[Chondropathia patellae--preoperative and postoperative demonstration of the patellar structure in plain roentgenography (author's transl)].

The study is based on 86 patients surgically treated for chondromalacia patellae, who in a 2 year follow-up were controlled by défilé-radiographs and clinical examination. While there was an improvement of all clinical symptoms examined, no significant change in the radiologic appearance of the patellar structure was observed as compared with the pre-operative status. This includes that neither significant changes indicating development of arthrosis nor aggravation of pre-existing arthrotic lesions could be found. However, it is commonly suggested from roentgenographic findings with type 3 lesions, that severe chondromalacia patellae may be precursor of osteoarthrosis.

[Retropatellar knee pain and chondropathy patella].

The phenomenon of the painful knee occurring at the patellofemoral joint of the adolescent knee is known as chondromalacia patellae. The constellation of signs and symptoms that fall with in the generally accepted criteria for this diagnosis may well cover more than one pathologic process. Chondromalacia patellae is a term descriptive of changes present on the patellar articular surface and not a single pathologic entity.

[Chondropathy of the patella. Clinico-radiologic study].

Totally 165 cases of chondromalacia patellae, taken from a series of 240 patients, are presented. The cause of the chondromalacia lies in imbalances, displacements, dysplasias of patellae and in femoro-tibial axial deviations. Overload is the constant intermediary between the different starting causes and the same final result. The precocity of the clinical-radiological diagnosis is essential for the timely carrying out of a corrective surgery, directed to remove the starting causes before the reversible chondromalacia develops into irreversible arthrosis.

[Diagnosis and operative treatment of chondromalacia patellae by osteotomy and ventral or ventral and medial transposition of the tuberositas tibiae (Maquet/Bandi-procedure) (author's transl)].

The clinical picture of chondromalacia patellae is outlined by the patellar syndrome. The diagnostic value of double contrast arthrography by tangential X-ray (skyline-view)-technique of the femoro-patellar-joint is stressed. If conservative treatment is not satisfactory, osteotomy and ventral transposition of the tuberositas tibiae should be performed. When the patella and the extensor apparatus are dislocated laterally, it is necessary to displace the tuberositas in medial direction. From 1976 to 1980 159 patients have been treated this way. Follow-up study in 86 cases (1 to 5 years after operation) shows excellent and good results in a percentage of 77.

Transverse fractures of the patella.

A technique termed longitudinal anterior band with or without cerclage (LAB/C) is recommended for internal fixation of transverse patellar fractures. The fixation is secure even with the existence of considerable comminution in one fragment. The security of this fixation allows early mobilization and the prevention of knee stiffness. The technique involves longitudinal drill holes through which a wire is pulled and looped over the anterior surface of the patella. When folded and tied over the patella, it allows excellent anterior compression and firm fixation. A cerclage wire is applied primarily for marked comminution. Sixteen patients have been treated in this manner with excellent results. To date there have been no complications from wire breakage or fragment distraction, and early motion has been achieved.

Painful medial bipartite patellae: a case report.

Bipartite patellae, though not unusual as an incidental roentgenographic finding, are rarely painful. This is a report of bilateral medial bipartite patella in a 20-year-old woman, which seems not to have been previously described in the literature. The symptoms were significantly relieved by an excision of the accessory ossification center.

[Operative treatment of lateral hyperpressure syndrome of the patella].

The lateral hyperpressure syndrome of the patella according to Ficat is characterized as a main cause of chondropathia patellae. Symptoms and course as well as roentgenology of this syndrome are illustrated. The logical method of treatment of this syndrome is the lateral release-operation according to Viernstein and Weigert. With this operation we gained good and very good results in 77 per cent of the 30 operated knee-joints.

Patella and patellofemoral resurfacing (37 cases report).

Chondromalacia and patellofemoral osteoarthritis are common diseases that cause pain and disablement of the knee. Conservative therapy is not always effective. Since 1983 we have used isolated polyethylene patellar prosthesis for patella resurfacing of 19 patients with patellofemoral osteoarthritis. After an average of 44.9 months follow-up, we found the treatment was not as satisfactory as earlier trials. The main reason is the wide erosion of femoral condyle caused by the polyethylene patella. To overcome this shortness, we designed a new type of patellofemoral prosthesis which is named Y-L-Q. From January, 1991 to November, 1993, we used this prosthesis to treat 16 knees of 13 patients with patellofemoral osteoarthritis. Most of those patients improved both symptomatically and functionally. The good to excellent results rate was 87.5% (14/16 knees) at the time of an average 16 months follow-up. The early results of our experience with patellofemoral resurfacing are encouraging, and extended follow-ups are in progress.

Acute fracture bipartite patella: case report and literature review.

Disorders of the patella are the most common cause of anterior knee pain. The etiologies of anterior knee pain are reviewed. A case report of an acute displaced patella fracture in a bipartite union is presented. Bipartite patellar development, incidence, radiographic findings, and clinical symptoms follow. Treatment of excision of displaced fragment provides an excellent result.

Linkage analysis of the nail-patella syndrome.

Nail-patella syndrome (NPS) is an autosomal dominant disorder characterized by dysplasia of nails and patella, decreased mobility of the elbow, iliac horns, and, in some cases, nephropathy. The disorder has been mapped to the long arm of chromosome 9, but the precise localization and identity of the NPS gene are unknown. Linkage analysis in three NPS families, using highly informative dinucleotide repeat polymorphisms on 9q33-q34, confirmed linkage of NPS to this chromosome. Recombinations were detected, by two-point linkage analysis, between NPS and the centromeric markers D9S60 and the gelsolin gene and the telomeric markers D9S64 and D9S66, in one of the families. Haplotype analysis suggested an additional recombination between NPS and the argininosuccinate synthetase (ASS) gene. These results localize the NPS gene to an interval on 9q34.1, distal to D9S60 and proximal to ASS, comprising a genetic distance of approximately 9 cM. This represents a significant refinement in the localization of the NPS gene.

[Long-term follow-up of patients with patella resurfacing].

Patellar resurfacing was performed by the authors. From 1983 to 1988, 19 cases (22 knees) with patello-femoral osteoarthritis were treated with this procedure in our hospital. Excellent and good results were recorded in 13 cases (59.1%) one year after operation, but were zero in the long-term follow-up (average 8.7 years) because of recurrent knee pain due to the wearing of the cortex of the femoral groove. Further operation was necessary for those cases, such as patellectomy or total knee replacement. Since 1991 Y-L-Q's type of patellofemoral replacement has been used in 11 cases in our department to alter the isolated resurfacing of the patella. The femoral groove was replaced with a metallic implant that allows sliding of a polyethylene patella to prevent the wearing of the femoral cortex. Preliminary results are good.

Patella baja after displacement of tibial tuberosity for patellofemoral disorders.

The change of patellar height after the displacement of tibial tuberosity and its effect on the improvement of clinical signs of knees was studied. The anterior displacement of tibial tuberosity was performed on fourteen knees and the anteromedial displacement on twenty knees with patellofemoral arthrosis of twenty-nine patients, four men and twenty-five women with the age at operation ranging from 47 to 79 years (average: 58.3). The anteromedial displacement of tibial tuberosity was performed on ten knees and medial displacement on fifteen knees with recurrent patellar dislocation (eleven knees) or patellar subluxation (fourteen knees) of twenty-four patients, two men and women with the an age range from 14 to 37, (average: 22.4). Patellar height was measured on lateral view by the Insall-Salvati method using Ligament/Patella ratio (Lt/Lp ratio). Ligament/Patella ratio of the older group with patellofemoral arthrosis was decreased significantly (p < 0.01) after surgery, from 1.10 to 0.94. The ratio of the younger group with patellar dislocation or subluxation showed no significant changes, but of seven knees with high riding patella, which ratio revealed more than 1.2 before operation, decreased significantly (p < 0.001) from 1.47 to 1.31. The postoperative patella baja had no poor effect on the improvement of clinical signs.

Intra-articular vertical dislocation of the patella: a case report of an irreducible patellar dislocation and unique surgical technique.

Intra-articular vertical dislocations of the patella are rare injuries that occur when a laterally or medially directed force is applied to the opposite patellar edge, causing it to rotate about its longitudinal axis. Diagnosis of this injury is not difficult with physical examination alone, but roentgenograms are necessary to rule out associated fracture and to define the direction of the articular surface. A Schanz screw is useful as a reduction tool to restore the patella to its normal relationship in the femoral groove if closed reduction proves impossible.

[Surgical treatment of habitual patella dislocations by combined soft tissue surgery].

Since January 1988 21 operations using a modified Madigan and Goldthwait method have been carried out on 18 patients suffering from recurrent dislocation of the patella. This soft tissue procedure is presented here in its individual operational steps. The average age of the patients was 22 years and the majority were female (6:1). The mean follow-up time was 19.3 months. One patient who had both knees operated on reported recurrent luxation 6 months after the operation in one knee and subluxation in the other. None of the other patients suffered further dislocations. In the opinion of the patients, 17 (81%) knee operations were evaluated as favorable (good/very good, satisfactory) while 4 (19%) were considered unsatisfactory. All patients had stable knee joints and a full range of motion. The modified soft tissue operation presented here can be recommended for all patients with recurring patella dislocations if previous conservative functional therapy remains unsuccessful.

Recurrent dislocation of the patella in Kabuki make-up syndrome.

Three patients with Kabuki make-up syndrome (KMS) who had recurrent dislocation of the patella are reported. In all, dislocation of the patella developed in adolescence, and they had generalized ligamentous laxity and dysplasia of the patellofemoral joints. In two of the three patients, surgical intervention was required; the operation was successful in one, but yielded poor results in the other. In the third patient, cardiac complications precluded operation. Orthopedists should be aware of this possible complication when treating patients with KMS.

Comminuted avulsion fractures of the inferior pole of the patella.

Treatment of comminuted avulsion fractures of the inferior pole of the patella is still an unsolved problem. Different methods of treatment have been introduced, some quite controversial, including immediate total patellectomy; a variety of partial patellectomies, and fixation by tension band wiring. Yet the problem of early development of arthrosis and a certain degree of limitation of motion due to the changed biomechanics, retropatellar incongruity and postoperative immobilization must still be resolved. We report two cases with comminuted avulsion fractures of the inferior pole of the patella, treated by fixation with a basket plate. The method allows for immediate postoperative mobilization of the knee joint and early weight bearing.

Rotatory intra-articular dislocation of the patella in two dogs.

Two young (8 and 13 week old) dogs were referred for evaluation of acute lameness. Physical examination, radiography, and surgical exploration of their stifles confirmed rotatory intra-articular dislocations of the patella. The dislocated patellas were stabilized with wire into their anatomically correct positions and allowed to heal. Follow-up radiography revealed patellar elongation attributable to excessive peripatellar mineralization. Patellar orientation, as evident on radiographs obtained after trauma, may indicate whether closed or open reduction of these injuries is necessary.
