from the anterior and posterior interpositus nuclei and terminate contralaterally in layer VII, layer VI, layer V, and the deep tier of layer IV of the superior colliculus. Cerebellotectal fibers from the posterior interpositus nucleus originate from the lateral part of the nucleus and end chiefly in the caudal part of the superior colliculus. Cerebellotectal fibers from the anterior interpositus nucleus arise from the ventral part of the nucleus and terminate mainly in the rostromedial part of the superior colliculus. Some neurons in the lateral cerebellar nucleus also send fibers contralaterally to the intermediate and deep layers of the superior colliculus, especially to its rostral and lateral parts. Few, if any, cerebellotectal fibers arise from the medial cerebellar nucleus.
