Management of the patient in respiratory distress requires an efficient and accurate diagnostic and therapeutic strategy. This article describes the approach to patients with respiratory compromise, including the indications and techniques for performing emergency surgical procedures. The clinical features of upper airway obstruction, thoracic wall trauma, and pleural space disease are discussed.
Common respiratory diagnostic imaging techniques using radioisotopes are ventilation-perfusion, perfusion, aerosol inhalation and tumor scintigraphy. The special features of respiratory imaging diagnostic techniques using radioisotopes are the possibility of early detection of disease and the non-invasiveness of the procedure. Also, no patient effort is required. The kinds of isotopes used are as follows: ventilation-perfusion (133Xe, 13N, 99mTc-pseudogas), perfusion (131I, 99mTc), aerosol inhalation (99mTc), tumor (67Ga, 201Tl). The advantages of these procedures are expected to lead to further developments in respiratory examinations using radio isotopes.
Allergy is a systemic disorder that can affect the nose, lungs, eyes, skin, and gastrointestinal tract, simultaneously or in succession, during the course of a patient's life. The reason for the variety in clinical expression of allergic disease is not known. Genetic predisposition, as well as environmental factors, contribute to the development of the allergic phenotype. Local and systemic inflammatory processes also seem to play a role; however, their exact contribution to the clinical manifestation of airway allergy still remains to be clarified. Although it is clear that allergen exposure of a sensitized target organ is associated with immunopathologic changes at the level of other target organs, the mechanisms underlying this relationship are far from being resolved. Today, most data suggest a systemic link between mucosal sites, involving the bloodstream, bone marrow, and mucosa-associated lymphoid tissue. This review will focus on the interaction between the nose and lungs in allergic disease and the possible implications for the gastrointestinal mucosa.
We studied changes in organism's sensitivity and resistance to hypoxic hypoxia under conditions of NMDA receptor blockade with MK-801. Breathing hypoxic gas mixture after administration of MK-801 sharply decreased the mean blood pressure in the systemic circulation and slightly improved lung ventilation in animals. Respiratory arrest was observed in 40% animals. NMDA receptor blockade decreased organism's sensitivity and resistance to hypoxic hypoxia.

In experiments on rats it was shown that injection to rats before hemorrhage of analeptic corasol or prior injections during three days of the inhibitory mediator sodium oxybutyrate decrease individual resistance of the respiratory and circulatory systems (arterial blood pressure, cardiac output, organ portal blood flow) to posthemorrhagic hypoxia. Injection to animals before the hemorrhage of GABA-A antagonist picrotoxin significantly raises individual resistance to massive bleeding. Individual resistance to acute hemorrhage correlates with individual sensitivity of cardiovascular and respiratory systems to corasol.

Some of the major unresolved questions in the respiratory physiology and pathophysiology of exercise are addressed. To solve many of these difficult basic problems and to understand how systems operate alone and in combination in an integrated fashion, truly experimental integrative physiological approaches are required in both humans and animals.
